Cardiac implantable electronic devices (CIEDs) store information continuously; however, the log of these devices is rarely analyzed in forensic practice. We retrospectively reviewed all cases referred for CIED interrogation by the Los Angeles County Department of Medical Examiner-Coroner between 2001 and 2020. According to the Department's practice, CIED interrogation may be requested for decedents in which details or cause of death are not clear from autopsy and clinical history. The CIED analysis was considered informative for the coroner's investigation either if it detected an arrhythmia or malfunction likely related to decedent's terminal event or if it was essential to determine time of death or identity of decedent. A total of 57 CIEDs were evaluated during the 20-year period. In almost half of cases (26/57: 45.6%), device analysis was informative for coroner's investigation. Arrhythmias likely related to terminal event were commonly detected (21/57: 36.8%). Device malfunction was identified as the likely cause of death in almost 10% of decedents (5/57: 8.8%), including three cases of battery depletion (3/57: 5.3%), one case of misclassification of ventricular tachycardia as supraventricular tachycardia with failure to deliver therapy (1/57: 1.7%), and one case of lead failure due to a broken pacing wire (1/57: 1.7%). Not infrequently, CIED interrogation was essential for determination of time of death (9/57: 15.8%), and there was one case (1/57: 1.7%) in which interrogation was essential for identifying the decedent. Our study shows that postmortem CIED interrogation can provide unique information regarding mechanism and time of death, and decedent's identity.

Despite an extensive number of existing methods, age estimation of human remains is still an unsolved matter in the field of forensic anthropology, especially when it comes to mature adults. The specific aim of this work was to propose a combined method for age estimation, for forensic purposes, by coupling the Suchey-Brooks method and the measure of the pubic bone density. For this purpose, we used an independent test sample comprising 339 CT scans of living individuals aged 15 to 99 years old. Measurement of bone density and staging according to the Suchey-Brooks phases were performed, followed by estimation of ages based on a combined method and an existing virtual reference sample. Results highlighted a significant negative correlation between bone density and age. Good accuracy was obtained for the measurement of pubic bone density for age estimation of men and women, especially concerning mature adults, with an absolute error ranging from 9 to 16 years for all individuals. The authors propose a practical combined method consisting of, first, allocating phases according to the scannographic approach of the Suchey-Brooks method. For phases I to IV, the age estimation is given using the Suchey-Brooks method. For phases V to VI, the pubic bone density measurement is used. Further study will be needed to assess the reproducibility of these results on cadavers and dry bones, as the post-mortem process could interfere with the measurement of mineral bone density.

Resume Dans un certain nombre de situations, les sequelles secondaires a une pathologie carcinologique necessitent une evaluation des prejudices dans un but indemnitaire. C’est notamment le cas quand des tiers responsables ont ete reconnus a l’origine de la maladie: pathologies professionnelles en cas de faute inexcusable de l’employeur, exposition a un risque radio induit par exemple dans le cadre de la reparation integrale des prejudices subis par les victimes des essais nucleaires francais ou enfin dans les sequelles liees a un retard diagnostic. Cet article n’aborde pas l’imputabilite des pathologies cancereuses a un evenement, mais propose une adaptation des methodes d’evaluation des prejudices, pour tendre vers une reparation integrale des dommages.

This study aims to determine if resting-state functional connectivity may represent a marker for the progression of posttraumatic stress disorder (PTSD) in women victims of sexual assault. Participants were 25 adult women recruited three weeks following exposure to sexual assault (T1) and 19 age-matched healthy, non trauma-exposed controls (HC). Among the victims, 10 participants met (PTSD) and 15 did not meet (trauma-exposed controls, TEC) DSM-IV criteria for PTSD six months post-trauma (T2). At both visits, patterns of intrinsic connectivity, a measure of network centrality at each voxel of the brain, were derived from resting-state functional magnetic resonance imaging. Compared to both the HC and TEC groups, victims who developed PTSD at T2 showed higher centrality in the right middle/superior occipital gyrus at T1, while reduced centrality of the posterior cingulate cortex (PCC)/precuneus at T1 was found for the TEC group, compared to the HC group only. There were no differences in intrinsic connectivity at T1 between the TEC and PTSD groups. There were no significant between-group differences in intrinsic connectivity at T2, and no significant group-by-time interaction. This study indicates that increased occipital centrality three weeks post-trauma exposure may represent a marker of the later development of PTSD. On the other hand, reduced centrality of the PCC/precuneus may represent a marker of resilience to trauma exposure.

In forensic anthropology, age estimation is a major element in the determination of a biological profile and the identification of individuals. Thus, many anatomical structures have been studied, such as the pubic symphysis, which is a source of major interest due to its late maturation. One of the most well-known methods of assessment is the Suchey-Brooks (SB) system based on the morphological characteristics of the pubic symphysis. The aim of this study was to propose linear regression formulae in order to deduce chronological age from bone density, using both Hounsfield unit (HU), and mean bone density (mBD) values of the pubic symphysis. Moreover, we intended to test the reliability and then to explore the feasibility of using HU instead of mBD values for age estimation. We built retrospectively a reference sample of 400 pubic symphyses using computed tomography at a French hospital and a test sample of 120 pubic symphyses. Equations were created to establish linear regression models for age estimation. Inaccuracy and bias were calculated for individuals aged more or less than 40 years. We highlighted homogeneous mean absolute errors for both HU and mBD values, most of them being less than 10 years. Moreover, we reported a moderate overestimation for younger individuals and a very small underestimation for older individuals. This study proposes a correlation between the bone density and age of individuals with a valuable level of reliability. Finally, HU measurements seem to be suitable for linking bone density with the age of individuals in forensic practice.

