El desarrollo de la psicología en Guatemala durante las últimas décadas se ha visto fortalecido por varios hitos en su historia como han sido los congresos, encuentros académicos y profesionales, colaboraciones internacionales y conferencias para nombrar algunos. Este número especial de nuestra revista da testimonio de uno de ellos. Gracias a la confianza, apoyo y mentoría de los grandes exponentes de las psicoterapias integrativas de América Latina y de la valiosa y comprometida colaboración de un comité organizador, pudo realizarse en Guatemala, de manera virtual con sólido apoyo tecnológico, el VIII Encuentro Latinoamericano de Psicoterapias Integrativas (ELAPSI). Si bien, el producto final fue de una excelente calidad académica, científica, profesional y sociocultural, también fue de inmenso valor el llegar a concretarlo. Desde el momento en que nos fue confiada la realización del mismo en el marco del VII Encuentro Latinoamericano que se llevó a cabo en Uruguay en 2018, sabíamos que estábamos abriendo brecha. Por primera vez, un ELAPSI sería realizado fuera de América del Sur, en donde históricamente se había congregado la comunidad académica y profesional de las psicoterapias integrativas. Por lo tanto, iniciar el proceso de organización implicaba no solo colocar a Guatemala en el mapa latinoamericano de la integración psicoterapéutica sino también consolidar un equipo organizador que representara las voces integrativas de las Américas particularmente de aquellos países donde hasta el momento estaban casi silenciadas. Los valores de solidaridad, apoyo, entusiasmo y compromiso se hicieron presentes para emprender una tarea que, aunque ambiciosa desde sus albores, fue apasionante.

Repeated herbicide applications in agricultural fields exert strong selection on weeds such as blackgrass ( Alopecurus myosuroides ), which is a major threat for temperate climate cereal crops. This inadvertent selection pressure provides an opportunity for investigating the underlying genetic mechanisms and evolutionary processes of rapid adaptation, which can occur both through mutations in the direct targets of herbicides and through changes in other, often metabolic, pathways, known as non-target-site resistance. How much target-site resistance (TSR) relies on de novo mutations vs. standing variation is important for developing strategies to manage herbicide resistance. We first generated a chromosome-level reference genome for A. myosuroides for population genomic studies of herbicide resistance and genome-wide diversity across Europe in this species. Next, through empirical data in the form of highly accurate long-read amplicons of alleles encoding acetyl-CoA carboxylase (ACCase) and acetolactate synthase (ALS) variants, we showed that most populations with resistance due to TSR mutations—23 out of 27 and six out of nine populations for ACCase and ALS , respectively—contained at least two TSR haplotypes, indicating that soft sweeps are the norm. Finally, through forward-in-time simulations, we inferred that TSR is likely to mainly result from standing genetic variation, with only a minor role for de novo mutations.

Fragility fractures of the pelvis (FFP) encompass two fracture entities: fracture after low-energy trauma and insufficiency fracture without trauma. It is unclear whether the two subgroups differ in terms of diagnosis and therapy. The aim of this retrospective study was to evaluate insufficiency fractures with regard to defined parameters and to compare specific parameters with the fractures after low-energy trauma.In the period from 2008 to 2017, 203 patients with FFP were recorded at our clinic (Level 1 Trauma Centre DGU, SAV approval). Of these, 25 had an insufficiency fracture and 178 had a pelvic ring fracture after low-energy trauma. Epidemiological, diagnostic and therapeutic parameters were examined.There was a relative increase in the insufficiency fracture within the FFP (2008 - 2009: 5.0% vs. 2015 - 2017: 17.8%). In these patients, osteoporosis tended to be more pronounced than in patients after low-energy trauma (t-value: - 3.66 vs. - 3.13). The diagnosis of insufficiency fractures showed increased use of MRI and DECT (60.9% vs. 26.0%) and a high proportion of type IV fractures after FFP (40.0% vs. 7.9%). In terms of therapy, surgical treatment of the insufficiency fracture was sought more often (68,2% vs. 52,1%), with a tendency towards increased use of combined osteosynthesis procedures (14.3% vs. 7.6%).We were able to show that as the number of cases increases, the insufficiency fracture becomes more important within FFP. If these patients tend to have more pronounced osteoporosis, particular attention should be paid to the diagnosis and adequate therapy of the osteoporosis, especially in the case of an insufficiency fracture. In addition to the increased diagnostic testing using MRI and DECT to detect oedema and the increased surgical therapy for this type of fracture, it is also noteworthy that the insufficiency fracture can cause higher-grade fractures after FFP.Innerhalb der Fragilitätsfrakturen des Beckens (Fragility Fractures of the Pelvis, FFP) existieren mit der Fraktur nach niedrigenergetischem Trauma und der Insuffizienzfraktur ohne Trauma 2 Frakturentitäten. Unklar ist, ob sich beide Subgruppen bez. Diagnostik und Therapie differenzieren. Ziel dieser retrospektiven Studie war es, die Insuffizienzfrakturen hinsichtlich definierter Parameter zu evaluieren und spezifische Parameter mit den Frakturen nach niedrigenergetischem Trauma zu vergleichen.Im Zeitraum von 2008 bis 2017 waren an unserer Klinik (überregionales TraumaZentrum DGU, SAV-Zulassung) 203 Patienten mit FFP zu verzeichnen. Davon hatten 25 eine Insuffizienzfraktur und 178 eine Beckenringfraktur nach niedrigenergetischem Trauma. Beleuchtet wurden epidemiologische, diagnostische und therapeutische Parameter.Es zeigte sich eine relative Zunahme der Insuffizienzfraktur innerhalb der FFP (2008 – 2009: 5,0% vs. 2015 – 2017: 17,8%). Bei diesen Patienten war tendenziell eine ausgeprägtere Osteoporose im Vergleich zu Patienten nach niedrigenergetischem Trauma (t-Wert: − 3,66 vs. − 3,13) festzustellen. Diagnostisch zeigte sich bei Insuffizienzfrakturen ein vermehrter Einsatz von MRT und DECT (60,9 vs. 26,0%) und ein hoher Anteil an Typ-IV-Frakturen nach FFP (40,0 vs. 7,9%). Therapeutisch wurde häufiger eine operative Versorgung der Insuffizienzfraktur angestrebt (68,2 vs. 52,1%) mit tendenziell vermehrtem Einsatz kombinierter Osteosyntheseverfahren (14,3 vs. 7,6%).Wir konnten zeigen, dass bei steigenden Fallzahlen die Insuffizienzfraktur innerhalb der FFP an Bedeutung gewinnt. Bei einer tendenziell ausgeprägteren Osteoporose dieser Patienten sollte insbesondere bei der Insuffizienzfraktur das Augenmerk auf die Diagnostik und adäquate Therapie der Osteoporose gelegt werden. Bemerkenswert ist neben der vermehrten Diagnostik mittels MRT und DECT zum Ödemnachweis und der vermehrten operativen Therapie bei dieser Frakturentität aber auch, dass die Insuffizienzfraktur höhere Frakturschweregrade nach FFP hervorrufen kann.

