The pathogenesis is strictly related to the association among the presence of genetic risk alleles and innate and acquired immune response with dramatic consequences on bone remodeling. Clinically, PsA patients may present heterogenicity of articular and periarticular manifestations that may be associated with the presence of comorbidities making treatment decision challenging in patients management. The identification of patient-targeted therapies is still a critical issue. Actually, several biological and synthetic drugs are promising in terms of efficacy and safety profile. National and international treatment recommendations support clinicians in the decision of the best treatment, although they may have limits basically related to updates and different outcomes included in the clinical studies evaluated. The aim of this narrative review is therefore to give guidance for clinicians for PsA patients treatment. For this purpose, we evaluated evidence on biological therapies efficacy used for PsA treatment. Specifically, we reviewed data on biological therapies, Janus kinases (JAK) inhibitors, and drugs with a new mechanism of action that are part of the treatment pipeline. PsA could be considered an enthesal disease because of the link between mechanical stress (entheses) and immunologically active tissue (synovium). Evidence of efficacy of anti-tumor necrosis factor alpha (TNF-α) is supported by reduction of histological vascularity and immune cell infiltrates in synovial tissue after treatment. Certolizumab pegol (CZP) is a polyethylene glycolylated (PEGylated) Fab′ fragment of a humanized monoclonal antibody that binds and neutralizes human TNF-α. The PEG moiety of the Fab fragment, markedly increases the half-life of CZP and confers to the drug a unique structure that differs from the other anti-TNF-α agents tested for the treatment of Crohn’s disease, rheumatoid arthritis, ankylosing spondylitis, axial spondyloarthritis, nonradiographic spondyloarthritis, PsO, and PsA. In contrast to other anti-TNF-α agents, CZP did not mediate increased levels of apoptosis, suggesting that these mechanisms are not essential for the anti-TNF-α efficacy in Crohn’s disease. As CZP, infliximab, and adalimumab, but not etanercept, almost completely inhibited lipopolysaccharide-induced interleukin-1 beta release from monocytes, this cytokine-production inhibition may be relevant for drug efficacy. Due to these characteristics, it has been demonstrated in clinical studies that CZP effectively improves signs and symptoms of arthritis and physical function and skin manifestations of PsO, with a safety profile similar to rheumatoid arthritis. This drug can be considered as a valid treatment in patients affected by PsA. The efficacy and tolerability profiles suggest CZP as a suitable antipsoriatic drug in the treatment of PsA.Keywords: psoriatic arthritis, certolizumab pegol, biological therapies, anti-TNF

Abstract

Objective Baricitinib, an oral Janus kinase (JAK) 1-2 inhibitor, is currently used along biologic DMARDs (bDMARDs) after the failure of methotrexate (MTX) in rheumatoid arthritis (RA). We investigated the efficacy and safety of baricitinib in real life. Methods We prospectively enrolled 446 RA patients treated with baricitinib from 11 Italian centres. Patients were evaluated at baseline and after 3, 6, and 12 months. They were arrayed based on previous treatments as bDMARD-naïve and bDMARD-insufficient responders (IR) after the failure or intolerance to bDMARDs. A sub-analysis differentiated the effects of methotrexate (MTX) and the use of oral glucocorticoids (OGC). Results Our cohort included 150 (34%) bDMARD-naïve and 296 (66%) bDMARD-IR patients, with 217 (49%) using baricitinib as monotherapy. Considering DAS-28-CRP as the primary outcome, at 3 and 6 months, 114/314 (36%) and 149/289 (51.6%) patients achieved remission, while those in low disease activity (LDA) were 62/314 (20%) and 46/289 (15.9%), respectively; finally at 12 months 81/126 (64%) were in remission and 21/126 (17%) in LDA. At all-timepoints up to 12 months, bDMARDs-naïve patients demonstrated a better clinical response, independently of MTX. A significant reduction in the OGC dose was observed at 3 and 12 months in all groups. The serum positivity for both rheumatoid factors (RF) and anti-citrullinated protein antibodies (ACPA) conferred a lower risk of stopping baricitinib due to inefficacy. Fifty-eight (13%) patients discontinued baricitinib due to adverse events, including thrombotic events and herpes zoster reactivation. Conclusion Real-life data confirm the efficacy and safety profiles of baricitinib in patients with RA and provide evidence that drug survival is higher in bDMARDs-naïve and seropositive patients.

