The objective of this preclinical study was to evaluate the feasibility and safety of transcatheter endocardial alginate hydrogel injection (TEAi) in a large animal model, utilizing the high-stiffness XDROP® alginate hydrogel in combination with the dedicated EndoWings® catheter-based system. All swine (n = 9) successfully underwent TEAi without complications. Acute results from a subset of animals (n = 5) demonstrated the ability of the catheter to access a wide range of endomyocardial areas and achieve consecutive circumferential hydrogel distribution patterns within the mid-left ventricular wall. Histological examinations at 6 months (n = 4) demonstrated that the XDROP® remained localized within the cardiac tissue. In addition, serial echocardiographic imaging showed that XDROP® had no adverse impacts on LV systolic and diastolic functions. In conclusion, this innovative combination technology has the potential to overcome the translational barriers related to alginate hydrogel delivery to the myocardium.

Acute pulmonary embolism (PE), often resulting from deep vein thrombosis (DVT), is the third most frequent cause of cardiovascular death and is associated with increasing incidence, causing considerable morbidity and mortality. This review aims to evaluate the efficacy, safety, and outcomes of treatment options in the management of acute PE and DVT, encompassing both established and emerging technologies, such as catheter-directed thrombolysis, aspiration thrombectomy, and other endovascular techniques. A comprehensive literature review was conducted, assessing clinical studies, trials, and case reports that detail the use of percutaneous interventions for PE and DVT and analyzing the advantages and disadvantages of each percutaneous system. Several percutaneous treatments have shown promising results, especially in cases where rapid thrombus resolution is critical, such as in high- and intermediate–high-risk patients. The incidence of major complications, such as bleeding, remains a consideration, though it is generally manageable with proper patient selection and technique. It is fundamentally important to tailor the specific treatment strategy to the clinical and anatomical characteristics of each patient. Percutaneous treatments for acute PE and DVT represent valuable options in the therapeutic arsenal, offering enhanced outcomes in appropriately selected patients. Background: The clinical characteristics and outcomes of patients who develop cardiogenic shock (CS) secondary to primary valvular dysfunction (valvular cardiogenic shock [VCS]) remain unclear. Objectives: The purpose of this study was to describe the cohort of patients with VCS and understand their outcomes compared to other forms of CS. Methods: All patients admitted to Cleveland Clinic cardiac intensive care unit between January 1, 2010, and December 31, 2021, with a diagnosis of CS were retrospectively identified. Characteristics and outcomes for shock patients with VCS were compared to those without VCS. Results: A total of 2,754 patients were admitted to our cardiac intensive care unit with CS, of which 442 (16%) had VCS. Introduction: There is a dearth of research available on the outcomes, complications, and recurrence rates of the modalities employed in treatment of diverticula after stone clearance by PCNL. We present our experience of various approaches employed in our institute for treatment of caliceal diverticulum after stone clearance by PCNL. We aimed to review the outcomes, complications, and recurrence rates of these procedures which can provide valuable insights into the effectiveness of these techniques in the treatment of stone containing caliceal diverticulum. Methods: A retrospective analysis of a prospectively maintained database of patients diagnosed with caliceal diverticular stone was conducted. The primary outcome was the stone-free rate (SFR) at the time of hospital discharge, determined by a combined nephroscopic/fluoroscopic assessment, and the obliteration of the diverticular sac. Secondary outcomes included the evaluation of operative time, duration of hospital stay and postoperative complications. Results: A total of 53 patients were evaluated. The mean diverticulum size was 23.2 mm, most common location was the superior calyx (30 (56.7%)). Group 1 (diverticular neck treatment + DJ stent) included 27 patients, group 2 (diverticular wall fulguration + PCN) included 18 patients and group 3 (PCN alone) included 8 patients. Mean operating time was highest in group 1 (80 min). Stone clearance was 100% in group 1, 91% in group 2 and 88% in group 3. Obliteration of caliceal diverticulum was highest in group 1 (90%). Mean duration of hospital stay was lowest in group 1 (3.2 days). Overall complications were lowest in group 2 (3/18). Conclusion: PCNL followed by combination of diverticular wall fulguration and PCN or treatment of diverticular neck and DJ stenting is safe and effective in causing diverticular obliteration. For the diagnosis of PS an integrated approach with echocardiography, cardiac computed tomography and cardiac magnetic resonance (CMR) is of paramount importance for the planning of the interventional treatment. In recent years, transcatheter approaches for the treatment of PS have increased however, meaning surgery is a possible option for complicated cases with anatomy not suitable for percutaneous treatment. Background: Percutaneous CT-guided cryoablation is an emerging technique for treating bone tumors. However, experience with using this procedure for osteoid osteomas in pediatric patients remains limited. Our study aims to assess its technical feasibility, clinical efficacy, and safety in children treated under conscious anesthesia. Methods: We conducted a retrospective study of consecutive pediatric patients who underwent CT-guided percutaneous cryoablation for osteoid osteomas at our institution between September 2017 and March 2021. All patients received conscious anesthesia. Data on peri-procedural VAS scores, post-procedural VAS scores, imaging findings, and nonsteroidal anti-inflammatory drug (NSAID) usage rates were collected for each patient. Technical success was defined as proper cryoprobe placement at the nidus center, while clinical success referred to pain relief without NSAID use. Intra- and post-operative complications were also evaluated. Results: Nine patients underwent CT-guided percutaneous cryoablation for osteoid osteomas under conscious sedation, with a 100% overall success rate with low peri-procedural and median VAS scores (p < 0.01). No complications were observed during or after the procedure. Conclusions: CT-guided percutaneous cryoablation of pediatric osteoid osteomas is an effective and safe minimally invasive procedure feasible under conscious anesthesia, holding promise as a valuable treatment option. Painful benign bone tumors often adversely influence quality of life primarily due to skeletal-related events such as unremittable pain, pathologic fracture, neurologic deficit, as well as skeletal growth disturbance. Substantial advances in percutaneous minimally invasive interventions for treatment of painful benign bone tumors beyond osteoid osteoma have been established as safe, efficacious, and durable treatments to achieve definitive cure. This article details the available armamentarium and most recent advances in minimally invasive percutaneous interventions and the role of radiologists for the management of patients with benign bone tumors beyond osteoid osteoma. Introduction: Percutaneous treatment is an effective alternative to surgery in suitable cases for liver hydatid disease. This study aimed to review the clinical findings and treatment methods of patients who developed cavity infections after percutaneous intervention and to evaluate the morbidity and mortality rates. Materials and Methods: Between January 2010 and December 2020, 560 (45%) of 1240 patients diagnosed with liver hydatid cysts were treated percutaneously by the interventional radiology clinic of our hospital. In 32 (5.71%) patients, the operation was planned as a result of the development of a cavitary infection. Demographic data, clinical and laboratory findings, diagnostic methods, surgical treatment methods, morbidity, mortality, and long-term results of the patients were evaluated retrospectively. Results: The main complaints of patients with infected liver cysts were pain and fever. Emergency surgery was planned for 3 patients. One patient died without being operated on due to rapidly developing cholangitis and septic shock. Two patients underwent emergency surgery, and 29 underwent elective surgery. Open surgery was performed in 28 patients, and laparoscopic surgery was performed in 3 patients. All patients underwent conservative surgery. Conclusion: Cholangitis and sepsis appear as serious clinical entities in patients who develop cavity infections after the percutaneous intervention. This rate is significantly higher than in patients operated on electively. These patients should be hospitalized, and pre-operative broad-spectrum antibiotics should be started and monitored closely. A new minimally invasive method for the management of catheter-related pseudoaneurysms (PSAs) using percutaneous temporary guidewire embolization (TGE) is presented. We performed percutaneous insertion of a flexible 0.018-in. guidewire into the PSA cavity under ultrasound guidance. Once thrombosis of the PSA cavity was achieved, the guidewire was removed. In all seven cases, TGE was technically feasible and achieved complete thrombosis of the PSA. The time required for PSA thrombosis from insertion to removal of the guidewire ranged from 5 to 40 minutes. TGE is a highly effective, safe, and minimally invasive treatment of PSA.