Die Versorgung schwangerer Traumapatientinnen stellt das gesamte medizinische Team vor eine besondere Herausforderung. Ziel unserer Studie war es, diese Daten zu erheben und die Unterschiede zu nichtschwangeren Traumapatientinnen zu ermitteln. Wir fuhrten eine retrospektive Datenanalyse aus dem TraumaRegister DGU® durch und verglichen 102 schwangere mit 3135 nichtschwangeren Patientinnen im gebarfahigen Alter (16–45 Jahre) aus den Jahren 2016–2018, welche in Traumazentren behandelt worden sind. Die Patientinnen sind jeweils uber den Schockraum aufgenommen und auf Intensivstation behandelt worden. In Deutschland, Osterreich und der Schweiz waren 3,2 % der Traumapatientinnen schwanger, d. h. 102 Frauen. Frauen im durchschnittlichen Alter von 29 Jahren erlitten am haufigsten ein Trauma infolge eines Verkehrsunfalls. Ein schweres Trauma („Injury Severity Score“ [ISS] ≥16 Punkte) erlitten 24,5 % der Schwangeren und 37,4 % der Nichtschwangeren. Bei schwangeren Patientinnen wurde nur in 32,7 % der Falle eine Computertomographie (Traumaspirale) durchgefuhrt – bei nichtschwangeren Frauen dagegen in 79,8 %. Infolge des Traumas verstarben 2,9 % der schwangeren und 3,5 % der nichtschwangeren Patientinnen. Die standardisierte Mortalitatsrate (SMR) hatte einen Wert bei Schwangeren von 0,42 und bei Nichtschwangeren von 0,63. Erstmalig liegen nun Daten fur die Lander Deutschland, Osterreich und Schweiz zu Inzidenz, Traumamechanismen, prahospitaler und innerklinischer Versorgungsphase sowie Intensivtherapiephase fur schwangere Traumapatientinnen vor. Weitere Untersuchungen zu Daten bezuglich des fetalen Outcomes und traumaassoziierten Verletzungen der schwangeren Traumapatientinnen waren wunschenswert. Gynakologen bzw. Geburtshelfer sollten hier im Bedarfsfall standardisiert in das Schockraumteam integriert werden konnen.

Bei Fragilitatsfrakturen des Beckenringes (FFP) vom Typ IV und den traumatischen Beckenfrakturen Typ C besteht haufig eine spinopelvine Instabilitat. Daraus ergibt sich die Indikation zur operativen Stabilisierung mittels einer spinopelvinen Abstutzung. Unklar ist aufgrund diverser operativer Verfahren zur spinopelvinen Abstutzung, welche Bedeutung hierbei eine minimal-invasive spinopelvine Schrauben-Stab-Osteosynthese haben kann. In die retrospektive, klinische Studie wurden uber einen Zeitraum von 2 Jahren 23 Patienten (Median 67 Jahre, 5 m und 18 f) mit instabiler Beckenfraktur (FFP Typ IV: n = 12, AO/OTA Typ C: n = 11) mittels triangularer minimal-invasiver spinopelviner Stabilisierung (TMSS) in die Studie eingeschlossen. Die Patientendaten wurden hinsichtlich der Parameter Geschlecht, Alter, Frakturmorphologie, intraoperativer Blutverlust, Operationszeit, postoperativer Infekt, postoperatives Repositionsergebnis (im CT) sowie Schraubenlockerung untersucht. Das Durchschnittsalter der 11 Typ-C-Frakturen lag bei 43 Jahren und das der 12 FFP Typ IV bei 80 Jahren. Der Nachuntersuchungszeitraum betrug im Durchschnitt 12,2 Monate. Die durchschnittliche Operationszeit lag bei 67 min, der Blutverlust bei 70 ml, es gab 2 postoperative Infekte und 4 Schraubenlockerungen. Die Reposition nach Matta war bei allen FFP

The change in allele frequencies within a population over time represents a fundamental process of evolution. By monitoring allele frequencies, we can analyze the effects of natural selection and genetic drift on populations. To efficiently track time-resolved genetic change, large experimental or wild populations can be sequenced as pools of individuals sampled over time using high-throughput genome sequencing (called the Evolve & Resequence approach, E&R). Here, we present a set of experiments using hundreds of natural genotypes of the model plantArabidopsis thalianato showcase the power of this approach to study rapid evolution at large scale. First, we validate that sequencing DNA directly extracted from pools of flowers from multiple plants -- organs that are relatively consistent in size and easy to sample -- produces comparable results to other, more expensive state-of-the-art approaches such as sampling and sequencing of individual leaves. Sequencing pools of flowers from 25-50 individuals at ∼40X coverage recovers genome-wide frequencies in diverse populations with accuracyr> 0.95. Secondly, to enable analyses of evolutionary adaptation using E&R approaches of plants in highly replicated environments, we provide open source tools that streamline sequencing data curation and calculate various population genetic statistics two orders of magnitude faster than current software. To directly demonstrate the usefulness of our method, we conducted a two-year outdoor evolution experiment withA. thalianato show signals of rapid evolution in multiple genomic regions. We demonstrate how these laboratory and computational Pool-seq-based methods can be scaled to study hundreds of populations across many climates.

Meniskusverletzungen fuhren zu einer fraglich erhohten Kniegelenksinstabilitat. Bisher ist aber unklar, ob eine relevante Innenmeniskusteilresektion zu einer vermehrten ventralen Tibiatranslation bei intaktem vorderem Kreuzband fuhrt. Ziel unserer Studie war es daher, die Stabilisatorfunktion eines um mindestens 30 % resezierten Innenmeniskus fur die anteriore Tibiatranslation klinisch zu untersuchen. Im Rahmen dieser prospektiven Studie wurden 18 Patienten mit unilateraler Innenmeniskuslasion vor und nach arthroskopischer Innenmeniskusteilresektion am gesunden und kranken Bein mittels zwei verschiedener apparative Verfahren (dynamische Translationsmessung mittels Hamstring-Reflexapparatur und KT-1000 Arthrometer) sowie einem funktionellen Test (computergestutzte dynamische Posturographie [CDP]) und einem klinischen Hop-Test untersucht und die Mittelwerte mittels nichtparametrischem Wilcoxon-Test auf Signifikanzen uberpruft. Nach Abschluss aller Untersuchungen konnten wir in unserer Studie keine signifikanten Unterschiede nachweisen, die auf eine vermehrte ventrale Instabilitat im Kniegelenk nach arthroskopischer Innenmeniskusteilresektion hinweisen. Die Innenmeniskusteilresektion fuhrt bei intaktem VKB zu keiner vermehrten ventralen Kniegelenksinstabilitat. Ob bei Patienten mit Instabilitat(sgefuhl) nach Meniskusteilresektion eine Rotationsinstabilitat die Ursache ist oder ob weitere Verletzungen bzw. Storungen im Kapsel-Band-Apparat vorliegen, muss in weiteren Studien untersucht werden. Eine ventrale Kniegelenksinstabilitat kann gemas unserer Studie nicht hinreichend erklart werden kann.

