5. A patient activation intervention in primary care for patients with chronic pain on long term opioid therapy: results from a randomized control trial

Abstract

Abstract Background Given significant risks associated with long-term prescription opioid use, there is a need for non-pharmacological interventions for treating chronic pain. Activating patients to manage chronic pain has the potential to improve health outcomes. The ACTIVATE study was designed to evaluate the effectiveness of a 4-session patient activation intervention in primary care for patients on long-term opioid therapy. Methods The two-arm, pragmatic, randomized trial was conducted in two primary care clinics in an integrated health system from June 2015—August 2018. Consenting participants were randomized to the intervention (n = 189) or usual care (n = 187). Participants completed online and interviewer-administered surveys at baseline, 6- and 12- months follow-up. Prescription opioid use was extracted from the EHR. The primary outcome was patient activation assessed by the Patient Activation Measure (PAM). Secondary outcomes included mood, function, overall health, non-pharmacologic pain management strategies, and patient portal use. We conducted a repeated measure analysis and reported between-group differences at 12 months. Results At 12 months, the intervention and usual care arms had similar PAM scores. However, compared to usual care at 12 months, the intervention arm demonstrated: less moderate/severe depression (odds ratio [OR] = 0.40, 95%CI 0.18–0.87); higher overall health (OR = 3.14, 95%CI 1.64–6.01); greater use of the patient portal’s health/wellness resources (OR = 2.50, 95%CI 1.42–4.40) and lab/immunization history (OR = 2.70, 95%CI 1.29–5.65); and greater use of meditation (OR = 2.72; 95%CI 1.61–4.58) and exercise/physical therapy (OR = 2.24, 95%CI 1.29–3.88). At 12 months, the intervention arm had a higher physical health measure (mean difference 1.63; 95%CI: 0.27–2.98). Conclusion This trial evaluated the effectiveness of a primary care intervention in improving patient activation and patient-reported outcomes among adults with chronic pain on long-term opioid therapy. Despite a lack of improvement in patient activation, a brief intervention in primary care can improve outcomes such as depression, overall health, non-pharmacologic pain management, and engagement with the health system. 6. Context matters in history textbook studies: A call to address the socio-political landscape of textbook production

Abstract

History textbooks are a tool of nation-building and often the only account of particular events, people and issues to which students will be exposed. This is one reason why it is important to examine, not only their content, but the context of their production. Research attention needs to be directed not only at disentangling the logics of textbook content, a purpose that dominates the field, but also at the social and political contexts of their development, including their production (publishing and authorship) and the processes by which they receive official approval. This work analyzed 100 history textbook studies in order to identify current trends in textbook research. This article focuses on one of the four major findings of the study: textbook studies often focus on the content of the textbook in isolation from the socio-political landscape of textbook production. The socio-political landscape refers, but is not limited to, the influence of local and geopolitics; the influence of the state, evident primarily in the official approval processes employed by governments; the economic dimension and publication parameters; and authorship factors. Among the relatively few studies that address the socio-political landscape, the authors often employ a historiographical or comparative perspective. The historiographical perspective, which is dominant, offers an analysis of textbooks over time in order to reveal both continuities and changes in the historical narratives produced by their authors. One example is a study by Yeow Tong Chia (2013), who examines conceptions of ‘Chineseness’ and China in Ontario high school history curricula and textbooks in the post Second World War to the 1980s period. More broadly, the paper locates the Ontario textbooks in the international context of the western-centric perspectives that were prevalent following the Second World War. The comparative perspective analyzes textbooks across regions in one historical moment. For example, Jason Nicholls (2006) addresses the role of national contexts in determining how the Second World War is portrayed in textbooks found in the United States, Italy, Sweden, Japan, and England.