We aimed to search for the efficacy of percutaneous transhepatic cholangiography-guided balloon sphincteroplasty, mechanical lithotripsy, and laser lithotripsy in the treatment of bile duct stones and share our experience in retrievable internal plastic biliary stents derived from drainage catheter. Twenty-nine patients, in whom endoscopic retrograde cholangiopancreatography was contraindicated or failed, were included in this retrospective study. Balloon sphincteroplasty and balloons were used as the first choice of percutaneous treatment for stone removal. In four patients (14%), combined treatment with balloon sphincteroplasty and laser lithotripsy was performed. Plastic internal biliary stent converted from drainage catheter was advanced in 14 patients (48%). In 18 (62%) patients, a single procedure was performed while two sessions were required for nine patients and three sessions for two patients. Complete stone removal was achieved in 86% of the patients, and small residual stones remained in the biliary duct in four patients (14%). Two patients (7%) had minor complications including pain and fever, and the symptoms were ameliorated with conservative management. Stent retromigration was encountered in one patient (3.4%). The median follow-up time of the study was 4 months (range 1–51 months). Percutaneous treatment choices in bile duct stones are low-cost and safe methods that can be used efficiently if indicated. Background: Coarctation of the aorta (CoA) is a common congenital heart defect that affects about 3-4 in every 10 000 live births. Despite clear signs on clinical examination, the diagnosis is sometimes not made until adulthood. An increasing number of patients with CoA are reaching child-bearing age. Unrepaired CoA or severe recurrent stenosis during pregnancy is a significant concern, as it poses a high risk of maternal and foetal complications and even death., Case Summary: A 21-year-old woman was referred to the cardiology department at 17 weeks' gestation for management of arterial hypertension and unexplained systolic murmur. She had been diagnosed with hypertension elsewhere the year before presentation, but unfortunately, this remained unexplored. She had been started on labetalol early in the pregnancy. Clinical examination showed a loud systolic heart murmur extending from parasternal to subclavicular and scapular areas. Pulses in the lower extremities were very weak, and blood pressure was slightly elevated with a significant gradient between the upper and lower extremities. Echocardiography showed remarkable absence of pulsatile flow in the abdominal aorta and narrowing just distal to the subclavian artery with typical diastolic tail pattern on suprasternal imaging. Cardiac magnetic resonance confirmed the presence of a severe coarctation distal to the subclavian artery and presence of multiple collaterals allocating this patient in the extremely high-risk category with a risk of up to 40%-100% of maternal cardiac event during pregnancy. An extensive multidisciplinary team meeting was convened. After initial medical optimization, increased claudication and signs of placental hypoperfusion necessitated an endovascular procedure under general anaesthesia at 23 weeks' gestation. A Bentley BeGraft Plus stent (16 × 38 mm) was successfully placed. Postoperative ultrasound showed biphasic placental perfusion and normalization of blood pressure and ankle-brachial indices. At 36 weeks' gestation, the patient gave birth to a healthy child., Discussion: Coarctation of the aorta should be considered in any young patient with arterial hypertension. Altered maternal haemodynamics during pregnancy resulted in severe symptomatic CoA and reduced placental flow necessitating percutaneous intervention during pregnancy. VCS was predominantly due to native valve dysfunction as compared to prosthetic valve dysfunction (71% vs 29%, P  < 0.001), with the aortic valve noted to be the most common valve affected. Patients with VCS had higher 1-year (44% vs 37%, P  < 0.001) and 30-day all-cause mortality (28% vs 20%, P  < 0.001) compared to those without VCS. When compared to percutaneous intervention and medical therapy alone, surgical intervention in VCS was associated with the best short- and long-term outcomes ( P  < 0.001)., Conclusions: VCS is associated with poor short and long outcomes. Native valvular dysfunction and aortic valve involvement account for the majority of patients with VCS. Purpose: To report techniques and results of 16 cryoablation procedures in 11 patients treated for recurrent head and neck cancer. Methods and Materials: This retrospective study reviewed 11 consecutive patients with head and neck cancer recurrence after primary treatment by surgery and radiotherapy, treated with cryoablation between 2016 and 2020. Efficacy was measured by local control rate evaluated on MRI or/and PET. Tumor characteristics, number of cryoprobes, thermoprotective measures and complications were documented. Results: Sixteen cryoablation procedures were performed in 11 patients with head and neck cancer recurrence after surgery or radiotherapy, deemed ineligible for classic salvage treatment. Among 11 patients, four were treated for an epidermoid carcinoma, four for an adenocarcinoma and three for other types: 1 muco-epidermoid carcinoma, 1 adenoid cystic carcinoma and 1 esthesioneuroblastoma, 10/11 patients had prior surgery, 7/11 patients had prior chemotherapy and 3/11 patients had prior radiotherapy. Median number of cryoprobes was 4, [IQR, 3–6 cryoprobes], thermoprotective measures to protect surrounding organs were required for 10/16 procedures. After cryoablation, local control rate was 45.4% at a mean follow-up of 11.7 months (range 3–34 months). Among the 16 cryoablation procedures, four resulted in complications, two were considered major