4. Signorini problem as a variational limit of obstacle problems in nonlinear elasticity

An energy functional for the obstacle problem in linear elasticity is obtained as a variational limit of nonlinear elastic energy functionals describing a material body subject to pure traction load under a unilateral constraint representing the rigid obstacle. There exist loads pushing the body against the obstacle, but unfit for the geometry of the whole system body-obstacle, so that the corresponding variational limit turns out to be different from the classical Signorini problem in linear elasticity. However, if the force field acting on the body fulfils an appropriate geometric admissibility condition, we can show coincidence of minima. The analysis developed here provides a rigorous variational justification of the Signorini problem in linear elasticity, together with an accurate analysis of the unilateral constraint.

We consider the topic of linearization of finite elasticity for pure traction problems. We characterize the variational limit for the approximating sequence of rescaled nonlinear elastic energies. We show that the limiting minimal value can be strictly lower than the minimal value of the standard linear elastic energy if a strict compatibility condition for external loads does not hold. The results are provided for both the compressible and the incompressible case.

We study an old variational problem formulated by Euler as Proposition 53 of his `Scientia Navalis' by means of the direct method of the calculus of variations. Precisely, through relaxation arguments, we prove the existence of minimizers. We fully investigate the analytical structure of the minimizers in dependence of the geometric parameters and we identify the ranges of uniqueness and non-uniqueness.

A new energy functional for pure traction problems in elasticity has been deduced in [23] as the variational limit of nonlinear elastic energy functional for a material body subject to an equilibrated force field: a sort of Gamma limit with respect to the weak convergence of strains when a suitable small parameter tends to zero. This functional exhibits a gap that makes it different from the classical linear elasticity functional. Nevertheless a suitable compatibility condition on the force field ensures coincidence of related minima and minimizers. 16. The gap in Pure Traction Problems between Linear Elasticity and Variational Limit of Finite Elasticity

A limit elastic energy for pure traction problem is derived from re-scaled nonlinear energy of an hyperelastic material body subject to an equilibrated force field. We show that the strains of minimizing sequences associated to re-scaled non linear energies weakly converge, up to subsequences, to the strains of minimizers of a limit energy, provided an additional compatibility condition is fulfilled by the force field. The limit energy is different from classical energy of linear elasticity; nevertheless the compatibility condition entails the coincidence of related minima and minimizers. A strong violation of this condition provides a limit energy which is unbounded from below, while a mild violation may produce a limit energy with infinitely many extra minimizers which are not minimizers of standard linear elastic energy and whose strains are not uniformly bounded. A relevant consequence of this analysis is that a rigorous validation of linear elasticity fails for compressive force fields that do not fulfil such compatibility condition.

18. Variational Problems for Foppl-von Karman plates

Some variational problems for a Foppl-von Karman plate subject to general equilibrated loads are studied. The existence of global minimizers is proved under the assumption that the out-of-plane displacement fulfils homogeneous Dirichlet condition on the whole boundary while the in-plane displacement fulfils nonhomogeneous Neumann condition. If the Dirichlet condition is prescribed only on a subset of the boundary, then the energy may be unbounded from below over the set of admissible configurations, as shown by several explicit conterexamples: in these cases the analysis of critical points is addressed through an asymptotic development of the energy functional in a neighborhood of the flat configuration. By a $\Gamma$-convergence approach we show that critical points of the Foppl-von Karman energy can be strongly approximated by uniform Palais-Smale sequences of suitable functionals: this property leads to identify relevant features for critical points of approximating functionals, e.g. buckled configurations of the plate. Eventually we perform further analysis as the plate thickness tends to 0, by assuming that the plate is prestressed and the energy functional depends only on the transverse displacement around the given prestressed state: by this approach, first we identify suitable exponents of plate thickness for load scaling, then we show explicit asymptotic oscillating minimizers as a mechanism to relax compressive states in an annular plate., Comment: 32 pages

