Objectives: To describe the clinical characteristics and outcomes of cycloserine (CS)-induced psychosis in adults diagnosed with drug-resistant tuberculosis (DR-TB). Materials and methods: A systematic review of case reports was carried out according to PRISMA guidelines. Subsequently, information was extracted concerning sociodemographic variables, clinical characteristics of psychosis, treatment, and clinical outcomes, as well as the quality of the articles using a standardized tool (Joanna Briggs Institute—JBI—Case Reports Tool). Results: Of 3416 articles, 20 reports from seven countries were included, encompassing 22 patients (68.18% male participants, mean age: 31.45 ± 10.88 years). Delusions (68.2%, primarily persecutory) were the most frequent psychotic symptom. The median duration of the psychotic episode was 13 days (interquartile range: 35). Other frequently appearing symptoms in CS-induced psychosis were aggressiveness (68.2%), insomnia (59.1%), hallucinations (54.5%), incoherent/disorganized speech (45.5%), and irritability (45.5%). After antipsychotic treatment (81.81% of the reported cases were treated with at least one antipsychotic), 95.5% presented improvement, while 4.54% died by suicide. Finally, after the quality assessment of studies using the JBI tool, 85% of the articles showed a low risk of bias. Conclusions: CS-induced psychosis is a rare presentation, generally of short duration, that includes delusions (mostly persecutory) as its main psychotic symptom and shows mostly a symptom improvement after medical treatment. RESUMEN Objetivos : Determinar la relación entre años de estudio y síntomas depresivos y ansiosos durante los primeros meses de la pandemia por COVID-19, en una muestra de estudiantes de medicina de Lima, controlando en particular las covariables sexo y enfermedades médicas previas. Material y métodos : Se realizó una encuesta en línea, que recolectó información de 250 estudiantes de medicina incluyendo datos generales, historia médica previa, síntomas de depresión (PHQ-9) y de ansiedad (GAD-7). Los datos se manejaron mediante un análisis multivariado. Resultados : Se encontraron diferencias entre años de estudios y los valores combinados de PHQ-9 y GAD-7 (Lambda de Wilks = 0,86; p = 1,68 x 10-4; η2p = 0,08). La presencia de enfermedades médicas previas mostró diferencias significativas en relación a los valores combinados de PHQ-9 y GAD-7 (Lambda de Wilks = 0,94, p = 4,43 x 10-4, η2p = 0,06). Las muestras univariadas mostraron diferencias en años de estudios para PHQ-9 (F6,241 = 4,12, p = 0,001, η2p = 0,09) y GAD-7 (F6,241 = 2,81, p = 0,01, η2p = 0,07). El análisis post hoc mostró diferencias estadísticamente significativas en los primeros años de estudio. Conclusiones : Estos resultados sugieren que estudiantes de medicina de los primeros años muestran mayores niveles de síntomas depresivos y ansiosos que los participantes de años superiores. Asimismo, la ocurrencia de enfermedades médicas previas explica también los valores altos de depresión y ansiedad. SUMMARY Objective : To determine the relationship between year levels of medical studies and depressive and anxious symptoms during the first months of the COVID-19 pandemic in a sample of medical students in Lima, controlling specifically the variables sex and presence of underlying medical conditions. Material and Methods : An online survey collected information from 250 medical students, covering general data, previous medical history and symptoms of depression (PHQ-9) and anxiety (GAD-7). Data were managed using multivariate analysis of covariance. Results : Differences were found between years of study and the combined values of PHQ-9 and GAD-7 (Wilks’ Λ = 0.86; p = 1.68 x 10-4; η2p = 0.08). Underlying medical conditions also showed significant differences for the combined values of PHQ-9 and GAD-7 (Wilks’ Λ = 0.94, p = 4.43 x 10-4, η2p = 0.06). The univariate test for year of study showed differences for PHQ-9 (F6.241 = 4.12, p = 0.001, η2p = 0.09) and GAD-7 (F6.241 = 2.81, p = 0.01, η2p = 0.07). The post hoc analysis showed statistically significant differences in the first years of study. Conclusions : These results suggest that medical students of the first years show higher levels of depression and anxiety symptoms than participants from more advanced years of the medical career. Likewise, the occurrence of previous medical conditions also explained high levels of depression and anxiety.

