Abstract

The relationship between allergen exposure and the onset of or protection from allergic diseases remains unclear. Many factors could be related to immunological responses, such as the age when the exposure occurs, type of allergen, timing, dose, and allergen route. In this study, we investigated whether exposure to respiratory allergens could occur in pregnancy or early life. In particular, we assessed whether Der p 1 and Blo t 5, as well as specific antibodies against these allergens, could be detected in 90 paired cord blood and colostrum samples. Der p 1 was detected in 58.6% of colostrum and 29% of cord blood samples, whereas Blot 5 was positive in 41.3% and 9.6% of the samples, respectively. Similar to specific IgA, which could be detected in all samples for both mites, specific IgG was found in a high number of colostrum samples, 93.5% and 94.8% for Dp and Bt, respectively. Although allergens were not detected in all cord blood samples, a high percentage of them (≥95%) were positive for specific IgM to both mites in cord blood samples, suggesting that neonates can be exposed and sensitized to airborne allergens during pregnancy. Many studies have attempted to correlate allergen exposure or its prevention in early infancy with the onset of or protection from allergic diseases. However, conflicting and inconsistent data do not show a clear correlation with or suggest a way to prevent allergen sensitization. Nevertheless, these unconvincing results could be better understood if the relationship with many aspects of allergen exposure after pregnancy could be clarified. Thus, it is necessary to address basic issues related to allergen exposure, including the development of reproducible, standardized and reliable methods, and to determine how and where the exposure occurs.

Abstract

In addition to being a source of nutrients for the developing newborn, human milk contains thousands of bioactive compounds, which influence infant health in the short-term as exemplified by its major benefits on infectious disease prevention. Many of the human milk compounds also have the required characteristics to instruct immune development and guide long-term health. Prebiotics, probiotics, and varied antimicrobial molecules all have the potential to shape the composition and function of the establishing gut microbiota, which is known to be a major determinant of immune function. Another and less explored way human milk can instruct long-term immunity is through antigen shedding. Here, we will review the evidence that antigens from maternal environment and more specifically from allergen sources are found in human milk. We will discuss data from rodent models and birth cohorts showing that allergen shedding in breast milk may influence long-term allergy risk. We will uncover the variables that may underlie heterogeneity in oral tolerance induction and allergy prevention in children breast-fed by allergen-exposed mothers. We will focus on the parameters that control antigen transfer to breast milk, on the unique biological characteristics of allergens in breast milk, and on the milk bioactive compounds that were found to influence immune response in offspring. We propose this understanding is fundamental to guide maternal interventions leading to lifelong allergen tolerance.

Abstract

Background Oral tolerance induction in early life is a promising approach for food allergy prevention. Its success requires the identification of factors necessary for its persistence. Objectives We aimed to assess in mice duration of allergy prevention by breastfeeding-induced oral tolerance and whether oral TGF-β supplementation after weaning would prolong it. Methods We quantified ovalbumin (OVA) and OVA specific immunoglobulins levels by ELISA in milk from the EDEN birth cohort. Since OVA specific Ig were found in all samples, we assessed whether OVA-immunized mice exposed to OVA during lactation could prevent allergic diarrhea in their 6 and 13-week-old progeny. In some experiments, a TGF-β enriched formula was given after weaning. Results At 6 weeks, only 13% and 34% of mice breastfed by OVA-exposed mothers exhibited diarrhea after 6 and 7 OVA challenges versus 44% and 72% in mice breastfed by naive mothers (p=0.02 and 0.01). Abstract

International audience; Background: Successful prevention of food allergy requires the identification of the factors adversely affecting the capacity to develop oral tolerance to food antigen in early life. Objectives: This study sought to determine whether oral exposure to Dermatophagoides pteronyssinus through breast milk affects gut mucosal immunity with long-term effects on IgE-mediated food allergy susceptibility. Methods: Gut immunity was explored in 2-week-old mice breast-fed by mothers exposed to D pteronyssinus, protease-inactivated D pteronyssinus, or to PBS during lactation. We further analyzed oral tolerance to a bystander food allergen, ovalbumin (OVA). In a proof-of-concept study, Der p 1 and OVA levels were determined in 100 human breast milk samples and the association with prevalence of IgE-mediated egg allergy at 1 year was assessed. Results: Increased permeability, IL-33 levels, type 2 innate lymphoid cell activation, and TH2 cell differentiation were found in gut mucosa of mice nursed by mothers exposed to D pteronyssinus compared with PBS. This pro-TH2 gut mucosal environment inhibited the induction of antigen-specific FoxP3 regulatory T cells and the prevention of food allergy by OVA exposure through breast milk. In contrast, protease-inactivated D pteronyssinus had no effect on offspring gut mucosal immunity. Based on the presence of Der p 1 and/or OVA in human breast milk, we identified groups of lactating mothers, which mirror the ones found in mice to be responsible for different egg allergy risk. Conclusions: This study highlights an unpredicted potential risk factor for the development of food allergy, that is, D pteronyssinus allergens in breast milk, which disrupt gut immune homeostasis and prevents oral tolerance induction to bystander food antigen through their protease activity. © 2020 American Academy of Allergy, Asthma & Immunology