Forensic age estimation is a challenging field in forensic sciences because of the increase of migratory flows. Medicolegal age assessment is a key point because it has many implications for authorities. Dental age estimation is an essential part of the global age assessment. The aim of this study was to evaluate and test the accuracy of Cameriere’s cutoff values of the third molar maturity index (I3M) in assessing legal adult age of 18 years in a French population. The sample was constituted of 431 orthopantomograms performed between January 2014 and August 2017 on patients aged between 14 and 22 years. The reproducibility and repeatability of the method were high. Age distribution gradually decreases as I3M increases in both sexes. 0.08 seemed to be the best I3M cutoff. For females, the sensitivity and specificity of the test were 74.51% and 88.23%, respectively. The sensitivity and specificity for males were 92.19% and 88.35%, respectively. The accuracies were 80.74% for female, 90.57% for male. Estimated post-test probabilities were 0.879 for female and 0.899 for male. To conclude, the specific cutoff value of I3M ˂ 0.08 may be a useful additional tool in discriminating adults and minors in French population.

Aim of the work In forensic anthropology, the estimation of age at death is required to establish a biological profile and facilitate individual identification. The pubic symphysis represents one of the most studied skeletal structures due to its late maturation. In this respect, the Suchey-Brooks (SB) system is one of the most commonly used methods of assessment based on the morphological characteristics of the pubic symphysis. However, the SB method has certain drawbacks, including frequently inaccurate estimation for individuals over 40 years old. The main aim of this work was to study the inter-observer agreement in the SB method then to test the errors of estimation by three observers with heterogeneous experience from a computed tomography sample for individuals aged between 15 and 98 years old. Patients and methods We retrospectively studied pubic bones from 50 males and 50 females undergoing clinical multislice computed tomography (MSCT) in a French hospital between November 2017 and April 2018. Results The inter-observer reliabilities, with or without the early and advanced patterns, were excellent between the observers (kappa values between 0.8 and 0.95). By focusing on the first three phases (I–III) versus the last three phases (IV–VI), there was no significant difference between the inter-observer reliabilities (kappa values between 0.44 and 0.85). Conclusion The inaccuracy rose with age, showing a growing underestimation from 40 years old and beyond. Besides, we highlighted a moderate overestimation for younger individuals. These errors of estimation were slightly dependent on the experience of the observer.

In a few situations, the consequences secondary to a carcinological pathology require an assessment of damages for compensatory purposes. This is particularly the case when liable parties have been found to be at cause of the disease: occupational pathologies in the case of inexcusable employer's fault, exposure to a radioactive risk, for example in the context of full compensation for damages suffered by the victims of nuclear experiments performed by France, or lastly, in the after-effects of late diagnosis. This article does not discuss the imputability of cancer pathologies to an event, but it proposes an adaptation of methods for assessing damages, in an attempt to provide full compensation for damages.

Forensic age estimation of a living individual is frequently performed in clinical forensic medicine. Medicolegal physicians are usually called upon by the authorities to estimate the age of a living person requesting asylum or to determine whether a suspected offender is subject to juvenile or adult criminal law. Dental age is one of the parameters studied to estimate biological age. Several methods are used, and among these, analysis of the eruption of the third molar (M3) on an orthopantomogram (OPG) is one of the common methods. The objective of the study was to analyse the contribution of M3 eruption to age estimation, in particular with regard to the threshold of 18 years of age, in a sample of individuals examined in a French university hospital. The stage of wisdom tooth eruption of all individuals aged between 15 and 24 years, who had an OPG performed between 1 January 2013 and 31 December 2015, was interpreted using a three-stage scoring system. In total, 557 OPGs (340 males and 217 females) were included. None of the stage scores allowed a distinction between minors and majors, for either sex. Overall, 85% of females and 98% of males with four M3s in stage 3 (complete emergence in the occlusal plane) were majors.

International audience; Age at death estimation is a major part of forensic anthropology, but is often poor in the latter decades of life and should benefit from specific population standards. The aim of this study was to test a virtual reference sample with a uniform age distribution in order to improve the accuracy of age estimation for individuals over 40~years of age. We retrospectively built a random virtual reference sample of pubic symphyses from 1100 clinical cases using computed tomography at two French hospitals; this was compared with a test sample (pubic symphyses from 75 corpses undergoing post-mortem computed tomography at a French forensic department) and with the sample originally used in the Suchey-Brooks (SB) system. Inaccuracy and bias were calculated and the proportions of cases in which the real age fell within~the estimated age \textpm~1 standard deviation were calculated. Compared to using the SB sample, using our French sample resulted in a lower inaccuracy for males over 55~years and bias showed that that for males aged 56 to 70~years were less underestimated. Compared to using the SB sample, using our French sample resulted in a lower inaccuracy for females over 70~years and bias showed that that for females aged 56 and older were less underestimated. This study presents a large data set of pubic symphyseal phases from a French virtual sample that allows for improving age estimation accuracy at death, particularly for individuals over 40~years. This kind of material can be useful to improve the age estimation accuracy in a specific region. However, the reliability remains poor and the variability of pubic symphysis morphology related to bone degeneration seems to be an unavoidable limit of the method.

Hypothermia is a potentially lethal pathology whose in vivo diagnosis is relatively easy, based on the observation of a body temperature below 35̊C. However, its post mortem diagnosis is much more complex because of a thermal decrease occurring systematically after the death, which renders most often uninterpretable the body temperature as a diagnostic argument for hypothermia. Moreover, none of the macroscopic or microscopic autopsy elements described as suspicious of hypothermia are pathognomonic of a hypothermic death. Currently, this diagnosis arises in the light of an association of concordant arguments including these autopsic and histological elements but also some biomarkers. The aim of this article is to illustrate by a practical case the diagnostic complexity of a death by hypothermia through the evocative elements currently described, by emphasizing the reflection on some biomarkers confirmed or under study.

