Vertaisarvioitu Lähtökohdat Tutkimuksessa verrattiin Suomen CP-vammaisten lasten ja nuorten lonkkien seuranta¬käytäntöjä Ruotsissa kehitettyyn CPUP-ohjelmaan. Ohjelman tarkoitus on ehkäistä lonkkien sijoiltaanmenoa varhaisella -konservatiivisella tai operatiivisella hoidolla. Menetelmät Valtakunnallisessa CP-hankkeessa kerättiin tiedot 362:sta alle 20-vuotiaasta potilaasta. ¬Tarkastelimme ¬erityisesti ensimmäistä lonkkaröntgenkuvaa ja lausuntoa, sekä selvitimme tehdyt kirurgiset ¬toimen¬piteet. ¬Selvitimme seurantakäytäntöjä myös kyselyllä lastenneurologian ylilääkäreille. Tulokset Lonkkaröntgenkuva oli otettu ainakin kerran 69 %:lta lapsista (n = 251), ensimmäisen kerran 0,2–15,2-¬vuotiaana. ¬CPUP-suosituksen mukaan lievimmissä liikuntavammoissa (GMFCS-luokka I) kuvausta ei tarvita rutiinin¬omaisesti. Kuitenkin 43 % tämän ryhmän lapsista oli kuvattu. Liikkumisen apuvälineitä käyttäville lapsille (GMFCS-luokat III–V) kuvausta suositellaan heti diagnoosin varmistuttua. Heistä 33 % oli kuvattu vasta yli 3-vuotiaana ja 6 oli jäänyt kuvaamatta. Päätelmät Lonkkien seuranta Suomessa poikkesi CPUP-ohjeistuksesta. Kansainvälisiin suosituksiin perustuva radiologinen seuranta mahdollistaa luksaatiokehityksen riittävän varhaisen toteamisen ja hoidon.

Background Tibial fracture is the third most common long-bone fracture in children. Traditionally, most tibial fractures in children have been treated non-operatively, but there are no long-term results. Methods 94 children (64 boys) were treated for a tibial fracture in Aurora City Hospital during the period 1980–89 but 20 could not be included in the study. 58 of the remaining 74 patients returned a written questionnaire and 45 attended a follow-up examination at mean 27 (23–32) years after the fracture. Results 89 children had been treated by manipulation under anesthesia and cast-immobilization, 4 by skeletal traction, and 1 with pin fixation. 41 fractures had been re-manipulated. The mean length of hospital stay was 5 (1–26) days. Primary complications were recorded in 5 children. The childrens’ memories of treatment were positive in two-thirds of cases. The mean subjective VAS score (range 0–10) for function appearance was 9. Leg-length discrepancy (5–10 mm) was found clinically in 10 of 45 subjects and rotational deformities exceeding 20° in 4. None of the subjects walked with a limp. None had axial malalignment exceeding 10°. Osteoarthritis of the hip and/or knee was seen in radiographs from 2 subjects. Interpretation The long-term outcome of tibial fractures in children treated non-operatively is generally good.

Background There is little information about the range of motion (ROM) and strength of the affected upper limbs of patients with permanent brachial plexus birth palsy. Patients and methods 107 patients who had brachial plexus surgery in Finland between 1971 and 1998 were investigated in this population-based, cross-sectional, 12-year follow-up study. During the follow-up, 59 patients underwent secondary procedures. ROM and isometric strength of the shoulders, elbows, wrists, and thumbs were measured. Ratios for ROM and strength between the affected and unaffected sides were calculated. Results 61 patients (57%) had no active shoulder external rotation (median 0° (-75–90)). Median active abduction was 90° (1–170). Shoulder external rotation strength of the affected side was diminished (median ratio 28% (0–83)). Active elbow extension deficiency was recorded in 82 patients (median 25° (5–80)). Elbow flexion strength of the affected side was uniformly impaired (median ratio 43% (0–79)). Median active extension of the wrist was 55° (-70–90). The median ratio of grip strength for the affected side vs. the unaffected side was 68% (0–121). Patients with total injury had poorer ROM and strength than those with C5–6 injury. Incongruity of the radiohumeral joint and avulsion were associated with poor strength values. Interpretation ROM and strength of affected upper limbs of patients with surgically treated brachial plexus birth palsy were reduced. Patients with avulsion injuries and/or consequent joint deformities fared worst.

