Vaccines combatting COVID-19 demonstrate the ability to protect against disease and hospitalization, and reduce the likelihood of death caused by SARS-CoV-2. In addition, monitoring viral loads in sewage emerges as another crucial strategy in the epidemiological context, enabling early and collective detection of outbreaks. The study aimed to monitor the viral concentration of SARS-CoV-2 in untreated sewage in a Brazilian municipality. Also, it attempted to correlate these measurements with the number of clinical cases and deaths resulting from COVID-19 between July 2021 and July 2022. SARS-CoV-2 viral RNA was quantified by RT-qPCR. Pearson's correlation was performed to analyze the variables’ relationship using the number of cases, deaths, vaccinated individuals, and viral concentration of SARS-CoV-2. The results revealed a significant negative correlation (p < 0.05) between the number of vaccinated individuals and the viral concentration of SARS-CoV-2, suggesting that after vaccination, the RNA viral load concentration was reduced in the sample population by the circulating concentration of wastewater. Consequently, wastewater monitoring, in addition to functioning as an early warning system for the circulation of SARS-CoV-2 and other pathogens, can offer a novel perspective that enhances decision-making, strengthens vaccination campaigns, and contributes to authorities establishing systematic networks for monitoring SARS-CoV-2. HIGHLIGHTS The presence of SARS-CoV-2 was detected in 100% of the wastewater samples.; Wastewater SARS-CoV-2 concentration negatively correlated with vaccinated individuals.; Wastewater-based epidemiology is a tool for the monitoring of virus during vaccination campaigns.;

Abstract The aim of this study was to identify the bio‐efficacy of sodium butyrate (NaBu) on preventing the development of peritoneal fibrovascular tissue induced by implantation of a synthetic matrix in the abdominal cavity. Polyether–polyurethane sponge discs were implanted in the peritoneal cavity of mice, which were treated daily with oral administration of NaBu (100 mg/kg). Control animals received water (100 μl). After 7 days, the implants were removed for assessment of inflammatory, angiogenic and fibrogenic markers. Compared with control values, NaBu treatment decreased mast cell recruitment/activation, inflammatory enzyme activities, levels of pro‐inflammatory cytokines, and the proteins p65 and p50 of the nuclear factor‐κB pathway. Angiogenesis, as determined by haemoglobin content, vascular endothelial growth factor levels and the number of blood vessels in the implant, was reduced by the treatment. In NaBu‐treated animals, the predominant collagen present in the abdominal fibrovascular tissue was thin collagen, whereas in control implants it was thick collagen. Transforming growth factor‐β1 levels were also lower in implants of treated animals. Sodium butyrate downregulated the inflammatory, angiogenesis and fibrogenesis axes of the fibroproliferative tissue induced by the intraperitoneal synthetic matrix. This compound has potential to control/regulate chronic inflammation and adverse healing processes in the abdominal cavity.

Objective To assess the predictive abilities of serum and urinary cystatin C levels for glomerular lesions in pregnant women with pre-eclampsia. Methods In this study, kidney function markers were compared between38 pregnant women with pre-eclampsia and 22 healthy pregnant women. Results The serum and urine levels of cystatin C and urea were significantly higher in the pre-eclampsia group than in the control group. Receiver operating characteristic curve analysis demonstrated that the serum cystatin C level (91.7%) had a superior diagnostic accuracy for pre-eclampsia than the other markers. Conclusion Serum cystatin C level maybe a significant marker of pre-eclampsia.

RESUMO O objetivo deste artigo foi apresentar um método qualitativo de abordagem da educação ambiental no processo de territorialização em saúde. O percurso metodológico que dá origem a este texto é uma pesquisa-ação cujo estudo é do tipo exploratório e descritivo de abordagem qualitativa por meio do desenvolvimento de oficinas. Foram realizados seis encontros para atender aos objetivos propostos. As atividades foram desenvolvidas por um moderador e contaram com a participação de 60 Agentes Comunitários de Saúde atuantes em um município de pequeno porte do interior e do litoral da Bahia. Os resultados apontam que, no processo de territorialização em saúde, é elementar a incorporação de novas práticas que demonstrem o território além de demarcações geográficas, valorizando a concepção ampliada de ambiente e suas relações com a saúde. Nesse ponto, surge como proposta a reorganização dos processos de trabalho, no sentido de buscar multidisciplinaridade, interdisciplinaridade e participação efetiva no processo de territorialização em saúde à luz da educação ambiental.

