Background Domiciliary medication reviews are thought to enable comprehensive medication reviews centred around the needs of individuals. However, there is no clear consensus on where the value of these services lie. Aim To determine the value of domiciliary medication reviews to service providers through semi-structured focus groups, interviews and thematic analysis. Method Study participants were recruited from domiciliary medication review services provided in the United Kingdom. Semi-structured focus groups and interviews were analysed using thematic analysis. Results Six themes were identified: the scope of domiciliary medication review services, the professional role, advantages over traditional settings, disadvantages of domiciliary medication reviews for the professional, levels of engagement and outcomes. Conclusion Pharmacy professionals believe that the domiciliary setting provides advantages over traditional healthcare settings when conducting medication reviews. They believe it enables a more in-depth review of an individual’s medications and needs. The traditional clinical outcomes recorded by services may not be capturing the holistic impact of domiciliary medication reviews.

Background Medication reviews are recognised as essential to tackling problematic polypharmacy. Domiciliary medication reviews (DMRs) have become more prevalent in recent years. They are proclaimed as being patient-centric but published literature mainly focuses on clinical outcomes. However, it is not known where the value of DMRs lies for patients who participate in them. Objective To determine the value of domiciliary medication reviews to service users. Setting Interviews took place with recipients of domiciliary medication reviews residing in the London boroughs of Islington and Haringey. Method Semi-structured interviews analysed using thematic analysis. Main outcome measure Themes and sub-themes identified from interview transcripts. Results Five themes were identified: advantages over traditional settings, attributes of the professional, adherence, levels of engagement and knowledge. Conclusion For many patients, the domiciliary setting is preferred to traditional healthcare settings. Patients appreciated the time spent with them during a DMR and felt listened to. Informal carers felt reassured that the individual medication needs of their relative had been reviewed by an expert.

Objectives Medication reviews in the domiciliary setting are becoming more prevalent internationally. Understanding the benefits of these reviews is essential to ensuring quality healthcare services. To date there has not been a systematic evaluation of the outcomes of these services and their impact on patients. A systematic review of the literature was undertaken with a view to understanding the impact of medication reviews in this setting. Controlled and uncontrolled studies were included. Outcomes were categorised according to the ECHO model. A narrative synthesis was developed. Key findings Nineteen out of 31 papers included demonstrated an improvement in outcome. Clinical outcomes were the most commonly measured and humanistic outcomes the least commonly measured. Domiciliary medication reviews (DMRs) services are presented as providing benefit. However, it is difficult to quantify the impact of services from the published outcomes. Summary Future work should focus on demonstrating the meaningful changes to patients that DMRs have enabled.

Twenty-eight empirical studies provided data for four meta-analyses on visual detection and discrimination inflight-decktypical tasks, at varying degrees of eccentricity relative to a central point of interest. The data revealed a general trend for poorer performance at increasing eccentricity, and greater degradation when eye movements were prevented. The data failed to reveal a systematic discontinuity of performance degradation beyond 15°, which defines the typical “primary field of view” in the cockpit, but they reveal a 14% miss rate and 21% discrimination error rate at that location. The results also point to the profound influence of moderator variables of expectancy and salience on eccentric visual performance.

This study examines the relationship between developmental outcomes of high-risk infants and parental perceptions and expectations. Parents of 209 consecutive high-risk infants were asked to provide their development interpretations and expectations before their infants received standard developmental assessments between April and October 1989. Moderate correlations between parents and professional assessments of motor and language skills were noted (p less than .05 to p less than .01). Most agreements occurred when infants were assessed as normal by professionals. Disagreements were common and occurred in all areas of development. These mismatches were not associated with gestational age at birth, neonatal complications, poverty, or estimates of parental experience. Professionals should take seriously any expressed developmental concerns by parents of high-risk infants. Expressed developmental concerns, however, cannot be relied on for developmental screening of high-risk infants.

- Abstract
De Corpore et Sanguine Domini is a seminal document in the history of a western tradition of teaching on the sacrament of eucharist. This teaching emphasizes the "real" or bodily presence of Christ on the altar as opposed to a "spiritual" or symbolic presence. The dissertation examines one aspect of several visions which comprise Chapter XIV of the document--the image of Christ present in the eucharist as a child--and shows that the document fosters a spirituality of dynamic, internal change within the participant, rather than a worship of Christ's body and blood. As part of the dissertation, Chapter XIV of De Corpore is translated from the Latin and incorporated in an Appendix. The early chapters of the dissertation give the theology and textual history of the treatise. The tribal religious concerns of its Saxon audience are briefly introduced, and the text of Chapter XIV is examined in some detail. The history of a Christian tradition of visions and miracles is also presented. Finally, the general psychological system of C. G. Jung is introduced in order to explain the subjective component of certain visions and to build a base of understanding for a process of personal transformation on which the remainder of the dissertation is focussed. The dissertation continues with an examination of the synthesis of personality which Jung terms the "Self," and adduces a pattern of human existence which includes wounding and transformation. This pattern of existence is shown to be typified in the figure of Christ, but is also present in the mythology of certain pagan gods. Radbert's use of a Divine Child symbol promotes a personal and emotional identification of the worshipper with the Christ Child. This placing of Christ as the center of one's spiritual life implies a corresponding removal of older, deeply entrenched Saxon pagan images.