Resume Introduction La taille est l’un des elements d’identification en medecine legale qui doit etre estime lorsque l’etat du corps n’en permet pas la mesure. La plupart du temps, cette estimation s’appuie sur les equations de Trotter et Gleser (ETG) etablies, il y a 70 ans sur une population americaine a partir de la mesure d’os longs d’individus de taille connue. L’objectif principal de notre etude etait de comparer des mesures scanographiques d’os longs effectuees sur des corps d’individus francais autopsies au modele issu des ETG puis de comparer la taille estimee par ces equations a la taille reelle. Materiel et methode Nous avons inclus l’ensemble des corps ayant beneficie d’une autopsie et d’un scanner a l’institut medicolegal de Tours en 2017. Nous avons mesure les os longs sur les scanners et les avons compares au modele des ETG. La taille estimee a ensuite ete comparee a la taille mesuree. Resultats Au total, 99 hommes et 43 femmes ont ete inclus. Les longueurs effectuees sur le femur etaient a 95,96 % (chez l’homme) et a 100 % (chez la femme) incluses dans l’intervalle de confiance a 95 % de l’ETG. L’estimation de la taille par les ETG tendait a sous-estimer la taille des individus les plus grands et a surestimer la taille des individus les plus petits. Conclusion Ce travail a confirme le femur comme l’os long le plus utile pour estimer la taille. Une etude sur un nombre plus important d’individus permettrait d’adapter les ETG a notre population.

In forensic anthropology, the estimation of age at death is mainly required to establish a biological profile and facilitate individual identification. The Suchey–Brooks (SB) system represents one of the most commonly used and tested methods of assessment based on the morphological characteristics of the pubic symphysis. However, this method has certain drawbacks, including frequently inaccurate estimation for older individuals. The aim of this work was to test the evolution of pubic bone mineral density (BMD) according to age, especially for individuals over 40 years old. We retrospectively studied pubic bones from males (n = 88) and females (n = 113) over 40 years of age undergoing clinical multi-slice computed tomography (MSCT) in a French hospital between November 2017 and April 2018. The results revealed a significant negative correlation between BMD and age for males (R = −0.62) and females (R = −0.55). The intra- and inter-observer reliabilities of the phase allocation were moderate for males (kappa values at 0.72 and 0.65) and strong for females (kappa values at 0.93 and 0.86). Moreover, a significant difference in BMD between stages 6-1 and 6-2 for males was observed. We hypothesize that BMD might help improve the reliability of the SB system for older individuals. In addition to the pubic bone, numerous other anatomical regions such as the area of Ward could represent interesting areas of study in order to relate bone density to age.

Resume Les fistules des arteres coronaires, communiquant soit avec une cavite cardiaque, soit avec un gros vaisseau, sont des entites rares. Elles peuvent etre congenitales ou acquises. Elles sont le plus souvent asymptomatiques, mais peuvent aussi entrainer des symptomes tels qu’une douleur thoracique ou une dyspnee. Des cas de mort subite ont ete decrits dans la litterature. Ces malformations peuvent etre diagnostiquees de maniere fortuite lors d’une coronarographie, ou plus rarement dans le cadre d’une autopsie. Nous rapportons le cas d’un individu qui a presente un malaise avec un trouble du rythme cardiaque aigu, complique d’un arret cardio-respiratoire. Apres ressuscitation et stabilisation de l’individu, une coronarographie a ete realisee, mettant en evidence une fistule entre l’artere interventriculaire anterieure et l’artere pulmonaire. L’evolution clinique a ete pejorative compte tenu des complications anoxo-ischemiques avec un passage en etat de mort encephalique. Un arret therapeutique a donc ete decide. Une autopsie a ete pratiquee confirmant la presence de la fistule entre l’artere interventriculaire anterieure et l’artere pulmonaire. L’imputabilite de cette malformation au deces est ensuite discutee par les auteurs.

Objective Exposure to sexual assault is a significant risk factor to develop post-traumatic stress disorder (PTSD) in females. The early neurobiological changes leading to the development of PTSD remain understudied and unclear in this population. Methods Participants were 27 adult females recruited within a month following exposure to sexual assault (T1) and 20 age-matched non-exposed controls. Among the victims, 10 participants met (PTSD+) and 15 did not meet (PTSD-) DSM-IV criteria for PTSD 6 months post-trauma (T2). At both visits, hippocampal and amygdala volumes were extracted from magnetic resonance imaging scans, and indices of total diurnal cortisol changes were derived from individual areas under the curve relative to the ground (AUCg). Measures at T1 were compared between groups at T1, measures at T2 between groups at T2, and measures at T1 between groups at T2. Results At T1, victims had significantly smaller bilateral hippocampal volumes, but not AUCg, than controls. At T2, neither hippocampal volume nor AUCg significantly differed among the groups. However, the PTSD+ group had significantly smaller hippocampal volumes at T1 than the control group, but not compared to the PTSD- group. Conclusions This study indicates that having smaller hippocampal volumes is a risk factor to develop PTSD in females exposed to sexual assault.

