A combination of thalidomide and pentoxyfylline therapy appears to provide marginal benefit for patients with rheumatoid arthritis. Effects on tumor necrosis factor alpha (TNFa) production were evaluated after 12 weeks of thalidomide and pentoxyfylline treatment among 12 patients with rheumatoid arthritis. TNFa is a component of the immune response. TNFa production decreased by half after treatment while production of other immune response factors including interleukin (IL) 12, IL-10, and IL-6 did not change significantly. Disease activity improved by 20% in five patients but most patients experienced one of several side effects.

Pentoxifylline may reduce production of tumor necrosis factor (TNF) in AIDS patients. TNF promotes replication of HIV and interferes with the effectiveness of zidovudine (AZT). TNF production was analyzed in 16 patients with advanced AIDS who took 400 milligrams of controlled-release pentoxifylline three times per day for eight weeks. TNF production decreased in 10 patients during treatment with pentoxifylline, was unchanged in four patients and increased in two patients. Average blood levels of triglycerides decreased after treatment with pentoxifylline. Lower blood levels of triglycerides are associated with decreased levels of TNF. No serious side effects occurred among the patients who completed treatment with pentoxifylline. Some patients had mild nausea during the first few days of treatment, and one patient had severe nausea.

Nonimmune persons infected with Plasmodium falciparum are frequent victims of fatal cases of cerebral malaria (CM). Elevated levels of a substance released by the immune system known as tumor necrosis factor (TNF) have been observed in severe cases. TNF levels declined as the patient recovered. Since TNF is a mediator in the development of CM, the use of drugs inhibiting TNF production may have therapeutic value in CM control. The ability of pentoxifylline, a drug that inhibits excess TNF synthesis, to control CM was studied in an animal model. Genetically susceptible mice were divided into treatment and control groups, and then infected with Plasmodium berghei ANKA (PB). The treatment group received pentoxifylline for 10 consecutive days; the control group received inert saline. Survival times and clinical responses were noted. Neurologic signs were indicators of CM. Death occurred within five hours after the onset of neural symptoms. Control animals developed symptoms and died within 9 to 15 days after infection. None of the treated mice died within 21 days after infection. TNF bioactivity levels were elevated in control animals, and undetectable in pentoxifylline-treated animals. The mechanism of TNF action in the development of CM remains unclear. While pentoxifylline appears to prevent CM in susceptible mice, no impact on parasitemia (presence of the parasite in the blood) was observed. All animals had developed greater than 60 percent parasitemia at death within four to five weeks after infection, without evidence of neurological involvement. Pentoxifylline seems to have potential as supportive therapy in cases of falciparum malaria to prevent death from cerebral malaria. (Consumer Summary produced by Reliance Medical Information, Inc.)

Leg ulcers commonly occur in elderly persons who have poor circulation or diabetes. They develop when the affected area does not receive enough oxygen and nutrients, and waste products are not adequately removed. The resulting infections are difficult to treat, often taking a long time to heal and sometimes never completely healing. A major complication of these ulcers is severe infection requiring amputation of part of the leg. A variety of treatments have been tried, but none are universally effective. A newer treatment involves using the drug pentoxifylline. This drug reduces the clotting abilities of the blood and makes the blood thinner. Studies have shown using this drug in conjunction with other therapies can greatly improve success in treating the ulcers. Leg ulcers usually involve the veins and are often associated with vascular disease. Recent research has indicated that white blood cells (WBCs) may play a role in decreased blood flow in leg capillaries, leading to ulcers. Ulcers involving the arteries are usually more painful and more defined. These often occur in diabetics, and result from an inadequate blood supply, usually caused by narrowed vessels. Treatment of leg ulcers requires increasing the blood supply to the tissues and keeping the tissues clean. Additional treatments may be used, depending on the particular case. The drug pentoxifylline is a useful adjunct to other treatments. In addition to its ability to thin the blood and decrease blood clotting, it also prevents white blood cells from clumping and may prevent inflammation. Its overall effect is to increase blood flow to the damaged tissue. Studies have found when it is added to treatment regimens, ulcer healing rates increase 61 to 88 percent. It is well tolerated and only produces a few side effects. Recently, in one clinic, nine patients were treated for leg ulcers that had not responded to local therapies. Pentoxifylline was given and the local therapies continued. Healing occurred in six of the patients and significant improvements were seen in the other three. The exact cause of leg ulcers is still unclear and thus a specific therapy remains elusive. Until then, local therapy with the addition of pentoxifylline therapy appears to be the best treatment.