Abstract

Increased risk of allergy during early life indicates deficient immune regulation in this period of life. To date, the cause for inefficient neonatal immune regulation has never been elucidated. We aimed to define the ontogeny of oral tolerance and to identify necessary conditions specific for this stage of life. Ovalbumin (OVA) was administered orally to mice through breast milk and efficiency of systemic tolerance to OVA was assessed in adulthood using a model of allergic airway inflammation. Oral tolerance induction was fully efficient starting third week of life. Inefficiency in neonates was a consequence of abnormal antigen transfer across the gut barrier and retinaldehyde dehydrogenase expression by mesenteric lymph node CD103(+) neonatal dendritic cells, resulting in inefficient T-cell activation. Neonates' serum retinol levels were three times lower than in adult mice, and vitamin A supplementation was sufficient to rescue neonatal defects and allow tolerance induction from birth. The establishment of oral tolerance required the differentiation of Th1 lymphocytes in both vitamin A-supplemented neonates and 3-week-old unsupplemented mice. This knowledge should guide the design of interventions for allergy prevention that are adapted to the neonatal stage of life such as vitamin A supplementation.

Resume Le systeme immunitaire du nouveau-ne est soumis a un defi considerable devant a la fois faire face a une stimulation antigenique massive du fait de la colonisation microbienne de la peau et du tractus digestif ainsi que de l’exposition a une multitude d’antigenes environnementaux. Outre ses vertus nutritives et son impact sur la prevention des maladies infectieuses, le lait maternel pourrait contribuer a l’education du systeme immunitaire vers l’acceptation des antigenes du soi et de l’environnement inoffensifs. En effet, des etudes epidemiologiques rapportent un effet protecteur de l’allaitement vis-a-vis de maladies dues a un defaut de tolerance immunitaire, telles que le diabete de type 1, la maladie cœliaque et les allergies. Cependant, les donnees de la litterature sont controversees et les mecanismes peu identifies. Dans cette revue, nous presenterons et discuterons les donnees qui suggerent que l’allaitement pourrait exercer des effets protecteurs a l’egard des maladies allergiques par induction de tolerance immunitaire suite au transfert d’allergene dans le lait. Nous presenterons les facteurs requis pour conferer ce potentiel tolerogene au lait maternel. Une meilleure comprehension des mecanismes de tolerance en debut de vie et de l’impact de la mere sur ce processus via l’allaitement devrait permettre de proposer de nouvelles strategies de prevention des maladies allergiques.

Abstract

There is strong evidence from animal models that placental and/or breast milk-mediated transfer of maternal allergen-specific IgG prevents allergic immune responses in the progeny. Both human and animal data also point to IgA as having an important regulatory role. In contrast, little is known about maternal transfer of IgG and IgA specific for respiratory allergens in humans. Dermatophagoides pteronyssinus (Der p) is an indoor allergen that is a major cause of asthma worldwide. We analysed maternal to child Der p-specific IgG and IgA transfer in a cohort of 77 paired maternal and child samples. We found Der p-specific IgG and its IgG1, IgG2 and IgG4 subclasses in all cord blood samples. Except for IgG1, cord levels were higher in newborns from atopic mothers (n = 29) compared to non-atopic mothers (n = 48). Der p-specific IgA was found in all colostrum samples and levels were independent of maternal atopic status. Notably, anti-Der p IgG was also found in colostrum and levels were higher in atopic mothers. We believe that our work is a critical first step in the identification of early factors that may impact asthma development and should guide the development of clinical studies that assess whether Der p-specific IgG and IgA protect children from allergy as demonstrated in animal models.