Background Survivors of sexual assault are vulnerable to long-term negative psychological and physical health outcomes, but few studies have investigated changes in cognition, emotional processing and brain function in the early stages after sexual assault. We used a multimodal approach to identify the cognitive and emotional correlates associated with sexual assault in women. Methods Twenty-seven female survivors of sexual assault were included within 4 weeks of the traumatic event, and they were compared with 20 age-matched controls. Participants underwent functional MRI while performing cognitive/emotional tasks (n-back, emotional go/no-go, mental imagery). We also measured diurnal salivary cortisol and conducted neuropsychological assessments of attention and memory abilities. Results Relative to the control group, the survivors group had lower levels of morning cortisol and showed attentional deficits. We observed no between-group differences in brain activation during the n-back or mental imagery tasks. During the emotional go/no-go task, however, the survivors group showed a lack of deactivation in the dorsal anterior cingulate cortex when processing emotional material, relative to neutral material. Exploratory analyses in the survivors group indicated that symptom severity was negatively associated with cerebellar activation when positive emotional (happy) content interfered with response inhibition, and positively associated with cerebellar activation when thinking of positive (happy) memories. Limitations The small sample size was the main limitation of this study. Conclusion Dysfunctions in the dorsal anterior cingulate cortex and the cerebellum may represent early functional brain modifications that alter higher cognitive processes when emotional material is involved.

Resume L’evaluation de la maturation osseuse de l’extremite sternale de la clavicule sur un scanner est une des methodes de l’estimation de l’âge chez le sujet vivant. Plusieurs classifications sont utilisees pour cette evaluation. But de l’etude L’objectif etait de savoir si cette analyse etait contributive pour determiner si un individu etait âge de plus ou moins de 18 ans. Patients et methodes Nous avons selectionne retrospectivement tous les patients âges de 15 a 30 ans ayant eu un scanner thoracique et/ou un body scanner dans un CHU francais, entre le 1er janvier 2009 et le 1er juillet 2014, et avons evalue la maturation osseuse de l’extremite sternale des clavicules en utilisant une classification en neuf stades. Resultats Trois cent dix-neuf scanners ont ete inclus. Dans notre echantillon, tous les individus dont la fusion metaphyso-epiphysaire de la clavicule etait superieure a un tiers etaient âges de 18 ans et plus. Discussion Les etudes similaires dans la litterature s’accordent sur le fait que tous les individus dont la clavicule est totalement fusionnee sont âges de 18 ans et plus. Une simplification des classifications existantes pourrait donc etre discutee pour limiter le risque d’erreurs qui augmente avec le nombre de stades.

We report an unusual case of homicidal stab wound of the cervical spinal cord, which illustrates the value of post-mortem computed tomography angiography (PMCTA) in cases of vascular injury. First, we noted a posterior and horizontal trajectory to the neck with complete section of the cervical spinal cord between the first and second cervical vertebrae. This lesion was accompanied by section of the right vertebral and right deep cervical arteries. We also noted an anterior cervical trajectory with an injury to the right internal jugular vein and an anterior right chest wound with a lung trajectory and section of the internal mammary vessels. Cases of spinal cord injuries secondary to stab wounds are rare in the literature. Only one large series has been published from Cape Town. Complete section of the cervical spinal cord accounts for only 4.5% of all cases. Furthermore, lethal cases are rare and classically victims survive and present neurological sequelae. We found only one similar case but despite the transection of the cervical spinal cord the patient survived. Some studies suggest that PMCTA may be very helpful in visualizing vascular system injuries. Our observations are consistent with this proposal. The use of different-time acquisitions was essential for detection of the injured vessels.

Resume But de l’etude Le controle medical est exerce par les medecins-conseils. Ils donnent des avis sur des situations medicales problematiques. La contestation de cet avis par l’assure entraine une expertise medicale de securite sociale selon l’article L. 141-1 du Code de securite sociale. L’epidemiologie de la population des assures concernes par cette procedure a deja ete etudiee, mais pas le profil des individus. Nous avons donc choisi d’etudier les profils individuels d’une telle population. Materiel et methodes Cette etude retrospective a concerne 103 dossiers d’expertises realisees entre juin 2011 et mars 2012 au service medical de la caisse primaire d’assurance maladie d’Indre et Loire (37). Un recueil de donnees a ete effectue afin d’en degager des profils. L’analyse statistique a consiste en une analyse des correspondances multiples suivie d’une classification ascendante hierarchique. Resultats Trois profils ont ete individualises. Le premier concerne des retraites et des professions superieures a qui l’on a refuse le versement d’indemnites journalieres pour une pathologie aigue en risque maladie. Le deuxieme concerne des femmes employees ou exercant une profession intermediaire sans penibilite, victimes d’une pathologie chronique. Enfin, le dernier concerne des ouvriers en arret de travail pour des troubles musculo-squelettiques survenant du fait de la penibilite physique de leur travail. Discussion Ce travail a consiste a completer les travaux preexistants sur la population de personnes expertisees selon l’article L. 141-1 du Code de securite sociale en s’attachant plus a l’individu qu’a la description de la population. Ces profils offrent une vision concrete des situations rencontrees dans la pratique de ce type d’expertise.

International audience; Medicolegal physicians are increasingly called upon to aid in determining the administrative age group affiliation of refugees with questionable unaccompanied minor claims. According to guidelines for forensic age assessment, age differentiation along the 18-year-old cut-off relies on clavicular ossification. The thin-slice computed tomography scan (TSCTs) of the medial clavicular epiphysis (MCE) is one of the methods contributing to this assessment, though it is not yet universally accepted. The aim of this systematic review was to identify scientific papers where age was assessed using TSCTs of the MCE and to observe whether this examination was reproducible and reliable in estimating a person's age relative to the 18-year-old threshold. A search algorithm was applied to several databases to identify articles in accordance with the PRISMA (Preferred Reporting Items for Systematic-Reviews and Meta-Analyses) statement. One boxplot per article was constructed, separating by stage of maturation and sex. The 13 articles selected represented a sample of 5605 individuals (3396 males, 2209 females) aged 10 to 35~years. All individuals classified as stages 4 and 5 were aged 18~years or older. The same result was obtained concerning stage 3c, except in one article. The results thus appear reliable and reproducible, in particular, with respect to the 18-year-old threshold; medicolegal physicians should be able to estimate that all individuals in stages 4 and 5 are at least 18~years old. Additional studies applied to several other populations in the world should complement the selected studies.

