- Abstract
The objective was to describe nursing care in the first hour of life of the newborn (NB). The method of choice was the integrative literature review carried out in the Regional Virtual Health Library (VHL) and Google Scholar, using the Health Sciences Descriptors (DECs): Newborn, Postnatal Care and the Boolean operator AND. In the VHL, 5,446 publications were evaluated by availability and publication in the last 5 years, resulting in 856 analyzed by title and abstract, resulting in 38 publications read in full and the selection of 1 for theoretical synthesis. Google Scholar found 10,400 publications filtered by publication in the last 5 years, resulting in 3,620 analyzed by title and theme, with 22 publications read in full and 6 selected. The results show that nursing care in the first hour of a newborn's life is essential to ensure a smoother adaptation to extrauterine life and, among the results of this practice, the reduction of neonatal mortality, the prevention of complications and the promotion of the bond between mother and child stand out. It is concluded that it is of utmost importance for health institutions to offer training and qualification to nursing professionals to qualify care in this critical period. [ABSTRACT FROM AUTHOR]

9. Experiencias y percepciones de la lactancia materna en mujeres postparto

Objetivo: Identificar las experiencias y percepciones de la lactancia materna en mujeres Guerrerenses. Materiales y métodos: Se llevó a cabo un estudio descriptivo y transversal en mujeres lactantes, con una muestra total de 75 participantes. Se diseñó un cuestionario con preguntas dicotómicas, como "¿Has visto a alguien lactar?" y "¿Percibes que la leche materna es beneficiosa para tu bebé?" Los datos fueron analizados utilizando SPSS v23.Resultados: Los resultados indican que la mayoría de las participantes tiene entre 23 y 27 años (40%) y un alto porcentaje posee estudios de preparatoria (66.7%). La mayoría optó por un parto vaginal (90.7%) y ha tenido uno o dos partos, sugiriendo que son madres recientes. El 88% elige lactancia materna exclusiva. En cuanto a la experiencia en lactancia, todas las mujeres de 32 a 35 años la tienen, mientras que solo el 55.5% de las de 18 a 22 años. Además, se encontraron asociaciones significativas entre variables sociodemográficas y la percepción de los beneficios de la lactancia. Conclusiones: La alta prevalencia de la lactancia materna entre las participantes refleja su educación y experiencias. Estos hallazgos subrayan la necesidad de promover la educación sobre la lactancia para mejorar la salud infantil.

Introduction. During pregnancy, at least 20% of the expectant mothers are affected by several forms of dermatosis, which involve a heterogeneous group of skin conditions with a variety of manifestations and evolution. The objective is to present a case of initial polymorphic eruption during the postpartum period with a brief review of the literature related to the pathology and its treatment. Case report. We present the case of a 38 year-old postpartum patient in her frst pregnancy, who exhibited a clinical case of skin eruption and pruritus that started in the abdominal area and extended to the lower limbs. The lesions consisted of erythematous papules that clustered together to form plaques. She was diagnosed with polymorphic eruption of pregnancy and was treated with antihistamines, which resolved the pathology. Discussion. Polymorphic eruption of pregnancy is a benign infammatory disorder of the skin It starts with the appearance of pruritic papules that cluster together to form erythematous plaques that initially appear in the abdomen (with the exception of the umbilical area) and generally start at the stretch marks and disseminate to the extremities. Treatment consists of the use of emollients and antihistamines to control the lesions and the pruritus. Conclusions. Although this is a benign, self-limiting condition, it is important to reach a correct diagnosis and initiate adequate medical treatment, as symptoms like pruritus can generate lesions from scratching that usually become infected and compromise the patients’ health. 12. Riesgo de depresión posparto en un primer nivel de atención.