Chronic distal biceps tendon ruptures are rare and conservative or operative treatment options are suitable. There is a consensus in the literature in case of acute traumatic ruptures the operative refixation should be preferred. Disagreement exists in the best way of care of old ruptures ( 4 weeks) of distal biceps tendon. Several kinds of refixation possibilities up to tendon grafts are described. Aim of this publication is showing an overview of the literature of the approved methods in reconstruction of the distal biceps tendon using autogenous and allogenic grafts, comparing the outcomes and transferring them on an own case.A literature research was carried out using the online medical database "PubMed" with the following keywords "chronic rupture distal biceps tendon, surgical techniques". 59 citations were found concerning the topic, 37 publications were relevant for this work.There is consensus that even in chronic ruptures the operative management of the distal biceps tendon generates the best results. Consistently the experiences and results of only little patient collectives are reported. Numerous techniques of surgery are described without predominance of one method. Reinsertions of the tendon butts are reported in different techniques: with achilles, palmaris longus, fascia lata, triceps, quadriceps and semitendinosus tendon grafts. All together they showed postoperative satisfactory results.With surgical treatment of chronic ruptured distal biceps tendons comparable outcomes can be achieved by primary refixation and graft augmentations. In case of graft augmentations several tissue options are available which showed in all cases satisfactory functional results in the end.Die chronische distale Bizepssehnenruptur ist immer noch eine Rarität, bei der konservative wie operative Behandlungsoptionen Anwendung finden. In der Literatur ist man sich einig, dass bei akut traumatischen Rupturen aufgrund des besseren funktionellen Outcomes der operativen Refixation Vorzug zu geben ist. Uneinigkeit herrscht vor allem hinsichtlich operativer Versorgungsoptionen veralteter ( 4 Wochen) Rupturen der distalen Bizepssehne. Beschrieben werden in der Literatur verschiedenste Refixationsmöglichkeiten bis hin zu diversen überbrückenden Sehnenplastiken. Ziel dieser Arbeit ist es, einen Überblick über die in der Literatur anerkannten Methoden zur Rekonstruktion der distalen Bizepssehne unter Verwendung autologer und allogener Transplantate zu geben, deren Outcome zu vergleichen und die Erkenntnisse auf einen aktuellen eigenen Fall zu übertragen.Es erfolgte eine Literaturrecherche mithilfe der medizinischen Onlinedatenbank „PubMed“. Nach englischer Eingabe der Suchbegriffe „chronic rupture distal biceps tendon, surgical techniques“ wurden 59 Literaturstellen gefunden (1990 – 2019). Für die Thematik der Rekonstruktion chronisch distaler Bizepssehnenrupturen konnten sodann 37 Publikationen im Volltext ausgewählt und für die aktuelle Veröffentlichung berücksichtigt werden.Konsens besteht darin, dass auch bei chronischen Rupturen die operative Versorgung der distalen Bizepssehne die besten Ergebnisse erbringt. Durchweg finden sich jedoch nur kleine Patientenkollektive, über deren Ergebnisse und Erfahrungen berichtet wird. Zahlreiche Operationstechniken werden berichtet, ohne dass sich eine den anderen überlegen zeigt. Dabei werden Re-Insertionen der Sehnenstümpfe mittels verschiedener Transplantattechniken berichtet: Anwendung finden Achillessehnen-, Palmaris-longus-, Fascia-lata-, Trizepssehnen-, Quadrizepssehnen- und Semitendinosussehnentransplantate. Allen gemein ist, dass postoperativ sehr zufriedenstellende Funktionen und nahezu altes Kraftpotenzial erzielt werden konnte.Durch die operative Versorgung chronisch rupturierter distaler Bizepssehnen können mittels primärer Refixation des Sehnenstumpfes wie aber auch durch Transplantataugmentation gleichwertige Ergebnisse erzielt werden. Im Falle notwendiger Transplantataugmentation sind zahlreiche Gewebeoptionen obligat, die im Rahmen kleiner Fallzahlen durchweg gute funktionelle Ergebnisse erwarten lassen.

As the average age of society increases, so does the number of cases of fragility fractures of the pelvis (FFP). Magnetic resonance imaging (MRI) can visualise associated oedema and is thus the gold standard for diagnosing such fractures. MRI, however, is costly, not always available, and involves certain exclusion criteria. Dual-energy computed tomography (DECT) appears to be a promising alternative. It is unclear, however, whether it could be used for diagnosing FFP with similar sensitivity/specificity. The aim of our study was thus to compare conventional CT and DECT with MRI in cases of suspected FFP. A total of 46 patients with suspected FFP underwent MRI, CT and DECT scans. There were three comparison groups for each of these patients: conventional CT image analysis without dual-energy modification (Arm 1), DECT analysis (Arm 2) and MRI as the gold standard (Arm 3). Diagnosis and FFP classification were performed by a radiologist in random order and without clinical information. The sensitivity and specificity of conventional CT and DECT were calculated in comparison with MRI as the reference standard. With 100% sensitivity and specificity, DECT is on par with MRI when it comes to diagnosing fragility fractures of the pelvis and is superior to conventional CT (90.3% sensitivity, 100% specificity). In terms of classification as well, there were no differences between DECT and MRI. On conventional CT, on the other hand, 16 patients were classified differently than they were on MRI. Our study shows DECT to be reliable and superior to conventional CT in terms of oedema detection and specific fracture classification in FFP. DECT thus combines the advantages of conventional CT (good visualisation of bone matter) and MRI (medullary cavity and visualisation of occult fractures).

Pelvic ring fractures type C present a special challenge due to their high instability, the possible accompanying injuries and the high mortality rate of up to 18.9%. The aim of this retrospective analysis was to use the data from the DGU pelvic register to identify changes in the epidemiology and therapy for type C pelvic ring fractures between 2004 and 2014.2,042 patients with type C pelvic ring injury were retrospectively included. Three time periods with roughly equal patient groups were specified and differences in epidemiology and the type of therapy were evaluated. For the surgical cases, the time of the operation, the duration of the operation, blood loss, the location of the fracture and the type of osteosynthesis were evaluated and the reduction result was recorded.For the period under review, there is an age shift in the incidence of a type C pelvic ring fracture towards older age. The isolated pelvic injury has increased, while the proportion of pelvic injuries in the context of polytrauma has steadily decreased. Complications and mortality decreased as a percentage. The tendency towards minimally invasive procedures could be shown in the surgical care. Navigated procedures in the area of the pelvic ring have so far not proven successful.We were able to show that the majority of the patients are increasingly old, that there is no relevant trauma in the history and that there is an increase in the isolated pelvic fracture type C and a decrease in the number of polytraumatised or multiply injured patients. In conjunction with mortality from pelvic ring injuries, the successes of standardised, pelvic-specific emergency management, an adapted time of operation outside the vulnerable phase and stable osteosynthesis care, which enable early functional follow-up treatment, are also evident.Beckenringfrakturen Typ C stellen aufgrund ihrer hohen Instabilität, den möglichen Begleitverletzungen und der hohen Letalitätsrate von bis zu 18,9% eine besondere Herausforderung dar. Ziel dieser retrospektiven Analyse war es, anhand einer Datenauswertung aus dem Beckenregister der DGU der Jahre 2004 bis 2014 Veränderungen hinsichtlich der Epidemiologie und Therapie bei Beckenringfrakturen Typ C herauszuarbeiten.Es konnten retrospektiv 2042 Patienten mit Beckenringverletzung Typ C nach AO eingeschlossen werden. Es wurden 3 Zeiträume mit etwa gleich großen Patientengruppen festgelegt und Unterschiede in Bezug auf die Epidemiologie und die Art der Therapie ausgewertet. Für die operativen Fälle wurde der OP-Zeitpunkt, die OP-Dauer, der Blutverlust, die Frakturlokalisation und die Art der Osteosynthese ausgewertet sowie das Repositionsergebnis erfasst.Für den betrachteten Zeitraum zeigt sich eine Altersverschiebung der Inzidenz einer Beckenringfraktur Typ C hin zu höherem Alter. Die isolierte Beckenverletzung hat dabei zugenommen, während der Anteil an Beckenverletzungen im Rahmen eines Polytraumas stetig abgenommen hat. Komplikationen und Mortalität nahmen prozentual ab. Bei der operativen Versorgung konnte die Tendenz zu minimalinvasivem Vorgehen aufgezeigt werden. Navigierte Vorgehensweisen im Bereich des Beckenringes haben sich aber bisher nicht bewährt.Wir konnten zeigen, dass die Mehrzahl der Patienten zunehmend höheren Alters ist, eine relevante Traumaanamnese fehlt und eine Zunahme der isolierten Beckenfraktur Typ C sowie ein Rückgang der polytraumatisierten bzw. mehrfach verletzen Patienten vorliegen. In Zusammenschau mit der Mortalität bei Beckenringverletzungen zeigen sich zudem die Erfolge durch ein standardisiertes, beckenspezifisches Notfallmanagement, einen angepassten Operationszeitpunkt außerhalb der vulnerablen Phase und die stabile osteosynthetische Versorgung, die eine frühfunktionelle Nachbehandlung ermöglichen.