RESUMEN Objetivo: Efectuar una validación de contenido por expertos del Índice de Distrés Peri-traumático relacionado a COVID-19 (CPDI) para su uso en el Perú. Material y Métodos: El CPDI fue traducido al castellano y luego adaptado en una primera ronda de expertos. En una segunda ronda (n = 13), se evaluó la validez del contenido mediante el uso de la V de Aiken (V). Se calculó la V para cada ítem, así como para el instrumento en su conjunto (VTOT). Resultados : De los 24 ítems originales, solo un ítem (S7) presentó valores no significativos para validez de contenido, razón por la que fue eliminado. La escala con 23 ítems no presentó alteraciones en el cálculo de la VTOT. Conclusiones : Sobre la base de su validez de contenido, el CPDI en castellano es adecuado para su aplicación en la población peruana. Quedan por definirse las características psicométricas así como la modificación propuesta del instrumento, mediante estudios cuantitativos en torno a la validez del constructo. SUMMARY Objectives : To perform an expert validation study of COVID-19 Peritraumatic Distress Index (CPDI) for its eventual use in Perú. Material and Methods: In a first round by experts, the CPDI was translated into Spanish and then adapted. In a second round, (n = 13), the content validity was evaluated by using Aiken’s V (V). The V was calculated for each item, as well as for the whole instrument (VTOT). Results : Of the 24 original items, only one (S7) presented non-significant values for content validity, reason for which it was removed. The final 23-items scale did not present any alterations for the VTOT calculation. Conclusions : On the basis of its adequate content validity, the Spanish version of CPDI is suitable for its application in the Peruvian population. The psychometric characteristics of the instrument, as well as its proposed modification remain to be defined through quantitative studies of the construct validity.

Objective: To describe different psycho-spiritual elements in a sample of stage IV cancer patients from a public hospital in Lima, Peru, through the application of the Functional Assessment of Chronic Illness Therapy - Spiritual Well-Being (FACIT-Sp) scale. Methods: Application of FACIT-Sp to a sample of patients (n = 100) diagnosed with stage IV cancer at the Oncology Service of the Hospital Nacional Arzobispo Loayza. The 33-question instrument, designed to evaluate psycho-spiritual elements, analyzes five areas: social well-being (SWB), emotional well-being (EWB), functional well-being (FWB), meaning and peace (MP) and faith (F). Results: The most prominent aspect was F (average score=13.53 ± 3.05, corresponding to 84.56% of the maximum possible score), and the lowest was FWB (average score=15.79 ± 6.89, corresponding to 56.39% of the maximum possible score). Conclusions: According to FACIT-Sp, the most developed psycho-spiritual aspect among the participating patients was the one related to Faith, whereas the least developed, the one linked to the area of Functional Well-Being. Objetivo: Describir los diferentes elementos psico-espirituales en pacientes con cáncer aplicando la escala Functional Assessment of Chronic Illness Therapy - Spiritual Well Being, FACIT-Sp (Evaluación Funcional de Terapia de Enfermedades Crónicas y Bienestar Espiritual). Métodos: Aplicación de la escala a una muestra de pacientes (n = 100) con diagnóstico de cáncer, estadio IV, del Servicio de Oncología del Hospital Nacional Arzobispo Loayza. El instrumento de 33 preguntas examina cinco áreas: bienestar social (SWB), bienestar emocional (EWB), bienestar funcional (FWB), sentido y paz (MP) y fe (F). Resultados: En la muestra estudiada, el aspecto más resaltante fue el relacionado con F (puntaje promedio=13,53 ± 3,05, correspondiente al 84,56% del puntaje máximo posible) y el más bajo fue el vinculado a FWB (puntaje promedio=15,79 ± 6,89, correspondiente al 56,39% del puntaje máximo posible). Conclusiones: De acuerdo a FACIT-Sp, el aspecto psico-espiritual más desarrollado en los pacientes de la muestra estudiada fue el relativo a la Fe y el menos desarrollado, el vinculado al área de Bienestar Funcional.