Abstract

The spatially resolved refractometer is an aberrometer used to measure the wave-front aberrations of the human eye. In its original form and the new configuration that we report, it uses the patient's perception in a psychophysical task to evaluate the wave-front errors at a variable number of loci (typically 40 or 160) across the cornea. This configuration includes pupil tracking and the ability to choose the measurement loci in software. An automated configuration that does not require patient input is also described. Abstract

The coherent anti-Stokes Raman scattering (CARS) was used to measure the temperature and concentration distributions of hydrogen in a hot-filament chemical vapor deposition reactor of diamond. Results show that the CARS spectra provided direct and accurate measurements of hydrogen gas temperature and concentration distribution. The concentration distribution of atomic hydrogen was also measured using a constant pressure condition and equilibrium between translational and rotational degrees in the system.

Abstract

High-resolution elevation models, palaeoseismic trenching, and Quaternary dating demonstrate that the Kenchreai Fault in the eastern Gulf of Corinth (Greece) has ruptured in the Holocene. Along with the adjacent Pisia and Heraion Faults (which ruptured in 1981), our results indicate the presence of closely-spaced and parallel normal faults that are simultaneously active, but at different rates. Such a configuration allows us to address one of the major questions in understanding the earthquake cycle, specifically what controls the distribution of interseismic strain accumulation? Our results imply that the interseismic loading and subsequent earthquakes on these faults are governed by weak shear zones in the underlying ductile crust. In addition, the identification of significant earthquake slip on a fault that does not dominate the late Quaternary geomorphology or vertical coastal motions in the region provides an important lesson in earthquake hazard assessment.

Abstract

The source parameters and slip distribution of the 2013 May 11 Mw 6.1 Minab earthquake are studied using seismology, geodesy and field observations. We observe left-lateral strike-slip motion on a fault striking ENE–WSW; approximately perpendicular to previously studied faults in the Minab–Zendan–Palami fault zone. The fault that ruptured in 2013 is one of a series of ∼E–W striking left-lateral faults visible in the geology and geomorphology. These accommodate a velocity field equivalent to right-lateral shear on ∼N–S striking planes by clockwise rotations about vertical axes. The presence of these faults can reconcile differences in estimates of fault slip rates in the western Makran from GPS and Quaternary dating. The longitudinal range of shear in the western Makran is likely to be controlled by the distance over which the underthrusting Arabian lithosphere deepens in the transition from continent–continent collision in the Zagros to oceanic subduction in the Makran.

Performance testing of the model-scale CPA was recently completed at the University of Texas Center for Electromechanics. A major part of the project was the development of design and simulation codes that would accurately represent the performance of pulsed alternators. This paper discusses the components of the system and its operational sequence. Details of the performance simulation model are presented along with test data. The test result is compared to the predicted data.

Abstract

Objective To compare mortality patterns for urban Aboriginal adults with those of urban non-Aboriginal adults. Methods Using the 1991–2001 Canadian census mortality follow-up study, our study tracked mortality to December 31, 2001, among a 15% sample of adults, including 16 300 Aboriginal and 2 062 700 non-Aboriginal persons residing in urban areas on June 4, 1991. The Aboriginal population was defined by ethnic origin (ancestry), Registered Indian status and/or membership in an Indian band or First Nation, since the 1991 census did not collect information on Aboriginal identity. Results Compared to urban non-Aboriginal men and women, remaining life expectancy at age 25 years was 4.7 years and 6.5 years shorter for urban Aboriginal men and women, respectively. Mortality rate ratios for urban Aboriginal men and women were particularly elevated for alcohol-related deaths, motor vehicle accidents and infectious diseases, including HIV/AIDS. For most causes of death, urban Aboriginal adults had higher mortality rates compared to other urban residents. Socio-economic status played an important role in explaining these disparities. Conclusion Results from this study help fill a data gap on mortality information of urban Aboriginal people of Canada. Abstract

