Abstract

Nell’area della città messapica di Muro Leccese (loc. Cuella) sono stati scavati, al di sotto di una struttura in “tufina”, tre contesti, probabilmente il residuo di sacrifici di fondazione, riempiti con ossa di pecora e databili agli inizi del IV secolo a.C. Si tratta di tre “fosse” contenenti i resti ossei non bruciati di animali. La prima fossa era caratterizzata da una deposizione accurata ed intenzionale delle ossa di una pecora, la seconda fossa era invece riempita dagli scheletri, in parziale connessione anatomica, di tre pecore. La terza conteneva anch’essa i resti di quattro pecora ma non in connessione anatomica. Il lavoro presenta le caratteristiche morfologiche degli animali, analizzando inoltre le pratiche rituali connesse ai tre differenti tipi di contesto archeologico.

The clinical efficacy of SLIT has been established in a number of clinical trials and meta-analyses. However, because SLIT is self-administered by patients without medical supervision, the degree of patient adherence with treatment is still a concern. The objective of this study was to evaluate the perception by allergists of issues related to SLIT adherence.Methods: We performed a questionnaire-based survey of 296 Italian allergists, based on the adherence issues known from previous studies. The perception of importance of each item was assessed by a VAS scale ranging from 0 to 10.Results: Patient perception of clinical efficacy was considered the most important factor (ranked 1 by 54% of allergists), followed by the possibility of reimbursement (ranked 1 by 34%), and by the absence of side effects (ranked 1 by 21%). Patient education, regular follow-up, and ease of use of SLIT were ranked first by less than 20% of allergists.Conclusion: These findings indicate that clinical efficacy, cost, and side effects are perceived as the major issues influencing patient adherence to SLIT, and that further improvement of adherence is likely to be achieved by improving the patient information provided by prescribers.Keywords: adherence, sublingual immunotherapy, efficacy, cost, side effects

Abstract

Nel presente lavoro è esaminato il campione di resti animali rinvenuti durante la campagna di scavo condotta nel 2007 a Castro dal Dipartimento di Beni Culturali dell’Università del Salento. Il contesto di riferimento (US 110) è stato interpretato come una discarica di età ellenistica, in giacitura secondaria, questa si trovava a ridosso di una struttura muraria, forse pertinente a fortificazioni messapiche o a fondazioni di un tempio, tagliate nella parte superiore dal successivo impianto delle mura aragonesi. I resti faunistici esaminati, pur nella loro esiguità, forniscono alcune utili informazioni sulle probabili attività cultuali che si svolgevano nel santuario. Si segnala la presenza di alcuni astragali, specialmente di ovicaprini, la cui peculiarità consiste nell’essere stati modificati mediante l’asportazione o l’abrasione di porzioni superficiali su una o più facce. Tali porzioni scheletriche possono essere interpretate come oggetti di particolare significato religioso, così come documentato in molti contesti cultuali e funerari dell’antichità. A conferma della natura cultuale concorrono, inoltre, anche altre caratteristiche dei reperti: come le frequenti tracce di macellazione e combustione, ma soprattutto l’età giovanile di abbattimento degli ovicaprini e suini, che potrebbe evidenziare una selezione degli animali per il sacrificio.

Abstract

Il campione faunistico del Castello Aragonese di Taranto proviene dalla cosiddetta galleria nord, indagata durante le campagne di scavo protrattesi tra il 2004 e il 2006. I materiali ceramici associati ai resti ossei si inquadrano cronologicamente tra il XVI e il XVII secolo. Il record osteologico presenta un ottimo stato di conservazione, tale da consentire la determinazione di una buona parte del materiale dal punto di vista sistematico e anatomico e fornire un quadro sufficientemente chiaro di quale fosse l’approvvigionamento alimentare del castello.

