Abstract

IntroductioneHealth seems promising in addressing challenges in the provision of care for Huntington’s disease (HD) across Europe. By harnessing information and communication technologies, eHealth can partially relocate care from specialized centers to the patients’ home, thereby increasing the availability and accessibility of specialty care services beyond regional borders. Previous research on eHealth (development) in HD is however limited, especially when it comes to including eHealth services specifically designed together with HD gene expansion carriers (HDGECs) and their partners to fit their needs and expectations.MethodsThis article describes the qualitative human-centered design process and first evaluations of the Huntington Support App prototype: a web-app aimed to support the quality of life (QoL) of HDGECs and their partners in Europe. Prospective end-users, i.e., HDGECs, their partners, and healthcare providers (HCPs), from different countries were involved throughout the development process. Through interviews, we captured people’s experiences with the disease, quality of life (QoL), and eHealth. We translated their stories into design directions that were further co-designed and subsequently evaluated with the user groups.ResultsThe resulting prototype centralizes clear and reliable information on the disease, HD-related news and events, as well as direct contact possibilities with HCPs via an online walk-in hour or by scheduling an appointment. The app’s prototype was positively received and rated as (very) appealing, pleasant, easy to use and helpful by both HDGECs and partners.DiscussionBy involving end-users in every step, we developed a healthcare app that meets relevant needs of individuals affected by HD and therefore may lead to high adoption and retention rates. As a result, the app provides low-threshold access to reliable information and specialized care for HD in Europe. A description of the Huntington Support App as well as implications for further development of the app’s prototype are provided.

Abstract

Background: Huntington's disease (HD) is an autosomal dominant neurodegenerative disease that affects the quality of life (QoL) of HD gene expansion carriers (HDGECs) and their partners. Although HD expertise centers have been emerging across Europe, there are still some important barriers to care provision for those affected by this rare disease, including transportation costs, geographic distance of centers, and availability/accessibility of these services in general. eHealth seems promising in overcoming these barriers, yet research on eHealth in HD is limited and fails to use telehealth services specifically designed to fit the perspectives and expectations of HDGECs and their families. In the European HD-eHelp study, we aim to capture the needs and wishes of HDGECs, partners of HDGECs, and health care providers (HCPs) in order to develop a multinational eHealth platform targeting QoL of both HDGECs and partners at home.Methods: We will employ a participatory user-centered design (UCD) approach, which focusses on an in-depth understanding of the end-users' needs and their contexts. Premanifest and manifest adult HDGECs (n = 76), partners of HDGECs (n = 76), and HCPs (n = 76) will be involved as end-users in all three phases of the research and design process: (1) Exploration and mapping of the end-users' needs, experiences and wishes; (2) Development of concepts in collaboration with end-users to ensure desirability; (3) Detailing of final prototype with quick review rounds by end-users to create a positive user-experience. This study will be conducted in the Netherlands, Germany, Czech Republic, Italy, and Ireland to develop and test a multilingual platform that is suitable in different healthcare systems and cultural contexts.Discussion: Following the principles of UCD, an innovative European eHealth platform will be developed that addresses the needs and wishes of HDGECs, partners and HCPs. This allows for high-quality, tailored care to be moved partially into the participants' home, thereby circumventing some barriers in current HD care provision. By actively involving end-users in all design decisions, the platform will be tailored to the end-users' unique requirements, which can be considered pivotal in eHealth services for a disease as complex and rare as HD.

Abstract

AimsTelomere attrition in cardiomyocytes is associated with decreased contractility, cellular senescence, and up-regulation of proapoptotic transcription factors. Pim1 is a cardioprotective kinase that antagonizes the aging phenotype of cardiomyocytes and delays cellular senescence by maintaining telomere length, but the mechanism remains unknown. Another pathway responsible for regulating telomere length is the transforming growth factor beta (TGFβ) signalling pathway where inhibiting TGFβ signalling maintains telomere length. The relationship between Pim1 and TGFβ has not been explored. This study delineates the mechanism of telomere length regulation by the interplay between Pim1 and components of TGFβ signalling pathways in proliferating A549 cells and post-mitotic cardiomyocytes.Methods and resultsTelomere length was maintained by lentiviral-mediated overexpression of PIM1 and inhibition of TGFβ signalling in A549 cells. Telomere length maintenance was further demonstrated in isolated cardiomyocytes from mice with cardiac-specific overexpression of PIM1 and by pharmacological inhibition of TGFβ signalling. Mechanistically, Pim1 inhibited phosphorylation of Smad2, preventing its translocation into the nucleus and repressing expression of TGFβ pathway genes.ConclusionPim1 maintains telomere lengths in cardiomyocytes by inhibiting phosphorylation of the TGFβ pathway downstream effectors Smad2 and Smad3, which prevents repression of telomerase reverse transcriptase. Findings from this study demonstrate a novel mechanism of telomere length maintenance and provide a potential target for preserving cardiac function.

