Abstract

The objective of this study is to explore the possibility of treating APL patients fully as outpatients. A total of 21 consecutive APL patients were identified over 30 years in the Centro de Hematología y Medicina Interna de Puebla, at Clínica Ruiz, but only 17 were studied, treated as outpatients, and followed for at least 1 month; they were observed for median of 95 months, their median age was 27 years and all were treated with ATRA, prednisone, and adriamycin as outpatients. Treatment was completed on an outpatient basis in 15/17 cases. Molecular remission was achieved in 16/17 patients. The median follow-up was 95 months (IQR 19 - 360). The median OS and LFS were not reached, and the 12-month LFS was 94%. We have confirmed that APL can be treated entirely on an outpatient basis: this observation is of utmost relevance in a resource-limited setting, such as those prevailing in low- and middle-income countries. Abstract

Background and Objectives: hypochloremia is an emerging risk factor for mortality in patients with chronic kidney disease. The pathophysiological mechanisms of this finding are not very clear. Some studies suggest the influence of inflammation as a synergistic factor, so we set out to analyse the association of a novel C-reactive protein-to-serum chloride ratio (CRP/Cl−) with the prognosis of maintenance haemodialysis patients and to assess its relationship with fluid status and body composition measured by bioimpedance. Materials and Methods: the present work is a retrospective cohort study of maintenance haemodialysis patients from our chronic outpatient haemodialysis programme between 1 January 2022 and 31 December 2022. (n = 281). Survival time was collected for all patients and analysed using the Kaplan–Meier method. A Cox proportional hazards regression model was used to evaluate survival probabilities. Variables included in the model were selected using a stepwise selection procedure based on the corrected Akaike information criterion (AICc), which balances model fit and complexity. Results: during a median follow-up of 306 days, 34 patients died. Patients in the fourth quartile of the CRP/Cl− (>0.118 mg/mEq) had higher overall mortality (log-rank test, p = 0.0011). In the Cox multivariate analysis, the variables significantly associated with higher mortality were higher modified Charlson index (MCI), lower body surface area (BSA), lower interdialytic weight gain (IDWG), and higher CRP/Cl− ratio. The latter variable was independently associated with higher overall mortality (adjusted hazard ratio = 1.027; 95% confidence interval [CI], 1.000–1.055 p = 0.0469). Conclusions: Higher CRP/Cl− ratio values were associated with higher all-cause mortality in our maintenance haemodialysis patients. [ABSTRACT FROM AUTHOR]

Abstract

El uso de la IA como herramienta didáctica en la educación primaria presenta prometedoras oportunidades para enriquecer el proceso educativo y cultivar el pensamiento crítico en los estudiantes. Sin embargo, su implementación efectiva requiere un enfoque reflexivo y cuidadoso que considere tanto los beneficios como los desafíos asociados con esta tecnología emergente. El objetivo principal del presente trabajo consiste en analizar como puede ser utilizada la IA como herramienta para el desarrollo del pensamiento crítico en las aulas de educación primaria, a través de su integración como herramienta didáctica en el aula. Con el objetivo también de desafiar y estimular las habilidades cognitivas y metacognitivas de los estudiantes, se propone el uso de algoritmos inteligentes y análisis de datos proporcionados por sistemas de IA mediante la utilización de actividades y ejercicios diseñados específicamente para ello, proporcionando así una retroalimentación inmediata y personalizada sobre el desempeño de los estudiantes, para fomentar la reflexión y el autoaprendizaje, e identificar áreas de mejora en el desarrollo del pensamiento crítico. Será necesario que la IA se adapte a cada contexto y las necesidades del alumnado, teniendo en cuenta los principios inclusivos, y asegurando la participación activa de todos los estudiantes en el proceso educativo. Confiamos en que esta propuesta contribuya a que los docentes puedan conocer las implicaciones que tiene el uso efectivo de la IA como herramienta didáctica para el desarrollo del pensamiento crítico en el contexto escolar. Por último, se considera que este trabajo es un punto de partida para la colaboración entre profesionales de la educación que busquen la innovación y mejora continua de la práctica educativa mediante el uso de herramientas emergentes como es la IA y dejen de verla como una amenaza para la enseñanza.

38. Social Inequalities in Fertility in Women Residing in Urban Neighbourhoods in Spain: A Multilevel Approach

Abstract

Background Reproductive health inequalities tend to be more marked in large cities because they include neighbourhoods with unequal social and community networks, resources and opportunities. The aim was to describe social inequalities in fertility patterns among women who gave birth between 2007 and 2016 in the city of Barcelona (Spain) by jointly evaluating the effect of individual and socioeconomic neighbourhood characteristics. Methods We conducted a multilevel study of women's individual characteristics (age, educational attainment, and country of origin) and neighbourhood characteristics (disposable household income, percentage of unemployment and percentage of foreigners). Results Multilevel modelling revealed significant variability in fertility across neighbourhoods after adjustment for individual explanatory variables. The fertility rate (FR) was higher in better educated women (adjusted relative risk: aRR 2.76), those aged 30-39 years (aRR 2.13), and in those born in Spain in relation to their respective reference groups. The FR was lowest in women with upper to post-secondary, non-tertiary education (aRR 0.86) who were born in high income countries (aRR 0.57). Women living in neighbourhoods with a low income (aRR 1.46) and with a higher unemployment (aRR 1.33) were more likely to have children. In contrast, women living in neighbourhoods with a lower percentage of foreigners had the lowest FR (aRR 0.81). Conclusions The neighbourhood's characteristics played an important role in fertility patterns, independently of women's individual characteristics. At the contextual level, the highest FR was found in the poorest settings, highlighting inequalities. Abstract

[EN] Dataset obtained from the CSIC's Library & Archive Network, which includes the digitalized documents from the collection “Manuscritos” (“Manuscripts”) preserved at the Tomás Navarro Tomás library belonging to the Centro de Ciencias Humanas y Sociales (CCHS) of the CSIC. The now disappeared Centro de Estudios Históricos (CEH) compiled a collection of miscellaneous documents. The complete collection contains 195 documents from private archives that were bound and gathered in codices and dossiers. It comprises mainly notary documents, testaments, property titles, papers related to administration and property management and accounting books, but it also includes work of a historical and literary character. This V1 of the dataset contains the 135 documents that have been digitalized to date., [ES] Dataset procedente del catálogo de la Red de Bibliotecas y Archivos del CSIC que recoge los documentos digitalizados del fondo “Manuscritos” conservados en el Archivo de la biblioteca Tomás Navarro Tomás del Centro de Ciencias Humanas y Sociales (CCHS) del CSIC. Se trata de una colección de documentos misceláneos reunidos durante la etapa del Centro de Estudios Históricos (CEH). La colección al completo está formada por 195 documentos agrupados en legajos y códices encuadernados procedentes de archivos privados. Predomina la documentación de carácter notarial, testamentos y títulos de propiedad, de administración y gestión de propiedades, los libros de cuentas pero también hay obras de carácter histórico y literario. Este dataset contiene los 135 documentos digitalizados hasta el momento.

Additionally, described is how the Library seeks to prepare its users so that they will be well positioned when, in the near future, open access to their publications and research data form part of the scientific evaluation.