International audience; We report on a comprehensive 25-year study on criminal medical liability in France, undertaken to establish trends in the number of cases being brought before the criminal courts. We did this by interrogating the database on the Légifrance website using a Boolean equation (' pénal' (criminal) AND ' médecin' (physician) OR ' docteur' (doctor)). In total 539 cases were selected, in which the status of the physician either influenced the punishment imposed, or was a prerequisite for the commission of the offence. The results of the search produced two outcomes: offences and the dates of the most recent criminal judgements (which had been left blank). Further data were also collected: references to court cases, hearing dates, offence dates, procedural time limits, numbers of accused parties, types of punishments and physician characteristics. The number of court hearings increased from the 1980s until the late 1990s. Since then, it has remained stable at around 25 cases per year. Of the defendants appearing before the courts, 39.2% have been found guilty. On average, 10 to 13 physicians every year - that is, one per month - are punished. Those most often punished are obstetrician-gynaecologists (13%), followed by intensive care anaesthetists (11%) and then by general practitioners (6.7%). The offences most frequently occurring are manslaughter (36.5%), illegal profits (12%), unintentional injuries (11.5%) and sexual offences (10.1%). The results are most reassuring in terms of the risks posed by the practice of medicine in France. Such a risk does indeed exist; however, it is at a low level and stable.

Virtual anthropology consists of the introduction of modern slice imaging to biological and forensic anthropology. Thanks to this non-invasive scientific revolution, some classifications and staging systems, first based on dry bone analysis, can be applied to cadavers with no need for specific preparation, as well as to living persons. Estimation of bone and dental age is one of the possibilities offered by radiology. Biological age can be estimated in clinical forensic medicine as well as in living persons. Virtual anthropology may also help the forensic pathologist to estimate a deceased person's age at death, which together with sex, geographical origin and stature, is one of the important features determining a biological profile used in reconstructive identification. For this forensic purpose, the radiological tools used are multislice computed tomography and, more recently, X-ray free imaging techniques such as magnetic resonance imaging and ultrasound investigations. We present and discuss the value of these investigations for age estimation in anthropology.

Resume L’estimation de l’âge chez le sujet vivant est un sujet important en medecine legale clinique, en raison de l’augmentation des flux migratoires. Des etudes se sont interessees a l’apport de l’imagerie par resonance magnetique (IRM) dans l’evaluation de la maturation osseuse, en raison du caractere non irradiant de cette technique d’imagerie. Le travail recent d’une equipe allemande sur l’analyse d’IRM d’extremite distale du femur a suggere que si un homme presentait une fusion metaphyso-epiphysaire de cette extremite, il etait âge d’au moins 18 ans. Nous avons voulu verifier ce resultat et avons selectionne des IRM de genou de 214 sujets de sexe masculin âges de 14 a 20 ans. Les images ont ete analysees par deux observateurs afin de determiner si la metaphyse et l’epiphyse de l’extremite distale du femur etaient completement fusionnees ou pas. Les resultats confirmaient ceux de la premiere etude, le sujet le plus jeune ayant une fusion complete visible sur les images d’IRM etant âge de 18,1 ans. Ainsi, meme si ces resultats ne concernent que les hommes de deux echantillons composes d’individus originaires de pays dont le niveau de modernisation est eleve, ils suggerent que le genou est une articulation dont l’analyse peut etre contributive dans l’estimation de l’âge et que l’IRM est une technique d’imagerie adaptee a cette evaluation.

Forensic age estimation of living individuals has become increasingly important in forensic practice with the constant increase of migration movements to developed countries. The method of Greulich and Pyle is one of the most frequently used methods for age determination. The aim of our study was to verify the reliability of this method on a French contemporary population. We retrospectively analyzed 190 frontal hand and wrist radiographs of living subjects aged between 10 and 19 years (100 males and 90 females) performed in the Radiology Department of the University Hospital of Tours. These radiographs were compared with the Greulich and Pyle atlas to determine the skeletal age (SA) and to compare it to the chronological age (CA). We calculated the coefficient of correlation and the coefficient of determination for each sex group. The SA of our subjects was also inserted in the graphs provided by Greulich and Pyle in their atlas. Intra- and inter-observer variabilities were good, demonstrating the reproducibility and repeatability of the method. The correlation coefficients were high (0.98 for males and 0.93 for females) and comparable with published data. The mean difference between CA and SA was -2.29 months for males and -6.44 months for females, showing an overestimation of CA for both sexes. No statistical difference was found between CA and SA for both male and female samples. We concluded that the method of Greulich and Pyle can be used on a contemporary French population but not without caution because of a tendency for this method to overestimate age. A fully developed hand and wrist does not allow to state that the 18th year of life is completed beyond a reasonable doubt. We recommend using it in association with other methods.

We report the case of a 5-year-old boy who died from complications of rheumatic heart disease with atypical presentation. He was hospitalized for recent inflammatory and neurological symptoms. He was diagnosed with viral encephalitis. He died the day after he was discharged. The macroscopic autopsy findings were unremarkable. Histology revealed typical rheumatic heart disease. Neuropathology showed cerebral infarction due to an embolic event linked with the rheumatic valvulitis. The cause of death was determined as heart failure due to rheumatic heart disease secondary to an undiagnosed acute rheumatic fever. It is related to an autoimmune response to infection with group A streptococcus. It mainly affects children in developing countries. In our case, viral encephalitis was consistent with the medical history and the proper diagnosis was made on histological analysis. Forensic pathologists should consider this diagnosis facing a sudden unexpected death in childhood, even in industrialized countries.

