The purpose of this case is to illuminate the nuances of one of the most important roles on a school campus—the role of the assistant principal; with a focus on Black, Indigenous, People of Color (BIPOC) women assistant principals. This case is for educational leaders interested in understanding the power dynamics between an assistant principal and multiple stakeholders, the nuances of the role of assistant principal, creating opportunities for networking and professional development, and viewing the assistant principal as an asset and integral part of the success of the school. Abstract: Ultrasound-assisted extraction (UAE) and direct immersion solid-phase microextraction (DI-SPME) were evaluated for the monoterpenic compounds determination in wine samples. The wine extracts obtained were analyzed by gas chromatography–mass spectrometry (GC–MS). The optimization of the variables affecting UAE and SPME methods was carried out in order to achieve the best extraction efficiency. Both UAE and SPME are quantitative (recoveries in the range 93–97% and 71.8–90.9%, respectively), precise (coefficients of variation below 5.5%), sensitive (limits of detection between 30–39μgL−1 and 11–25μgL−1, respectively) and linear over one order of magnitude. The application of both methods to red wine samples showed that UAE provided higher extraction of monoterpenic compounds than SPME. Although SPME remains an attractive alternative technique due to its speed, low sample volume requirements and solvent free character. [Copyright &y& Elsevier]

Introduction: Subependymal giant cell astrocytomas (SEGA) appear in 5-20% of patients with tuberous sclerosis complex (TSC) and are the most common brain tumours in TSC. They are benign tumours, of a glioneural stock, that develop mainly in the first two decades of life, generally close to the foramen of Monro, and can trigger hydrocephalus and intracranial hypertension. It is one of the leading causes of death in TSC. Recently mTOR inhibitors have proved to be a therapeutic alternative to surgical excision. AIM. To describe our experience of using everolimus to treat patients with SEGA and TSC., Patients and Methods: We conducted a prospective study of the responses of patients with TSC and at least one SEGA undergoing growth., Results: Three females and three males with a mean age of 12.3 years received treatment. One patient had previously undergone surgery due to SEGA with hydrocephalus. The maximum mean diameter of the SEGA on beginning treatment was 15.3 mm (range: 11.3-24.8 mm). Treatment was established with everolimus, 2.5 mg/day administered orally in patients with a body surface area < 1.2 m2, and 5 mg/day in patients with a body surface area > 1.2 m2. Two patients presented hypertriglyceridemia; one, anorexia; another, a mouth ulcer; and one, amenorrhoea. The mean reduction in the volume of the SEGA at three months of treatment was 46%, and the reduction remained steady in later control examinations (6-25 months)., Conclusions: Treatment with everolimus reduces the size of SEGA associated with TSC with an adequate safety profile, and constitutes an alternative to surgery in certain cases.

We report the case of a 9-year-old boy with delayed postanoxic encephalopathy. The patient improved after a period of coma and adult respiratory distress syndrome caused by benzodiazepine intoxication. Initial improvement appeared to be complete, but 9 days after discharge from the pediatric intensive care unit, extrapyramidal rigidity of the left arm became manifest. This event was followed by progressive neurologic impairment with involuntary, non-repetitive, but occasionally stereotyped movements, affecting distal, proximal and axial musculature in various combinations. On physical examination, passive movement of the left arm showed a plastic, lead-pipe rigidity with a ratchet-like cogwheel quality. Magnetic resonance imaging (MRI) was performed. Axial T 2 -weighted images of the brain showed characteristic signs of an ischemic area in the basal ganglia which were more evident in the right region, a finding consistent with anoxic-ischemic encephalopathy. Delayed postanoxic encephalopathy is a relatively uncommon and unexplained phenomenon which becomes manifest after an initial hypoxic-ischemic event that causes stupor or coma. The pathogenesis is unknown. Exceptionally, this syndrome can affect the basal ganglia more than the cerebral cortex and white matter. Initial improvement is followed after a variable period of time by relapse, progressing for weeks to months until the patient is left with permanent sequelae.