Background: Postpartum depression (PPD) is a severe mental disorder that can affect up to 56% of Latina women. PPD has multiple origins and affects maternal well-being, health and behavior, as well as child development. Objective: to analyze the risk of postpartum depression in a primary care center. Material and methods: observational, cross-sectional, retrospective study in patients entitled to primary care, who were in the postpartum period. Likert-type surveys were carried out, based on the Edinburgh Scale, collecting sociodemographic factors, in order to establish the associated risk factors. The association between the study outcome (risk of postpartum depression) with each independent variable was determined using Odds ratio and multivariate models. Results: 79 postpartum patients were included, the average age was 29 years (± 5), 46.8% with a high risk of depression, and 53.2% without risk of postpartum depression. The main factor associated with being able to present a high risk of postpartum depression was low socioeconomic level in 94.6%, other variables did not have statistical significance. Conclusions: the frequency of being at high risk for postpartum depression in our study was similar to that reported by the Pan American Health Organization 2018, with a frequency of 56% in Mexican and Latin American women. Objetivo: analizar la percepción de las puérperas sobre el papel de las enfermeras en el ciclo embarazo-puerperio a la luz de la Teoría de Peplau. Método: estudio descriptivo con abordaje cualitativo, utilizando la Teoría de Peplau como marco teórico-metodológico. Los datos fueron recolectados en un Centro de Parto Normal en Ceará - Brasil, de marzo a mayo de 2021. Se realizaron entrevistas semiestructuradas a 30 puérperas. Se analizaron según las fases de la teoría: orientación, identificación, exploración y resolución. Resultados: la información facilitada por las enfermeras no se comprendió satisfactoriamente y se restringió la autonomía de las mujeres durante el parto. No se exploraron eficazmente recursos como los planes de parto y los grupos de embarazadas. Las puérperas se mostraron mayoritariamente satisfechas con la resolución de sus cuidados. Conclusión: Se destacó la importancia de aplicar la teoría de Peplau a los cuidados de enfermería en el ciclo puerperal del embarazo para promover la autonomía de las mujeres en el proceso terapéutico.

17. Factores de la atención materna asociada a complicaciones obstétricas del periparto y postparto según encuesta demográfica y de salud familiar en el 2021

Objetivos: Determinar la asociación entre los factores de la atención materna y las complicaciones obstétricas del periparto y post parto. Material y métodos: Estudio cuantitativo, analítico, retrospectivo y transversal, basado en datos de la ENDES 2022. Resultados: Se analizó una muestra de 6 158 madres, el 22.2 % tuvieron complicaciones obstétricas. Las más frecuentes fueron fiebre postparto (12%) y sangrado periparto (9.9%). Los factores de la atención materna asociados con incremento de complicaciones del periparto fueron; violencia familiar (PRa=1.6; IC95%1.16-2.20; p=0.004), DM2 (PRa=4.11; IC95%1.85-9.10; p=0.001) y recibir ferroterapia (PRa=9.76; IC95%:1.88-50.64; p=0.007), por el contrario, un control prenatal brindado por personal calificado (PRa=0.29; IC95% 0.13 - 0.65; p=0.003) disminuirán estas probabilidades. Por otro lado, HTA (PRa=2.3; IC95%:1.10-4.82; p=0.027), DM2 (PRa=4), Anemia (PRa=2.11), violencia emocional (PRa= 2.22) aumentan la posibilidad de complicaciones mientras que el Control de Hb (PRa= 0.52; IC95%:0.31-2.76; p=0.014), atención posparto temprano (PRa=0.19) por personal calificado (PRa=0.66) disminuye la prevalencia de complicaciones post parto. Conclusiones: Factores de la atención materna asociados significativamente a complicaciones obstétricas del posparto y periparto fueron características del Control prenatal, las acciones de intervención y preventivo-promocionales, además de factores de riesgo como HTA, DM2, anemia y violencia familiar.