8. Classical causal models cannot faithfully explain Bell nonlocality or Kochen-Specker contextuality in arbitrary scenarios

Abstract

In a recent work, it was shown by one of us (EGC) that Bell-Kochen-Specker inequality violations in phenomena satisfying the no-disturbance condition (a generalisation of the no-signalling condition) cannot in general be explained with a faithful classical causal model -- that is, a classical causal model that satisfies the assumption of no fine-tuning. The proof of that claim however was restricted to Bell scenarios involving 2 parties or Kochen-Specker-contextuality scenarios involving 2 measurements per context. Here we show that the result holds in the general case of arbitrary numbers of parties or measurements per context; it is not an artefact of the simplest scenarios. This result unifies, in full generality, Bell nonlocality and Kochen-Specker contextuality as violations of a fundamental principle of classical causality. We identify, however, an implicit assumption in the former proof, making it explicit here: that certain operational symmetries of the phenomenon are reflected in the model, rather than requiring fine-tuned choices of model parameters. Abstract

RationaleUnderstanding and manipulating the cardiomyocyte cell cycle has been the focus of decades of research, however the ultimate goal of activating mitotic activity in adult mammalian cardiomyocytes remains elusive and controversial. The relentless pursuit of controlling cardiomyocyte mitosis has been complicated and obfuscated by a multitude of indices used as evidence of cardiomyocyte cell cycle activity that lack clear identification of cardiomyocyte "proliferation" versus cell cycle progression, endoreplication, endomitosis, and even DNA damage. Unambiguous appreciation of the complexity of cardiomyocyte replication that avoids oversimplification and misinterpretation is desperately needed.ObjectiveTrack cardiomyocyte cell cycle activity and authenticate fidelity of proliferation markers as indicators of de novo cardiomyogenesis in post-mitotic cardiomyocytes.Methods and resultsCardiomyocytes expressing the FUCCI construct driven by the α-myosin heavy chain promoter were readily and uniformly detected through the myocardium of transgenic mice. Cardiomyocyte cell cycle activity peaks at postnatal day 2 and rapidly declines thereafter with almost all cardiomyocytes arrested at the G1/S cell cycle transition. Myocardial infarction injury in adult hearts prompts transient small increases in myocytes progressing through cell cycle without concurrent mitotic activity, indicating lack of cardiomyogenesis. In comparison, cardiomyogenic activity during early postnatal development correlated with coincidence of FUCCI and cKit+ cells that were undetectable in the adult myocardium.ConclusionsCardiomyocyte-specific expression of Fluorescence Ubiquitination-based Cell Cycle Indicators (FUCCI) reveals previously unappreciated aspects of cardiomyocyte cell cycle arrest and biological activity in postnatal development and in response to pathologic damage. Compared to many other methods and model systems, the FUCCI transgenic (FUCCI-Tg) mouse represents a valuable tool to unambiguously track cell cycle and proliferation of the entire cardiomyocyte population in the adult murine heart. FUCCI-Tg provides a desperately needed novel approach in the armamentarium of tools to validate cardiomyocyte proliferative activity that will reveal cell cycle progression, discriminate between cycle progression, DNA replication, and proliferation, and provide important insight for enhancing cardiomyocyte proliferation in the context of adult myocardial tissue.