complications: one septic shock on inhalation pneumopathy during extubation, requiring intensive care; and one dysphonia due to a recurrent nerve injury. Conclusion: Cryoablation as a salvage treatment for recurrence of head and neck tumors after surgery and/or radiotherapy is an effective option, especially for patients that cannot benefit from salvage surgery. Lumbosacral hydatid disease (LHD), a rare skeletal parasitic disease that involves the lumbosacral region. In this study, we summarized the diagnostic and therapeutic procedures for patients with LHD to provide insights into managing this rare disease. Between 2000 and 2023, 16 patients diagnosed with LHD were retrospectively analyzed. Each patient's medical records and follow-up details, were carefully assessed. The average follow-up period was 11.25 ± 6.41 years, providing valuable insights into treatment durability and effectiveness. The diagnosis was confirmed via imaging, serological tests, and pathological examination. The clinical symptoms included lumbago with lower limb numbness (25 %) and urinary and fecal incontinence (25 %). All patients underwent surgery, with an average of 2.6 surgeries per patient. Thirteen (81.25 %) patients experienced recurrence postoperatively. LHD is a severe and complex skeletal parasitic disease with significant diagnostic and therapeutic challenges. Simple Summary: Percutaneous interventional radiological techniques offer many alternatives for treatment of Hepatocellular Carcinoma (HCC) using local anesthesia and sedation. These methods aim to destroy the malignant tumors locally without affecting the non-malignant liver. In this way, complications are kept low and patient recovery is quick. Indications depend on tumor size, type and stage, as well as patient's condition, liver function and co-morbidities. In recent years, a lot of research has been made in combining such approaches with immune therapy, but there is still much work to be done. This manuscript tries to analyze where we stand today and explain, using a comprehensive algorithm, the treatment options for each different clinical condition. Background: Hepatocellular carcinoma (HCC) can be treated by local and regional methods of percutaneous interventional radiological techniques. Indications depend on tumor size, type and stage, as well as patient's condition, liver function and co-morbidities. According to international classification systems such as Barcelona Clinic Liver Cancer (BCLC) classification, very early, early or intermediate staged tumors can be treated either with ablative methods or with transarterial chemoembolization (TACE), depending on tumor characteristics. The combination of both allows for individualized forms of treatment with the ultimate goal of improving response and survival. In recent years, a lot of research has been carried out in combining locoregional approaches with immune therapy. Although recent developments in systemic treatment, especially immunotherapy, seem quite promising and have expanded possible combined treatment options, there is still not enough evidence in their favor. The aim of this review is to provide a comprehensive up-to-date overview of all these techniques, explaining indications, contraindications, technical problems, outcomes, results and complications. Moreover, combinations of percutaneous treatment with each other or with immunotherapy and future options will be discussed. Use of all those methods as down-staging or bridging solutions until surgery or transplantation are taken into consideration will also be reviewed. Conclusion: Local and regional therapies remain a mainstay of curative and palliative treatment of patients with HCC. Currently, evidence on potential combination of the local and regional treatment options with each other as well as with other treatment modalities is growing and has the potential to further individualize HCC therapy. To identify the most suitable treatment option out of these new various options, a repeated interdisciplinary discussion of each case by the tumor board is of utmost importance. Introduction: Low back pain is one of the most common musculoskeletal problems among developed and developing nations. The prolapsed intervertebral disc (PIVD) is the most common pathology associated with treatable low back pain. The management protocol for PIVD has evolved from conservative treatment to surgical and finally to minimally invasive pain and spine interventions. Aims and Objectives: The aim of study was to evaluate the functional efficacy and patient satisfaction in PIVD patients with minimally invasive percutaneous endoscopic disc decompression (PEDD). Materials and Methods: All data was obtained from pain medicine OT records of tertiary hospital regarding epidemiological data, VAS score for pain, ODI score for functional outcome and Modified Macnab's criteria for patient satisfaction(Pre-op, 1 week, 1 month, 3 months and 1 year) were tabulated and statistically analyzed using SPSS version 21. Results: There were 30 male and 26 female patients in our series with the mean age of the 42.09±10.57 years. The VAS score showed statistically significant improvement 3 months (P=0.034) and at 1 year (P=0.012) postoperative. The modified ODI score showed statistically significant improvement during 3 months (P= 0.015) and 1-year score (P=0.008). The modified MACNAB scores (P= 0.001) showed significant improvement in patient satisfaction. One patient had a recurrence and opted for conventional surgical management. Few patients had transient side effects (dysesthesia-7, mild weakness-1) which improved with conservative measures. Conclusion: We conclude that percutaneous endoscopic disc decompression for single-level lumbar disc herniation is a safe, effective and low-cost definitive treatment modality with minimal complications.