As Leishmanioses constituem um grupo de doenças que refletem grave problema de saúde pública no Brasil. A fisiopatologia da leishmaniose visceral (LV) parece estar intimamente relacionada as diferentes espécies causadoras da doença e à estreita relação da resposta imune do indivíduo contra o parasita. Após a infecção inicial, alguns indivíduos podem evoluir com formas assintomáticas e cura espontânea, enquanto outros podem evoluir com forma graves. Indivíduos que desenvolvem alguma imunossupressão podem apresentar quadro de LV muito além do período habitual de incubação. Questiona-se a possibilidade da desregulação do sistema imunológico secundária a infecção grave por coronavírus (COVID 19) ter sido um fator facilitador para apresentação da LV clinicamente manifesta no caso descrito. O objetivo do estudo é descrever um caso de paciente jovem, pós COVID 19 grave, evoluindo com febre de origem indeterminada. Trata-se de paciente, 24 anos, com obesidade grau II (IMC:36), diagnosticado com COVID 19 através de teste rápido de antígeno de swab da nasofaringe. Em unidade de terapia intensiva (UTI) no 9º dia de sintomas, necessitou de intubação orotraqueal, protocolo de prona e uso de antibioticoterapia. Recebeu alta da UTI tolerando bem desmame de oxigenioterapia. No 26º dia iniciou com febre persistente, sem foco identificado apesar de propedêutica extensa. No 29º dia foi verificada pancitopenia, não presente em exames prévios com hemoculturas negativas. Evoluiu com hipotensão e novo choque séptico, uso de drogas vasoativas, intubação e injúria renal aguda com terapia de substituição renal. Propedêutica complementar evidenciou esplenomegalia leve, visualizada em tomografia computadorizada de abdome. Provas inflamatórias elevadas assim como desidrogenase láctica e hiperferritinemia importante (>400000) e provas de hemólise negativas e pancitopenia em piora. Realizado mielograma no 13° dia de febre mantida com resultado de PCR para Leishmania positivo. Iniciado tratamento com Anfotericina B lipossomal, durante 7 dias (dose total de 20 mg/kg). Recebeu alta com exames melhorados, assintomático. Os quadros de febre persistente em pacientes sob terapia intensiva são um desafio para a equipe assistencial. Apesar das infecções associadas a assistência serem a principal causa de febre nesse contexto, pacientes que apresentam sintomas típicos de outras doenças infecciosas, necessitam de investigação, considerando o contexto epidemiológico do nosso país.

Introdução: A toxoplasmose é a infecção do sistema nervoso central mais comum em pacientes com a síndrome da imunodeficiência adquirida, sem profilaxia adequada, geralmente com CD4 menor que 100 células. Descrição do caso: Paciente de 47 anos, sexo masculino, natural e procedente de zona rural em Minas Gerais (MG). Iniciou em maio de 2021, quadro de confusão mental, desorientação têmporo-espacial e queda da própria altura. Procurou atendimento médico na região de origem e foi diagnosticado HIV, com carga viral de 30.833 cópias (log 4,489) e CD4 de 500 células (17,24%). No mês seguinte, foi hospitalizado devido à piora das manifestações neurológicas e realizada ressonância nuclear magnética de encéfalo, a qual evidenciou lesões em tálamo direito e em núcleos da base à esquerda. A primeira lesão era heterogênea, com aspecto em alvo, extensão para a coroa radiada ipsilateral, áreas e focos de hipersinal na sequência ponderada em T1 e T2, com dimensões de 35 x 25 x 30 mm. A segunda lesão possuía aspecto semelhante à primeira, mas associada a efeito de massa e dimensões de 18 x 10 x 11 mm. O paciente foi transferido para hospital de referência, na capital de MG, no mês de julho, onde foi realizada sorologia para toxoplasmose, cujo resultado foi IgG reagente e IgM não reagente. Fundoscopia sem coriorretinite. Líquido cefalorraquidiano com discreta hiperproteinorraquia e testes moleculares para tuberculose, Epstein-Barr e toxoplasmose não detectáveis. Iniciado tratamento empírico para neurotoxoplasmose com Sulfadiazina, Pirimetamina e Ácido Folínico. Após 20 dias de tratamento, foi realizada nova ressonância de encéfalo, que além de realce periférico das lesões, mostrou que não houve redução das dimensões inicialmente descritas. Completadas 6 semanas de tratamento, nova ressonância de encéfalo revelou os mesmos achados do exame anterior. Assim, foi realizada biópsia estereotáxica, cujo anatomopatológico revelou achados de tecido cerebral com necrose e infiltrado inflamatório linfohistiocitário, sugerindo processo infeccioso. A imuno-histoquímica foi positiva para pesquisa de Toxoplasma em painel de anticorpos. O paciente recebeu alta para domicílio no mês de setembro, com proposta de extensão do tratamento diante da ausência de franca melhora e acompanhamento ambulatorial. Comentários: Neurotoxoplasmose é pouco provável em pacientes com lesão encefálica e CD4 > 200 células, mas essa hipótese não deve ser descartada, como exemplificado no caso descrito.

