2. Comparative efficacy of commercially available alcohol-based hand rubs and World Health Organization-recommended hand rubs: Formulation matters

Abstract

Background Use of alcohol-based hand rubs (ABHRs) effectively reduces transmission of pathogenic microorganisms. However, the impact of alcohol concentration and format on product efficacy is currently being debated. Methods Two novel ABHR formulations containing 70% ethanol were evaluated according to American Society for Testing and Materials E1174 (Health Care Personnel Handwash [HCPHW]) and European Norm (EN) 1500 global standards. Additionally, using E1174, the efficacy of these formulations was compared head-to-head against 7 representative commercially available ABHRs and 2 World Health Organization recommended formulations containing alcohol concentrations of 60% to 90%. Results The novel ABHR formulations met efficacy requirements for both HCPHW and EN 1500 when tested at application volumes typically used in these methods. Moreover, these formulations met HCPHW requirements when tested at a more realistic 2-mL product application. In contrast, the commercial ABHRs and World Health Organization formulations failed to meet HCPHW requirements using a 2-mL application. Importantly, product performance did not correlate with alcohol concentration. Conclusion Product formulation can greatly influence the overall antimicrobial efficacy of ABHRs and is a more important factor than alcohol concentration alone. Two novel ABHRs based on 70% ethanol have been formulated to meet global efficacy standards when tested at volumes more representative of normal product use in health care environments.

Abstract

Background The CDC Hand Hygiene Guidelines recommend washing hands with soap when hands are visibly soiled. Pending changes to the United States healthcare antiseptic regulations are decreasing the availability of antimicrobial soap active ingredients making it important to understand key performance differences across soap types. The purpose of this study was to investigate the germ removal properties of a novel, nonantimicrobial soap exhibiting improved interfacial tension properties, a measure of the interaction of the soap with skin. Methods The novel nonantimicrobial soap was compared with a control nonantimicrobial soap. In study 1, the soaps were tested according to ASTM E2755 to determine reduction of Serratia marcescens after one use where 5 mL of soap was applied to dry hands, lathered 30s and rinsed 30s (N = 12). Studies 2 and 3 compared the products under more realistic test conditions, including a more relevant healthcare pathogen, more realistic product application and in study three skin condition representative of healthcare worker skin. The second study compared the novel soap and the control soap for Staphylococcus aureus removal using ASTM E2755 with 1.8 mL of soap applied to dry hands, lathered for 30s and rinsed for 10s (N = 12). The third study used an ex vivo skin model of dry, irritated human skin to evaluate S. aureus removal. Statistical comparisons between soaps were made using a paired t-test (α = 0.05). Results In all three studies, the novel nonantimicrobial soap was superior to the control soap for bacteria removal. In study 1, the novel soap achieved a 2.26 log10 reduction compared with a 1.70 log10 reduction for the control soap (P < 0.0001). In studies 2 and 3, the nonantimicrobial soap achieved log10 reductions that were 0.34 (P = 0.0236) and 0.53 (P = 0.005) greater than the control soap, respectively. Conclusion This study indicates that a nonantimicrobial soap can achieve a high level of microbe removal (>99%) on skin. Additionally, product formulation appears to impact the microbial removal properties of nonantimicrobial soap on both healthy human subjects, and on dry irritated human skin. Abstract

Methods: Test specimens were ten cm squares of pebbled vinyl, brushed aluminum, Formica, stainless steel and Plexiglas. Ten specimens comprised a group. The disinfectant wipes was Birex (dual phenolic, Biotrol) and deionized water (same wipe fabric and liquid amounts). Used was a wipe-discard-wipe method with two towelettes per two, three or five soiled specimens. There were a total of 1920 disinfections (an estimated yearlong exposure) for each specimen. Tests included glossiness change, blood removal and residual surface ATP. After each 100 disinfections, 0.1 mL of defibrinated sheep blood went onto center 8 cm of each specimen, followed by an hour of drying, a single disinfection and then testing. Evaluation of specimen glossiness employed a Gloss Checker (Horibas). A PBSmoistened swab then sampled half the center area and went into 2.0 mL of PBS and vortexed. A Hemastix dipstick test then determined the amount of blood present. A 3M ATP swab then sampled the remaining area.