Aim To conduct a bibliographic analysis of the indexed literature relating to scope of practice in nursing so as to identify underlying patterns in journal publication, volume of scholarly work over time, countries of origin, central contributors, academic affiliation and the major dimension of the studies conducted. Methods A systematic search of the Scopus database provided data that was then extracted and utilized to undertake a bibliometric analysis of published work relating to scope of practice. In addition to identification of aggregated metrics relating to the most frequently occurring journals and most cited authors, a co-word analysis was conducted. Results A total of 2730 articles with the term Scope of Practice in the Title, Abstract or Keywords were identified. Co-word analysis revealed five major themes - Changing Regulatory Environment; Health Care Drivers; Competence & Role Implementation; Policy Context; and Role Evolution & Role Differentiation. Conclusions and policy implications From a policy perspective, we conclude that bibliographic analysis of the indexed literature is a useful technique that can augment our understanding of key regulatory issues such as scope of practice. However, the overemphasis on advanced practice in the scope of practice literature coupled with the increased interest in task shifting to support-personnel as governments pursue the goal of universal health coverage may leave nursing inadequately prepared to inform any evidence-based policy change. Abstract

Obesity is implicated in rheumatoid arthritis (RA) development, severity, outcomes, and treatment response. We estimated the independent effects of overweight and obesity on ability to achieve sustained remission (sREM) in the 3 years following RA diagnosis. Data were from the Canadian Early Arthritis Cohort, a multicenter observational trial of early RA patients treated by rheumatologists using guideline‐based care. sREM was defined as Disease Activity Score in 28 joints (DAS28) <2.6 for 2 consecutive visits. Patients were stratified by body mass index (BMI) as healthy (18.5–24.9 kg/m2), overweight (25–29.9 kg/m2), and obese (≥30 kg/m2). Cox regression was used to estimate the effect of the BMI category on the probability of achieving sREM over the first 3 years, controlling for age, sex, race, education, RA duration, smoking status, comorbidities, baseline DAS28, Health Assessment Questionnaire disability index, C‐reactive protein level, and initial treatment. Of 982 patients, 315 (32%) had a healthy BMI, 343 (35%) were overweight, and 324 (33%) were obese; 355 (36%) achieved sREM within 3 years. Initial treatment did not differ by BMI category. Compared to healthy BMI, overweight patients (hazard ratio [HR] 0.75 [95% confidence interval (95% CI) 0.58–0.98]) and obese patients (HR 0.53 [95% CI 0.39–0.71]) were significantly less likely to achieve sREM. Rates of overweight and obesity were high (69%) in this early RA cohort. Overweight patients were 25% less likely, and obese patients were 47% less likely, to achieve sREM in the first 3 years, despite similar initial disease‐modifying antirheumatic drug treatment and subsequent biologic use. This is the largest study demonstrating the negative impact of excess weight on RA disease activity and supports a call to action to better identify and address this risk in RA patients.

Abstract

The feasibility of breeding spring calving, single-suckled beef cows without the use of natural service was explored over two breeding seasons using repeated oestrus synchrony and fixed-time AI. Initially, cows were oestrus synchronised using subcutaneous norgestomet implants inserted for ten days with prostaglandin injection prior to implant removal. Cows were inseminated once, 56 hours after implant removal and 12 days later all cows were re-treated with norgestomet implants to allow a second synchronised service. Twenty-one days after the first synchronised AI, milk samples were drawn for progesterone assay and the norgestomet implants removed. Cows received a second service 56 hours later if the 21 day milk progesterone assay suggested they were not pregnant. All cows receiving a second service were re-treated with norgestomet implants as before to allow a third synchronised service as necessary. Pregnancy was later confirmed by manual rectal palpation. In year one, 48 cows entered the program and the pregnancy rates to the first, second and third synchronised services were 56%, 69% and 40% respectively with 17% of cows barren at the end of the breeding period. In year two, 69 cows entered the program and the pregnancy rates were 58%, 48% and 33% to each service with 20% of cows barren at the end of the breeding period. The accuracy of milk progesterone assay for pregnancy diagnosis was 84% and 87% in years 1 and 2 respectively. The study showed it is possible to achieve a calving rate of at least 80% following a 46 day breeding period using a combination of oestrus synchrony and fixed-time AI in beef cows at pasture., The Bovine Practitioner, No. 31.2 (1997 May)