14. ¿Un espacio para pensar alternativas? La academia latinoamericana de derecho internacional económico frente al orden económico global

Abstract

This article aims to analyse the role of the Latin American scholarship on international economic law in the formation of the global economic order. A large number of the problems that many Latin Americans face today relate to the global economy. Labour conditions, access to medicines, the use of natural resources, among others, are closely connected to the global economic order. In few instances, however, the debate on these problems recognises the fundamental role of the international economic law scholarship. Although the actual knowledge created by this scholarship does not explain why States behave in a certain way, it may serve to explain why they do not behave otherwise. This article argues that the scholarship on international economic law plays a crucial role in the creation and reproduction of the current global economic order. If this is correct, the regional scholarship can do more for Latin American than meeting the advisory and litigation needs of states. It can promote alternatives that can help the region and its people to find their place in the global economy., Esta contribución tiene por objetivo analizar el rol de la academia latinoamericana de derecho internacional económico frente al orden económico global. Gran parte de los problemas que hoy sufren los latinoamericanos se vincula a la globalización económica. Condiciones laborales, acceso a medicamentos, uso de recursos naturales, entre otros, están íntimamente ligados al orden económico global. En el debate sobre estos problemas, no obstante, pocas veces se reconoce el rol fundamental que tiene la academia de derecho internacional económico. Si bien es cierto que el conocimiento articulado por la academia no es suficiente para explicar por qué los Estados actúan de cierta forma, quizás sea suficiente para entender por qué no actúan de otra manera. En este marco, el argumento central de esta contribución es que la academia de derecho internacional económico tiene un rol clave en la creación y reproducción del actual orden económico global. Si esto es correcto, la academia regional puede hacer más por Latinoamérica que servir de apoyo a las necesidades estatales, tanto de asesoría como de defensa de sus intereses en controversias legales. Al tomar conciencia de su rol constitutivo del orden económico global, la academia regional puede ser un medio fundamental para articular alternativas que permitan a Latinoamérica y su población encontrar su lugar en la globalización.

En los últimos diez años, la inversión extranjera en agricultura se ha incrementado notablemente en Latinoamérica. Inversores extranjeros tanto privados como estatales están interesados en la producción agrícola, ya sea por el beneficio esperado o para asegurarse el abastecimiento de alimentos. Este aumento de la inversión puede ser positivo para los países de la región. No obstante, para realizar esta determinación, es necesario comprender, entre otros, los efectos legales de estos flujos de inversión.Este trabajo sostiene que los derechos de propiedad que adquieren los inversoresextranjeros sobre la tierra y los emprendimientos agrícolas puedenser diferentes de los derechos de los inversores nacionales Esto se debe a los tratados para la protección de inversión extranjera. El fortalecimiento de losderechos de los inversores extranjeros afecta la relación de estos inversorescon los Estados receptores. Este artículo analiza los efectos de losderechos de los inversores extranjeros en relación con cinco categorías deiniciativas estatales. Concretamente, 1) el derecho a exportar la produccióno al precio de exportación frente a medidas comerciales y tributarias; 2) elderecho a continuar el proyecto frente a cambios dentro del paradigma regulatorio;3) el derecho a continuar el proyecto frente a cambios de políticao nuevas preferencias sociales; 4) el derecho a recibir una compensaciónmayor frente a reformas agrarias o programas de reparación histórica;5) el derecho a mantener su plan de negocios frente a una crisis alimentaria.

29. Ultrasound-guided viscosupplementation of subacromial space in elderly patients with cuff tear arthropathy using a high weight hyaluronic acid: prospective open-label non-randomized trial.

Abstract

Objective: To assess if ultrasound (US)-guided viscosupplementation can reduce pain and improve function in elderly patients with cuff tear arthropathy.Patients and Methods: Ninety-three patients aged over 65 with grade 3 or above cuff tear arthropathy were included in this prospective open-label non-randomized trial. Institutional review board approval and informed consent were obtained. Thirty-three patients received intra-articular injections of sodium hyaluronate under US guidance. Sixty patients constituted the control group. Shoulder joint function was assessed with Constant scores (CS) and pain with a visual analogue scale (VAS).Results: Compared with controls, treated patients reported a significant decrease in symptoms at 1 (mean CS 66±3.1 vs 37±6.9; mean VAS 1.9±1.2 vs 6.9±2.2), 2 (mean CS 65±3.2 vs 35±7.2; mean VAS 1.7±1.2 vs 6.8±2.5), 3 (mean CS 66±3.4 vs 33 6.1; mean VAS 2.3±1.2 vs 6.6±1.9), and 4 (mean CS 62±3.0 vs 34±6.5; mean VAS 3.3±1.4 vs 7.8±3.1) months, p<0.001. After 5 months there were no differences.Conclusion: US-guided viscosupplementation is a beneficial therapeutic option in the first months of treatment. [ABSTRACT FROM AUTHOR]