Objetivos: comprender las experiencias sobre la lactancia materna desde la perspectiva de las mujeres, en un grupo de madres lactantes primerizas. Materiales y métodos: estudio cualitativo, bajo la perspectiva de etnografía, enfocado en un grupo de mujeres de la región oriente del departamento de Antioquia (Colombia). Por muestreo intencional y hasta la saturación de los datos se seleccionaron 16 mujeres lactantes primerizas, de 18 años o más, con hijos menores de 24 meses y que se encontraran amantando o hubiesen amantado de forma exclusiva al menos durante tres meses. Se realizaron entrevistas semiestructuradas y observación participante; el análisis de los datos se desarrolló por triangulación y codificación abierta y axial. Los resultados se presentan a través de la descripción de categorías. Resultados: se identificaron cuatro categorías frente a la experiencia del amamantamiento: el agotamiento y el dolor que se disipan con amor, la lactancia y el nuevo orden de las cosas, las emociones de una mujer en el cuerpo de una madre y la lactancia: un acto social. Conclusiones: proteger la lactancia materna requiere poner especial cuidado a la mujer como sujeto, más allá del amamantamiento, dando prioridad a su salud física y emocional como elementos esenciales que deben ser considerados por los profesionales en salud. Se requieren estudios que evalúen los efectos de la creación de grupos de apoyo que propicien el intercambio de saberes para acompañar a la mujer lactante.

Objetivo: Evaluar la asociación entre el apoyo social percibido y las características sociodemográficas y clínicas. Método: Estudio transversal, realizado en una maternidad del noroeste del estado de Rio Grande do Sul/Brasil, de noviembre de 2021 a abril de 2022. Se utilizaron un cuestionario sociodemográfico de caracterización clínica y una escala de apoyo social. Análisis descriptivo e inferencial. Resultado: Las mujeres postparto con estudios superiores obtuvieron puntuaciones medias más altas en apoyo emocional (p=0,015); las mujeres blancas obtuvieron puntuaciones medias más altas en apoyo material (p=0,009); y las que vivían en uniones estables obtuvieron puntuaciones medias más altas en apoyo emocional (p=0,0016), apoyo afectivo (p=0,035), información (p=0,019) e interacción positiva (p=0,032). Hubo diferencias significativas para las variables de que el embarazo fuera planificado, en las que las puérperas recibieron más apoyo material (p=0,015), y las que padecían hipertensión arterial sistémica, que recibieron más apoyo e interacción positiva (p=0,014). Conclusiones: Las mujeres puérperas tenían puntuaciones de apoyo social elevadas. Sin embargo, se observaron puntuaciones más altas entre las de niveles socioeconómicos más altos, en unión, que habían planificado sus embarazos y con hipertensión en el embarazo.

Objetivo: identificar factores que interfirieron en la atención posparto en una región fronteriza durante la pandemia de COVID-19. Método: investigación cualitativa, utilizando como marco metodológico la Teoría Fundamentada. Desarrollado en atención primaria, en una región de la triple frontera, Brasil, Argentina y Paraguay, con 30 participantes entre mujeres puerperales, profesionales de la salud y directivos. Recopilación y análisis de datos entre agosto de 2021 y mayo de 2022, siguiendo las etapas de codificación abierta, axial y selectiva, según la perspectiva straussiana. Resultados: se identificaron cuatro subcategorías: “acceso a las unidades de salud”, “comprensión de las visitas domiciliarias y búsqueda activa”, “intercambio de información insuficiente en la red de atención” y “evidencia de un sistema de salud sobrecargado”. Conclusión: se encontró que la mayoría de los factores que interfirieron en el cuidado de las mujeres puerperales ya existían, siendo agravados por la pandemia. Se recomienda implementar medidas que garanticen el intercambio de información y la contrarreferencia de manera oportuna.

