Although the pathogenesis of MI is still debated, cortical spreading depression and the consequent biochemical cascade and hemodynamic changes are presumed to play an important role. Here we describe a case of MI and systematically review the literature on the complex and possibly bidirectional relationship between migraine and stroke. A 44-year-old female with history of migraine with visual aura presented at the Emergency Department due to a sudden onset of left limb paresis and hypoesthesia. Brain magnetic resonance imaging revealed right fronto-parietal ischemic stroke. Two days after hospitalization, the patient experienced a prolonged visual aura and showed ultrasound evidence of intracranial artery vasospasm. To date, there have been 33 published articles on a total 119 patients with MI, although intracranial vasospasm has rarely been reported. Sustained hyperexcitability of cortical neurons, impairment of γ-aminobutyric acid inhibitory circuitry, altered serotonergic transmission, release of vasoconstrictive molecules, and cerebral blood flow changes have been proposed as pathogenic mechanisms of MI. The present case provides insight into the pathophysiological link between stroke and migraine, thus aiding clinicians in therapeutic decision-making although additional studies are needed to clarify the clinical, neuroradiological, and ultrasound findings that link MI and stroke-related migraine.Keywords: biochemical change, migrainous cerebral ischemia, pathogenesis, vasospasm

Abstract

Urban farming, also called urban agriculture (UA), represents a promising solution to achieve some of the SDGs (sustainable development goals) identified by United Nations. Indeed, UA is a multifunctional activity that presents interesting social, economic and environmental opportunities. The economic viability of UA is a hot topic, given the recent exponential growth in interest by companies and investors throughout the last ten years. The interest is also reaching public and private investors, being an opportunity for economic advancement in most countries, including EBRD (European Bank for Reconstruction and Development) countries of operation. Despite the growing interest and the manifested potential, some drawbacks are still hindering the development of UA, raising questions especially on the feasibility from a financial point of view. In particular, aspects such as the resource use efficiency, technological inputs and business models, still need to be expanded in order to correctly and efficiently apply and adapt UA to different local environmental and socio-economic conditions. The present paper aims to summarize the ongoing situation of UA, with respect to the main resources, technologies and business models to identify weaknesses and strengths for further improvement of the sector.

Abstract

Introduction: Autonomic nervous system activity in cirrhotic portal hypertension is linked to hyperdynamic circulation. Heart rate variability (HRV) is a validated noninvasive method to assess the sympathovagal balance. To investigate the correlation between HRV parameters and degree of portal hypertension, we studied a cohort of patients with cirrhosis accounting for etiology and treatments. Patients and Methods: In this cross-sectional, observational cohort study, 157 outpatients of both sex with nonalcoholic cirrhosis were assessed by upper gastrointestinal endoscopy to search for esophagogastric varices. Twenty-four-hour electrocardiogram Holter monitoring with 3 HRV parameters measurement [SD of the NN intervals, root mean square successive difference of NN intervals, and SD of the averages of NN intervals (SDANN)] according to time-domain analysis were performed in all patients. Sixteen patients with large esophagogastric varices underwent measurements of the HVPG and assessment of HRV parameters at baseline and after 45 days on carvedilol. Results: The liver dysfunction, expressed by Child-Pugh class or MELD score, was directly related to root mean square successive difference of NN intervals and inversely related to SDANN. Presence of ascites was inversely related to SDANN and to SD of the NN intervals. Treatment with carvedilol had an inverse relation with SDANN. Presence and size of esophagogastric varices had an inverse relation to SDANN and SD of the NN intervals. Upon multivariate analysis the associations between SDANN and Child-Pugh class, size of varices and ascites were confirmed. In the subgroup of 16 patients undergoing HVPG measurement, pressure gradient was unrelated to heart rate and HRV parameters. Conclusions: Time-domain HRV parameters in patients with cirrhosis, confirm the autonomic nervous system alteration, and their correlation to the degree of portal hypertension suggesting a role of the ANS in hepatic decompensation.

Abstract

Objective The full phenotypic expression of non-alcoholic fatty liver disease (NAFLD) in lean subjects is incompletely characterised. We aimed to investigate prevalence, characteristics and long-term prognosis of Caucasian lean subjects with NAFLD. Design The study cohort comprises 1339 biopsy-proven NAFLD subjects from four countries (Italy, UK, Spain and Australia), stratified into lean and non-lean (body mass index (BMI) 10 483 person-years), 4.7% of lean vs 7.7% of non-lean patients reported liver-related events (p=0.37). No difference in survival was observed compared with non-lean NAFLD (p=0.069). Conclusions Caucasian lean subjects with NAFLD may progress to advanced liver disease, develop metabolic comorbidities and experience cardiovascular disease (CVD) as well as liver-related mortality, independent of longitudinal progression to obesity and PNPLA3 genotype. These patients represent one end of a wide spectrum of phenotypic expression of NAFLD where the disease manifests at lower overall BMI thresholds. Lay summary NAFLD may affect and progress in both obese and lean individuals. Lean subjects are predominantly males, have a younger age at diagnosis and are more prevalent in some geographic areas. During the follow-up, lean subjects can develop hepatic and extrahepatic disease, including metabolic comorbidities, in the absence of weight gain. These patients represent one end of a wide spectrum of phenotypic expression of NAFLD.

