A fenotipagem eritrocitária pré-transfusional é um importante procedimento para aumentar a segurança das transfusões sangüíneas, sendo realizada rotineiramente no Hemocentro Regional de Uberaba-MG (HRU) desde 1996. O presente trabalho tem como objetivo geral avaliar a freqüência de anticorpos antieritrocitários irregulares em politransfundidos, de 1997 a 2005. Através de estudo retrospectivo foram levantados dados no arquivo do HRU de todos os pacientes aloimunizados, realizou-se análise estatística descritiva e comparam-se as proporções pelo teste "Z". Dos 23.220 transfundidos no período, com média de 5,7 transfusões por paciente, observou-se a ocorrência de aloimunização em 173 (0,75%). Os sistemas Rh e Kell juntos tiveram freqüência superior a 70%. A síndrome de Plummer-Vinson é caracterizada por disfagia cervical, deficiência de ferro e presença de membrana esofágica. Neste estudo, relatam-se dois casos dessa síndrome em irmãos adolescentes. Eles não obtiveram aumento dos níveis hematimétricos após reposição com ferro oral, o que, associado à eletroforese de hemoglobinas, sustentou o diagnóstico de talassemia concomitante. Devido ao quadro dos filhos, os pais foram também submetidos à eletroforese de hemoglobinas cujo diagnóstico do pai foi talassemia alfa/beta menor e da mãe, talassemia alfa menor. Os irmãos tiveram disfagia refratária e necessitaram de dilatação endoscópica. Ambos necessitaram de terapia com ferro venoso com melhora dos níveis hematimétricos.Plummer-Vinson syndrome is characterized by cervical dysphagia, iron deficiency and the presence of esophageal membranes. We report two cases of this syndrome present in adolescent brothers with associated thalassemia. After oral iron therapy, their hematimetric levels showed no increase, which associated with the results of hemoglobin electrophoresis, sustained the diagnosis of thalassemia. Due to the condition of the children, the parents were submitted to hemoglobin electrophoresis examinations; the father was diagnosed as having minor alpha/beta thalassemia and the mother as minor beta thalassemia. Both patients suffered from refractory dysphagia and required endoscopic dilatation. They both underwent venous iron therapy, which improved the hematimetric levels.

ABSTRACT Introduction: Sickle-cell anemia (SCA) is the most severe form of sickle-cell disease, and is characterized by homozygous hemoglobin S (α2βS2). Objective: Determine the haplotypes frequency in patients with SCA and their correlation with clinical and hematological profile. Method: We performed a retrospective descriptive study by reading the charts and a cross-sectional study for molecular analysis to determine the haplotypes of the gene βS globin in 61 patients with sickle-cell anemia (SS) by polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP), using restriction endonucleases Xmn I, Hind III, Hinf I and Hinc II for analysis of six polymorphic sites in the beta cluster. Result: The genotypes were Central African Republic (CAR)/CAR (60.8%), CAR/Benin type (BEN) (13.1%), CAR/ Cameroon type (CAM) (1.6%), CAR/atypical (ATP) (13.1%), BEN/BEN (13.1%), BEN/ATP (4.9%) and ATP/ATP (3.3%). Among the analyzed chromosomes, 64.8% were CAR type, 22.1% were BEN, 12.3% ATP and 0.8% CAM. Levels of fetal hemoglobin (HbF) were significantly lower in CAR/CAR than in ATP/ATP, BEN/ATP and CAR/BEN. No association was observed between the different genotypes and clinical manifestations. Conclusion: Despite the lack of association between genotypes and clinical profiles, higher frequency of clinical events was observed in patients with at least one type of CAR chromosome. A significant association was also observed between lower average levels of HbF and CAR/CAR genotype compared to other genotypes.
