Abstract: On 12 December 2023, the star $\alpha$ Orionis (Betelgeuse) will be occulted by the asteroid (319) Leona. This represents an extraordinary and unique opportunity to analyze the diameter and brightness distribution of Betelgeuse's photosphere with extreme angular resolution by studying the light curve as the asteroid occults the star from different points on Earth and at different wavelengths. Here we present observations of another occultation by Leona on 13 September 2023 to determine its projected shape and size in preparation for the December 12th event. The occultation observation campaign was highly successful. The effective diameter in projected area derived from the positive detections at 17 sites turned out to be 66 km $\pm$ 2 km using an elliptical fit to the instantaneous limb. The body is highly elongated, with dimensions of 79.6 $\pm$ 2.2 km x 54.8 $\pm$ 1.3 km in its long and short axis, respectively, at the occultation time. Abstract: This work aims at constraining the size, shape, and geometric albedo of the dwarf planet candidate 2002 MS4 through the analysis of nine stellar occultation events. Using multichord detection, we also studied the object's topography by analyzing the obtained limb and the residuals between observed chords and the best-fitted ellipse. We predicted and organized the observational campaigns of nine stellar occultations by 2002 MS4 between 2019 and 2022, resulting in two single-chord events, four double-chord detections, and three events with three to up to sixty-one positive chords. Using 13 selected chords from the 8 August 2020 event, we determined the global elliptical limb of 2002 MS4. The best-fitted ellipse, combined with the object's rotational information from the literature, constrains the object's size, shape, and albedo. Additionally, we developed a new method to characterize topography features on the object's limb. The global limb has a semi-major axis of 412 $\pm$ 10 km, a semi-minor axis of 385 $\pm$ 17 km, and the position angle of the minor axis is 121 $^\circ$ $\pm$ 16$^\circ$. From this instantaneous limb, we obtained 2002 MS4's geometric albedo and the projected area-equivalent diameter. Significant deviations from the fitted ellipse in the northernmost limb are detected from multiple sites highlighting three distinct topographic features: one 11 km depth depression followed by a 25$^{+4}_{-5}$ km height elevation next to a crater-like depression with an extension of 322 $\pm$ 39 km and 45.1 $\pm$ 1.5 km deep. Our results present an object that is $\approx$138 km smaller in diameter than derived from thermal data, possibly indicating the presence of a so-far unknown satellite. However, within the error bars, the geometric albedo in the V-band agrees with the results published in the literature, even with the radiometric-derived albedo.

Abstract: Within our program of physical characterization of trans-Neptunian objects and centaurs, we predicted a stellar occultation by the centaur (54598) Bienor to occur on January 11, 2019, with good observability potential. We obtained high accuracy astrometric data to refine the prediction, resulting in a shadow path favorable for the Iberian Peninsula. This encouraged us to carry out an occultation observation campaign that resulted in five positive detections from four observing sites. This is the fourth centaur for which a multichord (more than two chords) stellar occultation has been observed so far, the other three being (2060) Chiron, (10199) Chariklo, and (95626) 2002 GZ$_{32}$. From the analysis of the occultation chords, combined with the rotational light curve obtained shortly after the occultation, we determined that Bienor has an area-equivalent diameter of $150\pm20$ km. This diameter is $\sim30$ km smaller than the one obtained from thermal measurements. The position angle of the short axis of the best fitting ellipse obtained through the analysis of the stellar occultation does not match that of the spin axis derived from long-term photometric models. We also detected a strong irregularity in one of the minima of the rotational light curve that is present no matter the aspect angle at which the observations were done. We present different scenarios to reconcile the results from the different techniques. We did not detect secondary drops related to potential rings or satellites. Nonetheless, similar rings in size to that of Chariklo's cannot be discarded due to low data accuracy., Comment: Accepted for publication in A\&A

