Background: Cold renal perfusion (CRP) with 4[degrees]C crystalloid fluids has been described as a method to reduce renal injury during open surgical repair of complex aortic aneurysms (cAAs) (those requiring at least a suprarenal clamp site). We performed a meta-analysis to ascertain whether CRP improves kidney-related outcomes after open surgical cAA repair. Methods: Patients of any age or gender who had undergone open surgical repair of cAAs were included. Primary outcomes were the presence of postoperative kidney injury, the need for dialysis and mortality related to kidney injury. We compared patients who were treated with any intraoperative CRP strategy to a control population without CRP. We used a fixed-effects model to analyze derived odds ratios (ORs) and assess heterogeneity. We performed risk of bias analysis to identify potential confounding elements. Results: Among the 935 studies screened, 5 primary articles met the inclusion criteria. Cold renal perfusion significantly reduced postoperative acute kidney injury (OR 0.46 [95% confidence interval 0.32-0.68], Z = 3.98, p = 0.001). Only 1 study included data for the other primary outcomes. The data were considered homogeneous, with Cochrane Q = 0.23 and P of 0%. Conclusion: This meta-analysis showed reduced postoperative acute kidney injury with the use of CRP during open cAA repair. A prospective randomized controlled trial to perform further subgroup analysis and research the various types of CRP solutions may be warranted to identify further possible benefits. Nous avons procédé à une méta-analyse pour vérifier si la PRH améliore les paramètres rénaux après une réparation d'AAC par chirurgie ouverte. Méthodes : Nous avons inclus toutes les personnes (tous âges et sexes confondus) ayant subi une réparation d'AAC par chirurgie ouverte. Les paramètres principaux étaient atteinte rénale postopératoire, recours à la dialyse ou mortalité en lien avec l'insuffisance rénale. Nous avons comparé la population soumise à toute forme de PRH peropératoire à la population témoin non soumise à la PRH. Nous avons utilisé un modèle à effets fixes pour analyser les rapports des cotes (RC) dérivés et mesurer l'hétérogénéité. Nous avons effectué une analyse des risques de biais pour dégager de possibles variables de confusion. Résultats : Parmi les 935 études recensées, 5 articles principaux répondaient aux critères d'inclusion. La PRH a signifieativement contribué à réduire l'atteinte rénale postopératoire aiguë (RC 0,46 [intervalle de confiance de 95% 0,32-0,68], Z= 3,98, p = 0,001). Une seule étude incluait des données concernant les autres paramètres principaux. Les données ont été considérées homogènes, avec un test Q de Cochran = 0,23 et un coefficient P de 0%. Background and Aims: Changes in the sympathetic nervous system by pain can impact smooth muscle tone and can alter perfusion. This can be monitored by perfusion index (PI). It is a non-invasive, indirect, and continuous measure of peripheral perfusion. This study investigates the changes in PI due to painful stimuli under general anaesthesia. Methods: Twenty patients between the ages of 20 and 45 years, with informed consent, who were undergoing elective laparoscopic procedure, and belonging to the American Society of Anesthesiologists (ASA) physical status class I were connected with standard monitors along with SEDLINE, pulse oximetry (Root, Masimo Corporation®, Irvine, CA, USA) to monitor PI and Pleth-Variability Index (PVi). General anaesthesia was administered. PI, PVi, heart rate (HR), and non-invasive blood pressure were recorded pre-induction, during induction, before and after intubation, at the time of pneumoperitoneum (P0), and first laparoscopic port insertion (P1). Later, intravenous injection of fentanyl 0.5 μg/kg was administered and values were recorded at the second (P2) and third (P3) port insertion. The aforementioned parameters were recorded for up to 30 minutes. Statistical confirmation was done through paired t tests. Results: PI values after fentanyl increased from 5.33 ± 2.67 (P1) to 5.99 ± 2.8 (P2) (P < 0.001), and to 6.3 ± 2.88 (P3) (P < 0.001). This increase correlated with a decrease in HR, from 101.42 ± 12.53 (P1) to 87.93 ± 10.98 (P2) (P < 0.001), and to 83 ± 10.82 (P3) (P < 0.001). Conclusion: PI can be a tool to monitor the nociception in anaesthetised patients when administering analgesia. Background and Aims: Perfusion index (PI) is a new simple, objective and non-invasive method for evaluation of the success of central neuraxial and peripheral nerve blocks. So, we conducted a study with an aim to evaluate PI as an indicator for success of ultrasound-guided supraclavicular block (SCB). Methods: 65 patients of either sex, age 18-60 years, American Society of Anesthesiologists physical status I and II posted for upper limb surgery under ultrasound (US)-guided SCB were included. PI was recorded at baseline every 2 minutes till 10 minutes and then every 5 minutes till 30 minutes after block. PI ratio was calculated as the ratio between PI at 10 minutes and baseline PI. Sensory and motor blocks were assessed at 5-minutes intervals up to 30 minutes. Descriptive analysis was applied by mean and standard deviation for quantitative, frequency and