Abstract

The ice giants Uranus and Neptune are the least understood class of planets in our solar system but the most frequently observed type of exoplanets. Presumed to have a small rocky core, a deep interior comprising ~70% heavy elements surrounded by a more dilute outer envelope of H2 and He, Uranus and Neptune are fundamentally different from the better-explored gas giants Jupiter and Saturn. Because of the lack of dedicated exploration missions, our knowledge of the composition and atmospheric processes of these distant worlds is primarily derived from remote sensing from Earth-based observatories and space telescopes. As a result, Uranus's and Neptune's physical and atmospheric properties remain poorly constrained and their roles in the evolution of the Solar System not well understood. Exploration of an ice giant system is therefore a high-priority science objective as these systems (including the magnetosphere, satellites, rings, atmosphere, and interior) challenge our understanding of planetary formation and evolution. Here we describe the main scientific goals to be addressed by a future in situ exploration of an ice giant. An atmospheric entry probe targeting the 10-bar level, about 5 scale heights beneath the tropopause, would yield insight into two broad themes: i) the formation history of the ice giants and, in a broader extent, that of the Solar System, and ii) the processes at play in planetary atmospheres. The probe would descend under parachute to measure composition, structure, and dynamics, with data returned to Earth using a Carrier Relay Spacecraft as a relay station. In addition, possible mission concepts and partnerships are presented, and a strawman ice-giant probe payload is described. Abstract

Causal counterfactual theory provides clear semantics and sound logic for causal reasoning and may help foster research on, and clarify dissemination of, weather and climate-related event attribution. The current event attribution framework obeys the spirit of counterfactual logics. The author has proposed a methodology, which relies on in silico experimentation to derive both the factual and counterfactual probabilities. The attributable proportion are also used to name the same quantity. The FAR, as well as these similar terms, is used to communicate the idea, particularly relevant in epidemiology from which it originates, that the exposition to a given risk factor X translates into an increase of, say, the frequency of a given disease Y. In causal theory, the probability of necessary causation PN formally embeds the notion of causal attribution in its definition. Finally, at a more practical level, attribution studies applying causal theory require the availability of counterfactual model simulations.

Abstract

We experimentally study the jamming of quasi-two-dimensional emulsions. Our experiments consist of oil-in-water emulsion droplets confined between two parallel plates. From the droplet outlines, we can determine the forces between every droplet pair to within 8% over a wide range of area fractions $\phi$. We study three bidisperse samples that jam at area fractions $\phi_c \approx 0.86$. Our data show that for $\phi > \phi_c$, the contact numbers and pressure have power-law dependence on $\phi-\phi_c$ in agreement with the critical scaling found in numerical simulations. Furthermore, we see a link between the interparticle force law and the exponent for the pressure scaling, supporting prior computational observations. We also observe linear-like force chains (chains of large inter-droplet forces) that extend over 10 particle lengths, and examine the origin of their linearity. We find that the relative orientation of large force segments are random and that the tendency for force chains to be linear is not due to correlations in the direction of neighboring large forces, but instead occurs because the directions are biased towards being linear to balance the forces on each droplet., Comment: 14 pages, 13 figures, 4 tables

Abstract

In this work we systematically investigate the condensate properties, superfluid properties and quantum phase transitions in interacting Bose gases trapped in disordered optical potentials. We numerically solve the Bose-Hubbard Hamiltonian exactly for different: (a) types of disorder, (b) disorder strengths, and (c) interatomic interactions. The three types of disorder studied are: quasiperiodic disorder, uniform random disorder and random speckle-type disorder. We find that the Bose glass, as identified by Fisher et al [Phys. Rev. Abstract

We present a numerical study of dispersion manipulation and formation of matter-wave gap solitons in a Bose-Einstein condensate trapped in an optical superlattice. We demonstrate a method for controlled generation of matter-wave gap solitons in a stationary lattice by using an interference pattern of two condensate wavepackets, which mimics the structure of the gap soliton near the edge of a spectral band. The efficiency of this method is compared with that of gap soliton generation in a moving lattice recently demonstrated experimentally by Eiermann et al. [Phys. Rev. Lett. 92, 230401 (2004)]. We show that, by changing the relative depths of the superlattice wells, one can fine-tune the effective dispersion of the matter waves at the edges of the mini-gaps of the superlattice Bloch-wave spectrum and therefore effectively control both the peak density and the spatial width of the emerging gap solitons., Comment: 8 pages, 9 figures; modified references in Section 2; minor content changes in Sections 1 and 2 and Fig. 9 caption

Abstract

We analyze the Floquet-Bloch matter-wave spectrum of Bose-Einstein condensates loaded into single-periodic optical lattices and double-periodic superlattices. In the framework of the Gross-Pitaevskii equation, we describe the structure and analyze the mobility properties of dark matter-wave solitons residing on the background of extended nonlinear Bloch-type states. Abstract

We analyze the existence and stability of spatially extended (Bloch-type) and localized states of a Bose-Einstein condensate loaded into an optical lattice. In the framework of the Gross-Pitaevskii equation with a periodic potential, we study the band-gap structure of the matter-wave spectrum in both the linear and nonlinear regimes. We demonstrate the existence of families of spatially localized matter-wave gap solitons, and analyze their stability in different band gaps, for both repulsive and attractive atomic interactions.

