Abstract

BackgroundPatients with multiple sclerosis (MS) face several challenges in accessing clinical tools to help them monitor, understand, and make meaningful decisions about their disease course. The University of California San Francisco MS BioScreen is a web-based precision medicine tool initially designed to be clinician facing. We aimed to design a second, openly available tool, Open MS BioScreen, that would be accessible, understandable, and actionable by people with MS. ObjectiveThis study aimed to describe the human-centered design and development approach (inspiration, ideation, and implementation) for creating the Open MS BioScreen platform. MethodsWe planned an iterative and cyclical development process that included stakeholder engagement and iterative feedback from users. Stakeholders included patients with MS along with their caregivers and family members, MS experts, generalist clinicians, industry representatives, and advocacy experts. Users consisted of anyone who wants to track MS measurements over time and access openly available tools for people with MS. Phase I (inspiration) consisted of empathizing with users and defining the problem. We sought to understand the main challenges faced by patients and clinicians and what they would want to see in a web-based app. In phase II (ideation), our multidisciplinary team discussed approaches to capture, display, and make sense of user data. Then, we prototyped a series of mock-ups to solicit feedback from clinicians and people with MS. In phase III (implementation), we incorporated all concepts to test and iterate a minimally viable product. We then gathered feedback through an agile development process. The design and development were cyclical—many times throughout the process, we went back to the drawing board. ResultsThis human-centered approach generated an openly available, web-based app through which patients with MS, their clinicians, and their caregivers can access the site and create an account. Users can enter information about their MS (basic level as well as more advanced concepts), visualize their data longitudinally, access a series of algorithms designed to empower them to make decisions about their treatments, and enter data from wearable devices to encourage realistic goal setting about their ambulatory activity. Agile development will allow us to continue to incorporate precision medicine tools, as these are validated in the clinical research arena. ConclusionsAfter engaging intended users into the iterative human-centered design of the Open MS BioScreen, we will now monitor the adaptation and dissemination of the tool as we expand its functionality and reach. The insights generated from this approach can be applied to the development of a number of self-tracking, self-management, and user engagement tools for patients with chronic conditions.

Abstract

ObjectiveAdvances in medical discoveries have bolstered expectations of precise and complete care, but delivering on such a promise for complex, chronic neurologic care delivery requires solving last-mile challenges. We describe the iterative human-centered design and pilot process for multiple sclerosis (MS) NeuroShare, a digital health solution that brings practical information to the point of care so that clinicians and patients with MS can view, discuss, and make informed decisions together.MethodsWe initiated a comprehensive human-centered process to iteratively design, develop, and implement a digital health solution for managing MS in the routine outpatient setting of the nonprofit Sutter Health system in Northern California. The human-centered codesign process included 3 phases: discovery and design, development, and implementation and pilot. Stakeholders included Sutter Health's Research Development and Dissemination team, academic domain experts, neurologists, patients with MS, and an advisory group.ResultsMS NeuroShare went live in November 2018. It included a patient- and clinician-facing web application that launches from the electronic health record, visually displays a patient's data relevant to MS, and prompts the clinician to comprehensively evaluate and treat the patient. Both patients and clinicians valued the ability to jointly view patient-generated and other data. Preliminary results suggest that MS NeuroShare promotes patient-clinician communication and more active patient participation in decision-making.ConclusionsLessons learned in the design and implementation of MS NeuroShare are broadly applicable to the design and implementation of digital tools aiming to improve the experience of delivering and receiving high-quality care for complex, neurologic conditions across large health systems.

BackgroundIn persons with multiple sclerosis (MS), the Expanded Disability Status Scale (EDSS) is the criterion standard for assessing disability, but its in-person nature constrains patient participation in research and clinical assessments.ObjectiveThe aim of this study was to develop and validate a scalable, electronic, unsupervised patient-reported EDSS (ePR-EDSS) that would capture MS-related disability across the spectrum of severity.MethodsWe enrolled 136 adult MS patients, split into a preliminary testing Cohort 1 (n = 50), and a validation Cohort 2 (n = 86), which was evenly distributed across EDSS groups. Each patient completed an ePR-EDSS either immediately before or after a MS clinician's Neurostatus EDSS evaluation.ResultsIn Cohort 2, mean age was 50.6 years (range = 26-80) and median EDSS was 3.5 (interquartile range (IQR) = [1.5, 5.5]). 5. Staff perceptions of the link between complex trauma and offending behaviour in the youth justice population

Abstract

Introduction: The link between experiences of childhood trauma and youth offending behaviour is well documented. However, the relationship between trauma and offending is complex and few studies have examined the mechanisms that might account for this link. Youth Offending Team (YOT) staff work closely with many young people who have experienced traumas, which may allow them to gain a unique insight into these links. They also have the ability to significantly influence a young person’s future; therefore, an understanding of their perceptions of this cohort is critical. Consequently, this study aimed to obtain a detailed understanding of YOT staff’s knowledge and perceptions of the trauma-offending pathway. Methodology: Ten YOT workers from three YOTs in South Wales engaged in semi-structured interviews. Data was collected and thematically analysed, drawing on Constructivist Grounded Theory principles. Results: Three key themes were identified; ‘Staff perceptions of the mechanisms linking trauma and offending’, ‘Exits from offending’ and ‘The role of YOS and other services’ in supporting these young people. Discussion: This study helped bridge the gap between child welfare and juvenile justice research. It is argued that understanding the mechanisms that exacerbate or mitigate the link between trauma and offending can improve outcomes for young people and wider society. This study provides a detailed understanding of staff perceptions of the links between trauma and offending behaviour, which can help guide our understanding and inform future practice and research in the trauma-offending field.