Abstract: Within our international program to obtain accurate physical properties of trans-Neptunian objects (TNOs) we predicted a stellar occultation by the TNO (38628) Huya of the star Gaia DR2 4352760586390566400 (mG = 11.5 mag.) for March 18, 2019. After an extensive observational campaign, we updated the prediction and it turned out to be favorable to central Europe. Therefore, we mobilized half a hundred professional and amateur astronomers, and the occultation was finally detected from 21 telescopes located at 18 sites. This makes the Huya event one of the best ever observed stellar occultation by a TNO in terms of the number of chords. We determine accurate size, shape, and geometric albedo, and we also provide constraints on the density and other internal properties of this TNO. The 21 positive detections of the occultation by Huya allowed us to obtain well-separated chords which permitted us to fit an ellipse for the limb of the body at the moment of the occultation (i.e., the instantaneous limb) with kilometric accuracy. The projected semi-major and minor axes of the best ellipse fit obtained using the occultation data are (a', b') = (217.6 $\pm$ 3.5 km, 194.1 $\pm$ 6.1 km) with a position angle of the minor axis P' = 55.2 $\pm$ 9.1 degrees. From this fit, the projected area-equivalent diameter is 411.0 $\pm$ 7.3 km. This diameter is compatible with the equivalent diameter for Huya obtained from radiometric techniques (D = 406 $\pm$ 16 km). From this instantaneous limb, we obtained the geometric albedo for Huya (p$\rm_V$ = 0.079 $\pm$ 0.004) and we explored possible 3D shapes and constraints to the mass density for this TNO. We did not detect the satellite of Huya through this occultation, but the presence of rings or debris around Huya is constrained using the occultation data. Abstract: A stellar occultation by Neptune's main satellite, Triton, was observed on 5 October 2017 from Europe, North Africa, and the USA. We derived 90 light curves from this event, 42 of which yielded a central flash detection. We aimed at constraining Triton's atmospheric structure and the seasonal variations of its atmospheric pressure since the Voyager 2 epoch (1989). We also derived the shape of the lower atmosphere from central flash analysis. We used Abel inversions and direct ray-tracing code to provide the density, pressure, and temperature profiles in the altitude range $\sim$8 km to $\sim$190 km, corresponding to pressure levels from 9 {\mu}bar down to a few nanobars. Results. (i) A pressure of 1.18$\pm$0.03 {\mu}bar is found at a reference radius of 1400 km (47 km altitude). (ii) A new analysis of the Voyager 2 radio science occultation shows that this is consistent with an extrapolation of pressure down to the surface pressure obtained in 1989. (iii) A survey of occultations obtained between 1989 and 2017 suggests that an enhancement in surface pressure as reported during the 1990s might be real, but debatable, due to very few high S/N light curves and data accessible for reanalysis. The volatile transport model analysed supports a moderate increase in surface pressure, with a maximum value around 2005-2015 no higher than 23 {\mu}bar. The pressures observed in 1995-1997 and 2017 appear mutually inconsistent with the volatile transport model presented here. (iv) The central flash structure does not show evidence of an atmospheric distortion. Abstract: We predicted a stellar occultation of the bright star Gaia DR1 4332852996360346368 (UCAC4 385-75921) (m$_{\rm V}$= 14.0 mag) by the centaur 2002 GZ$_{32}$ for 2017 May 20$^{\rm th}$. Our latest shadow path prediction was favourable to a large region in Europe. Observations were arranged in a broad region inside the nominal shadow path. Series of images were obtained with 29 telescopes throughout Europe and from six of them (five in Spain and one in Greece) we detected the occultation. This is the fourth centaur, besides Chariklo, Chiron and Bienor, for which a multi-chord stellar occultation is reported. By means of an elliptical fit to the occultation chords we obtained the limb of 2002 GZ$_{32}$ during the occultation, resulting in an ellipse with axes of 305 $\pm$ 17 km $\times$ 146 $\pm$ 8 km. From this limb, thanks to a rotational light curve obtained shortly after the occultation, we derived the geometric albedo of 2002 GZ$_{32}$ ($p_{\rm V}$ = 0.043 $\pm$ 0.007) and a 3-D ellipsoidal shape with axes 366 km $\times$ 306 km $\times$ 120 km. This shape is not fully consistent with a homogeneous body in hydrostatic equilibrium for the known rotation period of 2002 GZ$_{32}$. The size (albedo) obtained from the occultation is respectively smaller (greater) than that derived from the radiometric technique but compatible within error bars. No rings or debris around 2002 GZ$_{32}$ were detected from the occultation, but narrow and thin rings cannot be discarded., Comment: Accepted for publication in MNRAS (8-Dec.-2020), 15 pages, 9 figures

