2. Evaluating the prevalence and predictive factors of vasomotor and psychological symptoms in prostate cancer patients receiving hormonal therapy: Results from a single institution experience

Abstract

Background & purpose: The impact of vasomotor symptoms (VMS) occurring in prostate cancer (PC) patients whilst on androgen deprivation therapy (ADT) has not been extensively researched. This longitudinal study sought to assess the VMS and identify any predictive factors. Material & methods: Data from 250 PC patients on ADT were prospectively evaluated between January 10 and August 13 using a physician-directed questionnaire, to assess the impact of VMS. Parameters including height, weight, body surface area (BSA), body mass index (BMI), duration/type of ADT, co-morbidities and ethnicity were recorded. Results: Fifty (20%) men reported no toxicity, whilst 171 (68.4%), and 29 (11.6%) reported mild to moderate and severe symptoms, respectively. Drenching sweats and hot flashes were common, and coexisted with sleep disturbances and fatigue. Patients with severe toxicity were younger (73 vs. 77 yrs; p = 0.04), had higher BMI (28 vs. 26; p = 0.02), and higher BSA (1.99 vs. 1.90; p = 0.04), when compared with those experiencing no toxicity. On multivariate analysis, younger age was predictive of sweats and hot flushes, whilst Afro-Caribbean men were twice as likely to experience sweats (OR 2.03, p = 0.05). Conclusions: The short-term side-effect profile of ADT for prostate cancer was favourable, though debilitating VMS can occur in a significant minority of cases. 5. Do Standardised Prognostic Algorithms Reflect Local Practice? Application of EORTC Risk Tables for Non-Muscle Invasive (pTa/pT1) Bladder Cancer Recurrence and Progression in a Local Cohort

Abstract

A risk calculator algorithm to allow prediction of probabilities of 1- and 5-year recurrence and progression rates in individuals with pTa/pT1 bladder cancer has been proposed by the European Organisation for Research and Treatment of Cancer (EORTC) and was incorporated into the European Association of Urology guidelines in 2006. We attempted to validate this algorithm in a cohort of patients with known outcome. Prognostic data were collected from a consecutively presenting cohort of 109 patients with non-muscle invasive (pTa/pT1) transitional cell cancer (TCC) at a single institution between 1983 and 1985. Using the same statistical models as in the EORTC original paper, predicted probabilities of 1- and 5-year recurrence and progression were calculated. Patients were divided into four risk groups for recurrence (Ir-IVr) and progression (Ip-IVp), respectively, using six prognostic criteria. These were then compared to the probabilities predicted in the EORTC algorithm. The predicted 1- and 5-year probabilities of recurrence were significantly higher in the study population as compared to the original EORTC algorithm for all four risk groups. The predicted 1-year probabilities for progression in groups Ip/IIIp and at 5-years for groups Ip/IIp were in accordance with the original algorithm, but were higher for the other progression groups. The concordance for the model of prediction using the study group for recurrence at 1 and 5 years was 62 and 63%, respectively, and for progression was 65 and 67, respectively. We were unable to validate the proposed algorithm in our group of patients. Although our study has limitations that prevent firm conclusions on the validity of the algorithm, it does expose some of the drawbacks of standardised nomograms when applied to local clinical practice.

6. Predictive Value of PTEN and AR Coexpression of Sustained Responsiveness to Hormonal Therapy in Prostate Cancer—A Pilot Study

Abstract

One limitation of current biochemical or histologic analysis of advanced prostate cancer (PC; T3/T4 ±Nx Mx) is the ability to identify on first diagnostic biopsy patients who will make a durable response to hormone ablation therapy. The aim of this study was to assess the predictive value (sustained response to hormonal therapy and clinical outcome (relapse-free and overall survival)) of phosphatase and tensin homolog (PTEN) and the androgen receptor (AR) immunoexpression in the presenting biopsy. Analysis was performed on 47 samples (10 cases of benign prostatic hyperplasia and 37 hormone-naive PCs). Patients selected represented two stages in the natural history of PC: The “clinical metastatic androgen–responsive” (androgen-dependent PC, ADPC) and the “clinical metastatic androgen–resistant” (androgen-independent PC, AIPC). Reduced immunoreactivity (IR) of either or both PTEN/AR in the initial hormone-naive PC samples was observed with increased frequency in AIPCs. In the ADPC group, low PTEN and/or AR-IR was associated with a shorter median relapse-free survival, i.e., at 30 months after surgery, the probability of relapse-free survival for high expressors of PTEN and AR was 85.7% (SEM = 9.3) compared with only 16.6% (SEM = 15.2) in low expressors. At 36 months, only 28.5% (SEM = 9.3) of ADPC high expressors had experienced a biochemical relapse compared with 100% of low expressors (hazard ratio, 4.6; 95% confidence interval, 4.7-146.8). Further studies analyzing the coexpression of PTEN and AR should be undertaken to validate this pilot study and the utility of these biomarkers in routine histopathologic workup of patients with PC.