Objective: to know the perception of the partner about the experience of paternity in the pregnancy-puerperal period. Method: qualitative research, developed between July and October 2021, in a virtual environment with 10 men, using the Technique of Creativity and Sensitivity called "Almanac", associated with individual semi-structured interview and thematic content analysis. Results: the paternal participation was evidenced in situations that covered the pregnancy-puerperal period and child development. Even when the presence of the partner was not effective, he sought some form of participation, demonstrating his concern to be available, assist and contribute in the care of the health of his partner and the baby. Conclusion: the exercise of paternity has been changing and overcoming the limiting historical and cultural aspects, resignifying the father's role in today's society. [ABSTRACT FROM AUTHOR]

Introduction: Infection due to SARS-CoV-2 virus has become a pandemic that has caused great number of complications and is usually fatal for pregnant and newlydelivered women. Objective: To describe the clinical and epidemiological characteristics of pregnant and newly-delivered women with Covid-19 admitted to intensive care units. Methods: An observational descriptive, cross-sectional study of 56 pregnant and newlydelivered women with confirmed diagnosis of Covid-19 was carried out; they were admitted to the Intensive Care Unit of Dr. Joaquín Castillo Duany Teaching Provincial Hospital in Santiago de Cuba during 2021. The analyzed variables were age, past health history, cause of admission, state when discharged from the institution and the main cause of death. For this purpose, the frequencies, percentages and case fatality rate were calculated. Results: There was a prevalence of pregnant women in all the age groups (58.9%) relative to the newly-delivered women (41.1%), mainly in the 31- 35 age group (30.3%). Hypertension was the most frequent comorbidity with 71.4 and 28.6%, for pregnant and newly-delivered women, respectively; as long as, 66.6% of the first ones and 33.3% of the second ones were admitted due to pneumonia. All the pregnant women were discharged alive and 7 newly-delivered women died. Case fatality rate was 12.5 and multiple organ dysfunction syndrome appeared in 57.2% of those affected. Conclusions: Hypertension was the most frequent comorbidity, viral pneumonia was the main admission diagnosis and multiple organ dysfunction syndrome, the first direct cause of death. Objective: investigating the presence of nipple trauma in postpartum women at the hospital discharge from joint accommodation. Method: cross-sectional, retrospective and descriptive study, in joint accommodation of a public hospital. Sociodemographic data and data on childbirth, birth, breastfeeding and characterization of nipple injury were collected. The study covered all months of 2018. For analysis, we used the statistical software Statistical Analysis System (SAS), version 9.4, with significance level of 5%. Results: of the sample of 480 postpartum women, 56.04% had nipple traumas, and abrasion was the most frequent lesion (28.13%). Exclusive breastfeeding prevailed at discharge (96.46%) and the use of milk formula was more present when nipple trauma occurred. Conclusion: the high frequency of nipple trauma indicates that it is essential the instrumentalization of health professionals to deal with the difficulties experienced by women in the breastfeeding process, also contemplating identification and classification of nipple trauma. Foundation: during the puerperium there are causes of maternal complications that compromise the woman’s life, a stage that requires continuous monitoring and successful action by health professionals. It is important in the world and in Cuba to monitor the maternal deaths causes’ behavior and the mater-nal patients morbidity in critical condition, for the application of organizational, control and action stra-tegies to improve this indicator.Objective: to characterize the maternal morbidity behavior during the puerperium.Methods: descriptive, cross-sectional study on the morbidity of puerperal women at the Cienfuegos Provincial Hospital, during the year 2020. The universe was composed of 3563 births that occurred. A form for data collection was developed. The analyzed variables were: maternal age, type of delivery, birth weight, gestational age, state of the newborn at birth, and severe and non-serious maternal morbi-dity.Results: there was women predominance between 20 and 34 years of age (76%); 57.9% had eutocic delivery. The postpartum women with maternal morbidity due to severe sepsis were 79 women (2.2%) and with non-serious morbidity 397 women (11.1%). Conclusions: severe sepsis, preeclampsia-eclampsia, and postpartum hemorrhage are the entities that were mostly observed in severe maternal morbidity; those with non-serious morbidity had low birth weight newborns and postpartum anemia.