In the normal lung, the only communications between the systemic and pulmonary arterial systems are the connections between the bronchial and pulmonary arteries that occur at the respiratory bronchioles, where pulmonary and bronchial capillaries freely anastomose. Rarely, anomalous connections can occur between normal or aberrant systemic arteries and pulmonary vessels. We performed a comprehensive literature review of all available manuscripts on PubMed and Google Scholar that included a case report or case series with diagnosis of systemic artery to pulmonary venous fistulas who underwent percutaneous treatment. Furthermore, we report three cases of children diagnosed and treated in our Pediatric Cardiology Center.

Objective: Thrombosis of a hemodialysis arteriovenous fistula (AVF) is a serious complication that needs urgent treatment. Most cases are treated surgically, but recently, endovascular strategies have become a viable alternative. This study is an evaluation of the success and patency rate of percutaneous balloon angioplasty of thrombosed hemodialysis fistulas using a drug-coated balloon (DCB) contrasted with a standard balloon (SB). Methods: The data of 33 patients with a thrombosed native hemodialysis AVF treated percutaneously in a tertiary care center were analyzed retrospectively. Success of the procedure was defined as restoration of flow with less than 30% residual stenosis and resumption of dialysis through the hemodialysis AVF. The success rate of the procedure and the patency rate at 1, 6, and 12 months were evaluated. The effect on patency of a DCB was compared to that of a SB. Results: Twenty-five radiocephalic and 8 brachiocephalic thrombosed hemodialysis AVFs were treated during the study period. Flow was restored in 23 thrombosed fistulas, a success rate of 69.7%. The patency rate of successfully treated fistulas was 95.6% at 1 month, 76.1% at 6 months, and 57.9% at 12 months. Ten of the 23 re-established AVFs were treated with a DCB and the remainder were treated with a SB. The patency of the fistulas treated with a DCB was similar to that of a SB at 1 month (100% vs 92.3%, respectively; p=0.393). The patency rate of a DCB was greater than that of a SB at 6 months (88.9% vs 66.7%, respectively; p=0.258) and 12 months (75% vs 45.4%, respectively; p=0.219). Conclusion: Percutaneous intervention for thrombosed hemodialysis AVFs is a safe, minimally invasive, and effective procedure. There was a positive trend in the patency rate of patients treated with a DCB at 6 and 12 months compared with a SB.