Febre Maculosa é doença infecciosa febril aguda, causada por bactérias gram negativas intracelulares obrigatórias do gênero Rickettsia, transmitida por meio de carrapatos. As manifestações clínicas podem variar de quadros leves a letais, sendo meningoencefalite uma manifestação grave com alta morbimortalidade. Relatamos o caso de paciente masculino, 35 anos, trabalhador rural, hígido. Participou de pescaria em 24/08/21. No dia 01/09/21 iniciou cefaleia holocraniana, vômitos, diarreia e inapetência. Procurou atendimento médico, foi liberado para domicílio com suspeita de COVID-19 e propedêutica foi negativa para SARS-COV-2. Em 06/09/21 apresentou piora da cefaleia, sonolência e febre alta. Foi internado e exames laboratoriais constaram leucocitose (23.200 cels/mm3, 32% bastonetes) trombocitopenia (93.000 cels/mm3), elevação de transaminases (TGO 167ui/L, TGP 136ui/L). Ao exame físico foi encontrado carrapato em dorso, cuidadosamente retirado, e iniciado tratamento empírico para Rickettsiose com doxiciclina em 06/09/21. Em 08/09/21 apresentou rigidez de nuca, abaixamento de nível de consciência, crises convulsivas reentrantes, desvio conjugado do olhar para baixo e nistagmo horizontal bilateral. Foi intubado e encaminhado ao Centro de Terapia Intensiva do Hospital Eduardo de Menezes com hipótese diagnóstica de meningoencefalite por Rickettsia e iniciado tratamento para status epilepticus. Em 08/09/21 tomografia de crânio evidenciou apagamento de sulcos, compatíveis com hipertensão intracraniana, e Liquor: proteínas 207mg/dL, glicose 50mg/dL (sérica 97mg/dl), 720 leucocitos, 77% polimorfonuclear, sem crescimento de microorganismos. Após 9 dias de tratamento, liberado resultado de RT-PCR para Rickettsia positivo, confirmando o diagnóstico de Febre Maculosa. Paciente teve melhora clínica e laboratorial e alta do CTI acordado e sem sinais neurológicos focais. Febre Maculosa tem apresentações clínicas diversas. Exantema é considerado sinal importante para o diagnóstico, por se manifestar em mais de 90% dos casos após cinco dias de doença. Quando ausente, a hipótese de Febre Maculosa não costuma ser aventada, o que atrasa o diagnóstico e aumenta a letalidade. No caso relatado, com manifestações neurológicas e sem observação de exantema, a suspeição baseada na apresentação clínica-epidemiológica, com a devida atenção à definição do MS, conduziu ao tratamento empírico sem que fosse necessário esperar por confirmação laboratorial.