Professional anxiety existed early in the coronavirus disease 2019 (COVID-19) pandemic with challenging infection prevention and control support. The aims of this study were to compare epidemiological features of healthcare workers (HCWs) within primary and secondary care with their serological evidence of infection. A prospective observational cohort of 1,916 HCWs completed a questionnaire, and their sera were assayed for detectable antibody to severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) nucleoprotein in the first wave of the pandemic. Datasets were compared between the two sub-cohorts in primary and secondary care and between the combined seropositive and seronegative cohorts. Curiosity of antibody status was high. Detectable antibody was 7% in the primary care and 5% in the secondary care workers at a time of 1.7% in the general community. Inappropriate personal protective equipment (PPE) was more common in primary care, and detectable antibody was twice as prevalent in HCWs who felt they did not have appropriate PPE. Contact tracing was perceived to be inadequate although it was more commonly performed in the seropositive cohort suggesting appropriate prioritisation. Both temperature and symptom checking alerts and work exclusion were significantly more prevalent in the seropositive cohort. The seroprevalence data support increased risk for HCWs, the importance of appropriate PPE and the usefulness of the daily temperature and symptom checks, particularly in primary care.

Spinopelvic instability is common in type IV fragility fractures of the pelvic ring (FFP) and type C traumatic pelvic fractures. This results in the indications for operative stabilization using a spinopelvic support. Due to the variety of surgical techniques for spinopelvic support it is unclear what importance a minimally invasive spinopelvic screw-rod osteosynthesis can have.In the retrospective clinical study over a period of 2 years, 23 patients (median age 67 years, 5 male and 18 female) with unstable pelvic fractures (FFP type IV: n = 12, AO/OTA type C: n = 11) treated by triangular minimally invasive spinopelvic stabilization (TMSS) were included in the study. The patient data were examined with respect to the parameters gender, age, fracture morphology, intraoperative blood loss, operating time, postoperative infection, postoperative reduction result in the computed tomography (CT) imaging and screw loosening.The average age of the 11 type C fractures was 43 years and that of the 12 FFP type IV fractures was 80 years. The follow-up period was on average 12.2 months. The average operation time was 67 min, the blood loss was 70 ml, there were 2 postoperative infections and 4 cases of screw loosening. The reduction according to Matta was 4 mm for all FFP and between 4-20 mm for traumatic pelvic fractures. Symptomatic pseudarthroses occurred in 3 cases.The triangular minimally invasive spinopelvic stabilization (TMSS) showed a stable and sufficient treatment of the type IV fragility fractures and in the slightly displaced type C traumatic pelvic fractures. Coarse fracture dislocations limit the procedure.EINLEITUNG: Bei Fragilitätsfrakturen des Beckenringes (FFP) vom Typ IV und den traumatischen Beckenfrakturen Typ C besteht häufig eine spinopelvine Instabilität. Daraus ergibt sich die Indikation zur operativen Stabilisierung mittels einer spinopelvinen Abstützung. Unklar ist aufgrund diverser operativer Verfahren zur spinopelvinen Abstützung, welche Bedeutung hierbei eine minimal-invasive spinopelvine Schrauben-Stab-Osteosynthese haben kann.In die retrospektive, klinische Studie wurden über einen Zeitraum von 2 Jahren 23 Patienten (Median 67 Jahre, 5 m und 18 f) mit instabiler Beckenfraktur (FFP Typ IV: n = 12, AO/OTA Typ C: n = 11) mittels triangulärer minimal-invasiver spinopelviner Stabilisierung (TMSS) in die Studie eingeschlossen. Die Patientendaten wurden hinsichtlich der Parameter Geschlecht, Alter, Frakturmorphologie, intraoperativer Blutverlust, Operationszeit, postoperativer Infekt, postoperatives Repositionsergebnis (im CT) sowie Schraubenlockerung untersucht.Das Durchschnittsalter der 11 Typ-C-Frakturen lag bei 43 Jahren und das der 12 FFP Typ IV bei 80 Jahren. Der Nachuntersuchungszeitraum betrug im Durchschnitt 12,2 Monate. Die durchschnittliche Operationszeit lag bei 67 min, der Blutverlust bei 70 ml, es gab 2 postoperative Infekte und 4 Schraubenlockerungen. Die Reposition nach Matta war bei allen FFP 4 mm, bei den traumatischen Beckenfrakturen zwischen 4 und 20 mm. Symptomatische Pseudarthrosen traten in 3 Fällen auf.Die TMSS zeigte eine stabile und suffiziente Versorgung der Fragilitätsfrakturen Typ IV und bei den gering dislozierten traumatischen Becken-C-Frakturen. Grobe Frakturdislokationen limitieren das Verfahren.

The choice of therapy for fragility fractures of the pelvis (FFP) is largely determined by the diagnosed fracture morphology. It is now unclear whether the change in diagnostic options - sensitive detection of fracture oedema in the sacrum using MRI and dual-energy computed tomography (DECT) - has an impact on the therapeutic consequences. The aim of this retrospective study was therefore to evaluate the change in the diagnostics used and the resulting therapy regimen in our patient population.We performed a monocentric-retrospective analysis of 196 patients with a fragility fracture of the pelvis in our clinic (national TraumaZentrumThe diagnostic procedures used are subject to a clear change towards oedema detection using MRI and DECT. The graduation has changed towards more severe forms of fracture after FFP. There is now also an increasing proportion of patients treated by surgery (2008 - 2009: 5.3% vs. 2015 - 2017: 60.3%).We were able to show that the introduction of sensitive diagnostic procedures coincided with a higher classification of the fractures. It is also noteworthy that the increase in operations is not only due to a higher degree of classification; also in relative terms, more patients are operated on within type FFP II.Die Wahl der Therapie bei Fragilitätsfrakturen des Beckens (FFP) wird im Wesentlichen bestimmt durch die diagnostizierte Frakturmorphologie. Unklar ist nun, ob auch der Wandel diagnostischer Optionen – sensitiver Nachweis von Frakturödemen im Sakrum mittels MRT und Dual-Energy-Computertomografie (DECT) – Einfluss auf die therapeutischen Konsequenzen hat. Ziel dieser retrospektiven Studie war es daher, die Veränderung eingesetzter Diagnostika und des resultierendem Therapieregimes bei unserem Patientengut zu evaluieren.Wir haben eine monozentrisch-retrospektive Analyse von 196 Patienten mit einer Fragilitätsfraktur des Beckens unserer Klinik (überregionales TraumaZentrumDie eingesetzten diagnostischen Verfahren unterliegen einem klaren Wandel hin zum Ödemnachweis mittels MRT und DECT. Die Graduierung hat sich dabei hin zu schwereren Frakturformen nach FFP verändert. Es zeigt sich nun auch ein steigender Anteil operativ versorgter Patienten (2008 – 2009: 5,3% vs. 2015 – 2017: 60,3%).Wir konnten zeigen, dass eine zeitliche Koinzidenz der Einführung sensitiver Diagnostikverfahren mit einer höheren Klassifizierung der Frakturen besteht. Bemerkenswert ist aber auch, dass die Zunahme der Operationen nicht nur auf einem höheren Klassifikationsgrad beruht; auch relativ betrachtet werden innerhalb des Typs FFP II mehr Patienten operiert.

