Orientador: Helder Antonio Rebêlo Pontes Dissertação (mestrado) – Universidade Estadual de Campinas, Faculdade de Odontologia de Piracicaba Resumo: O objetivo desse trabalho foi investigar as características clínicopatológicas da doença relacionada ao IgG4 (IgG4-RD) envolvendo a região oral e maxilofacial. Casos de IgG4-RD manifestando-se na região oral e maxilofacial foram recuperados de três instituições brasileiras. Os dados clínicos e sorológicos foram obtidos nos prontuários dos pacientes, enquanto os achados microscópicos e imunohistoquímicos foram revisados por patologistas orais. Os casos foram diagnósticos seguindo os critérios do American College of Rheumatology / European League Against Reumatismo. Foram encontrados sete casos, todos acometendo mulheres, com média de idade de 55,4 anos. A glândula submandibular foi acometida em quatro casos, a língua e a glândula parótida em dois casos cada e o palato em um caso. Em dois casos, a biópsia do lábio inferior foi utilizada como auxílio diagnóstico. O valor sérico de IgG4 variou de 185 a 5.930 mg / dL. Foi comum um infiltrado linfóide moderado a grave contendo plasmócitos e linfócitos, com aumento da razão IgG4 / IgG. O tratamento variou e corticóides foram a medicação mais utilizada (57,4%). Seis pacientes permaneceram vivos (quatro sem doença e dois com doença estável), enquanto um morreu de causa desconhecida. Embora as glândulas salivares maiores sejam comumente afetadas na IgG4-RD, a cavidade oral também pode estar envolvida, e a biópsia do lábio inferior pode ser uma ferramenta diagnóstica auxiliar Abstract: The aim of this study was to investigate the clinicopathological features of IgG4- related disease (IgG4-RD) involving the oral and maxillofacial region. Cases of IgG4-RD manifesting in the oral and maxillofacial region were retrieved from three Brazilian institutions. Clinical and serological data were obtained from the patients’ medical charts, while microscopic and immunohistochemical findings were revised by oral pathologists. Diagnoses followed the American College of Rheumatology/European League against Rheumatism criteria. Seven cases were found, all affecting women, with a mean age of 55.4 years. The submandibular gland was affected in four cases, the tongue and the parotid gland in two cases each, and the palate in one case. In two cases, lower lip biopsy was used as diagnostic approach. Serum IgG4 value ranged from 185 to 5,930 mg/dL. A moderate to severe lymphoid infiltrate containing plasma cells and lymphocytes, with an increased IgG4/IgG ratio was common. Treatment varied and steroids were the most frequently used (57.4%). Six patients remained alive (four free of disease and two with stable disease), while one died due to unknown cause. Proficiency testing is defined as the use of interlaboratory comparisons to determine the performance of individual laboratories for specific tests or measurements and to monitor laboratories‘ continuing performance. The number of PTs is increasing in the last years because the result of this exercise is useful not just for costumers, but also for other interested parties, such as regulators, laboratory accreditation bodies and other organizations who specify requirements for laboratories. Hence, it is very important to organize PT with criteria, focus and quality. The objective of this work was design of a quality system in Mat Control Laboratory of the University of Barcelona, according to the ISO/IEC DIS 17043 as a Proficient Test Provider. The quality system proposal for Mat Control was elaborated from September 2009 to January 2010 based on the attendance and participation in PT activities carried out in the Department of Analytical Chemistry of the Faculty of Chemistry, University of Barcelona. Fifteen documents were proposed to compose the structure of the quality system. Seven elaborated ―Standardized Operation Procedures‖ cover technical requirements of ISO/IEC DIS 17043. Instructions for a management equipment procedure shall be done considering mainly ISO/IEC 17025, and this will be a very important step to demonstrate Mat Control‘s competence to run analytical tests. Two ―General Quality Procedures‖ define elaboration, coding and management of documents. Other management requirements are contemplated in the quality manual and will be totally covered with the elaboration of instructions for four management procedures already structured.