A sífilis é doença infecciosa com várias formas de apresentação, mas frequentemente negligenciada. O acometimento ósseo na sífilis secundária é raro e existem poucos trabalhos sobre o tema. Este é um relato de caso de sífilis com várias manifestações de secundarismo, incluindo osteomielite sifilítica. Paciente do sexo masculino, 36 anos, previamente hígido, que iniciou quadro de cefaleia fronto-temporal bilateral, zumbido e rash cutâneo maculo-papular, este último com resolução espontânea. Após dois meses do início da cefaleia, o paciente evoluiu com baixa acuidade visual em olho esquerdo o que o motivou a procurar avaliação oftalmológica, quando foi vista uveíte. Foram realizados FTA-ABS com resultado positivo e VDRL reagente até a titulação de 1:4096. Diante de quadro de sífilis ocular o paciente foi encaminhado à internação hospitalar, e, na admissão, não apresentava alterações ao exame físico, exceto dor a palpação de região temporal bilateralmente. Exames laboratoriais foram coletados: teste de rápido de HIV e sorologias para hepatites virais foram negativas, PCR e VHS inalterados. O líquor era límpido, incolor, glicorraquia e proteinorraquia normais, haviam 5 células e pesquisas de BAAR, fungos e células neoplásicas foram negativas, O VDRL no Líquor foi não reagente. O FTA-ABS no líquor encontrava-se indisponível. Ressonância magnética de crânio e órbitas evidenciou comprometimento irregular da díploe craniana, notadamente frontoparietal esquerda, com aumento de partes moles extracranianas. A cintilografia de corpo inteiro com Gálio 67 demonstrou captação anormal discreta em região frontal direita e moderada a acentuada em região fronto-parietal esquerda, compatíveis com processo infeccioso. Assim ficou caraterizado quadro de sífilis ocular, e prováveis osteomielite sifilítica e sífilis otológica, todas manifestações dentro de um quadro de secundarismo, pois houve manifestação cutânea clássica e altos títulos de VDRL, denotando doença recente. Além do tratamento endovenoso com Penicilina G potássica por 14 dias, foi optado pelo sequenciamento oral com Doxiciclina por mínimo de 4 semanas, visto que a droga é ativa contra a espiroquetas e tem boa penetração em tecido ósseo. O paciente evoluiu com melhora progressiva dos sintomas e recebeu alta com proposta de realizar cintilografia de controle após 7 meses. Conclui-se que, apesar de raro, o diagnóstico de osteomielite por sífilis deve ser aventado nos casos de múltiplos órgãos acometidos pela doença.

Abstract Purpose To assess the action of pentoxifylline, administered by subcutaneous route, on skin flap tissue repair in rats, and to verify the histological aspects and biomarkers. Methods Thirty-two male Wistar rats were divided into four groups: control (CT) and treated with pentoxifylline (P1, P3 and P5). Modified McFarlane technique flap was used. Ten days later, the animals were euthanized and the areas of viable and necrotic tissue were evaluated. Hematoxylin/eosin staining was used to assess the morphometric characteristics of the number of vessels and epithelial thickness. Picrosirius red was used to assess collagen density. VEGF and TGF-?1 levels on the skin flap and serum of the animals were measured by the ELISA method. Results The macroscopic evaluation of the skin flap dimensions showed reduced necrotic tissue in the pentoxifylline (p < 0.05) treated groups. There was an increase in angiogenesis and reepithelization, demonstrated by analyses with an increased number of vessels (p < 0.05), VEGF and epithelial thickness. Fibrogenic effect showed decreased collagen density and TGF-β1 in the skin flap and serum. Conclusion The benefits of pentoxifylline administered by subcutaneous route, at dose 100 mg/kg, which was effective to improve the survival of skin flap by acting on tissue repair components, stimulating angiogenesis and reepithelization, in addition to reducing fibrogenesis

Versican and tumor-associated macrophages (TAMs) are involved in growth and metastases in several cancers. Here, we investigated the potential role of versican, a matrix proteoglycan, and its correlation with TAMs infiltrates in different stages of two different breast cancer models: spontaneous canine mammary gland carcinomas and the murine 4T1 breast cancer model. The stromal versican expression was correlated with TAMs accumulation in tumors with an advanced stage from spontaneous canine mammary carcinoma samples. Versican expression in mice, identified in late stages of tumor progression, was associated to a high number of peri-tumoral infiltrating TAMs. Indeed, TAMs were related to a pro-inflammatory and pro-angiogenic state in the primary tumor. Furthermore, TAMs accumulation was related to versican expression in the lungs and an increased number of pulmonary metastatic nodules with pulmonary mechanical dysfunction, which was due to leukocyte influx in the airways and elevated growth factor levels in the microenvironment. Thus, we suggest that versican and TAMs as attractive targets for breast cancer therapy.