Abstract

Increasing urbanization rates, loss of natural resources and the effects of climate change are challenging the sustainability of European cities. Therefore, ensuring resilient urban food systems is crucial to achieve food and nutrition security. Conventional systems are increasingly vulnerable to present and future disruptions, which urges us to change the way we produce, consume, and think about food. The development of local or regional food systems can play an important role because of the multifunctional services that are offered to the inhabitants of the city and its surroundings. The EU Project “Food systems in European Cities – FoodE” (H2020-862663) aims to encourage the development of such systems across the city-regional context by supporting their design, implementation and evaluation. Within the FoodE framework, 15 pilot case studies are being set up in 11 EU cities and serve as demonstration and first application of innovative solutions in market and social contexts, co-designed with citizens and relevant stakeholders. These local initiatives range from small-scale fisheries and low-tech market gardens to agricultural parks, semi-industrial high-tech greenhouses and vertical farms, in rural, coastal and urban agglomerations. This study reports and analyses the results of the community-based design approach of the pilot initiatives in the definition of priorities and optimal features to be implemented.

28. The impact of first and further decompensation in patients with metabolic-dysfunction associated compensated advanced chronic liver disease.

Abstract

The first and further decompensation mark the natural history and the risk of mortality in patients with cirrhosis. We assessed the cumulative incidence of first and further (acute and non-acute) decompensation and evaluated their impact on both liver-related death (LR-D) in patients with compensated advanced chronic liver disease (cACLD) due to metabolic dysfunction-associated steatotic liver disease (MASLD). Consecutive patients with clinical (LSM>10 kPa) or biopsy-proven (F3-F4 fibrosis) diagnosis of cACLD due to MASLD were included. First and further decompensation were defined according to Baveno VII criteria. The acute (AD) and non-acute (NAD)[MOU1] [VW(2] presentation of the decompensation was also evaluated. Competing risk analysis and cumulative incidence functions (CIF) [MOU3] were assessed by Fine and Gray. Cause-specific Cox models with baseline and time-dependent variables were applied. Multistate model was built to better assess the clinical course of cACLD due to MASLD. The cumulative incidence of the first decompensation was 3.5% at 5 years, increasing 20-times the risk of LR-D at cause-specific Cox analysis; the cumulative incidence of further decompensation was 44% at 5 years among patients with first decompensation, additionally increasing 1.6-times the risk of LR-D. Ascites, followed by variceal bleeding, were the most common events in both first and further decompensation. The impact of AD and NAD as both first or further event on LR-D was similar[MOU4]. Hepatocellular carcinoma (HCC) further independently increased the risk of LR-D of 3.2-times and 1.6-times, respectively, in the whole cohort of cACLD due to MASLD and in those who experienced first decompensation. The first and further decompensations (AD and NAD) represent tipping points in the clinical course of patients with cACLD due to MASLD, increasing 20-times and additionally 1.6-times the risk of LR-D. HCC is an independent predictor of LR-D in patients with cACLD due to MASLD, resulting in an additional risk of LR-D when associated with both first and further decompensation. Abstract

BACKGROUND: The main objective of this study was to evaluate physiological and quality changes of minimally processed broccoli sprouts illuminated during postharvest storage under blue, red and far-red LED lighting as compared to darkness or illumination with fluorescent light, as control treatments. RESULTS: Morphological and microbiological changes were determined during 15 days at 5 °C. In addition, total antioxidant activity and bioactive compound changes throughout the shelf life were also monitored. Results showed that far-red LED lighting increased hypocotyl and sprout length, decreased microbial growth and improved the total antioxidant and scavenging activities, compared to darkness and fluorescent lighting treatments. However, it did not stimulate the biosynthesis of phenolic acids. In contrast, blue LED light reduced by 50% the total antioxidant capacity of broccoli sprouts compared to far-red treatment, as well as morphological development. In addition, total scavenging activity was increased under far-red LED light compared with the other treatments by 12–10% (darkness and fluorescence) and 33–31% (blue and red LEDs). CONCLUSIONS: Our results suggest that minimally processed sprouts may benefit from LED lighting during shelf life in terms of quality, although further experiments should be conducted to optimize a proper exposure cycle and intensity aiming for use in the distribution chain. The results also open the way for further development towards the integration of this technology in the food distribution chain.