Abstract: On 28th January 2018, the large Trans-Neptunian Object (TNO) 2002TC302 occulted a m$_v= $15.3 star with ID 130957813463146112 in the Gaia DR2 stellar catalog. 12 positive occultation chords were obtained from Italy, France, Slovenia and Switzerland. Also, 4 negative detections were obtained near the north and south limbs. This represents the best observed stellar occultation by a TNO other than Pluto, in terms of the number of chords published thus far. From the 12 chords, an accurate elliptical fit to the instantaneous projection of the body, compatible with the near misses, can be obtained. The resulting ellipse has major and minor axes of 543 $\pm$ 18 km and 460 $\pm$ 11 km, respectively, with a position angle of 3 $\pm$ 1 degrees for the minor axis. This information, combined with rotational light curves obtained with the 1.5m telescope at Sierra Nevada Observatory and the 1.23m telescope at Calar Alto observatory, allows us to derive possible 3D shapes and density estimations for the body, based on hydrostatic equilibrium assumptions. The effective area equivalent diameter is $\sim$ 84 km smaller than the radiometrically derived diameter using thermal data from Herschel and Spitzer Space Telescopes. This might indicate the existence of an unresolved satellite of up to $\sim$ 300 km in diameter, to account for all the thermal flux, although the occultation and thermal diameters are compatible within their error bars given the considerable uncertainty of the thermal results. The existence of a potential satellite also appears to be consistent with other ground-based data presented here. From the effective occultation diameter combined with H$_V$ measurements we derive a geometric albedo of 0.147 $\pm$ 0.005, which would be somewhat smaller if 2002TC302 has a satellite. Abstract: Context. Pluto's tenuous nitrogen (N2) atmosphere undergoes strong seasonal effects due to high obliquity and orbital eccentricity, and has been recently (July 2015) observed by the New Horizons spacecraft. Goals are (i) construct a well calibrated record of the seasonal evolution of surface pressure on Pluto and (ii) constrain the structure of the lower atmosphere using a central flash observed in 2015. Method: eleven stellar occultations by Pluto observed between 2002 and 2016 are used to retrieve atmospheric profiles (density, pressure, temperature) between $\sim$5 km and $\sim$380 km altitude levels (i.e. pressures from about 10 microbar to 10 nanobar). Results: (i) Pressure has suffered a monotonic increase from 1988 to 2016, that is compared to a seasonal volatile transport model, from which tight constraints on a combination of albedo and emissivity of N2 ice are derived; (ii) A central flash observed on 2015 June 29 is consistent with New Horizons REX profiles, provided that (a) large diurnal temperature variations (not expected by current models) occur over Sputnik Planitia and/or (b) hazes with tangential optical depth of about 0.3 are present at 4-7 km altitude levels and/or (c) the nominal REX density values are overestimated by an implausibly large factor of about 20% and/or (d) higher terrains block part of the flash in the Charon facing hemisphere., Comment: 21 pages, 11 figures

13. Prevención y diagnóstico precoz de osteoporosis infantil: ¿estamos haciendo lo correcto?

Abstract: Objetivos: Evaluar la prevención, el diagnóstico precoz y la formación recibida de la osteoporosis en la Pediatría de nuestro medio. Material y métodos: Encuesta dirigida a facultativos de Pediatría de Atención Primaria (AP) y Atención Especializada (AE) que valora su actividad en prevención, detección y formación recibida en osteoporosis, y que fue difundida a través de las sociedades científicas pertinentes. Resultados: Participaron 420 pediatras (324 de AP y 96 de AE). El 93,5% de los pediatras de AP y el 89,6% de los de AE valoraban la actividad física de los pacientes; el 85,19% y 35,4% de ellos, respectivamente, la ingesta de lácteos. El 45,68% de AP y el 70,2% de AE suplementaban con calcio y vitamina D ante aporte nutricional bajo, realizándoles seguimiento el 39,2% de AP y el 47,2% de AE. El 39,6% de pediatras de AE solicitaba densitometría ósea ante enfermedad o tratamiento de riesgo, y el 47,9% medía los niveles de 25-OH-vitamina D. El 25,93% de AP y el 45,3% de AE preguntaban por la existencia de fracturas, el 90,4% y 96,8% valoraban el mecanismo etiopatogénico. El 40% de AP y el 86,2% de AE solicitaban una densitometría ósea o derivaban al especialista ante fracturas por traumatismos de baja energía, con criterios específicos en el 13,7% y 5,86%, respectivamente. El 92% de AP y el 82,3% de AE no habían recibido formación reciente en osteoporosis infantil. Conclusión: La detección, los circuitos de derivación y la formación de los pediatras respecto a la salud ósea en nuestro país es mejorable. Optimizar estos aspectos es fundamental para favorecer el pico de masa ósea en nuestra población.

