2. Access to health care among female prisoners in a penitentiary in Ceará, Brazil.

This study investigated access to health care among female prisoners in the state of Ceará, Brazil, and screened for common mental disorders. We conducted an analytical cross-sectional study in the only female prison in the state. Ninety detainees participated in the study. All participants were either pregnant or postpartum women or had one or more of the following health problems: hypertension, diabetes mellitus, tuberculosis, syphilis, hepatitis B, HIV/AIDS. The data were collected using a structured questionnaire. The findings reveal that access to health care was limited, violating the fundamental rights of the prisoners. Screening for diseases on admission to prison was limited, especially among non-pregnant women. Differences in health care were found between health conditions, with priority being given to pregnant and postpartum women. Most of the inmates (68.24%) were found to be at risk for common mental disorders (SRQ score > 7). A positive correlation was found between age and mental health problems (p = 0.0002). Despite legislation guaranteeing access to health care in prisons, the prison system is unprepared to meet the health needs of female prisoners with comorbidities and pregnant and puerperal women. This study aimed to identify the structuring elements guiding the establishment of the social representations of diabetic foot among people with diabetes mellitus. This qualitative study is based on the Social Representations Theory and was conducted in a capital of the Brazilian Northeast. The free word association test and a roadmap were used to characterize the sociodemographic and clinical profile to collect data. The analysis was performed using openEVOC software. The constituent elements of the diabetic foot's social representation "cure" and "really bad", revealing that living with a diabetic foot is challenging, but there is hope for a cure, which is a driving force in the daily search for care. The "prevention" element emerged in the representational field, denoting a more critical view and a capacity to transform the identified core elements. We found that the representational structure is based on subjective, valuating, and attitudinal contents. This knowledge can contribute to the design of interventions in the provision of care and diabetic foot screening in PHC services. [ABSTRACT FROM AUTHOR]

Home visits are a technology used in mental health services to address individuals with mental distress and their families. The intent is to analyze different types of knowledge and professional practices related to home visits, grounded on the analytical framework provided by the Psychiatric Reform's dimensions, namely: theoretical-conceptual, technical assistance, legal-political, and sociocultural. This case study with a qualitative approach was conducted in a Psychosocial Care Center (CAPS) located in Fortaleza, Ceará, Brazil. Observation and semi-structured interviews were held with workers and later analyzed from a critical hermeneutic perspective. The results show the territory covered by the service is acknowledged as a social space in which individuals are allowed to express their experience/suffering and a place where care is produced. Home visits require the reorganization of work processes and the development of deinstitutionalization strategies, which in turn promote the patients' autonomy and contractual power. There are, however, challenges to be overcome, such as insufficient human resources and a lack of material resources. Additionally, workers have to deal with urban violence and impediments to cultural change in the way society deals with mental illness. Home visits are essential within CAPS as a strategy to dismantle the traditional asylum apparatus. [ABSTRACT FROM AUTHOR]