Airports consume significant amounts of water which can be compared to the volume consumed by mid-size cities, thus practices aimed at reducing water consumption are important and necessar y. The objective of this study was to assess the reuse potential of sewage effluent produced at a mid-size international airport for nursery irri gation. The sewage treatment system consisted of a facultative pond followed by a constructed wetland, which were monitored during one hydrological year a nd the parameters COD, pH, solids, nitrogen, phosphorus and Escherichia coli we re analyzed. Removal efficiencies of 85% and 91% were achieved for C OD and solids, respectively. Removal efficiencies for ammonia nitrogen a nd total phosphorus were 77% and 59%, respectively. In terms of E. coli concen tration, the treated effluent met the recommendations by the World Health Organization for reuse in irrigation with the advantage of providing high levels of residual nutrient. The ornamental species Impatiens walleriana was irrigated with treated sewage effluent and plant growth characteristics were evalua ted. The experiment showed that reuse can enhance plant growth without signi ficantly affecting leaf tissue and soil characteristics. This study highlighted th e importance of simple technologies for sewage treatment especially in count ries which still do not present great investment in sanitation and proved that effluent reuse for landscape irrigation can provide great savings of water and financial resources for airport environments.

Based on the notion that inflammation favors tumorigenesis, our experiments comparatively assessed the influence of acute and chronic inflammation on the development of a murine mammary tumor (4T1). In addition, we characterized angiogenic and inflammatory markers in the tumor tissue and systemically. Subcutaneous implantation of polyether-polyurethane sponge discs in Balb/c mice was used to host 4T1 tumor cells (1x10(6)), which were inoculated intraimplant 24 h or 10 days post implantation. Flow cytometric analysis of enzyme-digested implants revealed that, after 24 hours, the population of leukocytes was primarily characterized by neutrophils (42.53% +/- 8.45) and monocytes (37.53% +/- 7.48), with some lymphocytes (16.27% +/- 4.0) and a few dendritic cells (1.82% +/- 0.36). At 10 days, macrophages were predominant (37.10% +/- 4.54), followed by lymphocytes (28.1% +/- 4.77), and monocytes (22.33% +/- 3.05), with some dendritic cells (13.60% +/- 0.55) and neutrophils (11.07% +/- 2.27). A mammary tumor grown in a chronic inflammatory environment was 2-fold when compared with one grown in acute inflammation and 5-fold when compared with tumor alone. The levels of pro-angiogenic cytokine (VEGF-Vascular Endothelial Growth Factor) were higher in implant-bearing tumor when 4T1 cells were grown in 10-day old implants as compared to the VEGF levels of the two other groups. Overall, the levels of the inflammatory markers evaluated (NAG -N-acetylglucosaminidase, TNF-α-Tumor Necrosis Factor-α) were higher in both groups of implant-bearing tumors and in serum from those animals when compared with the tumor alone levels. This inflammation-related difference in tumor growth may provide new insights into the contribution of different inflammatory cell populations to cancer progression.

ABSTRACT With the objective of promoting nature conservation, this study proposes an environmental zoning for a protected area that contains part of the remaining Atlantic Forest, a global hotspot that is home to several endangered species of flora and fauna. Conflicting therewith, the protected area is an important tourist attraction in the region and receives many visitors throughout the year. For the zoning preparation, the main factors and constraints of each objective (ecotourism and conservation) are established, and a multi-criteria analysis is used, in addition to the MOLA (Multi-Objective Land Allocation) algorithm, to establish the most suitable areas for each use. Extreme protection areas are defined through the intersection of endemic vegetation and soils and altitudes above 1,500 m. Thus, a fraction of 18% is obtained for extreme protection, 59% for conservation, and 23% for areas destined for ecotourism development. The results demonstrate the advantages of using these methods, due to their simplicity, which can support administrators in planning the protected area. The zoning is in accordance with the International Union for Conservation of Nature (IUCN) recommendation because the conservation area corresponds to approximately 77% of the park area, reinforcing the suitability and applicability of the methodology that is used for the zoning of a protected area.