Background and purpose The current treatment for femoral fractures in children is mostly operative, which contrasts with treatment of other long bone fractures in children. We analyzed treatment injuries in such patients in Finland in order to identify avoidable injuries. Our other aims were to calculate the incidence of these fractures and to describe the treatment method used. Methods The Patient Insurance Centre (PIC) provides financial compensation of patients who have sustained an injury in connection with medical care. We retrospectively analyzed incidence, treatment methods, and all compensation claims concerning treatment of femoral fractures in children who were 0–16 years of age during the 8-year period 1997–2004. Results The incidence of childhood femoral fractures in Finland was 0.27 per 1,000 children aged < 17 years, and two-thirds of the patients were treated operatively during the study period. 30 compensation claims were submitted to PIC during the 8-year study period. The compensation claims mainly concerned pain, insufficient diagnosis or treatment, extra expenses, permanent disability, or inappropriate behavior of medical personnel. Of the claims, 16 of 30 were granted compensation. Compensation was granted for delay in treatment, unnecessary surgery, and for inappropriate surgical technique. The mean amount of compensation was 2,300 euros. Of the injuries that led to compensation, 11 of 16 were regarded as being avoidable in retrospect. Interpretation The calculated risk of a treatment injury in childhood femoral fracture treatment in Finland is approximately 2%, and most of these injuries can be avoided with proper treatment.

The production of β-amylase is of great importance in two-rowed spring barley cv. Kymppi (Hordeum vulgare L.) in the Finland, where long-day conditions favour high enzyme activities. Nitrogen (N) fertilization of a crop is the main means of manipulating barley β-amylase activity for industrial purposes. In this study, leaf chlorophyll content determined with a portable chlorophyll meter (Minolta SPAD-502) in the field, was used to predict N availability of cv. Kymppi for β-amylase production. Critical chlorophyll meter readings (SPAD values) were calculated from data deriving from experiments with various N fertilizer levels using the Cate-Nelson procedure. According to the results of this study it can be stated that the critical SPAD values at pollination (i.e. pollen grains on well-developed stigmatic hairs, GS 52-58) are 37 SPAD units for grain yield and 41 SPAD units for β-amylase activity. The optimum grain yield occurred at 41 SPAD units and optimum P-amylase activity was reached at 45 SPAD units. Determination of leaf chlorophyll content using the chlorophyll meter led to more appropriate fertilizer application recommendations and subsequently increased β-amylase activity in grains. Grain protein concentration could be an effective diagnostic tool for post-harvest evaluation of grain β-amylase activity in cv. Kymppi., Tutkimuksessa määritettiin Kymppi-ohran lehtivihreäpitoisuuksien raja-arvoja parhaan mahdollisen sadonmuodostuksen ja β-amylaasi-aktiivisuuden kannalta. Lehtivihreämittaukset tehtiin kasvukaudella 1993 ja 1994 pelto-oloihin tarkoitetulla Minoltan SPAD-502 lehtivihreämittarilla. Mittauksissa käytettiin ylimpiä, täysin kehittyneitä lehtiä. Kenttäkokeina olivat typpitasokokeet (0 - 200 kg N ha-1). Lehtivihreämittaukset selittivät β-amylaasi-aktiivisuutta parhaiten tähkälletulovaiheessa (Zadoksin kasvuasteessa 52-58). Taulukossa 4 on yhteenveto Kymppi-ohran lehtivihreän raja-arvoista tässä kasvuasteessa. Lehtivihreän raja-arvo 37 SPAD rajoitti selvästi Kympin sadonmuodostusta. Sadon menetys oli tällöin 1868 kg ha-1. Jos lehtivihreämittaus oli alempi kuin 41 SPAD, laski sadon valkuaispitoisuus 4,7 prosenttiyksikköä ja β-amylaasi-aktiivisuus 143 W.K. Lehtivihreän raja-arvot optimisadolle olivat 37-41 SPAD-yksikköä, ja parhaalle mahdolliselle valkuaispitoisuudelle ja β-amylaasi-aktiivisuudelle 41- 45 SPAD-yksikköä. Tutkimus osoitti ohran jyvien β-amylaasin määrän nousevan voimakkaasti vasta sato-optimin jälkeen. Sato-optimin jälkeen jokaista kymmentä β-amylaasi-aktiivisuuden yksikköä kohden tarvitaan lisä-N noin 11 kg ha-1. Paras mahdollinen β-amylaasiaktiivisuus oli keskimäärin 252 W.K. ja vastaava valkuaispitoisuus 14,2 %. Sadonkorjuun jälkeen voidaan ohran jyvien valkuaispitoisuuden avulla arvioida epäsuorasti jyvien β-amylaasi-aktiivisuutta.