Abstract

Objective: To report baseline data of SLE patients enrolled in the Lupus Italian Registry (LIRE). Methods: Patients affected by SLE aged ≥ 16 years were consecutively recruited in a multicenter prospective study comparing two cohorts: patients starting biologic immunosuppressants (BC) and patients starting non-biologic immunosuppresants (NBC). Results: 308 patients were enrolled, 179 in NBC and 129 in BC. Mean age at disease onset and at diagnosis was significantly higher in NBC (p = 0.023, p = 0.045, respectively). Disease duration was longer in BC (p = 0.022). Patients in BC presented arthritis more frequently (p = 0.024), those in NBC nephropathy (p = 0.03). Quality of life was worse in BC (p = 0.031). Anti-dsDNA, low C3, were significantly more frequent in BC (p < 0.001, p = 0.009, respectively). Mycophenolate, methotrexate and azathioprine were the drugs more frequently prescribed in NBC, Belimumab and Rituximab in BC. Conclusion: The predominant organ involvement was different in the two cohorts: kidney involvement predominated in NBC, joint involvement in BC. Despite the younger age at disease onset, patients of the BC had a longer disease duration and more frequently had taken a cumulative prednisone dosage greater than 10 g. Even the pattern of clinical manifestations inducing to prescribe biological rather than conventional immunosuppressants was quite different. Abstract

In Sicilia, in occasione di numerose cerimonie religiose, soprattutto a Pasqua, entrano in scena statue con arti mobili (soprattutto Crocifissi e Madonne) e fantocci antropomorfi o teriomorfi animati da un manipolatore posto al loro interno. Se dal punto di vista fenomenologico, questi ultimi artefatti, costituiscono un ibrido, un incrocio tra umano e artificiale, dai punti di vista funzionale e simbolico, sia che rappresentino i santi apostoli sia che rappresentino eroi fondatori o animali leggendari, si pro-pongono, sempre e comunque, come entità altre dall’umano. I fantocci animati, pertanto, al pari dei mascherati e dei simulacri processionali dei santi, sono da considerarsi delle epifanie di una “alterità” con cui bisogna mantenere un sistema di relazioni positive al fine di garantire la ciclica ricostituzione dell’ordine naturale e sociale. Non a caso, quando irrompono nello spazio comunitario, sono caratte-rizzati da un lato da comportamenti a-normali, tesi a enfatizzarne la diversità e la non-umanità, dall’altro sono destinatari di offerte che rinnovano quel circuito del dono che sempre sostanzia ai rap-porti tra immanenza e trascendenza. In tutti casi, inoltre, le loro peculiarità morfologiche e i loro singo-lari comportamenti performativi, non di rado tricksterici e predatori, rivelano un modo “altro” di vivere il sacro e, più in generale, un modo di vivere “altro” che li propone come espressioni totemiche di una specifica appartenenza comunitaria. In Sicily, on the occasion of numerous religious ceremonies, especially at Easter, statues with movable limbs (especially Crucifixes and Madonnas) and anthropomorphic or theriomorphic puppets animated by a manipulator placed inside them enter the scene. If from a phenomenological point of view, these last artifacts constitute a hybrid, a cross between human and artificial, from a functional and symbolic point of view, whether they represent the holy apostles or that they represent founding heroes or legendary animals, they propose, always and in any case, as entities other than the human. The animated puppets, therefore, like the masked ones and the processional simulacra of the saints, are to be considered epiphanies of an "otherness" with which it is necessary to maintain a system of positive relationships in order to guarantee the cyclical reconstitution of the natural and social order. It is no coincidence that when they break into the community space, they are characterized on the one hand by a-normal behaviour, aimed at emphasizing their diversity and non-humanity, and on the other they are recipients of offers which renew that circuit of the gift which always substantiates to the relationship between immanence and transcendence. In all cases, moreover, their morphological peculiarities and their singular performative behaviors, often tricksteric and predatory, reveal an "other" way of experiencing the sacred and, more generally, an "other" way of living than proposes as totemic expressions of a specific community membership.

