1. From Hobby & Side-Hustle to Hopeful Habitat Exploring Student Services Practitioners and Undergraduate Students on Twitter

Abstract

Exploring the relationship between undergraduate students and student services practitioners is nuanced. By using Twitter as a platform, tool, and community, this dissertation explored this relationship. Previous research has indicated that social media and higher education have been explored broadly, however, relationships between two groups of users--specifically undergraduate students and student services practitioners--on one platform (such as Twitter) has not been significantly explored. Therefore, the purpose of the study was to examine undergraduate students and student services practitioners' relationship through Twitter. Utilizing case study as a qualitative research design, the study explored undergraduate students and student services practitioners from four institutions and the data was analyzed by within-case and cross-case analysis. Chapter 1 introduced the study and set the foundation for Twitter. Chapter 2 provided an overview of the literature and introduced the conceptual framework. Chapter 3 described the methodology. Chapter 4 presented an overview of each case as an institution and an update on Twitter. Chapters 5 -- 8 detailed the findings within each case while Chapter 9 highlighted cross-case findings. Chapter 10 discussed the findings, made connections back to the previous literature, reimagined the framework, addressed limitations, and provided recommendations. The study narrowed the scope of a large meta concept and added to what we know about Twitter and its unique idiosyncrasies, how student services practitioners use Twitter, and how students perceive their usage. Abstract

Background Accelerate Pheno blood culture detection system (AXDX) provides rapid identification and antimicrobial susceptibility testing results. Limited data exist regarding its clinical impact. Other rapid platforms coupled with antimicrobial stewardship program (ASP) real-time notification (RTN) have shown improved length of stay (LOS) in bacteremia. Methods A single-center, quasi-experimental study of bacteremic inpatients before and after AXDX implementation was conducted comparing clinical outcomes from 1 historical and 2 intervention cohorts (AXDX and AXDX + RTN). Results Of 830 bacteremic episodes, 188 of 245 (77%) historical and 308 (155 AXDX, 153 AXDX + RTN) of 585 (65%) intervention episodes were included. Median LOS was shorter with AXDX (6.3 days) and AXDX + RTN (6.7 days) compared to historical (8.1 days) (P =.001). In the AXDX and AXDX + RTN cohorts, achievement of optimal therapy (AOT) was more frequent (93.6% and 95.4%, respectively) and median time to optimal therapy (TTOT) was faster (1.3 days and 1.4 days, respectively) compared to historical (84.6%, P ≤.001 and 2.4 days, P ≤.001, respectively). Median antimicrobial days of therapy (DOT) was shorter in both intervention arms compared to historical (6 days each vs 7 days; P =.011). Median LOS benefit during intervention was most pronounced in coagulase-negative Staphylococcus bacteremia (P =.003). Conclusions LOS, AOT, TTOT, and total DOT significantly improved after AXDX implementation. Addition of RTN did not show further improvement over AXDX and an already active ASP. These results suggest that AXDX can be integrated into healthcare systems with an active ASP even without the resources to include RTN. [ABSTRACT FROM AUTHOR]

Abstract

Over the past ten years, there has been a consistent increase in opioid use, which has resulted in an increase in enrolment in methadone maintenance therapy [MMT]. With retention in MMT being a key factor, in order to understand the process of retention, it is important to gain an understanding of individual perceptions and experiences. No research in Ottawa, Ontario has addressed the perspective of MMT from people enrolled in MMT; therefore, nursing based research was undertaken. The objective was to understand the process and experiences associated with MMT from the perspective of persons who are enrolled in treatment. Twelve participants were engaged in semi-structured interviews. These participants described that, although MMT can positively affect the people who use such a treatment option, it continues to have a negative impact that repeatedly affects MMT initiation and delivery. The theoretical framework of Hardt and Negri’s “Triple Imperative of Empire” was used to analyze the research participants’ interviews within the current MMT program, to help develop a more inclusive healthcare service that addressed the current barriers hindering access and retention in treatment. The integration of this framework can help engage persons in treatment, tailor treatment to patient specific needs, and as a result increase access and retention in MMT programs.

Abstract

Methadone maintenance treatment (MMT) is a harm reduction approach for persons who wish to stop using opioids and is rather effective if used for a minimum of 12 months. Notably, research demonstrates that many persons enrolled in MMT programs discontinue care before this time, limiting its effects. To better understand this process, we undertook an exploratory descriptive qualitative study and interviewed 12 men and women who were using MMT. Using the theoretical work of Foucault and Hardt and Negri, the interview data highlighted that MMT continues to be strongly stigmatized, and that it is a system of care that involves rewards and penalties, based on if patients behave according to prescribed norms. These results suggest that MMT is a disciplinary mechanism, albeit one that impedes its own access. We consequently recommend that healthcare providers work to facilitate access to MMT, which means altering care delivery. [ABSTRACT FROM AUTHOR]