The author proposes that future research work on crop production should concentrate on alleviating the problems associated with overwintering and frost, early summer drought and rainy autumn in order to enhance the competitiveness of field crop production in Finland. Further, more detailed knowledge on the crops is required to understand the genotypic differences in potential yield formation in order to optimize management practices. Increasing the nitrogen (N) fixation efficiency of leguminous species should be considered as an important target. Naked oats, sunflower, and leguminous plants might be advantageous species for future crop production in Finland. The bioenergetic implications of increasing crop productivity are discussed., Kasvinviljelytutkimuksella on merkittävä osuus Suomen maatalouden kilpailuaseman kehittämisessä. Meidän kasvinviljelymme keskeisiä tutkimusongelmia ovat talvenkestävyys, alkukesän poutaisuuteen ja korjuukauden sateisiin oloihin sopeutuvien lajikkeiden ja viljelymenetelmien kehittäminen. Viljelytoimenpiteiden tehostaminen niin taloudellis- teknologisen kuin biologisen tuotannonkannalta on kuitenkin tehokkaimmillaan vasta, kun tunnemme viljelykasvien lajikekohtaisen sadonmuodostuspotentiaalin. Tämä tieto antaa selkeän käsityksen siitä, missä kasvien kehitysvaiheessa eri viljelytoimenpiteet ovat välttämättömiä. Kuorettoman kauran, auringonkukan ja palkokasvien tutkimiseen tulisi kiinnittää entistä enemmän huomiota. Tutkimuksessa on pohdittu erikasvilajien sadontuottokykyä ja typpitaloutta yhteyttämisessä tuotetun energian hyväksikäyttötehon perusteella.

Four units of an automatic small-scale 'home bakery' (Panasonic SD-BT2P, Japan) were tested for their suitability for rapid and simplified test baking. The results indicated that the four baking machines used produced loaves equal in volume. Loaf volume increased with increasing values of protein content, wet gluten content, sedimentation value, and with farinograph dough development time and stability values. Varietal differences in the relationship between quality and loaf volume were detected., Tutkimuksessa selvitettiin neljällä Panasonic (SDBT2P) kotileipurilla laitteen soveltuvuutta koeleivontaan. Koelajikkeina olivat kevätvehnät Reno, Luja, Ruso, Kadett, Drabant ja Tapio, joiden valkuaispitoisuus vaihteli 11—15 %. Tulokset osoittivat, että kotileipurilla saatiin toistettava leivontatulos. Leipätilavuus lisääntyi jauhojen valkuaispitoisuuden, kostean sitkon määrän, zeleny sedimentaatioluvun kasvaessa sekä farinogrammin tunnuslukujen parantuessa. Vehnälajikkeiden leivontatuloksissa oli eroja. Tutkimus kuitenkin osoitti, että lisätietoa tarvitaan leivontalaitteen kyvystä havaita pienehköjä eroja samantyyppisten lajikkeiden välillä.

Interrelationships between climatic factors and spring wheat yield and quality were examined with 21 years field experiments. The formation of gluten was less at dry conditions (total precipitation under 50 mm) and total precipitation exceeded 130—140 mm. The optimum daily temperature for gluten production was some 15—17°C during grain filling. The gluten content decreased if daily minimum and maximum temperatures exceeded 11—12°C and 21—22°C, respectively. The effect of temperature and rainfall were not, however, significant in early maturing varieties. The climatic factors and grain yield did not correlate. Grain yield and protein yield had strong positive relationship, which was perhaps a consequence of supply and utilization of nitrogen. It is concluded that climatic factors affecting yield to quality ration in wheat may be excessive rains before heading and high temperature during grain filling. Interaction between weather and nitrogen are discussed to optimize correct timing of nitrogen fertilization for amount and quality of economic wheat yield., Tutkimuksessa tarkastellaan, 21 vuoden mittaisen jakson perusteella, säätekijöiden vaikutusta kevätvehnän jyväsadon ja valkuaisen määrän muodostumiseen. Tutkimusaineistona olivat kevätvehnät: Ulla, Heta, Ruso, Kadett, Drabant sekä Tähti peruslannoitetuissa kenttäkokeissa. Yksittäiset säätekijät eivät selittäneet jyväsadon muodostusta. Kostean sitkon muodostusta kuitenkin rajoitti alkukesän kuivuus (sade alle 50 mm) tai liiallinen yli 130—140 mm sade, jonka seurauksena vehnän typenotto heikkeni. Yli 15—17°C keskilämpötila jyvän täyttymisjaksolla aiheutti vehnäsadon alhaisen sitkopitoisuuden. Säätekijät eivät kuitenkaan vaikuttaneet voimakkaasti aikaisin tuleentuviin lajikkeisiin. Kevätvehnän jyväsadon sekä valkuaissadon välillä oli voimakas positiivinen korrelaatio, mikä osoitti ilmeistä lisätyppilannoituksen tarvetta sellaisina kasvukausina, jolloin säätekijät vaikeuttavat peruslannoituksen hyväksikäyttöä.
