Abstract Background People with mental disorders have a higher prevalence of physical illnesses and reduced life expectancy as compared with the general population. However, there is a lack of knowledge across Europe concerning interventions that aim at reducing somatic morbidity and excess mortality by promoting behaviour-based and/or environment-based interventions. Methods and design HELPS is an interdisciplinary European network that aims at (i) gathering relevant knowledge on physical illness in people with mental illness, (ii) identifying health promotion initiatives in European countries that meet country-specific needs, and (iii) at identifying best practice across Europe. Criteria for best practice will include evidence on the efficacy of physical health interventions and of their effectiveness in routine care, cost implications and feasibility for adaptation and implementation of interventions across different settings in Europe. HELPS will develop and implement a "physical health promotion toolkit". The toolkit will provide information to empower residents and staff to identify the most relevant risk factors in their specific context and to select the most appropriate action out of a range of defined health promoting interventions. The key methods are (a) stakeholder analysis, (b) international literature reviews, (c) Delphi rounds with experts from participating centres, and (d) focus groups with staff and residents of mental health care facilities. Meanwhile a multi-disciplinary network consisting of 15 European countries has been established and took up the work. As one main result of the project they expect that a widespread use of the HELPS toolkit could have a significant positive effect on the physical health status of residents of mental health and social care facilities, as well as to hold resonance for community dwelling people with mental health problems. Discussion A general strategy on health promotion for people with mental disorders must take into account behavioural, environmental and iatrogenic health risks. A European health promotion toolkit needs to consider heterogeneity of mental disorders, the multitude of physical health problems, health-relevant behaviour, health-related attitudes, health-relevant living conditions, and resource levels in mental health and social care facilities.

Currently, one out of five Romanian people is affected by mental health problems, and at least 250000 Romanians suffer from a severe psychiatric disorder. Furthermore, it is estimated that in 2020 mental disorders will rank first regarding morbidity, overtaking cancer and cardiovascular diseases. The numeric insufficiency and deficient training of the specialists in rehabilitation and social inclusion have direct and indirect consequences for people with disabilities, for their families and for society as a whole. In 2005 it was founded the Romanian Association of Social Psychiatry, with the purpose of preparing, promoting and implementing projects at all levels for sustaining the community mental health assistance for people with psychiatric disorders, and also to support their social inclusion. The creation of a national network of specialists interested in implementing rehabilitation and social inclusion procedures will facilitate the experience exchange and knowledge transfer, and will become a basis for building partnerships and for mutual support in accessing and implementing community mental health services. Rehabilitation connects the personal relevant goals of the person to services that reduce disability and promote recovery. Achieving this goal requires the integration of medical and psychosocial models of interventions through a multidisciplinary team. In Romania the development of community alternatives to hospitalization and recovery-oriented services has been one of the main aims since 2005, as indicated by the Action Plan and Strategies of Ministry of Health. Despite that, little improvements were made, and health care is still mostly hospital based. In these problematic contexts, and drawing on previous experience of ACT interventions, an experimental case management program was implemented at Campulung Moldovenesc Community Mental Health Center in order to bridge the existent gap between hospital and community and support people with mental health problems not just improve their access to treatment services but also to improve their overall functioning and health. We have evaluated the impact of a Mental Health Program implemented Campulung Moldovenesc in comparison with standard treatment, on reducing symptoms and improving the overall level of functioning in a group of 91 participants diagnosed with depression and schizophrenia. Specifically compared with standard treatment, case management was associated with greater improvement in symptoms, fewer of hospital days, increased number of contacts between clients and professional staff, decrease of dropout rates from mental health services, greater improvement in the global and social functioning level, decreasing burden of care and increasing client and his family satisfaction with services and last, but not least, with lower total cost of care. Patients with depressive disorder may exhibit similar clinical symptoms, but their personal descriptors can be sometimes quite different in terms of existential expectations, basic needs, and motivational structure. And while it is true that medication can be widely used to treat same therapeutic class in order to restore neurotransmitter homeostasis, psychotherapy approach must take into account all the symptoms which are unique for each individual. In this regard, our expertise has emphasized two major needs to point out a patient with depressive disorder's response to therapy: the need for community integration, and the need for intimacy. We aim to demonstrate the finding that introverts (defined by a greater need for intimacy) tend to react better to individual therapy as opposed to extroverts (defined by a greater need for community integration) who tend to prefer group therapy. The subjective perception of quality of life of patients with chronic impairment depends on many influences coming from both mental and physical variables. The patient needs to adapt to new contexts such as loss of social roles, changes in the future plans, insecurity, self-esteem impairment. The aim of this study was to highlight the influence of mental variables over the perception of life quality in patients with multiple sclerosis. In order to acquire the data, we applied the following instruments: The State-Trait Anxiety Inventory (S.T.A.I.), Beck Depression Inventory (B.D.I.), Eysenck's Personality Inventory (E.P.I.), The Medical Outcomes Study 36-Items Short Form Health Survey (SF-36) and The Mini-Mental State Examination (M.M.S.E.). The research was performed on a group of 60 patients, diagnosed with multiple sclerosis. Subjects were informed that participation to the study required filling the test forms (depression, anxiety, emotional stability, cognition, and life quality inventories) and were evaluated individually. The data were statistically analyzed. The study revealed that the mental dimension of the life quality perception could be estimated by a model including depression and anxiety-state cumulated with extraversion and emotional stability, the latter two having a weaker power of prediction. Cognitive variables (orientation, attention, focus, memory and language) didn't prove their prediction significance on perceived quality of life. The study confirmed partially the hypothesis. The small study group and the medium level of disability did not allow us to generalize our conclusions, but the research opened up the perspective for further investigations. [ABSTRACT FROM AUTHOR]

The present study aims at highlighting the effects of introducing a supportive therapy intervention to develop self-esteem. Thus, we analyzed a group of 142 patients with recurrent depression who met the inclusion criteria and were randomly assigned into two study groups in equal numbers, following two different therapeutic interventions, namely: group A of 71 patients undergoing a 4-month social therapy program with a group session per week and a monthly social meeting consisting of techniques to strengthen the ego, positive reinforcement and self-valorization, and group B, which followed the support group psychotherapy. Patients in both groups followed the specific psychiatric treatment. We used the Rosenberg self-esteem scales, Hamilton Depression Rating Scale and SAS-SR for social adaptation. We found that the therapeutic intervention based on the development of self-esteem would have better results with respect to both self-esteem and social functioning dimensions than the intervention without this component. Regarding the level of depression, the intervention based on self-esteem development did not produce a statistically significant difference in comparison with the other group. Self-esteem score represents the predictor of the level of social functioning. Furthermore, the age of patients also has a predictive value on the response to therapy. In conclusion, self-esteem is a parameter of mental health that is worth considering when thinking about the social functioning of the patient, in relation to the level of depression and leading to the personal feeling of dignity.