Apophyseal avulsion fractures of the ischial tuberosity are rare injuries and therefore often not diagnosed in a timely manner. Healing may then result in massive hypertrophic ischial tuberosity. This can cause ischiofemoral impingement symptoms. Due to the low incidence and scarce literature, the optimal treatment and surgery is unclear.A literature search was carried out using the online medical database "PubMed". The findings of the literature were then applied to a clinical case of delayed diagnosis of the apophyseal avulsion fracture of the ischial tuberosity.There is no gold standard in the literature for the treatment of avulsion fractures on the ischial tuberosity. Nearly 90% are treated conservatively and a fragment dislocation of more than 2 cm is often the indication for surgical care. However, the surgical procedures described are very diverse. An ischiofemoral impingement symptom may result from excessive ossification of the ischial tuberosity, bringing the ischiofemoral distance to the critical limit of 2 cm.The timely correct diagnosis and initiation of a therapy is crucial for the later outcome of the patient. Ischiofemoral impingement symptoms may be the indication of bony displacement of the ischial tuberosity as a result of injury. Therapy is then surgical with partial resection of the ischial tuberosity and plate osteosynthesis.Apophysäre Avulsionsfrakturen des Tuber ischiadicum sind seltene Verletzungen und werden daher häufig nicht zeitgerecht diagnostiziert. Eine Ausheilung kann dann zu einer überschießenden Ossifikation und knöcherner Auftreibung der Tuber ischiadicum führen, woraus sich wiederum eine ischiofemorale Impingement-Symptomatik entwickeln kann. Aufgrund der geringen Inzidenz und kaum verfügbarer Literatur stellt sich die Frage nach der optimalen Therapie und Operationsmethode.Es erfolgte eine Literaturrecherche mithilfe der medizinischen Onlinedatenbank „PubMed“. Im Anschluss wurden die Erkenntnisse aus der Literatur auf einen klinischen Fall mit verspätet diagnostizierter apophysärer Avulsionsfraktur des Tuber ischiadicum übertragen.In der Literatur zeigt sich kein Goldstandard zur Therapie von Avulsionsfrakturen am Tuber ischiadicum. Knapp 90% werden konservativ behandelt und ab einer Fragmentdislokation von 2 cm wird häufig die Indikation zur operativen Versorgung gestellt. Die beschriebenen OP-Verfahren sind jedoch sehr vielfältig. Eine ischiofemorale Impingement-Symptomatik kann durch eine überschießende Ossifikation am Tuber ischiadicum entstehen, wodurch der ischiofemorale Abstand an die kritische Grenze von 2 cm kommt.Die zeitnahe korrekte Diagnosestellung und Einleitung einer Therapie ist entscheidend für das spätere Outcome des Patienten. Eine ischiofemorale Impingement-Symptomatik kann der Hinweis für eine knöcherne Auftreibung des Tuber ischiadicum als Folge der Verletzung sein. Die Therapie ist dann operativ mit Teilresektion des Tuber ischiadicum sowie einer Plattenosteosynthese.

Zusammenfassung Hintergrund Neben schmerztherapeutischer Wirkung zielt die Kyphoplastie auf eine Aufrichtung osteoporotisch frakturierter Wirbelkörper (WK) ab. Ein neueres Verfahren ist die Radiofrequenzkyphoplastie (RFK). Unklar ist, wie gut sich mit dieser Methodik im Vergleich zur älteren Ballonkyphoplastie (BKP) wieder eine anatomische WK-Aufrichtung erreichen lässt. Ziel unserer retrospektiven Studie war daher der Vergleich beider Verfahren bez. der Grund-/Deckplattenwinkel post- vs. präoperativ. Patienten und Methoden Anhand prä- und postoperativer Stehend-Röntgenaufnahmen wurden bei 87 Probanden (46 BKP und 41 RFK) bzw. 142 kyphoplastierten WK (67 BKP und 75 RFK) die Winkel von Grund- zu Deckplatte bestimmt. Hauptzielgröße war der Grad der Aufrichtung (Δpost−prä) bei der BKP vs. RFK. Außerdem wurden für die Bestimmung des sagittalen Alignments die angrenzenden gesunden Segmente ausgemessen. Ergebnisse Sowohl bei der BKP (BKPprä: 11,5 ± 6,0°, BKPpost: 6,2 ± 4,6°, p Schlussfolgerung Es konnte eine signifikant bessere Aufrichtung des Kyphosewinkels durch die BKP erreicht werden, wobei beide Verfahren für sich betrachtet zu einer hochsignifikanten Verbesserung des Kyphosewinkels geführt haben. Im angrenzenden Wirbelsäulensegment ist die Auswirkung auf den messbaren Winkel nicht signifikant ausgeprägt, sodass das Alignment durch den Eingriff tendenziell weniger beeinflusst wird.

Zusammenfassung Hintergrund Die Behandlung von Verletzten aus Konfliktländern mit Wunden mit multiresistenten Erregern steigt in Deutschland, vor allem in Bundeswehrkrankenhäusern. Häufig zeigen diese Patienten keine zufriedenstellende Vorbehandlung und hohe Keimbelastung mit seltenen Krankheitserregern. In Deutschland haben die Krankenhäuser diese Erreger per Gesetz zu erheben. Ziel dieser Studie war es, eine Übersicht über die aktuelle Keimsituation bei Komplikationswunden in Deutschland in den vergangenen 10 Jahren zu geben und über die Erfahrungen mit der Behandlung Verletzter mit Komplikationswunden aus Konfliktländern zu berichten. Patienten und Methoden Es wurde eine PubMed-Suche in Bezug auf alle multiresistenten Erreger, Antibiotikaresistenzen und Richtlinien in Deutschland von 2005 bis 2015 durchgeführt. Die Datenbanken und Empfehlungen des Robert Koch-Instituts, der Paul-Ehrlich-Gesellschaft und des Nationalen Referenzzentrums für Surveillance von nosokomialen Infektionen wurden analysiert und die Screeningergebnisse der verletzten Patienten aus Konfliktländern, die im Bundeswehrkrankenhaus Ulm behandelt wurden, eingeschlossen. Ergebnisse Das Spektrum von pathogenen Erregern in Bezug zu nosokomialen Infektionen änderte sich nicht, sehr wohl aber das Verhältnis der Krankheitserreger zueinander. Infektionen mit methicillinresistentem Staphylococcus aureus (MRSA) verringerten sich, vancomycinresistente Enterococcusinfektionen nehmen hingegen kontinuierlich zu und Enterococcus faecium (E. faecium) stellt heute den größten Teil der VRE dar. Obgleich die Datenbanken noch keine Ergebnisse in Bezug auf die relativ neue Klassifizierung der multiresistenten gramnegativen (MRGN) Krankheitserreger liefern können, spielen MRGN-Erreger bei Verletzten aus Konfliktländern die Hauptrolle. Schlussfolgerung In den letzten 10 Jahren traten Änderungen im Resistenzmuster und im Verhältnis multiresistenter Krankheitserreger zueinander auf. MRSA-Infektionen gingen in den letzten 10 Jahren zurück, der Einsatz von Reserveantibiotika stieg hingegen an. Infektionen mit VRE steigen und sogar einige Reserveantibiotika haben ihre Wirksamkeit verloren, weil der unkritische Einsatz moderner Antibiotika stetig zunimmt. In Bezug auf 3- und 4-MRGN gibt es noch keine validen deutschen Daten. Diese Erreger spielen die Hauptrolle bei Verletzten aus Konfliktländern, die in Deutschland behandelt werden. Routinescreening und umfassende Hygienesicherheitsmaßnahmen einschließlich Isolierung sind hier von entscheidender Bedeutung. Die chirurgische Wundtherapie tritt nun mehr und mehr in den Vordergrund und der unkritische und teure Einsatz von Antibiotika sollte – wenn nicht unbedingt erforderlich – in den Hintergrund treten.