The contribution of magnetic resonance imaging to forensic age estimation of living individuals is a subject of ongoing research. Several studies have focused on the wrist, clavicle, knee, and foot, and shown interesting results regarding the 18-year threshold. Authors have developed various staging systems for epiphyseal growth plate maturation. However, the procedure is observer-dependent and requires experience and a certain time-learning process. To reduce these pitfalls, we have developed an automatic method based on the analysis of variations of gray levels within the epiphyseal-metaphyseal junction. This method was tested on 160 MRI scans of the distal tibial epiphysis in a sample of individuals aged from 8 to 25 years old, after intensity non-uniformity correction of all images. Results showed that in our sample, 97.4 % of males and 93.9 % of females aged 18 years or more would be correctly classified using this method. To our knowledge, automatic methods for MRI analysis have not been used in the field of age estimation yet. Further studies should be performed to assess the validity of this procedure.

A suicide pact is the decision of two or more people to die together. This event is rare. The majority of suicide pacts victims are married, socially isolated, with a serious physical illness in one or both partners. We performed a retrospective study of all cases of suicide pacts leading to death between January 1, 2003 and December 31, 2010 referred to the Forensic Medicine Department of Tours (France). Six cases were recorded as suicide pacts. Firearms were the preferred means of death. Five suicide notes were discovered. For some cases, it was difficult to establish whether death was the result of a suicide pact or of a homicide-suicide without the consent of the victim. An objective element, such as a suicide note and motive, is required to prove that a suicide pact is involved. The prevalence of mental disorders is hard to assess. In France, psychological autopsies are not performed in such cases, as the investigation is usually very sparse after the death of both individuals.

Age estimation of living individuals is of critical importance in forensic practice, especially because of the increased migration in developed countries. Recently, the contribution of magnetic resonance imaging (MRI) to age evaluation has been studied, as it seems to be an efficient technique to analyze growth plate maturation and epiphyseal fusion. We developed an MRI staging system for the distal tibial epiphysis and the calcaneal epiphysis and evaluated its reliability on 180 MRI scans of the ankle and foot in a sample of individuals aged from 8 to 25 years old. For both bones, the degree of union between the metaphysis and epiphysis was classified in three stages. Intra- and inter-observer variabilities were good, showing the validity and reproducibility of the method. Our results were consistent with data in the literature indicating that both epiphyses mature earlier in females than in males. Bayesian predictive probabilities were used to assess the validity of our method in estimating the age of an individual in relation to the 18-year threshold. MRI of the ankle and foot can be used in association with other methods to estimate age in living individuals.

Investigation of deaths caused by penetrating gunshot wounds to the head often raises the possibility of foul play. The forensic pathologist may be asked if the victim was able to perform certain acts after the gunshot, and how quickly this person might have become incapacitated. The possibility of a suicidal act can depend on these answers. We report the case of a 45-year-old woman whose body was found with a right temporal entrance wound. A shotgun was found 60 ft from the body location. The question of knowing if this woman had been able to shoot herself in the head and then walk a distance of 60 ft before dying was essential for the investigation, as suicide was the first hypothesis. The autopsy and a careful neuropathology investigation allowed to answer this question. In the literature, multiple publications report cases of victims who were able to act following penetrating ballistic head injury.

Pacemakers and implantable cardioverter-defibrillators (ICDs) are implanted medical devices for the treatment of cardiac arrhythmias. These devices are now commonly encountered in the postmortem situation. The Los Angeles County Department of the Coroner uses the services of a cardiac electrophysiology consultant to interrogate pacemakers and ICDs, producing a detailed picture of the cardiac events recorded by the device. We have used this method to evaluate 20 cases where the ICD or pacemaker provided significant information. We report four forensic cases to illustrate the different applications of pacemaker interrogation and its contribution in forensic investigation. This technique was used to establish identification of the decedent, to determine the cause and time of death, and to determine whether device malfunction could have played a role in the death. We conclude that detailed evaluation of the pacemaker or ICD in selected cases may provide essential information to the forensic pathologist.

Homicide-suicides often attract media attention. Only one other study of homicide-suicide events in France have been conducted. American authors have emphasized the importance of developing a research strategy, based on reports of these events. Our study of the characteristics of all homicide-suicide in Tours between 2000 and 2005, is compared with published data. We have attempted to classify such homicide-suicides in accordance with other published studies.

Resume Objectifs Nous decrivons les caracteristiques d'une population de victimes d'abus sexuels examinees dans un centre de reference. Apres avoir obtenu les suites judiciaires de ces affaires, nous avons cherche l'existence d'une association entre certaines des donnees de l'entretien et de l'examen clinique, et l'issue de la procedure judiciaire. Materiel et methodes Cette etude retrospective a inclus toutes les victimes d'abus sexuels examinees a Tours sur une periode de sept ans. Les suites judiciaires de chacune des affaires ont ete obtenues au tribunal de grande instance. Un modele de regression logistique a ete utilise pour mettre en evidence une association statistique entre les circonstances de l'abus et les donnees medicolegales, et l'issue de la procedure judiciaire (condamnation de l'auteur des faits). Resultats Sept cent cinquante-six victimes ont ete incluses. 68,3 % d'entre elles etaient des mineurs de 15 ans au moment des faits. Des lesions traumatiques genitales ont ete constatees chez 6,8 % des victimes feminines et 6 % des victimes masculines de ce groupe. 31,7 % des victimes avaient plus de 15 ans au moment des faits. 11,3 % des femmes avaient des lesions traumatiques genitales. Au total, 36,3 % des agresseurs ont ete condamnes par un tribunal. Les deux elements associes a la condamnation de l'agresseur etaient le fait que l'examen soit effectue sur requisition des autorites judiciaires et le fait que la victime connaisse son agresseur. La presence de lesions traumatiques corporelles et/ou genitales n'etait pas associee a la condamnation de l'auteur des faits. Conclusion Le recueil des declarations de la victime par les medecins examinateurs doit etre systematique et rigoureux. La constatation de lesions traumatiques lors de l'examen clinique n'est pas determinante dans l'issue de la procedure judiciaire.