31. Finite Element and Finite Difference Methods in Engineering

Abstract

By means of a simple example, a stretched string under transverse load, finite element and finite difference methods which are so widely used in engineering are illustrated. The finite element method is shown to be an essentially modified Raleigh-Ritz procedure. The finite difference technique is applied directly to the string differential equation; an energy related approach is also discussed. The manner in which a combination finite element/finite difference solution can be effected for the same physical problem is treated. Application of both the finite element and finite difference methods to more complex problems as well as selected programs and depositories are mentioned.

Abstract

The title problem is solved by estimating the maximum strain rate field in a circular membrane associated with a modal velocity condition when one-half the kinetic energy in the system has been dissipated. The radial stress may only vary in the radial direction, but is taken as a constant with time at any given location. Internal plastic energy absorbed during the large deformation is found by integrating while accounting for rate sensitivity effects; this absorbed energy is equated to the initial kinetic energy in the system to obtain the final deformed configuration. Correlation of analytically determined results with a series of experiments reported elsewhere is very good. The procedure described here is potentially extendable to any planform membrane shape as well as to axisymmetric shells. The method is applicable to plate problems when membrane effects dominate over bending, that is for deformations of the order of two thicknesses or more. A simple general formula for final plate deformation is devised and also agrees well with experimental results.

33. A Simplified Method to Account for Plastic Rate Sensitivity With Large Deformations

Abstract

A string supported impulsively loaded mass is used to study large deformation rate sensitivity effects where membrane action is dominant. It is found that an overall correction factor can be devised using physical properties associated with the average strain rate. Maximum strain rate occurs with a velocity field corresponding to the deformation state wherein half the initial kinetic energy has been dissipated. (If V0 is initial velocity, V0/2 is associated with maximum strain rate.) Exact and approximate solutions for a broad range of parameters serve to illustrate and verify the procedure. A discussion is presented to show how the same methodology could also be applied via a modal approach to an arbitrary three-dimensional structure undergoing large deformations, if the primary mechanism for energy absorption is from membrane action.

34. A General Nonlinear Relaxation Iteration Technique for Solving Nonlinear Problems in Mechanics

Abstract

The nonlinear relaxation method, an iterative approach used in conjunction with finite-difference approximations, is illustrated via the solution to a very simple problem. Subsequently, the method is used to solve three geometrically nonlinear problems in mechanics: finite bending of a circular thin walled tube, the large deflection membrane response of a spherical cap, and finite deformations of a uniformly loaded circular membrane. Formulations for the three problems are quite different but this difference does not inhibit the use of the nonlinear relaxation technique. Solutions were obtained in approximately one man day per problem including the total time devoted to examining, planning, programming, debugging, etc. Solutions compare very favorably with results found elsewhere in the literature. The essential and important advantages of the nonlinear relaxation technique are (a) versatility and ease of application, (b) efficiency with respect to people and computer time utilized, (c) insensitivity to starting values as far as convergence is concerned, and (d) simplicity of logic that makes it a trivial task to learn how to employ it.

35. Asymmetric Buckling of Spherical Caps With Asymmetrical Imperfections

Abstract

The title problem has been solved using the nonlinear relaxation technique to treat the finite-difference representation of the differential equations governing cap response. For clamped spherical caps designated by λ = 6, buckling loads are somewhat lower for the same imperfection magnitudes than those obtained previously for asymmetric and axisymmetric buckling. The three differential equations of the Marguerre type are set up in terms of three displacement quantities as the dependent variables. The equations are formulated so as to allow arbitrary initial imperfections, boundary conditions, and loading. The computer program setup to solve these equations was checked by successfully solving simpler problems, the solutions of which are in the literature. Although the buckling loads for spherical caps with asymmetric initial imperfections appear reasonable, it is not possible to ascertain at this point whether the cap results are indicative of what the full shell might do.