Abstract: On 2023 December 12, the star α Orionis will be occulted by asteroid (319) Leona. This represents an extraordinary and unique opportunity to analyse the brightness distribution of Betelgeuse's photosphere with extreme angular resolution by studying light curves from different points on Earth and at different wavelengths. Here we present observations of another occultation by asteroid Leona, on 2023 September 13, whose goal was to determine Leona's projected shape and size in preparation for the December 12th event and its interpretation. The occultation campaign was highly successful with 25 positive detections from 17 different sites and a near miss. The effective diameter in projected area derived from the positive detections was 66 ± 2 km using an elliptical fit to the instantaneous limb. The body is highly elongated, with dimensions of 79.6 ± 2.2 km × 54.8 ± 1.3 km in its long and short axis, respectively, at occultation time. This result, in combination with dense time series photometry of Leona that we recently obtained, together with archival sparse photometry, allowed us to predict the angular size of the asteroid for the Betelgeuse event and to simulate the expected brightness change. Abstract: Context. The physical characterization of trans-Neptunian objects is essential for improving our understanding of the formation and evolution of our Solar System. Stellar occultation is a ground-based technique that can be successfully used to determine some of the TNOs’ fundamental physical properties with high precision, such as size and shape. Aims. This work is aimed at constraining the size, shape, and geometric albedo of the dwarf planet candidate (307261) 2002 MS4 through the analysis of nine stellar occultation events. Using multichord detection, we also study the object’s topography by analyzing the obtained limb and residuals between the observed chords and the best-fit ellipse. Metods. We predicted and organized the observational campaigns of nine stellar occultations by 2002 MS4 between 2019 and 2022, resulting in two single-chord events, four double-chord detections, and three events with between 3 and 61 positive chords. We derived the occultation light curves using differential aperture photometry, from which the star ingress and egress instants were calculated. Using 13 selected chords from the 8 August 2020 event, we determined the global elliptical limb of 2002 MS4. The best-fit ellipse, combined with the object’s rotational information from the literature, sets constraints on the object’s size, shape, and albedo. Additionally, we developed a new method to characterize the topography features on the object’s limb. Results. The global limb has a semi-major axis of a′ = 412 ± 10 km, a semi-minor axis of b′= 385 ± 17 km, and the position angle of the minor axis is 121° ± 16°. From this instantaneous limb, we obtained 2002 MS4’s geometric albedo of pV = 0.1 ± 0.025, using HV = 3.63 ± 0.05 mag and a projected area-equivalent diameter of 796 ± 24 km. Abstract: RESUMEN Introducción: La intensidad del dolor postoperatorio moderado-intenso después de una cirugía cardiaca (CC) varía, según los estudios, de un 45 a un 85 %. No existe evidencia sobre cuál es la pauta analgésica óptima en el postoperatorio de estos pacientes. El objetivo de este estudio es evaluar la efectividad de la aplicación de un protocolo analgésico multimodal, basado en la analgesia controlada por el paciente (PCA) con morfina, en pacientes sometidos a CC con circulación extracorpórea (CEC). Pacientes y métodos: Estudio prospectivo de todos los pacientes sometidos a CC con CEC, durante los primeros 3 días del postoperatorio (DPO). Se incluyeron 102 pacientes en dos periodos, noviembre de 2016, con analgesia convencional (AC) y enero-febrero de 2017 con PCA. Resultados: El dolor en reposo se mantuvo controlado (mediana escala numérica < 3). Se registró un 27 % de pacientes con dolor moderado e intenso. No hubo diferencias en la intensidad del dolor entre los pacientes con AC y los de PCA. El grupo de PCA precisó menos analgesia de rescate (63 vs. 44 %, p = 0,0487). La incidencia de dolor crónico postquirúrgico fue de un 39 % a los tres meses y un 3 % al año. El dolor y la ansiedad preoperatorios se correlacionaron con la intensidad del dolor dinámico (r = 0,287, p = 0,03). Conclusiones: La PCA con opioides a demanda y analgesia multimodal es una alternativa efectiva después de la CC. Se obtiene un buen control del dolor postoperatorio sin incrementar los efectos adversos y precisando menos analgesia de rescate administrada por enfermería. Abstract: Context. Deriving physical properties of trans-Neptunian objects is important for the understanding of our Solar