INTRODUCTION: Our aim was to evaluate percutaneous treatment of hydatid cyst cases of liver and to evaluate the efficacy, complication rate of percutaneous treatment and the reasons for failure in patients who did not respond to treatment. METHODS: Our study is a retrospective study and percutaneous treatment of hydatid cyst cases of liver was performed in our interventional radiology unit between January 2012 and November 2018 and the patients who were followed up by our clinic were included in the study. Patients were evaluated in terms of age, sex, localization of the cyst in the liver, number of cysts treated, cyst types and cyst size. Our unsuccessful treatment criteria are; increased cyst size without septation-calcification, no change in cyst stage, extravasation and recurrence. RESULTS: In our study, a total of 119 percutaneous treatment was applied to 104 patients [38 (36.5%) males and 66 (63.5%) females]. The percutaneous cysts were 4-16 cm in diameter and the average diameter was 7.8 cm. Ninety two (77%) cysts were compatible with type 1, 25 (21%) type 2 and 2 (1.6%) type 3 cyst hydatid. One hundred one treatment (85%) were successful; 18 treatment (15%) were not successful. Five (28%) of the unsuccessful treatment had cystobiliary fistulization, 2 (11%) had extravasation, 3 (17%) had inadequate medical treatment and 8 (44%) had recurrence. DISCUSSION AND CONCLUSION: Percutaneous treatment in hydatid cyst is a highly effective method because of can be applied in more than one cyst in the same session and low complication rate.

Abstract Background The spread of percutaneous arterial catheterization in diagnostic and therapeutic procedures has led to a parallel increase of vascular access site complications. The incidence of these events is between 0.2–1%. A detailed analysis of injuries by type of procedure shows a higher incidence of injuries after therapeutic procedures (3%) than those observed for diagnostic ones (1%), due to the greater size of the vascular devices used and the necessity to frequently administer anticoagulant and antiplatelet therapy during procedures. The iatrogenic arterial injuries requiring treatment are the pseudoaneurysm, arteriovenous fistula, arterial rupture and dissection. Less frequent complications include distal embolization of the limbs, nerve damage, abscess and lymphocele. Moreover, the use of percutaneous vascular closure devices (VCD) has further expanded the types of complications, with an increased risk of stenosis, thrombosis, distal embolism and infection. Our work aims to bring the personal 10 years’ experience in the percutaneous treatment of vascular access-site complications. Results Ninety-two pseudoaneurysms (PSA), 12 arteriovenous fistulas (AVF), 15 retrograde dissections (RD) and 11 retroperitoneal bleedings (RB) have been selected and treated. In 120/130 cases there were no periprocedural complications with immediate technical success (92.3%). Nine femoral PSA, treated with percutaneous ultrasound-guided thrombin injection, showed a failure to close the sac and therefore they were treated by PTA balloon inflation with a contralateral approach and cross-over technique. Only one case of brachial dissection, in which the prolonged inflation of the balloon has not led to a full reimbursement of the dissection flap, was then surgically repaired. At the 7 days follow-up, complications were two abscesses in retroperitoneal bleedings, treated by percutaneous drainage. At 3 months, acute occlusion of 3 covered femoral stents occurred, then treated by loco-regional thrombolysis and PTA. A total of 18 major complications was recorded at 2 years, with a complication rate at 2 years of 13.8%. Conclusions The percutaneous treatment of vascular access-site complications is the first-choice treatment. It represents a safe and effective option, validated by a high technical success rate and a low long-term complication rate, that allows avoiding the surgical approach in most cases.

Arachnoid cysts are benign intracranial lesions. They are usually located in the middle fossa, but can be found in other locations. We present a case of symptomatic Meckel cave (MC) arachnoid cyst - a very rare location - and a treatment strategy not elsewhere described before for this condition. A 54-year-old female with trigeminal neuralgia with previous history of radiofrequency rhizotomy treatment 6 years before admission had been experiencing pain recurrence with progression, which required successive increases in carbamazepine dosage. Magnetic Resonance Imaging (MRI) showed dilatation of the right MC with extension to the petrous apex. The lesion was compatible with arachnoid cyst, and due to the worsening of the clinical condition, surgical treatment was chosen. Percutaneous puncture of the cyst through the foramen ovale with injection of intracystic fibrin sealant was performed. The patient woke up from anesthesia with pain improvement and was discharged asymptomatic the next day. After 12 months of follow-up, she remained pain-free. In the literature review, we found only eight cases reported as MC arachnoid cyst. These are likely to progress and become symptomatic owing to their communication with the subarachnoid space and a unidirectional valve mechanism. Pain improvement with this technique is probably secondary to the interruption of these mechanisms.

ABSTRACT In this article, we analyse the changes occurred over the last 20 years in the percutaneous treatment of bifurcation lesions based on our own experience. We also analyse the more relevant papers recently published, as well as the strategies and guidelines recommended for the percutaneous management of bifurcation lesions. Technical aspects are relevant in this field and, for this reason, a significant section of this manuscript is dedicated to the technique used together with some tips and tricks also provided here. The technical advances made over the last years have significantly increased the success rate in the management of these complex lesions. At the same time, the technique of treatment has been standardized allowing the management of all type of bifurcations including distal left main disease.