Hintergrund: Depression ist eine der häufigsten psychiatrischen Diagnosen weltweit und wirkt sich nicht nur auf die Gesundheit des Individuums aus, sondern auch auf die Funktionalität. Eines der häufigsten Phänomene bei Depression ist Erschöpfung. In den letzten Jahren wurden verschiedene Modelle vorgeschlagen, um die Pathophysiologie von Depression sowie von Erschöpfung besser zu verstehen. Eines dieser vorgeschlagenen Modelle bezieht sich auf die Entzündungsaktivität, da sowohl Erschöpfung als auch Depression mit Entzündung einhergehen können. Darüber hinaus gibt es Hinweise, dass sich bestimmte Gehirnareale und/oder Gehirnfunktionen bei Depression verändern. Das Ziel dieser Doktorarbeit ist daher, Korrelationen zwischen entzündungsfördernden Parametern, Erschöpfung, zerebralen Endophänotypen bei Personen mit diagnostizierter Depression zu untersuchen. Methoden: Diese Monographie ist in zwei Teile gegliedert. Im ersten Teil wurden proinflammatorischen Plasmaparameter untersucht. Für die Messung der Erschöpfung wurde das Inventar FIS-D verwendet. Die gesamten Werte des BDI-FS-Inventars wurden für die Depressivität verwendet. Die Messungen wurden bei 50 Personen mit diagnostizierter Depression (PG) und 58 gesunden Personen (HC) durchgeführt. Im zweiten Teil wurden verschiedene proinflammatorische Plasmaparameter mit Werten aus einem computergestützten neurokognitiven Test und mit Werten des ereigniskorrelierten Potentials (ERP) mittels einer Elektroenzephalographie (EEG)-Messung untersucht und auf Korrelation geprüft. Die Messungen wurden bei 26 Personen mit diagnostizierter Depression und 33 gesunden Personen durchgeführt. Ergebnisse: Die Ergebnisse des ersten Teils zeigen positive Korrelationen zwischen Erschöpfung und Depression. BDI-FSTOT-Werte korrelieren positiv mit IL-1β und TNF-α in der PG, jedoch nicht in der HC. TNF-α korreliert positiv mit allen Erschöpfungswerten der PG. Im Gegensatz dazu korrelieren nur bestimmte Entzündungsfaktoren (IL-6, TNF-α, IL-1β) mit den Erschöpfungswerten in der HC. Im zweiten Teil werden unterschiedliche Korrelationen in der PG festgestellt. Die ERP-Latenzzeiten korrelieren positiv mit verschiedenen Erschöpfungsdimensionen und die P300-Amplitudenwerte des ERP korrelieren negativ mit einem proinflammatorischen Parameter (IL-1β). Schlussfolgerung: Die Ergebnisse dieser Doktorarbeit weisen auf eine Entzündungskomponente bei Depression und Erschöpfung hin. Der Einfluss von Entzündung auf die P300-Amplitude und ERP-Latenzzeiten ist jedoch bei Depression gering, obwohl der Einfluss von Erschöpfung auf die ERP-Latenzzeiten bei Depression erheblich ist.

Se examina el fenómeno clínico conocido como “fatiga”, resumiendo los principales hallazgos vinculados a su definición y aspectos históricos, información epidemiológica, conceptos neuroanatómicos y descripción psicopatológica. Las correlaciones entre los distintos tipos de daño estructural y funcional son numerosas generando dudas en torno a la existencia del síntoma y planteándose entonces una redefinición. La búsqueda de mecanismos fisiopatológicos y objetivos terapéuticos evidencian también el interés de los investigadores. La fatiga causa una carga ingente en la salud global debido a la disfuncionalidad resultante, por lo que se sugiere una variedad de análisis psicométricos y categorización de los componentes conocidos a fin de dejar atrás todos los elementos de confusión encontrados previamente. Finalmente, se plantea la necesidad de que futuros estudios, luego de definir qué es lo que realmente está detrás de fatiga, identifiquen marcadores biológicos y busquen daños estructurales micro- o macroscópicos de forma tal que se pueda mejorar la calidad de vida de la población que la padece. Clinical, historical and research aspects of the phenomenon known as “fatigue” are examined. A summary of the main findings regarding its history, epidemiological data, neuroanatomical and psychopathological descriptions, is presented. The correlations between the different types of structural and functional damage are numerous, which generates doubts about the symptom’s existence and forces a redefinition. The search of a physio-pathological mechanisms and therapeutic targets are also evidence of the interest of investigators. Fatigue is cause of significant impact on global health due to the resulting dysfunctionality; such is the reason for the suggestions of using psychometric and factorial analyses and a categorization of its known components in order to leave behind all the confusing elements previously found. It is necessary that future studies, after defining its underlying processes, focus on identifying biological markers and micro- and macroscopic structural damages, so that the quality of life of affected populations can be improved.

Objetivos: Describir la frecuencia de ansiedad, onicofagia y síndrome de piernas inquietas (SPI) en estudiantes de medicina y explorar la relación entre los mismos. Materiales y métodos: Participaron 315 estudiantes del primer al quinto año de medicina de una universidad privada de Lima. Se administraron la Escala de Ansiedad de Beck (BAI), una escala Likert para onicofagia, el Inventario de Estudios Epidemiológicos de SPI (García - Borreguero) y el Inventario de SPI (Grupo Internacional de SPI). Resultados: Se halló la frecuencia de ansiedad (61,3%), onicofagia (52,63%) y SPI (9,29%). Ser hombre es un factor protector para ansiedad (razón de prevalencia (RP) = 0,74; p = 0,001; IC 95% = 0,63 – 0,89). Existe asociación entre sintomatología ansiosa y SPI (RP =2,52; p = 0,036; IC 95% = 1,06 – 6); y entre sintomatología ansiosa y onicofagia (RP = 1,47; p = 0,002; IC 95% = 1,15 – 1,87). La presencia de onicofagia y SPI tenía asociación con sintomatología ansiosa (RP = 5,37; p = 0,023; IC 95% = 1,26 – 22,82). No se encontró asociación significativa entre SPI y onicofagia. Conclusiones: Existe una asociación entre ansiedad y SPI; y entre ansiedad y onicofagia; pero no entre SPI y onicofagia.