Background: Our aim is to evaluate the possible persistence of lung parenchyma alterations, in patients who have recovered from Covid-19. Design and methods: We enrolled a cohort of 115 patients affected by Covid-19, who performed a chest CT scan in the Emergency Department and a chest CT 18 months after hospital discharge. We performed a comparison between chest CT scan 18 months after discharge and spirometric data of patients enrolled. We obtained quantitative scores related to well-aerated parenchyma, interstitial lung disease and parenchymal consolidation. A radiologist recorded the characteristics indicated by the Fleischner Society and “fibrotic like” changes, expressed through a CT severity score ranging from 0 (no involvement) to 25 (maximum involvement). Results: 115 patients (78 men, 37 women; mean age 60.15 years old ±12.52). We study the mechanics of a reversible decohesion (unzipping) of an elastic layer subjected to quasi-static end-point loading. At the micro level the system is simulated by an elastic chain of particles interacting with a rigid foundation through breakable springs. Such system can be viewed as prototypical for the description of a wide range of phenomena from peeling of polymeric tapes, to rolling of cells, working of gecko's fibrillar structures and denaturation of DNA. We construct a rigorous continuum limit of the discrete model which captures both stable and metastable configurations and present a detailed parametric study of the interplay between elastic and cohesive interactions. We show that the model reproduces the experimentally observed abrupt transition from an incremental evolution of the adhesion front to a sudden complete decohesion of a macroscopic segment of the adhesion layer. As the microscopic parameters vary the macroscopic response changes from quasi-ductile to quasi-brittle, with corresponding decrease in the size of the adhesion hysteresis. At the micro-scale this corresponds to a transition from a `localized' to a `diffuse' structure of the decohesion front (domain wall). We obtain an explicit expression for the critical debonding threshold in the limit when the internal length scales are much smaller than the size of the system. The achieved parametric control of the microscopic mechanism can be used in the design of new biological inspired adhesion devices and machines.

Background: Traumatic brachial plexus injuries are rare but serious consequences of major traumas. Pre-ganglionic lesions are considered irreparable, while post-ganglionic injuries can be potentially treated if an early diagnosis is available. Pre-surgical diagnosis is important to distinguish low-grade from high-grade lesions and to identify their location. The aim of the review is to evaluate the diagnostic accuracy of magnetic resonance imaging (MRI) in the identification of adult post-ganglionic lesions due to traumatic brachial plexus injuries, compared to intraoperative findings. Methods: Research on the main scientific electronic databases was conducted. Studies of adults with traumatic post-ganglionic brachial plexus injuries were included. The index test was preoperative MRI and the reference standard was surgical exploration. Pooled sensitivity and specificity were calculated. Results: Four studies were included for the systematic review, of which three articles met the inclusion criteria for the meta-analysis. Pooled sensitivity and pooled specificity values resulted high. The sensitivity value is associated with a high heterogeneity index of the selected literature. Conclusion: MRI can be considered, despite the limits, the gold standard exam in morphological evaluation of brachial plexus injuries, particularly in the diagnosis of post-ganglionic traumatic injuries.

Primary hyperparathyroidism (PHPT) is a clinical condition characterized by overactive parathyroid gland secretion of parathyroid hormone with concurrent alteration of the phosphocalcemic metabolism. We present a literature review on primary hyperparathyroidism addressing key on clinical presentation, causes, medical and surgical treatment at the best of our knowledge. Based on this review we confirm the role of serum calcium and serum level examination, as well as we define the definitive treatment for PHPT being parathyroidectomy. In case of contraindication for surgery, medical treatment can play a relevant role.