Abstract

Despite multiple reports of overexpression in prostate cancer (PC), the reliance of PC cells on activated epidermal growth factor receptor (EGFR) and its downstream signaling to phosphoinositide 3'-kinase/Akt (PI3K/Akt/PTEN) and/or mitogen-activated protein kinase (MAPK/ERK) pathways has not been fully elucidated. In this study, we compared the role of EGF-mediated signaling in nonmalignant (BPH-1, PNT1A, and PNT1 B) and PC cell lines (DU145, PC3, LNCaP, and CWR22Rv1). EGF-induced proliferation was observed in all EGFR-expressing PC cells except PC3, indicating that EGFR expression does not unequivocally trigger proliferation following EGF stimulation. ErbB2 recruitment potentiated EGF-induced signals and was associated with the most pronounced effects of EGF despite low EGFR expression. In this way, the sum of EGFR and ErbB2 receptor phosphorylation proved to be a more sensitive indicator of EGF-induced proliferation than quantification of the expression of either receptor alone. Both Akt and ERK were rapidly phosphorylated in response to EGF, with ERK phosphorylation being weakest in PC3 cells. Extrapolation of these findings to clinical PC suggests that assessment of phosphorylated EGFR + ErbB2 together could serve as a marker for sensitivity to anti-EGFR-targeted therapies.

Abstract

In prostate cancer (PC), increasing evidence suggests that androgen receptor (AR) signalling is functional under conditions of maximal androgen blockade. PC cells survive and proliferate in the altered hormonal environment possibly by interactions between growth factor-activated pathways and AR signalling. The present review article summarizes the current evidence of this crosstalk and focuses on the interactions among the ErbB receptor network, its downstream pathways, the AR. The potential role of this crosstalk in the development of androgen independence and in relation to antiandrogen therapy is discussed. Such interactions provide insight into possible complementary or additional strategies in the management of PC.

Abstract

MUC1 mucin is transcriptionally regulated by estrogen, progesterone, and glucocorticoids. Our objective was to determine whether androgen receptor. (20AR) activation regulates expression of MUC1. The following breast and prostatic cell lines were phenotyped and grouped according to AR and MUC1protein expression: 1) AR+MUCi + [DAR17+19. (20AR transfectants of DU-145), ZR-75-1, MDA-MB-453, and T47D]; 2) AR-MUCi+ [DZeoi. (20AR- vector control), DU-145, BT20, MDA-MB231, and MCF7]; 3) AIR +MUCi -. (20LNCaP and LNCaP-r). Cell proliferation was determined using the MTT assay in the presence of synthetic androgen R1881, 0.1 pM to 1 µM. Cell surface MUC1expression was determined by flow cytometry in the presence or absence of oestradiol, medroxy progesterone acetate or R1881, with and without 4 hydroxy-flutamide. (204-OH), a nonsteroidal AR antagonist. The functional significance of MUC1expression was investigated with a cell-cell aggregation assay. Only AR+ MUC1 + cell lines showed a significant increase in MUC1expression with AR activation. (20P. (20range) =.01 to .0001), reversed in the presence of 4-OHF. Cell proliferation was unaffected. Increased expression of MUC1was associated with a significant. (20P. (20range) =.002 to .001) reduction in cell-cell adhesion. To our knowledge, this is the first description of androgen-dependent regulation of MUC1mucin. This is also functionally associated with decreased cell-cell adhesion, a recognised feature of progressive malignancy. These findings have important implications for physiological and pathological processes.