In the normal lung, the only communications between the systemic and pulmonary arterial systems are the connections between the bronchial and pulmonary arteries that occur at the respiratory bronchioles, where pulmonary and bronchial capillaries freely anastomose. Rarely, anomalous connections can occur between normal or aberrant systemic arteries and pulmonary vessels. We performed a comprehensive literature review of all available manuscripts on PubMed and Google Scholar that included a case report or case series with diagnosis of systemic artery to pulmonary venous fistulas who underwent percutaneous treatment. The clinical management of hepatic hydatidosis is complex and is fundamentally based on three pillars: surgical, percutaneous and pharmacological; which can sometimes be complementary. Therapy should be defined based on the characteristics, location and type of cyst; to which must be added the experience of the medical team, available resources and the adherence of patients to long-term monitoring. Surgery is the most used treatment, percutaneous techniques can possibly replace surgery in specific cases. However, despite advances in surgical techniques, in the use of chemotherapy and other treatments, the management of liver hydatidosis continues to be a major problem; mainly due to the lack of consensus on which procedure to use is better. The objective of this presentation is to present the wide range of existing therapies in the current management of hepatic hydatidosis and some suggestions in the use of each one of them. Cystic echinococcosis (CE) may be encountered in almost every site of the body, but bone involvement is relatively rare. The vertebral column and pelvis are the most affected areas. The combined medical and surgical approach is the main treatment option in current literature. Although percutaneous treatment of CE cysts located in the liver, spleen, kidney, and soft tissues has become a serious alternative to surgery, there is no bone CE cyst treated percutaneously in the literature. Step changes in angiographic imaging are not commonplace. Since the move from analogue to digital and flat detector plates, two-dimensional imaging technology has certainly evolved but not jumped forward. Of all the routine imaging techniques used in cardiology, angiography has been the last modality to embrace the third dimension. Although the development of rotational angiography was initially for the benefit of neuroimaging and fusion of cross sectional datasets was aimed at the treatment of descending aortic pathology, interventional physicians in congenital and structural cardiology have immersed themselves in this technology over the last 10 years. Like many disruptive technologies, its introduction has divided opinion. We aimed to explore the mindset of those in the field of interventional cardiology who are driving imaging forward. These structured interviews recorded during the 21st Pediatric and Adult Interventional Cardiac Symposium illustrate the challenges and sticking points as well as giving an insight into the direction of travel for three-dimensional imaging and fusion techniques. Covering a wide range of career development, seniority and experience, the interviewees in this article are probably responsible for the majority of the published literature on invasive three-dimensional imaging in congenital heart disease.

A pulmonary arteriovenous malformation (PAVM) is a rare anomaly that may have significant clinical complications. PAVMs are commonly seen in patients with hereditary hemorrhagic telangiectasia, while some 10% of PAVMs may be idiopathic. PAVMs can cause cyanosis, fatigue, polycythemia, and paradoxical thromboembolic complications. The diagnosis and treatment of a PAVM should be performed with great care, as the disorder may be fatal if not properly treated. Percutaneous closure (such as embolization) can be very beneficial. Presently described is the case of a 23-year-old man with an idiopathic PAVM who was treated percutaneously with 3 vascular plugs.

The article presents literature review about surgical treatment of liver hydatidosis. Particular attention is paid to the development of percutaneous treatment. Also modern trend of surgical treatment are described. We presented results of percutaneous treatment of liver hydatidosis in our clinic. We analized them and made the conclusion that percutaneous treatment is effective and safe method for liver hydatidosis. To reduce the risk of recurrence we have to do it right and all patients must take anti-relapse chemotherapy.