Meniscus injuries lead to increased knee joint instability. Currently, however, it is unclear whether a relevant medial meniscus part resection leads to an increased ventral tibia translation with intact anterior cruciate ligament. The aim of our study was therefore to clinically examine the stabilizer function of at least 30% resected medial meniscus for anterior tibial translation.In this prospective study, 18 patients with unilateral medial meniscus lesion were treated before and after arthroscopic medial meniscus resection. They were treated on the healthy and on the sick leg through the use of two different apparatus methods (dynamic translation measurement using hamstring reflex apparatus and KT-1000 arthrometers) as well as a functional test (computer-supported dynamic posturography (CDP)) and a clinical hop test. Further, the mean values for significance using non-parametric Wilcoxon test.After completing all the studies, we were not able to detect any significant differences in our study that would indicate increased ventral instability in the knee joint after arthroscopic medial meniscus resection.Inner meniscal partial resection does not lead to increased ventral knee instability in intact VKB. Whether in patients with instability (feeling) after partial meniscus resection, a rotation instability is the cause or whether further injuries or disturbances in the capsular ligament apparatus are present, must be examined in further studies. Anterior knee joint instability cannot be adequately explained according to our study.HINTERGRUND: Meniskusverletzungen führen zu einer fraglich erhöhten Kniegelenksinstabilität. Bisher ist aber unklar, ob eine relevante Innenmeniskusteilresektion zu einer vermehrten ventralen Tibiatranslation bei intaktem vorderem Kreuzband führt. Ziel unserer Studie war es daher, die Stabilisatorfunktion eines um mindestens 30 % resezierten Innenmeniskus für die anteriore Tibiatranslation klinisch zu untersuchen.Im Rahmen dieser prospektiven Studie wurden 18 Patienten mit unilateraler Innenmeniskusläsion vor und nach arthroskopischer Innenmeniskusteilresektion am gesunden und kranken Bein mittels zwei verschiedener apparative Verfahren (dynamische Translationsmessung mittels Hamstring-Reflexapparatur und KT-1000 Arthrometer) sowie einem funktionellen Test (computergestützte dynamische Posturographie [CDP]) und einem klinischen Hop-Test untersucht und die Mittelwerte mittels nichtparametrischem Wilcoxon-Test auf Signifikanzen überprüft.Nach Abschluss aller Untersuchungen konnten wir in unserer Studie keine signifikanten Unterschiede nachweisen, die auf eine vermehrte ventrale Instabilität im Kniegelenk nach arthroskopischer Innenmeniskusteilresektion hinweisen.Die Innenmeniskusteilresektion führt bei intaktem VKB zu keiner vermehrten ventralen Kniegelenksinstabilität. Ob bei Patienten mit Instabilität(sgefühl) nach Meniskusteilresektion eine Rotationsinstabilität die Ursache ist oder ob weitere Verletzungen bzw. Störungen im Kapsel-Band-Apparat vorliegen, muss in weiteren Studien untersucht werden. Eine ventrale Kniegelenksinstabilität kann gemäß unserer Studie nicht hinreichend erklärt werden kann.

Magnetic resonance and computed tomography (MRI, CT) has been known to compare the sensitivity for the detection of pelvic fractures with others. It is unclear whether MRI imaging beyond CT leads to therapy change. The aim of our study is to determine the information gained from MRI in the diagnosis of pelvic fractures and to reduce the effects on the form of therapy.In a retrospective, clinical study, 31 patients with pelvic fracture and CT and MRI imaging (median 81 ± 20 years, 22 female and 9 male) were examined. There was a classification according to AO classification for adequate or FFP classification for inadequate fractures. In addition, vascular, muscular, haematomatous and organic concomitant injuries as well as bone marrow edema and additional secondary findings requiring evaluation were evaluated. The type of therapy (conservative vs. surgical) and a possible type of therapy change were documented for each patient. Exact test according to Fisher was tested orienting.Overall, MRI showed a greater fracture rate of pelvic fractures in 29% (n = 9) patients than CT. Four type I fractures according to FFP classification were identified as type II fractures and 4 type II fractures as type IV fractures. One type B1 fracture according to AO classification was found to be C2 fracture on MRI. Fisher's Exact Test found that the parameters "adequacy of trauma" and "fracture type change" by MRI were p = 0.38. MRI showed a total of 82 concomitant injuries, CT 31. Overall, MRI gained information in 75% (n = 24) of all patients examined. A change from conservative to operative after MRI took place in 2 patients. No patient was surgically changed from planned surgery to conservative. The extent to which MRI caused changes within one form of therapy (conservative, operative) could not be determined retrospectively. In 18% of patients with an inadequate fracture, however, according to the literature, the treatment regimen would have changed pro forma.In summary, it can be stated that the MRI in our study provided an information gain in the case of adequate and inadequate pelvic fractures as well as their accompanying injuries and that a possible therapeutic relevance of this information gain could be obtained specially at the inadequate fractures showed.Zwar wurden Magnetresonanz- und Computertomografie (MRI, CT) bereits hinsichtlich ihrer Sensitivität zur Detektion von Beckenfrakturen miteinander verglichen. Ziel unserer Studie war es daher, den Informationszugewinn aus der MRT gegenüber der CT in der Diagnostik von Beckenfrakturen zu erfassen und eventuelle Auswirkungen der Mehrinformation auf das Therapieregime herauszustellen.Im Rahmen einer retrospektiven, klinischen Studie wurden 31 Patienten mit Beckenfraktur und CT- sowie MRT-Bildgebung (Median 81 ± 20 Jahre, 22 weiblich und 9 männlich) untersucht. Es erfolgte eine Einteilung nach AO-Klassifikation für adäquate bzw. FFP-Klassifikation für inadäquate Frakturen. Zudem wurden vaskuläre, muskuläre, hämatomatöse und organische Begleitverletzungen sowie Knochenmarködeme und abklärungsbedürftige Nebenbefunde evaluiert. Die Art der Therapie (konservativ vs. operativ) und ein eventueller Therapieartwechsel wurden für jeden Patienten dokumentiert. Mithilfe des Exakten Tests nach Fisher wurde orientierend getestet.Insgesamt zeigte die MRT bei 29% (n = 9) der Patienten ein größeres Frakturausmaß der Beckenfrakturen als die CT. Vier Typ-I-Frakturen nach FFP-Klassifikation wurden als Typ-II-Frakturen und 4 Typ-II-Frakturen als Typ-IV-Frakturen erkannt. Eine Typ-B1-Fraktur nach AO-Klassifikation zeigte sich in der MRT als C2-Fraktur. Der Exakte Test nach Fisher ergab für die Parameter „Adäquatheit eines Traumas“ und „Änderung des Frakturtyps“ durch die MRT p = 0,38. Die MRT zeigte insgesamt 82 Begleitverletzungen an, die CT 31. Insgesamt zeigte sich ein Informationszugewinn durch die MRT bei 75% (n = 24) aller untersuchten Patienten. Ein Wechsel von konservativ auf operativ nach der MRT fand bei 2 Patienten statt. Kein Patient wurde von geplant operativ auf konservativ umgestellt. Inwiefern die MRT Änderungen innerhalb einer Therapieform (konservativ, operativ) bewirkte, konnte retrospektiv nicht festgestellt werden. Bei 18% der Patienten mit inadäquater Fraktur hätte sich aber pro forma laut Literatur mit dem Klassifikationstyp auch das Therapieregime ändern müssen.Zusammenfassend kann man festhalten, dass die MRT in unserer Studie einen Informationszugewinn bei adäquaten und inadäquaten Beckenfrakturen sowie deren Begleitverletzungen lieferte und sich eine mögliche Therapierelevanz dieses Informationszugewinnes vor allem bei den inadäquaten Frakturen zeigte.