En una búsqueda permanente de innovación educativa, surge el presente proyecto de trabajo interinstitucional, que contó con apoyo de la Facultad de Ciencias (UDELAR). El objetivo principal del mismo es que los estudiantes conozcan y se familiaricen con una investigación científica de comportamiento animal y de esa forma intentar responder a la pregunta: “Todos somos primates, pero ¿qué tan primates somos?” El proyecto se basa en el estudio del comportamiento en cautiverio de dos especies de primates, Papio hamadryas y Lemur catta, en el Zoológico “Villa Dolores”, Montevideo, Uruguay. Se realizó una valoración de aspectos positivos y negativos (tipo de recinto, ambientación y cuidados, estructura de grupo), habilitando una nueva dimensión de análisis: el “Enriquecimiento ambiental”. A partir de la construcción conjunta de un Etograma (entre estudiantes de los diferentes Centros Educativos de Secundaria) se realizó un análisis de los comportamientos más frecuentes registrados, en una comparación con el comportamiento humano. La Evaluación, considerada un aspecto fundamental, ha sido continua y coordinada durante el proceso. El trabajo deja abierto el debate sobre los objetivos y los roles que deberían cumplir los Parques Zoológicos, las Reservas. ¿Podríamos hablar de “Centros de Recuperación de Fauna”? ¿Quiénes deberían participar en la toma de decisiones al respecto?

Introduction: Adenoid cystic carcinomas (ACCs) account for 10% of all malignant salivary tumours. They are slow-growing but locally aggressive. Reports of liver and renal metastases are rare.Case description: A 58-year-old woman who had undergone resection of a left submandibular ACC in 1995 was referred to our centre for follow-up in 2018. A computed tomography scan revealed two lesions: one on segment six of the liver and the other on a kidney. A hepatic wedge resection and right nephrectomy were performed. The postoperative course was uneventful. At 2-year follow-up, the patient was alive and well with no evidence of disease.Conclusion: Management of ACC is a therapeutic challenge because of its tendency for distant metastases. The literature recommends regular follow-up imaging and radical surgical treatment but specific guidelines for the approach to recurrence are lacking.

37. The impact of COVID-19 lockdown on infectious diseases epidemiology: The experience of a tertiary Italian Pediatric Emergency Department.