6. Time Dependence and Density Inversion in Simulations of Vertically Oscillated Granular Layers

Abstract

We study a layer of grains atop a plate which oscillates sinusoidally in the direction of gravity, using three-dimensional, time-dependent numerical solutions of continuum equations to Navier-Stokes order as well as hard-sphere molecular dynamics simulations. For high accelerational amplitudes of the plate, the layer exhibits a steady-state "density inversion" in which a high-density portion of the layer is supported by a lower-density portion. At low accelerational amplitudes, the layer exhibits oscillatory time dependence that is strongly correlated to the motion of the plate. Abstract

Life in Transit: Travel Narratives of the British Governess argues that on entering the profession of governessing, women embarked on a new, more mobile existence of travel and relocation on a local and global scale. At a time when gentlewomen rarely travelled far without a chaperone, governesses left home and travelled unaccompanied across counties, countries and even continents for the purpose of work. Some relocated to wealthy households in Britain, some toured with families on the Continent, and others voyaged out to the colonies to work for expatriates or members of the Eastern aristocracy. Previously, however, scholars have tended to consider the governess in light of her unusual social status between the middle and working class. Studies of this kind do much to highlight the complexity of the governess’s situation, but by developing new theoretical perspectives which focus on the governess’s mobility, this thesis demonstrates how the impact of travel is fundamental to this. Highlighting the interplay between the governess of fact and fiction, Life in Transit defines the ‘governess travel narrative’ as a literary strand present in the canonical novel, and a sub-genre of women’s travel writing. Beginning with a re-reading of the governess novel, it considers Charlotte Brontë’s Jane Eyre (1847) and William Thackeray’s Vanity Fair (1848) to explore the governess’s journey in England. Moving its focus across the Channel, it then examines how the semi-autobiographical governesses of Anna Brownell Jameson’s Diary of and Ennuyée (1826) and Brontë’s Villette (1853) experience life on the Continent. Crossing the border of fact and fiction into the genre of travel writing, the thesis considers the work of the lesser-known Emmeline Lott and Ellen Chennells, and examines governess travel narratives produced at the height of the British Empire. Finally, it analyses the journeys of Sarah Heckford and Anna Leonowens, who travelling in the 1870s and 80s, reached as far as South Africa and Siam, extending the scope of women’s travel and pushing the boundaries of the governess profession. In this way, Life in Transit re-reads the governess’s plight as both a physical and psychological journey in which she attempts to understand her place in the world. Incorporating theories of travel, space, translation and ‘things’ into a framework through which to examine her experience, it builds on Marxist and feminist approaches to the governess’s position. Allowing for further analysis of the governess’s unusual status, this approach shows how, from within the liminal space of her displacement, the governess experiences her life through spatial above social relations, and provides a unique voice in nineteenth-century Britain’s conception of self and world.

12. The Most Effective Way of Delivering a Train-the-Trainers Program: A Systematic Review

Abstract

Introduction: Previous literature has shown that multifaceted, interactive interventions may be the most effective way to train health and social care professionals. A Train-the-Trainer (TTT) model could incorporate all these components. We conducted a systematic review to determine the overall effectiveness and optimal delivery of TTT programs. Methods: We searched 15 databases. Reference lists and online resources were also screened. Studies with an objective follow-up measure collected over 1 week after the intervention were included. The intervention had to be based on a TTT model for health and social care professionals. Results: Eighteen studies met the inclusion criteria. TTT interventions varied greatly, ranging from didactic presentations to group discussions and role-plays. The heterogeneity of the studies and limited data prevented meta-analysis. A narrative review found that the TTT programs in 13 studies helped to increase knowledge, improve clinical behavior, or produce better patient outcomes. One study showed no effect. Three studies showed possible effect and one study showed that a CD-ROM training method was more effective than a TTT training method in improving participants' knowledge. Ratings of the studies' methodologies suggested moderate risk of bias, which limits interpretation of the results. Discussion: There is evidence that using a blended learning approach to deliver TTT programs--combining different techniques such as interactive, multifaceted methods and accompanying learning materials--can help to effectively disseminate and implement guidelines and curricula to health and social care professionals. However, further research is needed to determine the optimum "blend" of techniques. (Contains 2 figures and 1 table.)

14. Development of a core outcome set for quality of life for adults with drug‐resistant epilepsy: A multistakeholder Delphi consensus study.