Existem fortes evidências de que a supressão da hipersensibilidade nos recém nascidos pode ser mediada pela transferência de anticorpos maternos, dependendo de sua concentração e especificidade, no entanto carece estudos sobre a eficácia em humanos. Realizamos este estudo a fim de caracterizar qualitativa e quantitativamente a transmissão de anticorpos direcionados ao principal alérgeno da poeira domiciliar (Der p) via placenta e colostro materno, assim como investigar o efeito da sensibilização materna ao Der p na transferência passiva destes anticorpos. Para tais objetivos, analisamos amostras de sangue materno, cordão umbilical e colostro de puérperas sensibilizadas. Demonstramos pela primeira vez que S-IgA anti-Der p pode ser transferida ao lactente em concentrações bastante variáveis e com alto índice de avidez, independente da sensibilização materna ao mesmo ácaro. Demonstramos também que o nível de IgG específica ao Der p é mais elevado em recém nascidos de mães sensibilizadas quando comparado aos de mães controle não sensibilizadas. Já a avidez específica da IgG anti-Der p foi muito semelhante entre as amostras pareadas de cordão umbilical e soro materno, assim como em amostras do grupo estudo e grupo controle. It is known that the incidence of allergic disease has been rising very fast in the last decade and nowadays affects thousands of children worldwide. For this reason, it is of great interest that efficient strategies of prevention of atopy should be applied in the first years of life or even before birth. The are strong evidences that the suppression of hypersensitivity in newborn can be mediated by the transference of maternal antibodies, depending on their concentration and specificity, however still little is known about the mechanisms involving, in special in humans. We made this study aiming to characterize qualitatively and quantitatively the antibodies transmission directed to the main home dust allergen (Dermatophagoides pteronyssinus; Der p) through placental transference and maternal breastfeeding as well as to investigate the maternal sensitizing effect against to Der p in passive transference of these antibodies. For those objectives, we quantified by ELISA, IgG anti-Der p in paired samples of maternal blood and umbilical cord and anti-Der p S-IgA in colostrums of sensitized mother (n=13) and not sensitized (n=26); and we analyzed the functional activity of the same antibodies by avidity assays. The sensibility was determined in maternal sera by specific RAST (Cap System® Pharmacia). We show by the first time that anti-Der p S-IgA is transferred to the infant in very variable concentrations and with high levels of avidity, but is not dependent of maternal sensitization. We believe that breastfeeding is important, because it supplies S-IgA with the capacity to neutralize in a specific manner and block the entrance of Der p through mucosa, in infant of RAST+ mothers as well as of RAST-. We also demonstrate that total and specific to Der p IgG levels are more elevated in newborns of sensitized mothers when compared to of those of control mothers and non-sensitized indicating that the maternal sensitizing can influence the fetal immune response. In the other hand, the specific avidity of anti-Der p IgG was very similar between paired samples of umbilical cord and maternal sera, as well as in samples of study group and control group, suggesting thar the avidity index of IgG does not influence on placental transfer of specific antibodies. Once that the maternal antibody transference represent a important mechanism for immunomodulation of allergic response, we expect that a better understanding of the influence of maternal sensitivity on passive transfer of specific antibodies to babies will contribute with advances in the elaboration of adequate strategies of prevention of allergic sensitivity with more efficient therapeutic results.

Abstract

International audience; Introduction.– Impact of exposure to environmental allergens during early life on allergic sensitization and disease development is controversial. Here, we investigated whether airborne allergen from Dermatophagoides pteronyssinus (Der p) house dust mite (HDM), a major cause of allergic asthma, is found in human colostrum and milk samples and set up a mouse model to assess its impact on allergic outcome in offspring. Methods.– Der p 1was quantified in human milk (1 and 6 months postpartum samples from Australia, n = 71 and n = 161, respectively) and colostrum from Brasil (n = 76), France (n = 97) and Australia (n = 34) by ELISA. A basophil degranulation assay was used to confirm immunogenicity of ELISA-detected Der p. Balb/c mice were fostered by mothers exposed intra-nasally to Der p extract before pregnancy and during lactation. Progeny response to Der p was measured at 6 weeks and included assessment of allergic sensitization, airway inflammation and lungs function. Results.– Der p 1 was present in 58% Brazilian, 70% French, and 78% Australian colostrum. Median [Der p 1] was similar between countries (96 pg/mL; IQR 50.2–201.6 pg/mL). In mature milk, Der p1 was found in 52% of 1-month and 6-month samples. Median [Der p 1] in milk was 65.9 pg/mL; IQR 41.5–172 pg/mL) and was significantly lower than in colostrum (P = 0.0001). Der p 1-containing milks were able to induce degranulation of human anti-Der p IgE coated basophils. Mice breastfed by Der p-exposed mothers had 5-fold increased levels of Der p specific IgE and IgG1 compared to mice breastfed by naïve mothers and their allergic airway inflammation and lung function was not affected. Conclusion.– This study demonstrates that early life exposure to ubiquitous respiratory allergens can take place through breastfeeding. An animal model mimicking the human situation shows early life exposure to Der p through milk primes the immune system rather than protect it as we reported with OVA. These data highlight that antigen administration to the neonate through the oral route may contribute to child allergic sensitization and have important implications for the design of studies assessing early oral antigen exposure for allergic disease prevention.