Considering the current changes in social conditions brought about by the psychiatry reform, effects of these transformations are also expected on the scientific production in this field. The article aimed at analyzing studies published in journals between 2000 and 2005 discussing mental health and its dimensions. Nursing and Collective Health national and international journals, classified in Brazil as B or C, were reviewed. In the documental analysis of the material the following categories emerged: organizations and management of services, training of mental health professionals, psychosocial rehabilitation strategies and results, care of people with psychiatric suffering, and perceptions on mental illness. It noticed that the knowledge production in mental health is strongly influenced by practice. A partir de los cambios ocurridos en la realidad social con la reforma psiquiátrica, se supone que los reflejos de estas transformaciones se hayan presentado también en la producción científica de esta área. El estudio ha tenido como objetivo analizar publicaciones de periódicos que enfocan la salud mental y sus dimensiones, en recorte temporal de 2000-2005. Han sido consultadas cinco revistas clasificadas en Brasil como B y C en el área da enfermería y salud colectiva y de circulación nacional o internacional. En el análisis documental del material surgieron las siguientes categorías: organizaciones y gestión de los servicios, enseñanza y formación de profesionales en el área de salud mental, estrategias y resultados de la rehabilitación psicosocial, el cuidado a la persona en sufrimiento psíquico y las percepciones sobre la enfermedad mental. Estas han permitido percibir que la producción de conocimiento acerca de salud mental, sufre la influencia de la práctica. A partir das mudanças ocorridas na realidade social, com a reforma psiquiátrica, supõe-se que os reflexos destas transformações tenham se apresentado também na produção científica desta área. O estudo teve como objetivo analisar publicações de periódicos que abordam a saúde mental e suas dimensões, em recorte temporal de 2000 a 2005. Foram consultadas cinco revistas classificadas como Qualis B de circulação nacional ou Qualis B ou C de circulação internacional nas áreas da Enfermagem e Saúde Coletiva. The Family Health Strategy should be focused on the family unit and constructed operationally within the community sphere. The research assessed the family focus and community orientation as attributes of Primary Health Care, comparing if the responses differed among users, professionals and managers. It is an evaluative study of a population-based quantitative approach conducted between January 2010 and March 2011 in São Luís in the state of Maranhão. The study involved a population of 32 managers and 80 professionals with more than six months experience in the Family Health Strategy, and 883 users were selected by means of cluster sampling. Questionnaires validated in Brazil were used based on the components of the Primary Care Assessment Tool (PCATool). The composite index of the family focus was 2.7 for users, 4.9 for professionals and 5.3 for managers. In the posttest phase, differences were detected between users and professionals, and users and managers. The composite index of community orientation was 2.9 for users, 3.9 for professionals and 4.8 for managers (p < 0.001). Managers attributed higher percentages in all indicators, followed by professionals and lastly users. Both attributes were rated as being unsatisfactory in the perception of the users. A Estratégia Saúde da Família deve ser focada na unidade familiar e construída operacionalmente na esfera comunitária. A presente pesquisa avaliou o enfoque familiar e a orientação para a comunidade como atributos da Atenção Primária em Saúde, comparando se as respostas diferem entre usuários, profissionais e gestores. Trata-se de pesquisa avaliativa de abordagem quantitativa de base populacional, realizada de janeiro de 2010 a março de 2011 no município de São Luís (MA). Trabalhou-se com população de 32 gestores e 80 profissionais com mais de seis meses de experiência na Estratégia e com amostra de 883 usuários selecionados por conglomerados. Foram utilizados questionários validados no Brasil a partir dos componentes do Primary Care Assessment Tool (PCATool). O índice composto do enfoque familiar foi de 2,7 para os usuários, 4,9 para os profissionais e 5,3 para os gestores. No pós-teste as diferenças foram entre os usuários e os profissionais, e usuários e gestores. O índice composto da orientação para a comunidade foi de 2,9 para os usuários, 3,9 para os profissionais e 4,8 para os gestores (p < 0,001). Os gestores atribuíram percentuais mais altos em todos os indicadores, depois os profissionais e por último os usuários. Os dois atributos tiveram avaliação insatisfatória na percepção dos usuários. - Abstract
Objetivo: analisar a organização do processo de trabalho do Núcleo de Apoio à Saúde da Família (NASF). Método: pesquisa qualitativa, com 11 pro ssionais de duas equipes de NASF em Fortaleza, Ceará. A coleta de dados foi por meio de entrevista semiestruturada e observação livre, com análise baseada na Hermenêutica-Dialética. Resultados: evidenciaram-se descompassos da formação em relação à proposta de trabalho do nasf, precárias condições laborais e di culdades de articulação e pactuações de apoio à Estratégia Saúde da Família-esf, bem como potencialidades como trabalho em equipe, à cogestão e compartilhamento das ações. O NASF foi reconhecido como apoio à esf, entretanto, ainda não consegue atuar de modo a efetivar as diretrizes propostas pelo Ministério da Saúde. Conclusão: há necessidade de discussão crítica e repactuação dos processos de trabalho no nasf, com vistas a materializar suas diretrizes, orientadas pelo atendimento às necessidades de saúde da população.