Isolated pelvic fractures are relatively rare with an incidence of 3 - 6% of all fractures, but their incidence in polytraumatized individuals increases to 25%. The S3 guideline Polytrauma gives a clear recommendation for diagnostics by means of pelvic radiography (X-ray) and computed tomography (CT). A recommendation for the diagnosis by means of magnetic resonance tomography (MRI) especially in patients with low energetic/missing trauma does not currently exist. It is unclear on the basis of which criteria the MRI can be indicated in pelvic fractures. The aim of our study was therefore to retrospectively record indications for the indication of MRI in pelvic fractures - with adequate as well as inadequate trauma.In a retrospective clinical study, a total of 140 patients (median 68 years, range 15 - 97, 75 female, and 66 male) with a pelvic fracture were included in the study over a period of three years. Overall, the trauma mechanism revealed 73 adequate and 67 inadequate fractures. 31/140 patients had undergone MRI of the pelvis in addition to a CT/X-ray scan. The two subgroups "with MRI" and "without MRI" were analyzed with regard to the parameters "sex", "age", "adequacy of the trauma", "fracture localization", "duration of admission to imaging", "type of therapy" and "duration to surgery" compared.It was shown that the MRI diagnosis was performed especially in female, elderly patients (81 years, range 19 - 94 years). Patients with inappropriate trauma have received MRI more frequently (74%) than patients with adequate trauma (26%). With regard to fracture localization no differences could be shown. The MRI was performed on a median 4 days after the CT examination. Regarding the decision "conservative" vs. "operationally" our two groups without and with MRT tended to differ not. Patients with MRI were operated on median 2 days later than patients who did not receive MRI.Pelvic fracture MRI should be performed primarily in elderly female patients without adequate trauma. In patients with adequate trauma, MRI is of low value, especially as MRI diagnostics are performed with delay, resulting in later surgery.Isolierte Beckenfrakturen sind mit einer Inzidenz von 3 – 6% aller Frakturen relativ selten, ihre Inzidenz steigt bei Polytraumatisierten jedoch bis auf 25% an. Unklar ist, anhand welcher Kriterien die MRT bei Beckenfrakturen indiziert werden kann. Ziel unserer Studie war es daher, retrospektiv Hinweise zur Indikation einer MRT bei Beckenfrakturen – sowohl mit adäquatem als auch inadäquatem Trauma – zu erfassen.Im Rahmen einer retrospektiven klinischen Studie wurden über einen Zeitraum von 3 Jahren insgesamt 140 Patienten (Median 68 Jahre, Range 15 – 97, 75 weiblich und 65 männlich) mit einer Beckenfraktur, die an unserer Klinik behandelt worden waren, in die Studie eingeschlossen. Patienten mit einem inadäquaten Trauma haben häufiger ein MRT erhalten (74%) als Patienten mit einem adäquaten Trauma (26%). Hinsichtlich der Frakturlokalisation konnten keine Unterschiede dargestellt werden. Das MRT wurde im Median 4 Tage nach der CT-Untersuchung durchgeführt. Hinsichtlich der Entscheidung „konservativ“ vs. „operativ“ unterschieden sich unsere beiden Gruppen ohne und mit MRT tendenziell nicht. Patienten mit MRT wurden im Median 2 Tage später operiert, als Patienten, die kein MRT erhalten haben.Das MRT bei Beckenfrakturen sollte vor allem bei älteren, weiblichen Patienten ohne adäquates Trauma durchgeführt werden. Bei Patienten mit einem adäquaten Trauma hat das MRT einen geringen Stellenwert, zumal die MRT-Diagnostik mit Verzögerung durchgeführt wird und dadurch auch zu einer späteren OP führt.

Einleitung Talusluxationsfrakturen vom Typ IV nach Marti und Weber sind auserst seltene Verletzungen mit besonderer Herausforderung fur den Operateur und haufig schlechten Heilungsergebnissen. Aufgrund geringer Inzidenz und kaum verfugbarer Literatur stellt sich die Frage nach der optimalen Behandlungsoption zur Erlangung bestmoglicher Therapieergebnisse und Minimierung der haufig assoziierten Komplikationen. Material und Methoden Es erfolgte eine Literaturrecherche mithilfe der medizinischen Onlinedatenbank „PubMed“. Im Anschluss wurden die Erkenntnisse aus der Literatur auf einen klinischen Fall mit Typ-IV-Talusluxationsfraktur nach Marti und Weber ubertragen. Ergebnisse Moderne Osteosyntheseverfahren mit groszugigen und der Situation angepassten Zugangswegen sind gegenuber den fruher priorisierten Arthrodese- und Talektomieverfahren zu bevorzugen. Prioritat hat die schnellstmogliche Reposition der Fraktur unter Schonung der Weichteil- und Durchblutungsverhaltnisse. Auch wenn davon ausgegangen werden kann, dass mit zunehmendem Luxationsgrad das Talusnekroserisiko steigt, korreliert dies nicht zwangslaufig mit dem klinischen Outcome. Schlussfolgerung Bei Talusluxationsfrakturen liegt der Fokus auf einer zeitnahen Reposition und suffizienten osteosynthetischen Versorgung. Trotzdem ist mit einer hohen Komplikationsrate zu rechnen. Die im klinischen Beispiel beschriebene mediale Explantation und laterale Re-Implantation des Taluskorpers wurde in der Literatur so noch nie beschrieben und zeigte einen zufriedenstellenden Verlauf.

Background: Cement leakage is regarded as a typical complication of cement-based augmentation of vertebral fractures. The gold standard is balloon kyphoplasty (BK). Recent methods, such as radiofrequency kyphoplasty (RFK), must be compared with BK in terms of therapeutic success and complication rates. It is unclear whether the cement leakage rate in RFK is lower than with BK and whether this has any clinical relevance. Therefore, the aim of our prospective clinical study was to compare RFK with BK with respect to cement leakage rates and associated clinical complications. Patients and Methods: After prospective randomisation, 100 patients (76 women and 24 men with an average age of 78.5 years) or 162 vertebral bodies were treated by BK (n = 79) or RFK (n = 83). We evaluated the parameters “localisation of cement leakage” (epidural, intradiscal, extracorporal, intravascular) and “clinical relevance”. Results: More cement is used in BK (5.2 ml) than in RFK (4.0 ml, p = 0.001). Cement leakage was found in 48/79 patients (60.8 %) with BK and 53/83 patients (63.9 %) with RFK (p = 0.420). Even subanalysis by location showed no significant difference between the two methods. Despite the high leakage rates, we experienced only two cases (1× BK, 1× RFK) with intravascular leakage in the inferior vena cava, with interventional endovascular salvage. Conclusion: The two examined kyphoplasty methods (BK vs. RFK) have the same high rates of cement leakage, but are rarely associated with clinically manifest complications. Clinically relevant differences between the two compared kyphoplasty methods could not be found.

