Abstract

Item does not contain fulltext Although Dutch can be considered a transparent orthography, a substantial number of children in the Netherlands suffer from severe reading and spelling problems. Over the past decades, research has focused on the question of how problems with learning to read and spell in Dutch can best be understood. Importantly, policymakers have recognized the results of ongoing research efforts and protocols are now being followed nationwide for the diagnosis of developmental dyslexia and conduct of interventions.In this chapter, we start with a short introduction to the Dutch language and its orthography. We then review what is known about the processes of learning to read and spell in Dutch as evidenced by research with typical children (see also Verhoeven, 2017). Next, we sketch the historical and cultural context leading up to the identification of problems with learning to read in Dutch and the diagnosis of developmental dyslexia in the Netherlands.

Item does not contain fulltext In this final chapter, we provide our view on some of the main conclusions that can be drawn from the research on developmental dyslexia across languages. The overarching questions concern how our understanding of reading disability benefits from considering a broad array of languages. If there is a universal biological basis of developmental dyslexia, there should be shared observations on reading disability across languages and writing systems. Or, so one might expect. It is possible, however, that observations of differences in the manifestations of dyslexia - whether subtyping within a language (e.g., phonological vs surface dyslexia) or comparisons across languages (e.g., alphabetic vs morphosyllabic) - reflect some unitary cause or combinations of multiple causes.

In learning to read, children implicitly learn how their writing system encodes their spoken language and how they can decode printed words into spoken words to derive meaning (see Verhoeven & Perfetti, 2017). However, many children around the world encounter problems learning to read, fail to develop fluent decoding, and are thus diagnosed as dyslexic. A large body of research supports the conclusion that a phonological deficit underlies most developmental dyslexia. Much of the existing evidence, however, is based on studies of children learning to read in English. It is important to note that English has an opaque orthography that creates challenges beyond those facing children who read more transparent orthographies. In recent years, the research base for developmental dyslexia has broadened across languages, allowing the question of differences and similarities across languages and writing systems to receive attention.

Abstract

Background There are no Italian data regarding the strategies for preventing neonatal group B streptococcal (GBS) infection. We conducted a national survey in order to explore obstetrical, neonatal and microbiological practices for the GBS prevention. Methods Three distinct questionnaires were sent to obstetricians, neonatologists and microbiologists. Questionnaires included data on prenatal GBS screening, maternal risk factors, intrapartum antibiotic prophylaxis, microbiological information concerning specimen processing and GBS antimicrobial susceptibility. Results All respondent obstetrical units used the culture-based screening approach to identify women who should receive intrapartum antibiotic prophylaxis, and more than half of the microbiological laboratories (58%) reported using specimen processing consistent with CDC guidelines. Most neonatal units (89 out of 107, 82%) reported using protocols for preventing GBS early-onset sepsis consistent with CDC guidelines. Conclusions The screening-based strategy is largely prevalent in Italy, and most protocols for preventing GBS early-onset sepsis are consistent with CDC guidelines. However, we found discrepancies in practices among centers that may reflect the lack of Italian guidelines issued by public health organizations.

Abstract

A pesar que los paragangliomas pueden ser observados desde la base del cráneo hasta la vejiga o a lo largo de la cadena ganglionar del simpático, sólo constituyen el 0,012 % de todos los tumores diagnosticados y su hallazgo en cualquier región anatómica representa una rareza. El objetivo del presente trabajo es hacer un análisis de tres casos clínicos con diagnóstico de paraganglioma vistos en nuestro hospital. De los tres pacientes, uno no fue sometido a cirugía por la localización del tumor, ameritando biopsia e inmunohistoquímica para confirmar el diagnóstico, tras el cual recibió tratamiento paliativo, falleciendo año y medio después del mismo, los otros dos pacientes tras el tratamiento quirúrgico se mantienen asintomáticos. Al diagnóstico de estos tumores como en otros, se llega inicialmente con la experiencia del clínico y su sospecha, como con los estudios que permitan documentarlos, tras lo cual su tratamiento como el seguimiento es fundamental. Although the paragangliomas can be observed from the base of the skull until the bladder or along the chain ganglionic of the nice, alone they constitute 0.012 % of all the diagnosed tumors and their discovery in any anatomical region it represents a rarity. The objective of this work is the analyses of three clinical cases with diagnostic of a paraganglioma wirer in our hospital. Of the three patients, one was not subjected to surgery for the localization of tumor, needing biopsy and inmuno-histochemistry confirm the diagnosis, after which received palliative treatment, dying year and half after the same one, the other two after patients after the surgical treatment stay asymptomatic. Diagnostic of these tumors like in other, you arrives initially with the experience of the clinical one and their suspicion, like with the studies that allow to document them, after that which their treatment as the pursuit is fundamental.

