1. A personalized model and optimization strategy for estimating blood glucose concentrations from sweat measurements

Abstract

Background and objective: Diabetes is one of the four leading causes of death worldwide, necessitating daily blood glucose monitoring. While sweat offers a promising non-invasive alternative for glucose monitoring, its application remains limited due to the low to moderate correlation between sweat and blood glucose concentrations, which has been obtained until now by assuming a linear relationship. This study proposes a novel model-based strategy to estimate blood glucose concentrations from sweat samples, setting the stage for non-invasive glucose monitoring through sweat-sensing technology. Methods: We first developed a pharmacokinetic glucose transport model that describes the glucose transport from blood to sweat. Secondly, we designed a novel optimization strategy leveraging the proposed model to solve the inverse problem and infer blood glucose levels from measured glucose concentrations in sweat. To this end, the pharmacokinetic model parameters with the highest sensitivity were also optimized so as to achieve a personalized estimation. Our strategy was tested on a dataset composed of 108 samples from healthy volunteers and diabetic patients. Results: Our glucose transport model improves over the state-of-the-art in estimating sweat glucose concentrations from blood levels (higher accuracy, p<0.001). Additionally, our optimization strategy effectively solved the inverse problem, yielding a Pearson correlation coefficient of 0.98 across all 108 data points, with an average root-mean-square-percent-error of 12%+/-8%. This significantly outperforms the best sweat-blood glucose correlation reported in the existing literature (0.75). Conclusion: Our innovative optimization strategy, also leveraging more accurate modeling, shows promising results, paving the way for non-invasive blood glucose monitoring and, possibly, improved diabetes management.

2. Discretised microfluidics for noninvasive health monitoring using sweat sensing.

Abstract

Using sweat instead of blood for monitoring chemical biomarker concentrations of hospitalised patients offers several advantages for both the patients and healthcare workers. Unlike blood, sweat can be noninvasively and continuously sampled without direct involvement of a professional, and sweat contains a rich composition of biomarkers. However, patients in resting state have extremely low sweat rates and they produce correspondingly small sweat volumes, which makes sweat sensing of hospitalised patients highly challenging. We propose a unique solution that enables the use of sweat as a viable biofluid for noninvasive health monitoring, by actively transporting the sweat in a discretised manner. Our device uses electrowetting-on-dielectrics (EWOD) to create and move sweat droplets with a volume of around 1 nanolitre from a sweat gland to sensors integrated in the device. We present the first wearable device with integrated EWOD, and we show that it can collect and transport sweat on-body, while measuring sweat rate, under conditions typical for individuals at rest. Abstract

Sweat is a biomarker-rich fluid with potential for continuous patient monitoring via wearable devices. However, biomarker concentrations vary with the sweat rate per gland, posing a challenge for sweat sensing. To address this, we propose an algorithm to compute both the number of active sweat glands and their individual sweat rates. We developed models of sweat glands and a discrete sweat-sensing device to sense sweat volume. Our algorithm estimates the number of active glands by decomposing the signal into patterns generated by the individual sweat glands, allowing for the calculation of individual sweat rates. We assessed the algorithm's accuracy using synthetic datasets for varying physiological parameters (sweat rate and number of active sweat glands) and device layouts. The results show that device layout significantly affects accuracy, with error rates below 0.2% for low and medium sweat rates (below 0.2 nL min−1 per gland). However, the method is not suitable for high sweat rates. The suitable sweat rate range can be adapted to different needs through the choice of device. Based on our findings, we provide recommendations for optimal device layouts to improve accuracy in estimating active sweat glands. This is the first study to focus on estimating the sweat rate per gland, which essential for accurate biomarker concentration estimation and advancing sweat sensing towards clinical applications. Abstract

Research targeting the development of on-body sensors has been significantly growing in recent years - an example is on-skin sweat sensing. However, the wide inter and intra person variability of skin characteristics make in vivo testing of these sensors and included materials such as skin adhesives difficult, which hampers especially the initial development phase of such wearables. Besides the development of wearable sweat sensors, companies developing deodorants, cosmetics, medical adhesives and wearable textiles now need to perform expensive human subjects testing with little control over the exact sweat mechanisms. Hence, there is a need for a realistic, adaptable and stable test platform, or artificial skin. We present a versatile artificial skin platform that faithfully recapitulates skin topography, active sweat pores, skin wetting behaviour and sweat rate, and that can be tuned to mimic the specifications of the targeted body location and sweating characteristics. The developed artificial skin is capable of generating sweat rates as low as 0.1 nL min−1 pore−1 and as high as 100 nL min−1 pore−1, spanning the whole range of physiological sweat rates. Specifically, the platform can be used for the development of sweat sensors for sedentary persons whose sweat rates are commonly lower than currently delivered by any other artificial skin platform.

Abstract

Sweat is a biomarker-rich fluid that contains information that can provide insights into the metabolic activity of the patient. To interpret the information from certain biomarkers the sweat rate of each gland needs to be known. An agent-based model was created to explore the potential improvement of current methods to obtain the sweat rate per gland. In a digital sweat sensing device, the continuous sweat flow is discretized into individual droplets. These droplets are detected by a sensor to create a signal. The model simulates these signals for different device designs and parameters. The model shows how different design choices can influence the signal. The results demonstrate the utility of the model, for both the development of a signal analysis method and the subsequent optimization of the device design.

Typical droplet volumes range from several microliters to milliliters, and when downscaling is desired the system is usually filled with oil. Here, we demonstrate actuation of sub-nanoliter droplets in air.