36. A Numerical Solution for the Nonlinear Deflection of Membranes

Abstract

A numerical method of solution for the nonlinear deflection of thin flat membranes subjected to normal forces as well as forces in the plane of the membrane has been given by Hencky. The equations he solved were first derived by Föppl, and they also follow directly from the von Kármán nonlinear plate equations on formally making the plate stiffness zero. Föppl’s equations are two in number, one being of fourth order and the other second. The unknown quantities are a stress function and the normal displacement. Hencky’s method of solution does not seem capable of easy generalization. The same class of membrane problems is reconsidered here. By casting the problem entirely in terms of displacement components three simultaneous nonlinear second-order partial differential equations are obtained, and a technique is here devised by means of which these equations can be solved without difficulty using finite-difference approximations in conjunction with a relaxation-iteration procedure. Various simple preliminary examples are discussed, after which an example involving a rectangular region is considered in complete detail. The technique devised can be used for a region of any shape, including the case where the boundary is curved, and also for any arbitrary given load system.

Abstract

A new energy based finite difference analytical technique is introduced. The method incorporates certain energy concepts and the ability to use arbitrary, irregular meshes within the framework of the Finite Difference Method. This formulation reduces any governing partial differential equations to a set of difference equations containing partial derivatives up to and including the second order. Further certain strong simularities with the popular Finite Element Method are shown and the ability to solve problems with irregular boundaries is discussed. Abstract

Mitral annular calcium (MAC) is a condition that often occurs in patients with systemic hypertension. To evaluate the effectiveness of nifedipine in preventing MAC, 223 patients with systemic hypertension of recent onset and without MAC were selected and randomly enrolled in 3 groups: group 1 (76 patients) received nifedipine; group 2 (72 patients) received enalapril; and group 3 (75 patients) received atenolol. After 5 years, these treatments significantly reduced systolic (p0.001) and diastolic (p0.05) blood pressure (BP) in 3 treated groups. M-mode echocardiography revealed MAC only in 2 patients in the nifedipine group (2.6%), in 13 in the enalapril group (18%) and in 15 in the atenolol group (20%). The degree of MAC was mild (5 mm) in the 2 patients in group 1, in 5 of the 13 in group 2, and in 6 of the 15 in the group 3, whereas it was severe (5 mm) in the remaining 8 in the enalapril group and in the other 9 in the atenolol group. There was also a significant correlation in the degree of MAC, left atrial enlargement and mitral regurgitation. In addition, atrial fibrillation and atrioventricular conduction defects were associated with severe MAC. These results indicate that nifedipine is an effective drug both in the long-term management of systemic hypertension and in preventing or delaying MAC.

Abstract

Objective To identify the sequence and significance of left ventricular filling abnormalities associated with progressive aging in humans. Design Cross-sectional study comparing three age groups. Setting: Department of Geriatrics at University of Naples. Participants Seventy-five healthy subjects in three age groups: 25 subjects from 25 to 45 years (Group I), 25 subjects from 46 to 65 years (Group II), and 25 subjects from 66 to 85 years (Group III). Intervention None. Main Outcome Measures All underwent pulsed-doppler echocardiography under color guide to measure the following parameters: peak velocity flow for early (E wave) and late (A wave) mitral flow; A/E wave ratio; deceleration time of mitral flow (MDt); isovolumic relaxation time (IRT); ratio of velocity time integrals of the A wave to the velocity time integrals of the entire mitral spectrum (VTIA/VTIM). Results Peak velocity of the E wave was slightly lower in Group II and III compared to Group I; in contrast, peak velocity of the A wave was greater (P < 0.005) in aged individuals. Also A/E wave ratio, MDt, and IRT were significantly greater with advancing age (P < 0.01; P < 0.05 and P < 0.001, respectively). Finally, VTIA/VTIM tended to be greater in the oldest group, but not significantly so. Conclusions Left ventricular relaxation is progressively impaired in late middle age and old age, presumably an early marker of cardiac aging. Increased left atrial pressure is compromised only in the oldest group, presumably representing a compensation for impaired left ventricular relaxation.