System. This requires observational efforts and the development of techniques suitable for these studies. Aims. Our aim is to characterize the large trans-Neptunian object (TNO) 2002 TC302. Methods. Stellar occultations offer unique opportunities to determine key physical properties of TNOs. On 28 January 2018, 2002 TC302 occulted a mv ~ 15.3 star with designation 593-005847 in the UCAC4 stellar catalog, corresponding to Gaia source 130957813463146112. Twelve positive occultation chords were obtained from Italy, France, Slovenia, and Switzerland. Also, four negative detections were obtained near the north and south limbs. This represents the best observed stellar occultation by a TNO other than Pluto in terms of the number of chords published thus far. From the 12 chords, an accurate elliptical fit to the instantaneous projection of the body can be obtained that is compatible with the near misses. Results. The resulting ellipse has major and minor axes of 543 ± 18 km and 460 ± 11 km, respectively, with a position angle of 3 ± 1 degrees for the minor axis. This information, combined with rotational light curves obtained with the 1.5 m telescope at Sierra Nevada Observatory and the 1.23 m telescope at Calar Alto observatory, allows us to derive possible three-dimensional shapes and density estimations for the body based on hydrostatic equilibrium assumptions. The effective diameter in equivalent area is around 84 km smaller than the radiometrically derived diameter using thermal data from Herschel and Spitzer Space Telescopes. This might indicate the existence of an unresolved satellite of up to ~300 km in diameter, which is required to account for all the thermal flux, although the occultation and thermal diameters are compatible within their error bars given the considerable uncertainty of the thermal results. Abstract: Context. Pluto's tenuous nitrogen (N2) atmosphere undergoes strong seasonal effects due to high obliquity and orbital eccentricity, and has been recently (July 2015) observed by the New Horizons spacecraft. Goals are (i) construct a well calibrated record of the seasonal evolution of surface pressure on Pluto and (ii) constrain the structure of the lower atmosphere using a central flash observed in 2015. Method: eleven stellar occultations by Pluto observed between 2002 and 2016 are used to retrieve atmospheric profiles (density, pressure, temperature) between $\sim$5 km and $\sim$380 km altitude levels (i.e. pressures from about 10 microbar to 10 nanobar). Results: (i) Pressure has suffered a monotonic increase from 1988 to 2016, that is compared to a seasonal volatile transport model, from which tight constraints on a combination of albedo and emissivity of N2 ice are derived; (ii) A central flash observed on 2015 June 29 is consistent with New Horizons REX profiles, provided that (a) large diurnal temperature variations (not expected by current models) occur over Sputnik Planitia and/or (b) hazes with tangential optical depth of about 0.3 are present at 4-7 km altitude levels and/or (c) the nominal REX density values are overestimated by an implausibly large factor of about 20% and/or (d) higher terrains block part of the flash in the Charon facing hemisphere., 21 pages, 11 figures

Abstract: RESUMEN El control eficaz del dolor postoperatorio se ha convertido en una parte esencial de los cuidados perioperatorios y su adecuado tratamiento, junto a otros factores como la movilización y la nutrición precoz, se relacionan directamente con la disminución de las complicaciones postoperatorias y de la estancia hospitalaria. En la actualidad se presentan diversos retos en el campo del tratamiento del DAP, que precisarán de nuevos enfoques y nuevas alternativas terapéuticas. Los protocolos analgésicos específicos para cada tipo de intervención quirúrgica, adaptados al contexto organizativo y de práctica clínica hospitalaria, son una garantía para individualizar los tratamientos y responder adecuadamente a las demandas analgésicas de cada paciente. Entre las recomendaciones de las recientes Guías del manejo del Dolor Agudo Postoperatorio de la American Pain Society (APS) destacamos: el uso de la analgesia multimodal, la analgesia regional y epidural en procedimientos específicos, las mínimas dosis de opioides y la preferencia de la vía oral frente a la intravenosa, y la modalidad de PCA. Ante la epidemia de consumo de opioides, las recomendaciones actuales se basan en minimizar la dosis de opioide postoperatorio, aplicar pautas multimodales y retirarlos precozmente cuando puedan ser sustituidos por otros analgésicos. En los programas de cirugía fast-track, el inicio precoz de la deambulación, de la fisioterapia o de la rehabilitación