proportion for categorical variables. Results: Mean PI increased continuously from baseline and reached the maximum at 10 minutes and then slightly decreased up to 30 minutes, but values at subsequent time intervals were quite high as compared to baseline. In case of successful blocks, median PI started increasing 2 minutes after the block and then increased in a linear fashion till 10 minutes, whereas in case of failed blocks, it only increased minimally. Conclusion: PI is an objective and faster indicator for evaluating success of US-guided SCB. A cut-off value of 3.25 for PI and 3.03 for PI ratio showed a fairly good ability with high sensitivity and specificity for predicting the success of SCB. Introduction The mechanism of fast inspiratory flow rate (V.sub.I') induced lung injury is unclear. As fast V.sub.I' increases hysteresis, a measure of surface tension at the air-liquid interface, surfactant release or function may be important. This experimental study examines the contribution of impaired surfactant release or function to dynamic-VILI. Methods Isolated perfused lungs from male Sprague Dawley rats were randomly allocated to four groups: a long or short inspiratory time (Ti = 0.5 s; slow V.sub.I' or Ti = 0.1 s; fast V.sub.I') at PEEP of 2 or 10 cmH.sub.2O. Tidal volume was constant (7 ml/kg), with f = 60 breath/min. Forced impedance mechanics (tissue elastance (Htis), tissue resistance (Gtis) and airway resistance (Raw) were measured at 30, 60 and 90 min following which the lung was lavaged for surfactant phospholipids (PL) and disaturated PL (DSP). Results Fast V.sub.I' resulted in a stiffer lung. Concurrently, PL and DSP were decreased in both tubular myelin rich and poor fractions. Phospholipid decreases were similar with PEEP. In a subsequent cohort, laser confocal microscopy-based assessment demonstrated increased cellular injury with increased V.sub.I' at both 30 and 90 min ventilation. Immune checkpoint blockade (ICB) has demonstrated curative potential in several types of cancer, but only for a small number of patients. Thus, the identification of reliable and noninvasive biomarkers for predicting ICB responsiveness is an urgent unmet need. Here, we show that ICB increased tumor vessel perfusion in treatment-sensitive EO771 and MMTV- PyVT breast tumor as well as CT26 and MCA38 colon tumor models, but not in treatment-resistant MCaP0008 and 4T1 breast tumor models. In the sensitive tumor models, the ability of anti-cytotoxic T lymphocyte-associated protein 4 or anti-programmed cell death 1 therapy to increase vessel perfusion strongly correlated with its antitumor efficacy. Moreover, globally enhanced tumor vessel perfusion could be detected by Doppler ultrasonography before changes in tumor size, which predicted final therapeutic efficacy with more than 90% sensitivity and specificity. Mechanistically, [CD8.sup.+] T cell depletion, IFN-[gamma] neutralization, or implantation of tumors in IFN-[gamma] receptor knockout mice abrogated the vessel perfusion enhancement and antitumor effects of ICB. These results demonstrated that ICB increased vessel perfusion by promoting [CD8.sup.+] T cell accumulation and IFN-[gamma] production, indicating that increased vessel perfusion reflects the successful activation of antitumor T cell immunity by ICB. Objective To investigate the short- and medium-term renal hemodynamic and functional responses to both short and sustained hypoperfusion. Subjects Eleven Merinos ewes. Setting Animal laboratory of the University Physiology Institute. Design Prospective observational study. Interventions Studies were performed in conscious sheep after unilateral nephrectomy with a vascular occluder and flow probe implanted on the remaining renal artery. In five sheep, renal blood flow (RBF) was reduced by 25, 50 and 75%, respectively, by acute vascular occlusion for 30 min at weekly intervals. In another six sheep, RBF was reduced by 80% for 2 h. Measurements and results After 25, 50 or 75% renal hypoperfusion for 30 min, there was no associated extended loss of renal function. During 2 h of 80% hypoperfusion, urine output decreased from 80 to 17 ml, and creatinine clearance from 32 to 3 ml/min, whereas plasma creatinine increased from 103 to 132 [mu]mol/l, and fractional excretion of sodium and urea increased. Release of occlusion induced brief hyperemia before all measured variables returned to normal within 8 h and remained normal for the following 72 h. At autopsy, the kidneys were histopathologically normal. Conclusions Various degrees of renal hypoperfusion for 30 min did not induce prolonged changes in renal function or blood flow. Even with sustained severe hypoperfusion, there was rapid recovery to baseline function and flow. Methods Anesthetized rats were hemodiluted (180 ml kg.sup.-1) with low (P.sub.50 ~70 mmHg) or high affinity (P.sub.50 ~14 mmHg) HRC 101 at hemoglobin (Hb) concentrations near 100 or 70 g l.sup.-1 (n = 6--8). Hippocampal tissue oxygen tension ([P.sub.Br]O.sub.2), blood flow, arterial blood gases, Hb, hematocrit (Hct) and lactate were measured. Data (mean +- SD) were analyzed by two-way ANOVA. Results Hemodilution reduced the hematocrit to 1 +- 1% in all groups. [P.sub.Br]O.sub.2 was best maintained after hemodilution with low affinity HRC 101 at Hb 100 and 70 g l.sup.