Objective: To evaluate efficacy, safety and the long-term results of percutaneous imaging-guided treatment of hydatid cysts (HC) in children and to discuss the differences of applied percutaneous interventional techniques. Material and Methods: Our retrospective study was conducted with the approval of the local ethics committee. Between October 2009 and April 2014, 34 children (22 female, 12 male; mean age 9.1±3.2 years) with type WHO-CE 1 and CE 3a, 60 HC underwent percutaneous treatment. For 32 cysts smaller than 6 cm in longest diameter the method of treatment was 'Puncture, Aspiration, Injection, Reaspiration" (PAIR), for 28 cysts with diameters larger than 6 cm the method of treatment was catheterization. Catheter was leaved to free drainage in 16 cysts, out of which 12 was ended in single session. All patients were followed first month and every 3 months during the 1st year, every 6 months during the 2nd year, and after then yearly. Results: Thirty two cysts with a diameter of less than 6 cm were treated with the PAIR method, and 28 cysts with a diameter greater than 6 cm were treated with catheterization method. While the catheter was left in free drainage in 16 cysts, the catheter was removed at the end of the procedure in 12 cysts, and the treatment was terminated in one session. Percutaneous treatment of HC was successful in all patients. Mean volume reduction was 74.6% with PAIR, 78.7% with single session catheterization, and 57.9% in catheterization -- free drainage methods. The most potent method is single session drainage followed by catheterization -- drainage and lastly PAIR (p < 0.000). Mean of hospitalization periods were 16 hours for both of those patients treated with the PAIR technique, and single session catheterization, 2.1 days for the patients treated by catheterization -- free drainage. The mean follow-up was 93.6 months. Conclusion: Percutaneous HC treatment is a highly efficient, successful and safe procedure. Percutaneous treatment should be preferred in WHO CE type 1 and 3a cysts especially in pediatrics. Performing the catheterization without keeping it in the cavity makes it advantageous in pediatrics because of convenience of approach, low pain, high patient adherence, and short hospitalization period. Intervertebral disc degeneration (IDD) stands for the most frequent cause of low back pain. Finding a cure for this disease is an important challenge as current conservative treatments and surgical interventions fail to bring a solution to this disease. Ozone therapy (O 2 -O 3 ) has yielded outstanding outcomes in intervertebral disc pathology. The ozone's efficacy in the treatment of IDD remains unconfirmed. This study aimed to assess the effectiveness of intradiscal ozone injection on IDD induced in a rat. Effects of ozone therapy on the viability of nucleus pulposus cells were evaluated by CCK-8 assays. Macrophage immunoreactivity was detected by immunohistochemical, the expression of collagen type II was evaluated by western blot, and measurement of oxidative stress parameters was realized. Molecular docking studies were carried out in order to predict the interaction formed between O 3 and the target enzymes, on the one hand, O 3 with PI3K and, on the other hand, O 3 with COX-2. IRM, X-ray, hematoxylin-eosin, and bleu alcian staining were realized to assess the therapeutic impacts of ozone in the puncture-induced rat model of IDD. In vivo, O 3 ameliorated the IDD in the early stage of this disease. It was also displayed in molecular docking that O 3 might bind to PI3K to suppress the PI3K/Akt/NF-κB signaling pathway. This study's results show that the O 3 should be administered at the low grade of IDD and at an early stage because it cannot restore the advanced inflammatory alteration of the IVD. Introduction: Spinal epidural abscess (SEA) incidence is rising. However, most series do not differentiate between SEAs associated with pyogenic infectious spondylodiscitis (PS) and SEAs limited to the epidural space.Methods: We retrospectively reviewed the records and radiological images of all patients admitted to our institutions with a diagnosis of SEA not associated with PS between January 2013 and December 2018.Results: We found three males and four females; five of the seven were intravenous drug users. All patients presented with pain: in six, it was associated with acute motor and sensory deficits, while one had only pain and paresthesias. Staphylococcus aureus was cultured from abscesses and/or from multiple blood cultures in four patients. Abscesses were localized to the cervical spine in one patient, thoracic in three, lumbar in one, and in two, the SEAs involved multiple segments. All patients but one underwent urgent open surgery. This patient had a multisegmental abscess and was successfully treated by percutaneous aspiration when pain became intractable. After abscess evacuation, the neurological deficits improved in all patients except one. The patients that were treated without spine instrumentation did not develop delayed kyphosis or instability at follow-up.Conclusion: Patients with SEAs not associated with PS are likely to present with pain and motor deficits, appear to benefit from urgent abscess evacuation, and seem to be less dependent on spine instrumentation to avoid delayed spinal deformities compared to SEA associated with PS. Finally, the lack of initial involvement of bone and intervertebral disks may suggest that at least some of the SEAs without PS originate from infection of epidural lymphatic vessels that are not present inside those structures.

Pulmonary thromboembolism (PTE) is usually caused by deep vein thrombosis (DVT) in the lower extremities; which can be as varied clinical spectrum as asymptomatic embolism detected incidentally to serious disease with massive embolism causing death. A 44 year-old female patient was admitted to emergency department with complaints of general condition impairment, hypotension and marked dyspnea. She had a fracture on the right femur proximal region after falling a month ago. Lower extremity Doppler ultrasonography revealed findings consistent with acute deep vein thrombosis in the right lower extremity. Emergency pulmonary CT angiography revealed bilateral massive pulmonary thromboembolism extending especially from the main pulmonary artery to the right pulmonary artery. IV thrombolytic was contraindicated as a result of head trauma and subdural hematoma history a month ago. The patient was taken to the catheter laboratory and we performed a selective thrombus aspiration and fragmentation. The vital signs and hemodynamics of the patient improved rapidly after the procedure. This case report is important for demonstrating rapid percutaneous management of a young female patient with a life-threatening condition and favourable outcome of percutaneous intervention despite many comorbid conditions.