La hemofilia A adquirida es un trastorno hemorrágico poco frecuente a nivel mundial, y se caracteriza por la presencia de autoanticuerpos inhibidores dirigidos hacia un factor de la coagulación, en la mayoría de ocasiones el factor VIII. Las etiologías son variadas, entre las que se encuentra el posparto. Se presenta el caso de una paciente de 34 años con dolor lumbar, hematuria y hematoma en región glútea derecha, sin antecedentes previos de sangrado. Por extensión de las manifestaciones hemorrágicas es transferida al servicio de emergencia. Los exámenes auxiliares de perfil de coagulación, prueba de mezclas y medición de los títulos de inhibidores del factor VIII permitieron confirmar el diagnóstico. El caso resalta la importancia de considerar esta patología en una paciente puérpera con persistencia de sangrado por herida operatoria, hematoma extenso y sin historia de sangrado previo.

41. El Cuidado del Cordón Umbilical en un Grupo de Mujeres en Estado Puerperal en Luanda

Las prácticas de cuidado con el muñón umbilical varían mucho dependiendo del país, de la región, de la cultura y de las personas que cuidan. El cuidado con el muñón umbilical se realiza, normalmente, en el ámbito del domicilio, teniendo como cuidadores personas de la familia o aquellas próximas (normalmente mujeres) y lo que rige este cuidado no siempre tiene como base evidencias científicas, más bien está basado en creencias que son transmitidas de generación en generación. Muchas de esas prácticas culturales de cuidado con el muñón umbilical entran en conflicto con las prácticas basadas en el conocimiento científico, debido a que se usan substancias o medidas que exponen a los recién nacidos a un riesgo elevado de desarrollar, principalmente, tétano neonatal e infecciones. Así, el objetivo de este estudio fue conocer las prácticas de cuidado del muñón umbilical realizadas en mujeres en estado puerperal que hicieron sus partos en la Maternidad Lucrécia Paim. Fue realizado un estudio de abordaje cualitativo. El local del estudio fue la Maternidad Lucrécia Paim, en Luanda, Angola. Hicieron parte del estudio 29 mujeres que tuvieron sus partos en la Maternidad Lucrecia Paim. Los datos fueron recolectados a través de una entrevista semi estructurada; las entrevistas fueron grabadas y transcritas en su totalidad. La muestra se hizo por saturación de los datos. Para el análisis de los datos fue utilizado el análisis de contenido, modalidad temática. Los resultados mostraron tres temas: 1) Cómo cuidar del muñón umbilical; 2) Objetivos del cuidado del muñón umbilical; y 3) Personas de referencia para el cuidado del muñón umbilical. Se observó que las mujeres en estado puerperal usaban el alcohol y siendo que otras usaban substancias/productos culturalmente conocidos, para facilitar la caída rápida y la desinfección del muñón umbilical. La mayoría de esas mujeres aprendieron a cuidar el muñón umbilical en el medio familiar y en la sociedad con el conocimiento popular que es muy valorizado por esas mujeres; una minoría aprendió con profesionales de la salud. También se destacó que, los profesionales de la salud no orientaban a las mujeres en estado puerperal sobre el cuidado del muñón umbilical del recién nacido.

- Abstract
Foundation: during the puerperium there are causes of maternal complications that compromise the woman's life, a stage that requires continuous monitoring and successful action by health professionals. It is important in the world and in Cuba to monitor the maternal deaths causes' behavior and the mater-nal patients morbidity in critical condition, for the application of organizational, control and action stra-tegies to improve this indicator. Objective: to characterize the maternal morbidity behavior during the puerperium. Methods: descriptive, cross-sectional study on the morbidity of puerperal women at the Cienfuegos Provincial Hospital, during the year 2020. The universe was composed of 3563 births that occurred. A form for data collection was developed. The analyzed variables were: maternal age, type of delivery, birth weight, gestational age, state of the newborn at birth, and severe and non-serious maternal morbi-dity. Results: there was women predominance between 20 and 34 years of age (76%); 57.9% had eutocic delivery. The postpartum women with maternal morbidity due to severe sepsis were 79 women (2.2%) and with non-serious morbidity 397 women (11.1%). Conclusions: severe sepsis, preeclampsia-eclampsia, and postpartum hemorrhage are the entities that were mostly observed in severe maternal morbidity; those with non-serious morbidity had low birth weight newborns and postpartum anemia.