28. Laparoscopic repair of a large interstitially incarcerated inguinal hernia.

Abstract

A 68 year old female presented for elective repair of an abdominal wall hernia. Preoperative CT imaging revealed a right inguinal hernia defect with hernia contents coursing cephalad between the external and internal abdominal oblique muscles. This was consistent with an interstitial inguinal hernia, a rare entity outside of post- traumatic hernias. At operation the hernia contents were reduced laparoscopically. The hernia was then repaired by transitioning to the totally extraperitoneal (TEP) approach using a 15cm X 15cm piece of polyester mesh. The patient had an uneventful recovery. Interstitial hernias are rare, difficult to diagnose and potentially dangerous if left untreated. There is no consensus on the ideal repair of these unique hernias. This represents a minimally invasive repair of an unusual hernia, with a novel approach to diagnose and manage the hernia and its redundant sac.

Abstract

Traditional databases commonly support efficient query and update procedures that operate in time which is sublinear in the size of the database. Our goal in this paper is to take a first step toward dynamic reasoning in probabilistic databases with comparable efficiency. We propose a dynamic data structure that supports efficient algorithms for updating and querying singly connected Bayesian networks. In the conventional algorithm, new evidence is absorbed in O(1) time and queries are processed in time O(N), where N is the size of the network. We propose an algorithm which, after a preprocessing phase, allows us to answer queries in time O(log N) at the expense of O(log N) time per evidence absorption. The usefulness of sub-linear processing time manifests itself in applications requiring (near) real-time response over large probabilistic databases. Abstract

Single photon detectors are indispensable tools in optics, from fundamental measurements to quantum information processing. The ability of superconducting nanowire single photon detectors (SNSPDs) to detect single photons with unprecedented efficiency, short dead time, and high time resolution over a large frequency range enabled major advances in quantum optics. However, combining near-unity system detection efficiency (SDE) with high timing performance remains an outstanding challenge. In this work, we fabricated novel SNSPDs on membranes with 99.5-2.07+0.5% SDE at 1350 nm with 32 ps timing jitter (using a room-temperature amplifier), and other detectors in the same batch showed 94%-98% SDE at 1260-1625 nm with 15-26 ps timing jitter (using cryogenic amplifiers). The SiO2/Au membrane enables broadband absorption in small SNSPDs, offering high detection efficiency in combination with high timing performance. With low-noise cryogenic amplifiers operated in the same cryostat, our efficient detectors reach a timing jitter in the range of 15-26 ps. We discuss the prime challenges in optical design, device fabrication, and accurate and reliable detection efficiency measurements to achieve high performance single photon detection. As a result, the fast developing fields of quantum information science, quantum metrology, infrared imaging, and quantum networks will greatly benefit from this far-reaching quantum detection technology.

Abstract

The Cassini spacecraft orbiting Saturn carries the composite infrared spectrometer (CIRS) designed to study thermal emission from Saturn and its rings and moons. CIRS, a Fourier transform spectrometer, is an indispensable part of the payload providing unique measurements and important synergies with the other instruments. It takes full advantage of Cassini's 13-year-long mission and surpasses the capabilities of previous spectrometers on Voyager 1 and 2. The instrument, consisting of two interferometers sharing a telescope and a scan mechanism, covers over a factor of 100 in wavelength in the mid and far infrared. It is used to study temperature, composition, structure, and dynamics of the atmospheres of Jupiter, Saturn, and Titan, the rings of Saturn, and surfaces of the icy moons. CIRS has returned a large volume of scientific results, the culmination of over 30 years of instrument development, operation, data calibration, and analysis. As Cassini and CIRS reach the end of their mission in 2017, we expect that archived spectra will be used by scientists for many years to come.