Abstract

Pregnancy requires a special management in women with inflammatory rheumatic diseases (RDs), with the aim of controlling maternal disease activity and avoiding fetal complications. Despite the heterogeneous course of RDs during pregnancy, their impact on pregnancy largely relates to the extent of active inflammation at the time of conception. Therefore, accurate evaluation of disease activity is crucial for the best management of pregnant patients. Nevertheless, there are limitations in using conventional measures of disease activity in pregnancy, as some items included in these instruments can be biased by symptoms or by physiological changes related to pregnancy and the pregnancy itself may influence laboratory parameters used to assess disease activity. This article aims to summarize the current literature about the available instruments to measure disease activity during pregnancy in RDs. Systemic lupus erythematosus is the only disease with instruments that have been modified to account for several adaptations which might interfere with the attribution of signs or symptoms to disease activity during pregnancy. No modified-pregnancy indices exist for women affected by other RDs, but standard indices have been applied to pregnant patients. The current body of knowledge shows that the physiologic changes that occur during pregnancy need to be either adapted from existing instruments or developed to improve the management of pregnant women with RDs. Standardized instruments to assess disease activity during pregnancy would be helpful not only for clinical practice but also for research purposes.

Summary: It is evident that our technological society is undoubtedly linked to the diffusion of images. Abstract

Autoimmune diseases (AIDs) share similar serological, clinical, and radiological findings, but, behind these common features, there are different pathogenic mechanisms, immune cells dysfunctions, and targeted organs. In this context, multiple lines of evidence suggest the application of precision medicine principles to AIDs to reduce the treatment failure. Precision medicine refers to the tailoring of therapeutic strategies to the individual characteristics of each patient, thus it could be a new approach for management of AIDS which considers individual variability in genes, environmental exposure, and lifestyle. Precision medicine would also assist physicians in choosing the right treatment, the best timing of administration, consequently trying to maximize drug efficacy, and, possibly, reducing adverse events. In this work, the growing body of evidence is summarized regarding the predictive factors for drug response in patients with AIDs, applying the precision medicine principles to provide high-quality evidence for therapeutic opportunities in improving the management of these patients. © 2020

Abstract

Cosa sia una collezione è noto a tutti e tuttavia, se ci interroghiamo su che cosa tenga insieme attività e oggetti molto diversi fra loro sotto il segno della collezione, le cose non sono più tanto chiare: si tratta di pratiche culturali diverse che presentano somiglianze ma anche profonde discontinuità. Collezionare è molto di più che raccogliere e accumu- lare degli oggetti, è una pratica di costruzione del sapere attraverso la quale si dà forma alla realtà, la si rappresenta e la si espone in forma pubblica o privata: è un modo di darsi ragione del mondo, di costruire un microcosmo tra reale e immaginario che ci parla tanto degli altri come di noi. E un pensiero concreto che si fa con le cose. È lo spazio in cui si mette ordine nel mondo o nella propria vita, ma anche un modo per esplorare altre possibilità. È uno dei luoghi in cui gruppi e persone articolano la propria biografia e costruiscono la propria identità e quella degli altri. Partendo dalle ricerche etnografiche svolte in Italia e in Camerun, Ivan Bargna riflette sulle forme contemporanee del “collezionare” nella quotidianità e nell’arte, dai musei alla discarica, dall’accumulazione compulsiva di cose ed esperienze all’arte di disfarsene, dalle cabine armadio delle nostre case ai tesori dei re dei Grassfields camerunesi. Il collezionismo appare così non la mania di persone eccentriche (o una prerogativa esclusiva dell’occidente) ma come indicatore di dinamiche sociali e culturali più ampie, localmente diversificate e attraversate da flussi globali.