We describe the medico-legal findings in a population of sexual assault cases assessed in an urban French referral centre, analyse the subsequent legal dispositions in each case and determine whether the characteristics of the assault and the medico-legal findings were associated with conviction of the assailant. We performed a retrospective study of medico-legal reports in all the sexual assault cases reported in Tours (France) during a seven-year period. We defined two groups of victims: children under 15 years old and victims aged 15 years or more. Legal outcomes were obtained from courtroom proceedings. The relationship between the outcomes and the circumstances of the case was analyzed by logistic regression. We enrolled a total of 756 cases during the study period. The mean age of the study population was 16.5 years and 68.3% of the cases involved children under 15 years old. In 57% of these cases, the assailant was a family member. 31.7% of all the victims were aged 15 years or more. The assailant was an acquaintance of the victim in 62.2% of the cases. Drug-facilitated assault was suspected in 2.9% of the cases. In 46.2% of the cases, formal criminal charges were not filed due to insufficient evidence; 36.3% of the assailants were convicted. Examination at the request of the police authorities and previous acquaintance of the assailant by the victim were significantly associated with conviction. Allegations of penetration, the presence of general body trauma and the presence of genital trauma were not necessarily associated with conviction. Medical examiners need to be circumspect when they record non-medical variables. Physical evidence of trauma was neither predictive nor essential for conviction. Successful prosecution depends on the quality of the testimony provided by the victim.

Forensic age estimation of living individuals is a controversial subject because of the imprecision of the available methods which leads to errors. Moreover, young persons are exposed to radiation, without diagnostic or therapeutic advantage. Recently, non-invasive imaging techniques such as magnetic resonance imaging (MRI) have been studied in this context. The aim of this work was to study if the analysis of wrist/hand MRI enabled determination of whether a subject was 18 years old. Two observers retrospectively analyzed metaphyseal–epiphyseal fusion of the distal epiphysis of the radius and the ulna and the base of the first metacarpus in wrist/hand MRI of living people between 9 and 25 years of age. A three-stage scoring system was applied to all epiphyses. Intra- and inter-observer variability was excellent. Staging of the distal radial epiphysis allowed the subjects to be correctly evaluated with regard to the 18-year-old threshold in more than 85 % of cases. Analysis of the radius alone was as good as the analysis of the three epiphyses together. Evaluation of the metaphyseal–epiphyseal fusion of the distal radius in wrist MRI gave good results in forensic age estimation. Wrist MRI could meet ethical expectations with regard to the link between the benefit and risk of practicing radiologic examination on individuals in this context.

This retrospective study examined homicides in two French departments located in the West of France (Indre-et-Loire and Loir-et-Cher) for a four-year period from 2000 to 2003. During this period 63 homicidal deaths were investigated at the Institute of Forensic Science of Tours (Indre-et-Loire). There were 45 male and 18 female victims with an average homicide rate of 1.55 per 100,000 persons. The mean age of the victims was 42 years-old. Forty-five assailants were identified; their mean age was 38 years-old. The most common method of homicide was the use of firearms (40% of the cases), followed by blunt-force injury (36%) and sharp-force injury (16%). 51% of the victims knew their assailant, a family member in 26% of the cases. Spousal homicides occurred in 16% of the cases. Dyadic death occurred in six cases. A review of the literature compares these findings to other populations.

Cet article a pour objet de synthetiser les connaissances actuelles sur la soumission chimique, un mode d'agression ayant fait l'objet de nombreuses publications scientifiques ces dix dernieres annees. Les effets recherches par les agresseurs sont essentiellement la desinhibition, la sedation et l'amnesie anterograde dans le but de causer a la victime un prejudice. Les produits les plus utilises sont l'alcool ethylique et les benzodiazepines. Certaines substances comme le flunitrazepam et l'acide gamma-hydroxybutyrique (GHB) ont ete tres mediatisees. L'analyse toxicologique des prelevements de sang, d'urine ou de cheveux, permet de mettre en evidence la substance utilisee. Les prelevements doivent etre effectues le plus rapidement possible apres les faits. La chromatographie en phase gazeuse avec detection par spectrometrie de masse est la methode analytique de reference.

Acute promyelocytic leukemia (APL) is a subtype of acute myelogenous leukemia frequently associated with clotting abnormalities and severe hemorrhagic diathesis. The disease is associated with a high incidence of early fatal hemorrhage. We report the sudden death of a 40-year-old male without significant medical history in which foul play had been initially suspected. A thorough postmortem investigation performed on the decedent lead to the diagnosis of APL. Cause of death was a cerebellar hematoma. Underlying APL should be considered in the differential diagnosis when unexplained bleeding is encountered in a decedent. This case emphasizes the value of routinely collecting bone marrow during an autopsy to enable accurate testing and diagnosis.