Although superior mesenteric artery stenosis is a relatively common situation, it is rarely symptomatic due to the fact that in a significant number of cases an adequate collateral circulation exists. The aim of this study is to report a case in which arterial reconstruction was needed due to the absence of such a patent collateral circulation. The 47-year-old patient was investigated for chronic postprandial pain and was diagnosed with superior mesenteric artery stenosis. Percutaneous treatment was the initial option of choice but the patient rapidly became symptomatic again. Therefore surgery was performed, the segment of arterial stenosis was resected and the arterial continuity was re-established by using a synthetic prosthesis. The postoperative outcome was uneventful, the patient was discharged in the seventh postoperative day under anticoagulant therapy. In conclusion, superior mesenteric artery reconstruction by using a synthetic prosthesis can be useful in cases presenting chronic mesenteric ischemia and failure of percutaneous treatment. Purpose: To evaluate and compare the results of puncture, aspiration, injection and re-aspiration (PAIR) and catheterization techniques for treatment of CE1 and CE3a liver hydatid cysts according to World Health Organization classification.Materials and Methods: Forty patients (29 females) with 56 liver CE1and CE3a cysts were prospectively randomized and enrolled into 2 groups by sealed envelope method. Procedures were performed under general anesthesia. Several parameters including technical success (completing procedure steps), clinical success (lack of recurrence on follow-up), major and minor complications, long-term changes of cyst cavities and length of hospital stay were compared between two groups.Results: As in 2 patients with 3 cysts, PAIR technique had to be changed to catheterization technique due to technical reasons. The technical success rates were 91.9% and 100% for PAIR and catheterization groups, respectively. Volume decrease rates were 78.5% and 86.8% in PAIR and catheterization groups, with a mean follow-up of 78.1 and 71 months, respectively. There was no mortality, anaphylactic shock or intraabdominal dissemination. The rate of major complications such as abscess, cysto-biliary fistula and recurrence was 2.94% and 36.84% in PAIR and catheterization groups, respectively (p = 0.002). Median length of hospital stay was shorter in PAIR group (1 vs 4 days) (p = 0.015).Conclusion: PAIR technique should be preferred to catheterization technique for treatment of liver CE1 and CE3a cysts due to lower rates of major complications and length of hospital stay. Aim: We aimed to determine predictive factors for predicting cystobiliary fistulas (CBF) in children after treatment of liver hydatid cyst (LHD).Methods: The records of patients who were treated for LHD between 01.06.2009 and 1.06.2019 were retrospectively reviewed. Age, sex, laboratory test results, size and number of cysts, method of first intervention (percutaneous or surgery), whether or not CBF developed and how it was treated were investigated. Among findings, those could be predictive were investigated. Data were evaluated with SPSS 21.0 program, p < 0.05 was considered significant.Results: Of the 97 patients, 48 (49.5%) were male, 49 (50.5%) female, the mean age was 11.2 years, Eighty patients had right (82.5%), 13 had left, and 4 had bilobar involvement. As first intervention, surgery was performed in 39 (40.2%); percutaneous treatment was performed in 58 (59.8%) patients. In 8 patients (20.5%) in surgery group and in 6 patients (10.3%) in percutaneous group, totally in 14 patients (14.4%), CBF developed. The mean cyst diameter of CBF-developed group was 114.36 mm, and of CBF-undeveloped group was 74.30 mm. There was no statistically significant differences between groups in terms of age, sex, involved lobe, other organ involvement, and preoperative results (p > 0.5). There was a significant relationship between the cyst diameter and the rate of CBF development in both surgical and percutaneous groups (p < 0.05). ROC analysis was performed, and the cut-off value for the development of CBF detected as 69 mm for children. Since obstructive jaundice seen in adults is not common in children, an increase in liver function tests and bilirubin levels were not seen in our patients.Conclusion: A significant correlation was found only between the size of the cyst and developing CBF. Cysts greater than 69 mm have a higher risk of developing CBF after both percutaneous and surgical treatment and should be closely monitored. [ABSTRACT FROM AUTHOR]

PURPOSE: To evaluate therapeutic efficacy of two minimally invasive surgical methods in managing acute ureteral obstruction and severe infection caused by upper urinary tract calculi (UUTC). PATIENTS AND METHODS: Data of 47 patients diagnosed with acute upper urinary tract obstruction and severe infection caused by ureteral calculus using X-ray CT between September 2014 and January 2019 were retrospectively analyzed. All patients were treated with immediate renal drainage and, after infection and ureteral obstruction were relieved, UUTC removal. Renal drainage was performed by ultrasound-guided percutaneous nephrostomy and retrograde ureteral catheterization was performed using cystoscopy. Kidney and ureteral stones were removed; renal function and the urinary tract were examined by X-ray during follow-up. RESULTS: Percutaneous nephrostomy was performed in 29 patients in a critical condition including intolerance to surgery, high-grade hydronephrosis, or failure of retrograde ureteric stent placement. In other 18 patients diagnosed with small stones (≤10 mm) and low-grade hydronephrosis, indwelling double-J ureteral stents were temporally installed by a cystoscope. Acute infection and ureteral obstruction were relieved; white blood cell counts returned to normal values within 3 to 7 days after drainage in all patients. In the second-stage treatment, percutaneous nephrolithotomy (PCNL), ureteroscopic lithotripsy, extracorporeal shock wave lithotripsy and nephrectomy were performed in 24, 10, 8 and 5 patients, respectively. No patients developed severe complication after stone removal surgery. All patients were followed up for 3 months to 4.5 years. Renal function was significantly recovered; 17/29 (59%) patients with elevated serum creatinine returned to normal and serum creatinine in 12/29 (41%) patients improved significantly after drainage, with a pre-operation level of 285±169μM vs 203±91μM post-operation (P = 0.014). Five patients were lost during follow-up. CONCLUSION: This study demonstrated an optimal approach for relieving upper urinary tract obstruction and acute infection in which percutaneous nephrostomy drainage is preferred for patients with severe pyonephrosis, large stones (>10 mm) with high-grade hydronephrosis, steinstrasse, or failure in retrograde ureteric stent placement, while retrograde ureteral catheterization using cystoscopy is suitable for patients diagnosed with small stones (≤10 mm) and low-grade hydronephrosis.