-1 (25.2 +- 7.6 and 16.6 +- 8.3 torr, respectively). [P.sub.Br]O.sub.2 decreased (9.5 +- 9.3 torr, P < 0.05) and serum lactate levels increased (5.0 +- 1.7 mmol l.sup.-1, P < 0.05) following hemodilution with the high affinity HRC 101 (Hb 100 g l.sup.-1). Conclusions HRC 101 with a lower oxygen affinity favored tissue perfusion and maintained [P.sub.Br]O.sub.2 after near complete blood volume exchange in rats. Design A prospective, randomised and controlled animal study. Setting Animal laboratory in a university hospital. Subjects Sprague-Dawley male rats. Interventions The rats were anaesthetised with urethane and ketamine, mechanically ventilated and haemodynamically monitored. Lidocaine or saline were infused continuously via thoracic epidural catheters followed by a continuous intravenous infusion of Escherichia coli lipopolysaccharide (1.5 mg/kg per h). Densities of perfused and non-perfused capillaries (i.e., with and without erythrocyte perfusion, respectively) as well as erythrocyte velocity in both the mucosa and the muscularis of the terminal ileum were determined using intravital microscopy. Measurements and results Measurements were performed at baseline, after 30 min of epidural infusion as well as after 60 and 120 min of lipopolysaccharide infusion. In animals receiving TEA, mean arterial pressure and heart rate were significantly reduced throughout the experiment. In the muscularis the endotoxaemia-induced increase in non-perfused capillaries was absent with epidural lidocaine (0 [0/0] versus 39 [36/137] cm.sup.-1, median [25.sup.th/75.sup.th percentile]), whereas in the mucosa perfused capillary density declined to a greater extent than in controls (-47 [-53/-23]%) versus -19 [-34/+10]%, p Conclusions Microvascular perfusion data during endotoxaemia show a redistribution of blood flow towards the mucosa. TEA seems to impede this redistribution resulting in improved muscularis and worsened mucosal microvascular perfusion. Design: Prospective observational substudy, part of a randomised controlled trial of early treatment of ruptured intracranial aneurysms. Setting: Intensive care unit (ICU) of a university hospital. Patients: A consecutive sample of 26 patients [13 surgical (7/6 Hunt & Hess Grade I--II/H & H Gr IV--V) and 13 endovascular (3/10 H & H Gr I--II/H & H Gr IV--V)] out of 56 SAH patients randomly assigned to either endovascular or surgical treatment during the substudy period between 1 May 1995 and 31 August 1996. All patients were treated within 72 h after SAH. Measurements and results: After treatment of a ruptured aneurysm, hemodynamics and gastric intramucosal pCO.sub.2 were measured during the first 4 h and between 6 h and 12 h after aneurysm treatment. In the whole sample, neither the gastric intramucosal-arterial pCO.sub.2 difference (pCO.sub.2 gap) (1.5+-1.9 kPa and 1.7+-1.2 kPa, NS) nor gastric intramucosal pH (7.28+-0.12 and 7.29+-0.08, NS) changed during the study. There were no differences in pCO.sub.2 gap or gastric intramucosal pH between treatment groups or Hunt & Hess grade groups during the study period. Conclusions: Splanchnic tissue perfusion may be insufficient even though there is no systemic hemodynamic disturbance in patients after SAH. Neither the therapeutic treatment nor pre-treatment Hunt & Hess grade is associated with a specific pattern of pCO.sub.2 gap. In the ten patients with preserved autoregulation, CBF decreased from 31 +- 3 to 28 +- 3 ml/100 g/min in the two patients with impaired autoregulation CBF increased respectively from 16 to 35 and from 21 to 70 ml/100 g/min. SjO.sub.2 did not change significantly from baseline. TCD remained within the normal range.APConclusions: During CPP management norepinephrine can be used to increase MAP without potentiating hyperemia if pressure autoregulation is preserved. The assessment of pressure autoregulation should be considered as a guide for arterial pressure-oriented therapy after HI. The coronary arteries are the major blood vessels supplying the heart, which may become narrowed or obstructed under abnormal conditions, leading to myocardial ischemia, an insufficient supply of blood to the heart. If untreated, ischemia may result in myocardial infarction, or heart attack, which involves deterioration of heart tissue. Providing the coronary artery with a nutrient-containing fluid in a procedure called autoperfusion has been shown to improve short episodes of ischemia during coronary angioplasty, a procedure for repairing the obstructed coronary artery. The effectiveness of autoperfusion in protecting the heart after prolonged coronary artery occlusion was examined in 24 dogs. Thirteen dogs were subjected to coronary artery occlusion alone, whereas 11 dogs were treated with autoperfusion. The area receiving insufficient blood supply was similar in size in both groups. However, the area of deteriorating or infarcted tissue was greater in the group of animals that were not autoperfused than in the group receiving autoperfusion. The results show that autoperfusion of the coronary artery for six hours after coronary artery occlusion does not prevent, but reduces the extent of deterioration of the heart tissue in the dog model.