son factibles con un nivel moderado de dolor. Pretender eliminar totalmente el dolor en estos programas de fast-track puede asociarse a inmovilidad del paciente o a efectos secundarios de los analgésicos que retrasen la recuperación. Se han desarrollado nuevos dispositivos de administración de opioides no invasivos o "needel-free", con el fin de eliminar las desventajas de la morfina intravenosa. Las ventajas teóricas se basan en una mayor movilidad y satisfacción del paciente, en la autoadministración y en un mejor perfil farmacológico. Son opioides de inicio rápido y acción prolongada, sin metabolitos activos, por lo que teóricamente tienen un perfil farmacológico más eficaz y seguro. Estas nuevas alternativas podrían sustituir a la administración de la PCA de morfina a bolos en la cirugía mayor laparoscópica o en la cirugía de columna vertebral, entre otras. También podrían jugar un papel de analgesia de transición, en la retirada precoz de los catéteres epidurales o paravertebrales en cirugía torácica o en cirugía vascular. ABSTRACT Effective control of postoperative pain, as well as other factors such as early mobilization and nutrition, are directly related to the reduction of postoperative complications and hospital stay, and have become an essential part of perioperative care There are actually several challenges in the field of the treatment of DAP, which require new approaches and therapeutic alternatives. The specific analgesic protocols for each type of surgery, adapted to the organizational context and clinical practice, are a guarantee to individualize the treatments and appropriately respond to the analgesic demands of any patient. Among the recommendations of the recent Guidelines for the Management of Acute Postoperative Pain of the American Pain Society (APS), we highlight: the use of Multimodal Analgesia, Regional and Epidural Analgesia in specific procedures, the minimum possible doses of opioids, the preference of the oral versus intravenous route, and PCA modality. Faced with the epidemic use of opioids, the current recommendations are based on minimizing the dose of postoperative opioids, together with, applying multimodal guidelines and early withdrawing, when they can be replaced by other analgesics. In fast-track surgery programs, the early onset of ambulation, physical therapy and/or rehabilitation are feasible with a moderate level of pain. Attempting to eliminate completely the pain in these fast-track programs can be associated with patients immobility, or analgesic side effects that may delay patients recovery. New devices for administering non-invasive opioids or "needel-free" have been developed in order to eliminate the disadvantages of intravenous morphine. The theoretical advantages are based on greater mobility and patient satisfaction, self-administration and a better pharmacological profile. They are fast onset opioids with a prolonged action and without active metabolites, which offer a theoretically more effective and safe pharmacological profile. These new alternatives could replace the administration of morphine PCA to boluses, in major laparoscopic surgery or in spinal surgery, among others. They may also play a role in the transition analgesia, in situations such as early withdrawal of epidural or paravertebral catheters in thoracic or vascular surgery.

Abstract: Resumen: Introducción: el volumen urinario bajo (< 1 ml/kg/h) es el factor etiológico más frecuente en la litiasis renal. Frecuentemente, se asume que los niños que refieren beber mucho tienen una diuresis protectora. En este estudio analizamos el hábito de consumo de líquidos en niños sanos y su relación con el volumen urinario. Metodología: se estudiaron 84 niños, de 5 a 17 años. Se registró la percepción de consumo habitual de líquidos como: "poco", "normal" o "mucho" y los tipos de bebida habitual. Se cuantificó el volumen en orina de 24 h y se relacionó con la percepción de la ingesta de líquidos. Resultados: la ingesta habitual de líquidos fue calificada como "poca" en el 23% de los casos, "normal" en un 56% y "mucha" en un 21%. El 39% de los niños (33/84) presentó una diuresis > 1 ml/kg/h que, por grupos, supone el 38,9% de los que refirieron una ingesta escasa, el 63% de los de consumo "normal" y el 73,7% de los que refirieron ingesta elevada. El 56% de los niños de los grupos de ingesta "normal" y "elevada" mostraron una diuresis < 1 ml/kg/h. Conclusiones: la mayoría de niños mostraron una diuresis por debajo del límite recomendado, incluso entre los que refieren una ingesta "normal" o "elevada" de líquidos. Estos hallazgos hacen visible la necesidad de promover estrategias encaminadas a aumentar el consumo de agua en la población.