42. Can quality of movement be measured? Rasch analysis and inter-rater reliability of the Motor Evaluation Scale for Upper Extremity in Stroke Patients (MESUPES)

Abstract

OBJECTIVE: Clinical scales evaluating arm function after stroke are weak at detecting quality of movement. Therefore a new scale, the Motor Evaluation Scale for Upper Extremity in Stroke Patients (MESUPES), was developed, comprising 22 items pertaining to arm and hand performance. The scale was investigated for validity and unidimensionality using the Rasch measurement model, and for inter-rater reliability. SETTING: Twelve hospitals and rehabilitation centres in Belgium, Germany and Switzerland. PATIENTS: There were 396 patients (average age 63.38+/-12.89 years) in the Rasch study and 56 patients (average age 65.68+/-12.75 years) in the reliability study. MAIN MEASURES: The scale was examined on its fit to the Rasch model, thereby evaluating the scale's unidimensionality and validity. Differential item functioning was performed to test the stability of item hierarchy on several variables. Inter-rater reliability was examined with kappa values, weighted percentage agreement and intraclass correlation coefficients (ICC). RESULTS: Based on Rasch analysis, five items were removed. The MESUPES was divided in two tests: the MESUPES-arm test (8 items) and MESUPES-hand test (9 items). Both scales fitted the Rasch model. All items were stable among the subgroups of the sample. ICCs were 0.95 (95% confidence interval (CI) 0.91 -0.97) and 0.97 (95% CI 0.95-0.98) for the total score on arm and hand test respectively. The scale was also reliable at item level (weighted kappa 0.62 -0.79, weighted percentage agreement 85.71 -98.21). CONCLUSION: The MESUPES-arm and MESUPES-hand meet the statistical properties of reliability, validity and unidimensionality. Both tests provide a useful clinical and research tool to qualitatively evaluate arm and hand function during recovery after stroke. [ABSTRACT FROM AUTHOR]

Abstract

Blood pressure was measured in 392 healthy, full-term, appropriate-weight infants and was measured again at 6, 15, 24, 36, 48, and 60 months of age. Systolic pressure rose from birth to six months of age, remained at a plateau until 15 months of age, and from 24 to 60 months rose an average of 1 mmHg per year. Diastolic pressure rose from birth to 24 months but showed little change from 24 to 60 months. The correlation of systolic with diastolic pressure decreased seriatim from birth to 60 months. Blood pressure did not vary as a function of ethnic group, sex, or socioeconomic status during the first five years of life. The association of body size with blood pressure was no more than modest during this period, but seemed to be closer for females than for males. A closer child-parent association was found for heart rate than for blood pressure. The child's heart rate was inversely associated with the parent's blood pressure although the child's blood pressure was not associated with the parent's pressure. During the first five years of life, a low heart rate may be a more significant risk factor for adult hypertension than elevated blood pressure.

Heart rate was measured in 392 healthy, full-term, appropriate-weight infants and was measured again at 6, 15, 24, 36, 48, and 60 months of age. Heart rate increased from newborn to six months of age and then decreased seriatim. Heart rate did not vary as a function of socioeconomic status or of sex. Blacks had higher heart rates than whites as newborns and at six months of age; at the older ages the difference was no longer significant. The slowing of heart rate with age was greater among blacks. Heart rate was inversely associated with weight and with height, but showed no association with blood pressure. A significant child-mother correlation of heart rate was found at two ages. No significant child-mother correlations of systolic or of diastolic blood pressure were found. Thus, a closer child-parent association was found for heart rate than for blood pressure. Offspring of a parent with a history of hypertension had slower heart rates than offspring neither of whose parents had a history of hypertension. Neither the child's systolic nor diastolic blood pressure varied with the presence or absence of a parental history of hypertension. Results suggest that during the first five years of life a child's slower heart rate may be a better index of risk of adult hypertension than a child's elevated blood pressure.

Abstract

Blood pressure (BP) was measured on the third postnatal day in 392 healthy, full-term, appropriate weight infants, and again in 318 infants at six months, in 277 infants at 15 months, and in 232 infants at 24 months of age. Differences in average BP between white infants and black infants were small; BP did not vary significantly as a function of socioeconomic class or sex. BP, measured during sleep, had increased from birth to six months of age, but showed no change from six to 15 months, a period of rapid growth, during which average weight increased 37% and average height increased 16%. BP continued to show to change from 15 to 24 months, when waking BP measurements at 24 months were adjusted for sleep-waking differences in BP. Cross-sectional and analyses revealed only modest weight-BP relationships at 15 and 24 months. Five per cent of the infants exhibited systolic BPs which remained above the 80th percentile at six, 15, and 24 months of age. This was six times the number expected by chance if BP was assumed to be uncorrelated from measurement to measurement. This group of infants showed physical maturation, measured by weight change, which was almost identical with that of the total cohort.

Abstract

In this paper will be reported the results of the clinical evaluation of the microprocessor based device S.P.A.C.2, for the rehabilitation of the superior and inferior limb in the hemiplegic subjects. The results, referred to a period of 7 months, were obtained during the first two years of a group of 7 patients, all injured to the left hemisphere. The device S.P.A.C.2 has been realized during the first two years of the MPI 40% Project "Analysis of the dynamics of the induced and voluntary movements for the prevention, diagnosis and rehabilitation" and its design characteristics have been previously presented (9,10).