Introduction: The aim of this study was to describe the rate and types of community-acquired respiratory infections observed in a pediatric ED during the SARS-CoV-2 related lockdown in Italy and to compare data with the same period of previous year.Methods: A retrospective analysis of medical charts of patients arrived at the ED of Gaslini Children's Hospital from 10th March 2020 to 30th April 2019 and the same frame of 2020 were performed. We compared two groups by demographics, duration of fever before ED admission, triage code, number of patients hospitalized after ED evaluation. We calculated proportion and incidence rate for airborne infections, fever, and urinary tract infections (UTI), appendicitis, and gastroenteritis for control.Results: 1362 children arrived at the ED during the lockdown compared to 5628 in the same period of 2019 (-75,8%). No difference was noticed (27.7% vs 28.4%) in the total amount of infectious episodes. A significant reduction in rate of incidence and proportion were observed for upper respiratory tract infections (21,4% vs 28%), otitis (2,6% vs 16,2%), streptococcal infections (0,5% vs 5,2%) and bronchiolitis (2,1% vs 5,7%). Conversely, FUO (27,8 vs 11,1%), infectious mononucleosis (2,6% vs 0,4%), UTI (7,4% vs 2,9%) and appendicitis (6,8% vs 1,1%) significantly increased. Median time from the onset of fever and arrival in ED was significantly lower in 2020 group.Conclusion: Our results demonstrated a reduction in community-acquired respiratory infections during the lockdown for COVID-19. The increase in rate of FUO and febrile conditions, together with the short time from fever onset and ED visit could be related to the fear for a SARS-CoV-2 infection. Background. Radioimmunoguided surgery (RIGS) by means of radiolabeled monoclonal antibodies and a probe has been reported to be useful in recognizing subclinical tumor deposits during operation. Aim of this study was to understand the limits of this technique and to access the potential diagnostic use of RIGS in colorectal cancer surgery. Methods. Monoclonal antibody B72.3 reacting with TAG 72 antigen, labeled with iodine, 125, was injected in 32 patients with primary tumors and in 22 patients with recurrent colorectal cancer. One hundred thirty-three suspected tumor sites were evaluated during operation by means of probe and resected with immunohistochemistry as control. Results. Over the last decade, lymphatic mapping and sentinel lymph node (sN) biopsy have greatly increased the possibility of identifying nodal metastasis in clinically node-negative patients with melanoma and breast cancer, thus improving the accuracy of pathologic staging. Recently, sN biopsy has been applied also in colorectal cancer. This prospective study aimed to assess its feasibility and accuracy in predicting regional lymph nodes metastases in colorectal cancer patients as well as the impact on treatment decision-making.Lymphatic mapping was accomplished by means of blue dye, which was intraoperatively injected into the subserosa overlying the tumor site in 26 patients undergoing colorectal cancer surgery. Following bowel resection, the operative specimen was inspected to identify each blue-stained node, the sN, which was sent separately to the pathologist. One half of each sN was examined by multiple 200 microm sections, while the second half was examined by standard bi-valving technique with hematoxylin-eosin (H and E) staining; all the other regional non-sentinel nodes were routinely examined by standard bi-valving technique and H and E staining.At least one sN was detected in 24 of 26 patients (92.3%); two patients with rectal cancer had no sN identified. Overall, 70 sN were retrieved into the operative specimens, with a mean of 2.9 sNs/patient, and 19 sNs were tumor-positive. An agreement between sN and regional lymph-node status was observed in 20 of 24 patients (83.4%). The sN was histologically negative in two of nine patients with positive regional nodes (sensitivity = 77.8%; false-negative rate of 22.2%); in two of seven patients with tumor-positive sN (28.6%), the sN was the exclusive site of regional nodal metastasis. The negative predictive value was 88.2% (15 of 17 patients), and the overall accuracy was 91.7% (22 of 24 patients). As regards the contribution to the detection of nodal metastasis according to the pathologic technique, standard H and E bi-valving technique detected 16 of 19 tumor-positive sNs (84.2%) while, by means of serial sectioning, metastases were detected in the remaining 3 of 19 sNs (15.8%).The sN biopsy proved feasible, with a rather short learning curve. The focused analysis of the sN by means of serial sectioning improved the detection rate of nodal metastasis compared to standard bi-valving examination, so that a more accurate nodal staging should be expected; finally, an elective localization of metastasis within the sN was observed in almost one third of regional node-positive patients.

50. Two-stage Hepatectomy in Two Regional District Community Hospitals: Perioperative Safety and Long-Term Survival

- Abstract
Introduction Surgical resection offers the best chance of cure for patients with colorectal liver metastases (CRLMs). Two-stage hepatectomy (TSH) has been demonstrated to be safe and effective to obtain curative resection in patients with multiple, bilobar CRLMs that are unresectable in a single procedure. Up to now TSH has been the prerogative of dedicated liver surgery centers. The aim of this study was to assess the safety and effectiveness of TSH also in community hospitals.Methods Of 294 patients operated on for CRLMs between September 1997 and June 2012 in 2 district community hospitals (belonging to the same regional healthcare district), 43 (14.6%) were scheduled for TSH. Thirty-eight/43 received neoadjuvant and/or bridge chemotherapy (2 neoadjuvant only, 4 neoadjuvant and bridge, 32 bridge only).Results The mean follow-up was 35.74 ± 29.53 months. Five-year overall survival (OS) was 31.4%, with a median survival time of 31 months. Twenty-nine patients completed the planned procedure (OS: 42.9%; median 47 months), while 14 did not because of disease progression (OS: 0%; median 13 months). No operative mortality occurred within the first 90 days either after the first or second stage.Conclusions Our results suggest good efficacy and safety of TSH even when performed in a community hospital setting. Shifting patient selection from neoadjuvant to bridge chemotherapy had no impact on outcome once the clearing of the liver had been achieved. In patients presenting with synchronous CRLMs, simultaneous colorectal resection and clearing of the less involved hemiliver as the first surgical step is feasible without any negative impact on outcome.
