Skills for Safer Living is a 20-week group intervention for people with recurrent suicide attempts. This article details how it pivoted from in-person to virtual groups due to the COVID-19 pandemic. Concerns of privacy, client safety, and how to deliver the program virtually are explored, along with lessons learned.

In the 1980s, the response rate of major depressive disorder with psychotic features (MDD-Psy) to placebo pills was reported to be close to 0%. To our knowledge, this placebo response rate has not been systematically reassessed. We undertook a systematic review of randomized controlled trials (RCTs) that have used a placebo or sham control group for MDD-Psy.We searched MEDLINE and identified 9 relevant publications reporting on 10 studies comparing a placebo or sham interventions versus an active intervention. We extracted reported rates of response or of dropout for all causes associated with placebo versus active intervention(s) and aggregated response and dropout rates across trials.Two sham-controlled electroconvulsive therapy (ECT) trials did not provide response rates. In the 3 pharmacotherapy studies published in the 1980s, 0 of 12 participants (0%) responded to placebo versus 13 of 38 (34.2%) responding to the active interventions. In contrast, 5 RCTs published in the 2000s, 114 of 339 participants (33.6%) randomized to placebo responded versus 149 of 373 participants (39.9%) randomized to active interventions; dropout rates were 71/236 (30.1%) for placebo versus 84/282 (29.8%) for the active interventions.As expected, response rates to placebo pills in RCTs for MDD-Psy increased markedly from the 1980s to the 2000s. Methodological issues in the design and conduct of more recent RCTs may have contributed to the high placebo response. However, one needs to consider this placebo response rate when interpreting the result of recent trials of MDD-Psy, which typically have not included a "pure" placebo condition.

The combination of a-benzoin-oxime (aboH2) in the coordination chemistry of NiII leadsto the formation of octanuclear clusters which contain an unusual peroxo anion derivedby the reduction of atmospheric oxygen while the reducing agent appears to be theorganic ligand (aboH2) which was found within the clusters in its oxidized form as theanion of a-benzil monooxime (abmoH). The in-depth study of the above reaction systemand the chemical reactivity of the octanuclear cages led to the synthesis of a family ofNiII clusters which counts over 20 family members built around a ι6-δ 3:δ 3-Ο2 2-. Thecombination of pseudochalides and other organic ligands in the above reaction systemdid not alter the cores of the octanuclear clusters but caused significant structuralchanges which in turn drastically changed the magnetic properties of the clusters. Theclusters were characterized by X-ray crystallography, spectroscopic and physicalmethods while their magnetic properties were also investigated.In the second part of this thesis, was completed the study of the magnetic properties ofa family of hexanuclear MnIII cages, while was also studied the coordination chemistryof other oxime ligands in combination with nickel, iron and manganese.

Despite its importance in the real-world, manual (hand) dexterity is often ignored in medical-based virtual training environments that have traditionally focused on cognitive and affective skills development. Psychomotor (technical) skills, particularly those related to manual dexterity, are fundamental to various medical procedures and ignoring them in virtual based training tools can lead to a sub-optimal training experience. Here, we present a novel, consumer-level data glove that provides accurate user interactions involving the proximal and medial phalanges, interactions that are relevant in many manual dexterity tasks. We also outline how this novel data glove is being incorporated into an existing serious gaming platform for anesthesia training that currently focuses on cognitive and affective skills development only. The addition of psychomotor skills development through the adoption of simulated tactile feedback will provide a more complete serious gaming training platform.

While most patients with depression, anxiety, or at-risk drinking receive care exclusively in primary care settings, primary care providers experience challenges in diagnosing and treating these common problems. Over the past two decades, the collaborative care model has addressed these challenges. However, this model has been adopted very slowly due to the high costs of care managers; inability to sustain their role in small practices; and the perceived lack of relevance of interventions focused on a specific psychiatric diagnosis. Thus, we designed an innovative randomized clinical trial (RCT), the Primary Care Assessment and Research of a Telephone Intervention for Neuropsychiatric Conditions with Education and Resources study (PARTNERs). This RCT compared the outcomes of enhanced usual care and a novel model of collaborative care in primary care patients with depressive disorders, generalized anxiety, social phobia, panic disorder, at-risk drinking, or alcohol use disorders. These conditions were selected because they are present in almost a third of patients seen in primary care settings. Innovations included assigning the care manager role to trained lay providers supported by computer-based tools; providing all care management centrally by phone - i.e., the intervention was delivered without any face-to-face contact between the patient and the care team; and basing patient eligibility and treatment selection on a transdiagnostic approach using the same eligibility criteria and the same treatment algorithms regardless of the participants' specific psychiatric diagnosis. This paper describes the design of this RCT and discusses the rationale for its main design features.

A assistência pré-natal é de grande importância para uma gestação saudável, prevenindo e detectando precocemente possíveis complicações inerentes à gravidez e diminuindo, assim, a morbimortalidade materna e fetal. Sendo uma das intercorrências mais prevalentes, a diabetes mellitus gestacional tem grande impacto na saúde materno-fetal e pode causar complicações a curto e longo prazo. Com o fim de verificar a incidência e a mortalidade das complicações da gravidez, do parto e do puerpério e a mortalidade decorrente de diabetes mellitus na gestação no Brasil. Realizou-se a coleta de dados no Departamento de Informática do Sistema Único de Saúde (DATASUS) do período de 2017 a 2019, baseando-se no Sistema de Informações Hospitalares do SUS (SIH/SUS) e no Sistema de Informações sobre Mortalidade (SIM). Diante disso, foi obtido que na mortalidade materna decorrente da diabetes mellitus na gravidez no Brasil houve um aumento de cerca de 18,18% de 2014 para 2015 e de 15,38% de 2015 a 2016. Já de 2016 a 2017, houve um decréscimo percentual de 40%, seguido de um aumento de 40% entre 2017 e 2018 e posterior decréscimo entre 2018 e 2019 de 26,66%. Em suma, a diabetes mellitus gestacional não foi considerada a maior responsável pelos óbitos no período estudado, no entanto, é um distúrbio que deve ser tratado e detectado precocemente, devido ao fato de que pode levar ao desenvolvimento de complicações tanto à gestante, quanto ao feto.

Background We sought to understand poor uptake of the Primary Care Assessment and Research of a Telephone Intervention for Neuropsychiatric Conditions with Education and Resources study (PARTNERs), a pragmatic randomized controlled trial of a collaborative care intervention for people experiencing depression, anxiety or at-risk drinking. We explored primary care providers' experience with PARTNERs, and preferences regarding collaborative care models and trials. Methods In this qualitative study, we interviewed primary care providers across Ontario who had participated in PARTNERs, using stratified sampling to reach high-, low- and nonreferring providers in urban and rural settings. We audio-recorded, transcribed and thematically analyzed the interviews between May and December 2017, collecting and analyzing data concurrently until achieving saturation. Results We interviewed 23 primary care providers. They valued the unique availability of telephone-based coaching for patients but desired greater integration of the coach into their practice. They appreciated expert psychiatric recommendations but rarely changed their practices. Sites varied in organizational adoption and implementation of the study, including whether they designated a local champion, proactively identified eligible patients, integrated the study into existing workflows and reflected on (and revised) practices. These behaviours affected continuing awareness of the study and referral rates. Interpretation Study uptake was influenced by the limited relationship between PARTNERs coaches and primary care providers, and variable attention to leadership, training and quality improvement as vital elements of collaborative care. Study designs focusing on implementation could promote reach and penetration of novel interventions in the practice setting and more successfully advance collaborative care implementation.