Abstract

Evergreen species of temperate regions are dominant in low-nutrient soils. This feature is attributed to more efficient mechanisms of nutrient economy. Nevertheless, the cashew (Anacardium occidentale- Anacardiaceae), a deciduous species, is native to regions in Brazil with sandy soil, whilst the annatto (Bixa orellana- Bixaceae), classified as an evergreen species native to tropical America, grows spontaneously in regions with more humid soils. Evergreens contain robust leaves that can resist adverse conditions for longer. The physical aspects of the leaves and mechanisms of nutrient economy between the two species were compared, in order to verify whether the deciduous species had more efficient mechanisms that might explain its occurrence in regions of low soil fertility. The mechanisms of nitrogen economy were also compared for the two species at available concentrations of this nutrient. The following were analysed: (i) leaf life span, (ii) physical leaf characteristics (leaf mass per area, and rupture strain), (iii) nitrogenous compounds (nitrogen, chlorophyll, and protein), (iv) nitrogen conservation mechanisms (nitrogen resorption efficiency, resorption proficiency, and use efficiency), and (v) nitrogen conservation mechanisms under different availability of this mineral. The higher values of leaf mass per area and leaf rupture strain found in A. occidentale were related to its longer leaf life span. A. occidentale showed lower concentrations of nitrogen and protein in the leaves than B. orellana. Under lower nitrogen availability, A. occidentale had higher nitrogen resorption proficiency, nitrogen use efficiency and leaf life span than B. orellana. These characteristics may contribute to the adaptation of this species to sandy soils with low nitrogen content.Perenifólias de clima temperado são dominantes em solos pouco férteis. Essa característica é atribuída a mecanismos mais eficientes de economia de nutrientes. O cajueiro (Anacardium occidentale - Anacardiaceae), uma espécie decídua, é nativo de região do Brasil com solos arenosos, enquanto que o urucuzeiro (Bixa orellana - Bixaceae), uma perenifólia, nativa da América tropical, é espontâneo em regiões com solo mais úmido e fértil. As perenifólias apresentam folhas robustas, adaptadas a resistir a adversidades por mais tempo. Compararam-se aspectos físicos das folhas e mecanismos de economia de nutrientes entre as duas espécies, a fim de verificar se a espécie decídua apresenta mecanismos mais eficientes, que expliquem sua ocorrência em locais pouco férteis. Foram também comparados os mecanismos de economia de nitrogênio sob disponibilidades diferentes deste nutriente. Foram avaliados: (i) longevidade foliar, (ii) características físicas das folhas (massa por área e tensão de ruptura), (iii) concentração de compostos nitrogenados nas folhas (nitrogênio, clorofila e proteínas), (iv) mecanismos de conservação de nitrogênio (eficiência de reaproveitamento, proficiência de reaproveitamento e eficiência do uso), (v) mecanismos de conservação de nitrogênio sob disponibilidades diferentes deste elemento. A maior longevidade foliar de A. occidentale foi relacionada a valores maiores de massa foliar específica e tensão de ruptura foliar. A. occidentale apresentou menores concentrações de nitrogênio e proteínas. Sob menor disponibilidade de nitrogênio A. occidentale apresentou maiores valores de proficiência de reaproveitamento e eficiência do uso de nitrogênio e maior longevidade foliar. Essas características podem ter contribuído para a adaptação dessa espécie a locais com solos arenosos mais pobres em nitrogênio.

Abstract

Results Der p 1 was present in 58% Brazilian, 70% French, and 78% Australian colostrum. Median [Der p 1] was similar between countries (96 pg/mL). In mature milk, Der p1 was found in 55% of samples, median [Der p 1] was 65·9 pg/mL and was significantly lower than in colostrum (p=0·0001). Der p 1-containing milks were able to induce basophils degranulation. Mice breastfed by Der p-exposed mothers had 5-fold increased levels of Der p specific IgE and IgG1 compared to mice breastfed by naive mothers. Their allergic airway inflammation was not affected.