Abstract

I was busy conducting experiments, writing papers, and trying to finish my dissertation. But when I was asked to serve on a faculty job search committee, I felt I couldn't say no. I thought it would be a great opportunity to learn how I might eventually secure a faculty position of my own. However, as I read through reams of impressive applications, reality struck: It would take a lot of time and many sacrifices to build a CV that would be competitive in a faculty job search—and I could still end up empty-handed. For the first time, I began to question the academic career path I was on. > “I was startled to learn that academic achievements were not always what mattered most.” Until then, I hadn't had any reason to doubt it. I was passionate about science, research, and teaching. I already had a respectable handful of publications, awards, and grants. I certainly knew I couldn't immediately jump to a faculty position—I'd need to do a postdoc or two first—but I was confident I was on the right path to securing one. But reading through the stack of more than 200 faculty job applications shook my confidence. Each told a story of a strong devotion to science. Nearly all of them had daunting lists of publications, citations, accomplishments, and awards. Many of the applicants had magnificent letters of recommendation. And some held tenured faculty positions. A few did not stand a chance because of too little experience, sparse accolades, or unflattering letters of recommendation. But the majority were exceptionally well qualified—and they were all seeking one position. I spent a week sorting the applications into piles marked “definitely yes,” “unsure,” and “definitely no,” ultimately landing on a short list of 10 applicants. Then I entered a conference room to meet with the four professors serving on the committee with me. As each of us made a case for who should be invited for an interview and why, I was startled to learn that academic achievements were not always what mattered most. Many candidates whom I found exceptional were eliminated quickly because some committee members felt their specialty wasn't needed in the department. I was also disheartened to find that a few committee members who had well-funded labs and domineering personalities exerted a disproportionate influence on decisions. One professor strongly advocated for his own postdoc, even though his CV was not at all competitive. After considerable debate—and without consensus—he was invited for an interview anyway. As I returned to the lab, I envisioned a bleak future. I could spend years making considerable sacrifices—working long hours, uprooting my wife for a series of postdocs, putting off starting a family—only to fall short of securing a faculty position due to factors outside my control. Around that time, I also got my first exposure to working in a corporate setting. I started a part-time internship at a telecommunications company, which paid better than a teaching assistantship, allowing me to work fewer hours and spend more time writing my dissertation. I appreciated that the company valued data analyses and critical thinking, and I found it gratifying to see the impact of my work. With my interest piqued, I sat in on courses in my university's business program and read as much as I could about marketing, strategy, and business research. I began to see a career in business as an appealing alternative to the personal sacrifices, hypercompetitiveness, and arbitrary hiring decisions I would encounter in academia. After I graduated, I made the leap. It has now been well over 20 years, and I have no regrets. At first, I worried that once I learned how to be successful in business, the work would not satisfy my intellectual curiosity. However, my career, which has taken me to a variety of companies and to my current role managing a consulting business, has delivered plenty of intellectual challenges. I have also benefited in my personal life. Because my consulting work affords flexibility, I am able to live in the city of my choice and be actively involved in my kids' activities. My experience on the faculty job search committee may not have been what I expected. But I'm thankful it sent me on a path to reevaluating my future and moving toward a different—and extremely rewarding—career.

Abstract

Introduction Erythema nodosum is often associated with a distressing symptomatology, including painful subcutaneous nodules, polyarthropathy, and significant fatigue. Whilst it is a well-documented side-effect of estrogen therapy in females, we describe what we believe to be the first report in the literature of erythema nodosum as a result of estrogen therapy in a male. Case presentation A 64-year-old Afro-Caribbean man with locally advanced carcinoma of the prostate agreed to participate in a randomized controlled trial comparing estrogen patches with luteinizing hormone-releasing hormone analogs to achieve androgen deprivation, and was allocated to the group receiving estrogen patches. One month later he presented with tender lesions on his shins and painful swelling of his ankles, wrists, and left shoulder. This was followed by progressive severe fatigue that required hospital admission, where he was diagnosed with erythema nodosum by a rheumatologist. Two months after discontinuing the estrogen patches the erythema nodosum, and associated symptoms, had fully resolved, and to date he remains well with no further recurrence. Conclusion Trial results may establish transdermal estrogen as an alternative to luteinizing hormone-releasing hormone analogs in the management of prostate cancer, and has already been established as a therapy for male to female transsexuals. It is essential to record the toxicity profile of transdermal estrogen in men to ensure accurate safety information. This case report highlights a previously undocumented toxicity of estrogen therapy in men, of which oncologists, urologists, and endocrinologists need to be aware. Rheumatologists and dermatologists should add estrogen therapy to their differential diagnosis of men presenting with erythema nodosum.

Abstract

Thievery of food items among colonies of a ponerine ant,Ectatomma ruidum was common; nonnestmates in colonies or near the colony entrances receive incoming food items and carry them to their own colony. In our study area 7 of 10 colonies were victimized by thief ants. Colonies have discrete home ranges and home range size is correlated with the number of workers in the colony. Worker ants discriminate nestmates from non-nestmates when non-nestmates are presented at colony entrances, but individuals from different colonies were not observed to engage in agonistic interactions away from nest entrances. Non-nestmates gain entrance to colonies when the entrance is unguarded. Many instances of non-nestmates being removed from colonies by residents were observed. The costs and benefits of theft under these circumstances are considered.