Abstract

Objective: In 2017, the American Academy of Neurology (AAN) convened the AAN Quality Measurement Set working group to define the improvement and maintenance of quality of life (QOL) as a key outcome measure in epilepsy clinical practice. A core outcome set (COS), defined as an accepted, standardized set of outcomes that should be minimally measured and reported in an area of health care research and practice, has not previously been defined for QOL in adult epilepsy. Methods: A cross‐sectional Delphi consensus study was employed to attain consensus from patients and caregivers on the QOL outcomes that should be minimally measured and reported in epilepsy clinical practice. Candidate items were compiled from QOL scales recommended by the AAN 2017 Quality Measurement Set. Inclusion criteria to participate in the Delphi study were adults with drug‐resistant epilepsy diagnosed by a physician, no prior diagnosis of psychogenic nonepileptic seizures or a cognitive and/or developmental disability, or caregivers of patients meeting these criteria. Results: A total of 109 people satisfied inclusion/exclusion criteria and took part in Delphi Round 1 (patients, n = 95, 87.2%; caregivers, n = 14, 12.8%), and 55 people from Round 1 completed Round 2 (patients, n = 43, 78.2%; caregivers, n = 12, 21.8%). One hundred three people took part in the final consensus round. Consensus was attained by patients/caregivers on a set of 36 outcomes that should minimally be included in the QOL COS. Of these, 32 of the 36 outcomes (88.8%) pertained to areas outside of seizure frequency and severity. Significance: Using patient‐centered Delphi methodology, this study defines the first COS for QOL measurement in clinical practice for adults with drug‐resistant epilepsy. This set highlights the diversity of factors beyond seizure frequency and severity that impact QOL in epilepsy. [ABSTRACT FROM AUTHOR]

24. Studies of some magneto-optical properties of ferromagnetic films

Abstract

This thesis deals with the detection and measurement of the Faraday and Voigt effects in monocrystalline (110) films of ferromagnetic nickel. The Faraday and Voigt rotations were measured as a function of four parameters: applied field, specimen thickness, wavelength and polarization azimuth of the incident light. Nickel films were grown epitaxially on a monocrystalline copper layer deposited on rock salt. The specimens were selected, using optical and electron microscopy, to be as near as possible to the theoretical ideal of pure, parallel-sided, monocrystalline films. The thickness of each specimen was measured using a multiple beam Fizeau fringe technique. The magneto-optical rotations were measured using a photoelectric polarimeter, accurate to [one-sixth] of arc, which is described6later in the thesis. The saturation dispersion and azimuthal behaviour of the Faraday and Voigt effects are discussed and compared with available theory.

Abstract

Intravenous gamma globulin and erythropoietin may be effective treatment for sickle cell aplastic crisis in children whose parents oppose blood transfusions. Sickle cell aplastic crisis occurs when there is a deficiency of all formed elements of the blood. Patients in sickle cell aplastic crisis are usually treated with blood transfusions. The case of an 11-month-old infant whose mother was a Jehovah's Witness is discussed. The infant was initially admitted to the hospital with a fever and treated for a suspected urinary tract infection. Her hematocrit, a measure of the red blood cells, was low at 15%. The following day her hematocrit dropped to 10%. On physical exam, the infant was pale but did not have an enlarged liver or spleen. Infection with parvovirus B19 was suspected. The infant was treated with 1 gram per kilogram (kg) of body weight of gamma globulin, an immunizing agent found in blood plasma. She was also treated with 100 units/kg of erythropoietin. Erythropoietin stimulates and regulates the production of hemoglobin, the major component of blood. The infant was discharged on the fifth day and received further erythropoietin treatments on the seventh and 10th days.

Abstract

The world's busy air traffic corridors pass over hundreds of volcanoes capable of hazardous explosive eruptions. The risk to aviation from volcanic activity is significant. In the United States alone, aircraft carry about 300,000 passengers and hundreds of millions of dollars of cargo near active volcanoes each day. Costly disruption of flight operations in Europe and North America in 2010 in the wake of a moderate-size eruption in Iceland clearly demonstrates how eruptions can have global impacts on the aviation industry. This book examines national and international efforts, and partnered with the U.S. Geological Survey Volcano Hazards Program, which is playing a leading role in the global effort to reduce the risk posed to aircraft by volcanic eruptions.

Abstract

This mixed-methods study explored the volunteer work of newly-certified, unemployed teachers in Ontario by asking the research question: How do newly-certified, unemployed teachers understand their volunteer work in schools? Quantitative data pertaining to unemployed volunteers and their volunteer behaviour from the National Survey of Learning and Work was analyzed using descriptive statistics in order to provide context for qualitative interviews. Six newly-certified, unemployed teachers completed a semi-structured interview that asked about volunteer tasks, learning, and access to teaching. Findings demonstrated that volunteer-teachers perform a wide range of tasks, engage in informal learning that is both volunteer-related and job-related; and perceive that volunteer work helps them gain access to teaching through growing a professional network and providing insight into the hiring process. Furthermore, how teachers understand their volunteer work is differentiated, seemingly dependent on having previously paid teaching work and pre-existing relationships in a school community. Recognition of a distinct type of volunteer work in the contemporary Canadian volunteer landscape is also presented. Policy and practice implications include disseminating this information for aspiring and unemployed teachers in Ontario, as well as recognizing the adult learning that takes place at the site of volunteer work.
