8. Geometric Markov Chain Monte Carlo

Abstract

This thesis discusses Markov Chain Monte Carlo (MCMC) sampling algorithms which make use of differential geometry to improve performance. In particular, new sampling algorithms are introduced which exploit the dual geometry of statistical models developed by Amari, Chentsov and other authors in the field of Information Geometry. Previous work on differential-geometric MCMC has only used Riemannian geometry, i.e. without the dual structures studied in Information Geometry. Background chapters of the thesis discuss the development of Information Geometry by formulating statistical models as manifolds of data-generating measures. We review how Bregman divergences on such manifolds give rise to the dually-flat structure of Information Geometry, with examples provided by exponential families. Furthermore, Bayes' theorem is recast in a co-ordinate free form. Two novel sampling algorithms are introduced, called Orthogonal Gibbs and Orthogonal Gradient, which make use of dually-flat geometry. The Orthogonal Gibbs algorithm samples primal variables conditionally on dual variables and vice versa; similarly Orthogonal Gradient moves along the primal geodesic whose tangent vector is proportional to the gradient of the target log-density, and samples recursively on an m-flat submanifold orthogonal to the e-flat geodesic. We show that both algorithms move in directions in parameter space which are orthogonal with respect to the Riemannian metric. Furthermore, we show that when applied to a multivariate Gaussian target distribution, the Orthogonal Gibbs and Orthogonal Gradient algorithm respectively reduce to the Gaussian Elimination and Conjugate Gradient solvers for linear systems with added stochastic terms in the updates to the solution. The latter finding reflects work by previous authors on the connection between MCMC and optimization, including that of Fox and Parker. We show that the Orthogonal Gibbs algorithm produces independent samples for multivariate Gaussian targets, and produce numerical results suggesting the analogous result for Orthogonal Gradient. Finally, we present numerical results for a non-Gaussian target distribution for which the novel algorithms have favourable effective sample sizes compared to the No U-Turn Sampler (NUTS).

Abstract

The association among pathological response, recurrence-free survival (RFS) and overall survival (OS) with neoadjuvant therapy in melanoma remains unclear. In this study, we pooled data from six clinical trials of anti-PD-1-based immunotherapy or BRAF/MEK targeted therapy. In total, 192 patients were included; 141 received immunotherapy (104, combination of ipilimumab and nivolumab; 37, anti-PD-1 monotherapy), and 51 received targeted therapy. A pathological complete response (pCR) occurred in 40% of patients: 47% with targeted therapy and 33% with immunotherapy (43% combination and 20% monotherapy). pCR correlated with improved RFS (pCR 2-year 89% versus no pCR 50%, P < 0.001) and OS (pCR 2-year OS 95% versus no pCR 83%, P = 0.027). In patients with pCR, near pCR or partial pathological response with immunotherapy, very few relapses were seen (2-year RFS 96%), and, at this writing, no patient has died from melanoma, whereas, even with pCR from targeted therapy, the 2-year RFS was only 79%, and OS was only 91%. Pathological response should be an early surrogate endpoint for clinical trials and a new benchmark for development and approval in melanoma. Exciting advances in melanoma systemic therapies have presented the opportunity for surgical oncologists and their multidisciplinary colleagues to test the neoadjuvant systemic treatment approach in high-risk, resectable metastatic melanomas. Here we describe the state of the science of neoadjuvant systemic therapy (NAST) for melanoma, focusing on the surgical aspects and the key role of the surgical oncologist in this treatment paradigm. This paper summarizes the past decade of developments in melanoma treatment and the current evidence for NAST in stage III melanoma specifically. Issues of surgical relevance are discussed, including the risk of progression on NAST prior to surgery. Technical aspects, such as the definition of resectability for melanoma and the extent and scope of routine surgery are presented. Other important issues, such as the utility of radiographic response evaluation and method of pathologic response evaluation, are addressed. Surgical complications and perioperative management of NAST related adverse events are considered. The International Neoadjuvant Melanoma Consortium has the goal of harmonizing NAST trials in melanoma to facilitate rapid advances with new approaches, and facilitating the comparison of results across trials evaluating different treatment regimens. Our ultimate goals are to provide definitive proof of the safety and efficacy of NAST in melanoma, sufficient for NAST to become an acceptable standard of care, and to leverage this platform to allow more personalized, biomarker-driven, tailored approaches to subsequent treatment and surveillance.

36. Effects on Activity Outcomes of an e-Health Smartphone Intervention to Reduce Office Workers' Sitting Time

Abstract

Background: Occupational sedentary time is the largest contributor to total weekday sitting time among office workers. Purpose: This feasibility study evaluated the effects of an individual-level intervention to target office workers total and prolonged sedentary behaviour during working hours, using an e-health smartphone application. Methods: A three-arm (Prompt-30 or 60 min Intervention arm and a No-Prompt Comparison arm), quasi-randomised intervention was conducted over 12 weeks. Behavioural outcomes (worktime sitting, standing, stepping, prolonged sitting, and physical activity) were monitored using accelerometers and anthropometrics measured at baseline, 6 weeks and 12 weeks. Cardiometabolic measures were taken at baseline and 12 weeks. Results: Fifty-six office workers (64% female) completed baseline assessments. The Prompt-60 arm was associated with a reduction in occupational sitting time at 6 (-46.8 min/8 h workday [95% confidence interval = -86.4, -6.6], p < 0.05) and 12 weeks (-69.6 min/8 h workday [-111.0, -28.2], p < 0.05) relative to the No-Prompt Comparison arm. Sitting was primarily replaced with standing in both arms (p > 0.05). Both Intervention arms reduced time in prolonged sitting bouts at 12 weeks (Prompt-30: -27.0 [-99.0, 45.0]; Prompt-60: -25.8 [-98.4, 47.4] min/8 h workday; both p > 0.05). There were no changes in steps or cardiometabolic risk. Conclusions: Findings highlight the potential of a smartphone e-health application, suggesting 60 min prompts may present an optimal frequency to reduce total occupational sedentary behaviour. Abstract

Globally, over three million women participate in rugby union, yet injury prevention and training strategies are predominantly based on androcentric data. These strategies may have limited generalisability to females, given the cervical spine is more susceptible to whiplash and less adept at resisting inertial loading. A total of 53 university rugby union players (25 female, 28 male, 20.7 ± 1.8 years) had their isometric neck strength measured. Bespoke instrumented mouthguards were used to record the magnitude of head impact events in six female and seven male competitive matches. Mean female maximal isometric neck strength was 47% lower than male. Independent samples Mann–Whitney U tests showed no significant differences for peak linear head acceleration (female: median 11.7 g, IQR 7.9 g; male: median 12.5 g, IQR 7.0 g p=.23) or peak rotational head acceleration (female: median 800.2 rad·s−2, IQR 677.7 rad·s−2; male: median 849.4 rad·s−2, IQR 479.8 rad·s−2; p=.76), despite the mean male body mass being 24% greater than female. Coded video analysis revealed substantial differences in head-impact mechanisms; uncontrolled whiplash dominated >50% of all recorded female impact events and <0.5% in males. Direct head-to-ground impacts comprised 26.1% of female and 9.7% of male impacts, with whiplash occurring in 78.0% and 0.5%, respectively. Overall, the data provided in this study do not support the generalisation of male-derived training and injury-prevention data to female rugby athletes. These results suggest a considerable research effort is required to identify specific weakness of female rugby players and derive appropriate training, injury prevention and return to play protocols.
