Background: Recurrent chronic idiopathic urticaria (RCIU) is a common skin condition that affects 0.1–3% of the population in the USA and Europe and accounts for nearly 75% of all ‘ordinary’ chronic urticaria (CU) cases. Methods: We studied 838 consecutive patients with RCIU referred to hospital between 1998 and 2003. Patients with known causes of CU were excluded. Clinical history, physical examination, and symptom diaries were evaluated during two periods, a diet-free period (1 week) and a food-additive-free diet (FAFD) period (4 weeks), respectively, and two double-blind placebo-controlled (DBPC) challenges of six food additives were administered. The first DBPC challenge included a mixture of the six food additives (DBPCmixed) given to all patients. The second DBPC challenge comprised the single food additives, administered at increasing doses (DBPCsingle) to patients with a positive DBPCmixed test and 105 patients with a negative DBPCmixed test, as a control. Results: The DBPCmixed challenge was positive in 116 patients. None of the 105 control patients had a positive DBPCsingle test. Only 31 DBPCsingle tests were positive in patients with positive DBPCmixed challenge. Twenty-four of the 116 patients showing a positive DBPCmixed challenge also had a positive DBPCsingle result. Conclusions: Our results confirmed that food additive hypersensitivity reactions occurred in few RCIU patients using DBPCsingle challenge. The combination of the results of FAFD and DBPCmixed challenge seems to be of considerable practical interest for allergists, internists and dermatologists, rather than the data of clinical history and the results of DBPCsingle challenge, in patients with RCIU.

Refractory temporal lobe epilepsy (TLE) is associated with a dysfunction of inhibitory signaling mediated by GABA A receptors. In particular, the use-dependent decrease (run-down) of the currents (I GABA ) evoked by the repetitive activation of GABA A receptors is markedly enhanced in hippocampal and cortical neurons of TLE patients. Understanding the role of I GABA run-down in the disease, and its mechanisms, may allow development of medical alternatives to surgical resection, but such mechanistic insights are difficult to pursue in surgical human tissue. Therefore, we have used an animal model (pilocarpine-treated rats) to identify when and where the increase in I GABA run-down occurs in the natural history of epilepsy. We found: ( i ) that the increased run-down occurs in the hippocampus at the time of the first spontaneous seizure (i.e., when the diagnosis of epilepsy is made), and then extends to the neocortex and remains constant in the course of the disease; ( ii ) that the phenomenon is strictly correlated with the occurrence of spontaneous seizures, because it is not observed in animals that do not become epileptic. Furthermore, initial exploration of the molecular mechanism disclosed a relative increase in α4-, relative to α1-containing GABA A receptors, occurring at the same time when the increased run-down appears, suggesting that alterations in the molecular composition of the GABA receptors may be responsible for the occurrence of the increased run-down. These observations disclose research opportunities in the field of epileptogenesis that may lead to a better understanding of the mechanism whereby a previously normal tissue becomes epileptic.

BACKGROUND: During inflammation, activated vascular endothelial cells and other cell types express various adhesion molecules, which facilitate the binding of circulating leukocytes and their extravasation in surrounding tissue (i.e. renal tissue). The serum concentration of circulating soluble adhesion molecules is supposed to reflect the degree of this activation. OBJECTIVE: In the first part of the study, we determined if the serum levels of the soluble intercellular adhesion molecule (sICAM)-1 and the soluble endothelial cell-leukocyte adhesion molecule (sELAM)-1, in patients affected by microscopic polyangiitis (MPA), associated with myeloperoxidase (MPO)-anti-neutrophil cytoplasmic antibodies (ANCA), were related to the active and the inactive vasculitis phase. In the second part of the study, we examined the changes in circulating sICAM-1 and sELAM-1 levels and the clinical outcome of renal function in these patients. METHODS: We examined 20 MPO-ANCA-positive MPA patients in an acute phase and in a remission phase, after 6 months of treatment, and 50 subjects as controls, 30 with autosomal dominant polycystic kidney disease (ADPKD) in stable chronic renal failure (CRF) and 20 healthy volunteers (HS) with normal renal function. RESULTS: Regarding serum creatinine (Cr) concentration, no significant differences were found comparing active and inactive phases in the MPA group and the CRF group. Mean serum adhesion molecule levels in the MPA group were higher in the active phase compared to the inactive phase and to the CRF and HS groups. In addition, considering the outcome of serum Cr concentrations in the MPA group, the serum adhesion molecule levels were higher and decreased more slowly in patients with final high serum Cr concentrations than in patients with final normal serum Cr concentrations. CONCLUSION: Our data suggest that in MPO-ANCA-positive MPA patients, higher sICAM-1 and sELAM-1 levels during the active phase and their slower decline during the treatment period, could be a prognostic risk factor for CRF development.

This paper has as main objective to explore, adopting a historical and critical perspective, the release of film and anime TV in UK. This would be a first step towards the studio of the peculiar implementation of manganime Culture in Britain.\ud \ud Compared with other European countries, UK has shown to be slower and even reluctant in importing Japanese television products. Thus, while major markets of anime such as France, Italy or Spain expanded considerably during 1975-1995 period, in a recurrent synergy of television markets, and technological publishing, the implantation of the principal channel (televised anime) in UK has been irregular and unstable. Even today, the catalogue of broadcasting anime is limited to some high success movies, late night television on thematic channels and quite recently, video-on-demand services (Netflix). The offer cannot be compare in importance and diversity to other European countries. This fact, far from being anecdotal, has had an impact on the subsequent implantation of media Japanese cultures such as manga, anime, video games and cosplay.\ud \ud What can be the reason of this irregular development of Japanese visual culture in United Kingdom? Characteristic having the television market and / or the UK audience? Main hypothesis in relation to these issues can be considered to be of sociological character, but are reflected in the idiosyncrasies of British television culture and production system.\ud \ud Thus, compared to other Western markets (including the US) which saw the opportunity to purchase economic products for children’s television audiences in the late 70s and early 80s, the British ‘telly’ already offered a broad catalogue (Roobarb, Super Ted, Danger Mouse, etc. ...). The only exception to this children’s ‘made in Britain’ programming was the co-production model. This caused a leak of few products that were not even considered as “Japanese” (Seven Cities of Gold, Godzilla) but mere ‘cartoons’. This competition with the British children production as well as the wide catalogue of other forms of British Popular Culture would explain why the film, domestic video and later adults programming would be the marginal routes of entry for manganime.

This paper contextualises the mutual perceptions between Spain and Japan through the historical background and some examples within several creative industries. Subsequently, the paper reviews the ways in which transnational popular culture has been regarded as non-official National Branding tool, with special attention to the Spanish case and the proliferation of these images in the (mis)representation of its tangible and intangible cultural heritage. Japanese anime is chosen as the most representative global industry among Japanese Popular Culture. Finally, three case studies, especially relevant because of its combination of elements fictionality and creation of national images are discussed.

SummaryBackgroundEpidemiologic studies have demonstrated that elderly patients with fixed airflow obstruction can be affected by asthma or chronic obstructive pulmonary disease (COPD).MethodsWe studied 49 consecutive elderly outpatients, presenting fixed airflow obstruction, by clinical history (smoking), pulmonary function tests, blood gas analysis, and induced sputum.ResultsThe age was not different in patients with COPD (n=28) and asthma (n=21) (70.2±3.9 years vs. 69.6±3.7 years), also the degree of fixed airflow obstruction was similar (FEV1: 58.3±1.5% vs. 59.0±1.4% of predicted).