Abstract: RESUMEN El control eficaz del dolor postoperatorio se ha convertido en una parte esencial de los cuidados perioperatorios y su adecuado tratamiento, junto a otros factores como la movilización y la nutrición precoz, se relacionan directamente con la disminución de las complicaciones postoperatorias y de la estancia hospitalaria. En la actualidad se presentan diversos retos en el campo del tratamiento del DAP, que precisarán de nuevos enfoques y nuevas alternativas terapéuticas. Los protocolos analgésicos específicos para cada tipo de intervención quirúrgica, adaptados al contexto organizativo y de práctica clínica hospitalaria, son una garantía para individualizar los tratamientos y responder adecuadamente a las demandas analgésicas de cada paciente. Entre las recomendaciones de las recientes Guías del manejo del Dolor Agudo Postoperatorio de la American Pain Society (APS) destacamos: el uso de la analgesia multimodal, la analgesia regional y epidural en procedimientos específicos, las mínimas dosis de opioides y la preferencia de la vía oral frente a la intravenosa, y la modalidad de PCA. Ante la epidemia de consumo de opioides, las recomendaciones actuales se basan en minimizar la dosis de opioide postoperatorio, aplicar pautas multimodales y retirarlos precozmente cuando puedan ser sustituidos por otros analgésicos. En los programas de cirugía fast-track, el inicio precoz de la deambulación, de la fisioterapia o de la rehabilitación son factibles con un nivel moderado de dolor. Pretender eliminar totalmente el dolor en estos programas de fast-track puede asociarse a inmovilidad del paciente o a efectos secundarios de los analgésicos que retrasen la recuperación. Se han desarrollado nuevos dispositivos de administración de opioides no invasivos o "needel-free", con el fin de eliminar las desventajas de la morfina intravenosa. Las ventajas teóricas se basan en una mayor movilidad y satisfacción del paciente, en la autoadministración y en un mejor perfil farmacológi, Effective control of postoperative pain, as well as other factors such as early mobilization and nutrition, are directly related to the reduction of postoperative complications and hospital stay, and have become an essential part of perioperative care There are actually several challenges in the field of the treatment of DAP, which require new approaches and therapeutic alternatives. The specific analgesic protocols for each type of surgery, adapted to the organizational context and clinical practice, are a guarantee to individualize the treatments and appropriately respond to the analgesic demands of any patient. Among the recommendations of the recent Guidelines for the Management of Acute Postoperative Pain of the American Pain Society (APS), we highlight: the use of Multimodal Analgesia, Regional and Epidural Analgesia in specific procedures, the minimum possible doses of opioids, the preference of the oral versus intravenous route, and PCA modality. Faced with the epidemic use of opioids, the current recommendations are based on minimizing the dose of postoperative opioids, together with, applying multimodal guidelines and early withdrawing, when they can be replaced by other analgesics. In fast-track surgery programs, the early onset of ambulation, physical therapy and/or rehabilitation are feasible with a moderate level of pain. Attempting to eliminate completely the pain in these fast-track programs can be associated with patients immobility, or analgesic side effects that may delay patients recovery. New devices for administering non-invasive opioids or "needel-free" have been developed in order to eliminate the disadvantages of intravenous morphine. The theoretical advantages are based on greater mobility and patient satisfaction, self-administration and a better pharmacological profile. They are fast onset opioids with a prolonged action and without active metabolites, which offer a theoretically more effective and safe pharmacological profile. T

Abstract: Objectives: To assess prevention, early diagnosis and training received regarding osteoporosis among the pediatrics professionals in our area. Material and methods: Survey directed to physicians of pediatricians of Primary Care (PC) and Specialized Care (SC) in order to evaluate their activity in prevention, detection and training received in osteoporosis. The survey was disseminated through the relevant scientific societies. Results: 420 pediatricians participated (324 from PC and 96 from SC). 93.5% of PC pediatricians and 89.6% of SC pediatricians valued the physical activity of the patients; 85.19% and 35.4% of them, respectively, the intake of dairy products. 45.68% of PC and 70.2% of SC recommended calcium and vitamin D supplements in the case of low nutritional intake, whereas 39.2% of PC and 47.2% of SC favored follow-up. 39.6% of SC pediatricians requested bone densitometry for this disease or risk treatment, and 47.9% measured the levels of 25-OH-vitamin D. 25.93% of PC and 45.3% of SC asked about the existence of fractures, 90.4% and 96.8% requested etiopathogenic mechanism. 40% of PC and 86.2% of SC requested a bone densitometry or referred to the specialist for fractures due to low trauma energy, with specific criteria in 13.7% and 5.86%, respectively. 92% of PC and 82.3% of SC had not received recent training in childhood osteoporosis. Conclusion: Detection, derivation circuits and the training of pediatricians regarding bone health in our country can be improved.