Computed tomography (CT) scan of the medial clavicular epiphysis is one of the methods recommended by the Study Group on Forensic Age Diagnostics to estimate the age of living individuals. The aim of our study was to assess the relationship between the skeletal maturation of the sternal end of the clavicle and the chronological age in a sample of French individuals, using a nine-stage classification. We retrospectively reviewed 319 chest CT-scans of individuals aged 15-30 years old (252 males, 67 females). Among males and females, all individuals with a complete fusion, or an ongoing fusion of more than one third of the total surface of the metaphysis were at least 18 years old. Our results were consistent with data in the literature indicating that individuals with a complete fused clavicle were at least 18 years old. Similar studies with the same methods allow for creating a database of samples from different countries to confirm the validity of this method and its excellent results in forensic age estimation of living individuals.

Age estimation of living individuals is a regular activity in medico-legal practice. Among the available tools for determining skeletal age, some authors have stated that the disappearance of epiphyseal scars could be a useful marker. The aim of the present study was to assess whether the presence of an epiphyseal scar on the knee, as seen on a plain X-ray, was linked to biological age. A total of 988 frontal X-rays of individuals (509 females and 479 males) aged between 15 and 40 years were analyzed to see whether a scar was visible or not on each of the three epiphyses of the knee. A scar was visible for 96 % of the females and 98 % of the males. For each sex, scars were visible at each year of age, from 15 to 40 years. In younger females, there were 15 individuals with no scar visible on the fibula, 16 on the tibia, and 20 on the femur. For males, the ages were respectively 16, 17, and 18 years. On a frontal X-ray, the persistence of epiphyseal scars was not a marker of a recent fusion. All individuals with fully ossified knee that had no scar on the femur were older than 18 years. Further studies focusing on epiphyseal scars on MR and CT scans could be useful, as these techniques allow the more precise analysis of the epiphysis.

A case is presented of a 30-year-old man suffering from borderline personality disorder who committed suicide by means of smothering. He was hospitalized at the psychiatric department of a public hospital for several months and was found dead in the seclusion room. The death resulted from an obstruction of the upper aerodigestive tract after ingurgitation of pellets of toilet paper. A literature review revealed several cases of suicidal asphyxia by smothering of patients with a history of psychiatric illness. Our patient's operating process had not been reported before to our knowledge.

In the case reported here, the antemortem computed tomography scan (CT scan) was essential in the forensic investigation. A 32-year-old man was found fully awake with a facial abrasion, after what seemed to be a car accident. He lost consciousness suddenly one hour after initial management. Successive CT scan showed a facial fracture and a metallic foreign body in the carotid canal associated with an occlusion/dissection of the left internal carotid, a pseudoaneurysm, and a carotid-cavernous fistula. The victim died from a stroke. Autopsy confirmed that the facial abrasion was a gunshot entrance wound, the metallic foreign body being a projectile. Intracranial vascular injuries linked with gunshot wounds are most of the time isolated and due to pelet embolism. The observed vascular injury association has never been described in the existing literature. The CT scan provided a better understanding of the chronology of events that led to death.

Strychnine-related death has been described since the 19th century. This alkaloid was discovered in 1818. Historically, strychnine was used by the South-East Asian autochthones on their arrows. However, its production was modified by legislation, which was used to protect people against accidental intoxications. Here, we present the case of a 69-year-old man who was found dead at home. During the autopsy, we found a blue substance in the stomach. Toxicological analysis measured strychnine at 0.29 μg/mL in the blood sample, which is a relatively low level in comparison with the results given in the literature. However, histologic examination and toxicological findings permitted the conclusion of strychnine poisoning.

Postmortem investigation often reveals various conditions, which may or may not have played a part in the death of the individual. The case of a 32-year-old woman is reported, with a long history of drug addiction. She was found dead in her bed. The autopsy revealed diffuse pulmonary edema with congestion of the lungs, brain, liver, and spleen. Microscopic examination of the lungs showed multiple intra-alveolar and interstitial foamy macrophages and extracellular fat droplets surrounded by polynuclear giant cells. Death was attributed to acute polydrug intoxication. As microscopic examination had revealed severe pulmonary lesions, lipoid pneumonia was considered as a contributing factor to death. Lipoid pneumonia is an uncommon entity with the characteristic radiograph features and histologic findings of alveoli filled with vacuolated, lipid-laden histiocytes. It can be either exogenous or endogenous in cause, based on the source of the lipid. Exogenous lipoid pneumonia usually results from aspiration or inhalation of fat-like material, such as mineral oil or petroleum-based lubricants and decongestants, resulting in pulmonary inflammatory reactions.

The Suchey-Brooks method is commonly used for pubic symphyseal aging in forensic cases. However, inter-population variability is a problem affected by several factors such as geographical location and secular trends. The aim of our study was to test the reliability of the Suchey-Brooks method on a virtual sample of contemporary French males. We carried out a retrospective study of 680 pubic symphysis from adult males undergoing clinical Multislice Computed Tomography in two hospitals between January 2013 and July 2014 (Toulouse and Tours, France). The reliability of the Suchey-Brooks method was tested by the calculation of inaccuracy and bias between real and estimated ages, and the mean age for each stage and the mean stage for each 10-years age interval were compared. The degree of inaccuracy and bias increased with age and inaccuracy exceeded 20 years for individuals over 65 years of age. The results are consistent with an overestimation of the real age for stages I and II and an underestimation of the real age for stages IV, V and VI. Furthermore, the mean stages of the reference sample were significantly lower for the 14–25 age group and significantly higher for individuals over 35 years old. Age estimation is potentially limited by differential inter-population error rates between geographical locations. Furthermore, the effects of secular trends are also supported by research in European countries showing a reduction in the age of attainment of indicators of biological maturity during the past few decades. The results suggest that the Suchey-Brooks method should be used with caution in France. Our study supports previous findings and in the future, the Suchey-Brooks method could benefit from re-evaluation of the aging standards by the establishment of new virtual reference samples.