Hintergrund: Die peridurale Infiltrationstherapie (PDI) ist ein bewahrtes Verfahren zur konservativen Behandlung des unteren Ruckenschmerzes. In vorhergehenden Studien konnte gezeigt werden, dass bei Patienten mit chronischen Schmerzen der unteren Lendenwirbelsaule die posturale Stabilitat, die fur den aufrechten Stand und Gang essenziell ist, schmerzbedingt vermindert ist. Ziel unserer Studie war es, mithilfe computergestutzter dynamischer Posturografie (CDP) herauszufinden, ob die Schmerzlinderung durch PDI zur Verbesserung der posturalen Kontrolle fuhrt. Patienten und Methoden: Es nahmen 32 Patienten mit chronischem Schmerzsyndrom der unteren Lendenwirbelsaule teil. Die CDP erfolgte jeweils vor Therapiebeginn und in definierten Zeitabstanden wahrend und nach der PDI-Serie von insgesamt 3 Injektionen. Hauptzielgrose war der Gesamtstabilitatsindex (OSI) im pra- und postinterventionellen Vergleich. Ergebnisse: Es zeigte sich, dass die Schmerzintensitat auf der visuellen Analogskala von 4,3 ± 2,5 Punkten auf 1,6 ± 1,9 Punkte abnahm. Dies entspricht einer relativen Schmerzreduktion um 62,8 %. Der OSI verbesserte sich ebenfalls signifikant um 21,6 % von 3,7 ± 1,7° auf 2,9 ± 1,4° (Tag 5; p = 0,019). Schlussfolgerung: Wir konnten zeigen, dass eine Schmerzlinderung durch eine PDI-Serie zu einer Verbesserung der posturalen Kontrolle fuhrt. Damit geht die PDI-Therapie nicht nur mit einer nachweislichen Schmerzreduktion einher, sondern erhoht auch die Standstabilitat des Achsenskeletts. Dies ist im Rahmen der anschliesenden physiotherapeutischen Beubung und Rehabilitation von groser Bedeutung.

Purpose Cement leakage is a typical complication of kyphoplasty for vertebral fractures. It is unclear if cement application intraoperatively can be improved by using other techniques of visualization and which kind of postoperative imaging should be recommended to detect cement extravasation accurately. Objective To compare the rates of cement leakage detected by intraoperative fluoroscopy, postoperative radiography, and postoperative computed tomography (CT) in a retrospective study. Patients and Methods The study included 78 patients (60 women and 18 men; 115 vertebral bodies) who were treated with two kinds of kyphoplasty. The patients underwent intraoperative fluoroscopy and postoperative radiography and CT. After surgery, the images were evaluated to compare cement leakage rates and locations in the three visualization techniques. Leakage locations were described as epidural, intradiskal, extravertebral, or intravascular. Results Compared with CT, intraoperative fluoroscopy regularly detected intradiskal leakage (75%) but had a considerably lower sensitivity for visualizing epidural (21%), extravertebral (31%), and intravascular (51%) cement leakages. A comparison of radiography and CT showed that radiography had a high sensitivity for detecting intradiskal (82%) and intravascular (70%) cement extrusions but a lower sensitivity in identifying epidural (42%) and extravertebral (50%) leaks. Therefore, the CT scan overall was best in detecting location and accuracy. Conclusion CT detected more cement leaks than any of the other investigated techniques, especially epidural, extravertebral, and intravascular cement leakages. To achieve the best accuracy, only CT provides complete information.

Hintergrund: Ziel der operativen Versorgung von Becken- und Azetabulumfrakturen ist eine rasche Mobilisierung des Patienten mit Wiederherstellung der Stand- und Gangstabilitat (posturale Kontrolle), da diese signifikant mit dem Outcome korreliert. Die Regulation der stabilen Standfahigkeit wird dabei wesentlich durch die Weiterleitung propriozeptiver Reize gesteuert. Zudem hat das Becken als Verbindung zwischen Beinen und Wirbelsaule eine hohe Bedeutung fur die mechanische Stabilisierung. Unklar ist bisher, ob nach operativer Versorgung von Beckenring- und Azetabulumfrakturen die posturale Kontrolle beeintrachtigt ist. Ziel dieser Studie war es daher, mittels computergestutzter dynamischer Posturografie (CDP) den Einfluss von operativ versorgten Beckenring- und Azetabulumfrakturen im Mittel nach 35 Monaten auf die stabile Standfahigkeit zu untersuchen und mit einem gesunden Kollektiv zu vergleichen. Patienten und Methoden: Im Rahmen einer retrospektiven Fall-Kontroll-Studie wurden 38 Patienten nach operativ versorgter Beckenring- oder Azetabulumfraktur sowie 38 gesunde Probanden mittels CDP untersucht. Der Zeitpunkt der Nachuntersuchung lag im Mittel bei 35 (12–78) Monaten. Den wichtigsten Outcome-Parameter stellte hierbei der Gesamtstabilitatsindex (OSI) dar. Als Nebenzielgrosen wurde die Huftgelenkbeweglichkeit, die gesundheitsbezogene Lebensqualitat (SF-12) und Schmerzen untersucht. Ergebnisse: Es zeigte sich, dass operativ versorgte Beckenring- und Azetabulumfrakturen keinen Einfluss auf die stabile Standfahigkeit haben. Der OSI betrug bei der Patientengruppe 1,9° und bei den Probanden 2,1°. In der Nebenzielgrose Huftgelenkbeweglichkeit konnten keine wesentlichen Unterschiede in den Gruppen festgestellt werden. 52 % der Patienten zeigten keine oder nur leichte Schmerzen. Die gesundheitsbezogene Lebensqualitat zeigte Durchschnittswerte, die denen der Gesamtbevolkerung entsprechen. Schlussfolgerung: Becken- und Azetabulumfrakturen mit anschliesender operativer Versorgung fuhren mittelfristig nicht zu einer beeintrachtigten Standstabilitat, was fur eine rasche Mobilisierung der Patienten von groser prognostischer Bedeutung ist. Damit ist ebenso auch keine Erhohung des Sturzrisikos nach stattgehabter und ausversorgter Fraktur zu erwarten.

Kyphoplasty is used to alleviate pain and to restore the initial height of osteoporotic vertebral fractures (OVF). One of the most recent procedures is radiofrequency-targeted vertebral augmentation (RFTVA). We investigated whether restoration with this method is similar and as adequate as with the established procedure of balloon kyphoplasty (BKP), as assessed by the anatomical height of the vertebral body. The aim of our study was to compare the intravertebral angles (base-endplate) post- and preoperatively with these two procedures.The base and endplate angles were measured on 142 vertebral bodies treated by kyphoplasty (67 BKP and 75 RFTVA), on the basis of pre- and postoperative X-rays in the upright position in 87 volunteers (46 BKP and 41 RFTVA). The main object was to detect the degree of correction (Δpost-preop) with BKP compared to RFTVA. Furthermore, the sagittal alignment of the adjacent heathy levels were measured.Significant correction was detected with both BKP (BKPpre: 11.5 ± 6.0°, BKPpost: 6.2 ± 4.6°, p 0.001) and RFTVA (RFTVApre: 9.9 ± 6.2°, RFTVApost: 6.3 ± 4.4°, p 0.001). Potential correction was greater with BKP than with RFTVA (Δpost-pre BKP: - 5.3 ± 4.4°, Δpost-pre RFK: - 3.6 ± 4.4°, p = 0.03). Neither procedures gave a significant change in the sagittal angle in the adjacent segment (segment BKPpre: 13.8 ± 8.0°, Segment BKPpost: 12.5 ± 9,2°, p = 0.638; Segment RFTVApre: 18,8 ± 14,3°, Segment RFTVApost: 15.0 ± 13.2°, p = 0.330).BKP gave significantly better correction, even though both methods were able to restore significant improvement in the kyphotic angle. In the adjacent levels, correction of the sagittal angle was not significant, although the influence of the intervention on alignment tended to be less.Neben schmerztherapeutischer Wirkung zielt die Kyphoplastie auf eine Aufrichtung osteoporotisch frakturierter Wirbelkörper (WK) ab. Ein neueres Verfahren ist die Radiofrequenzkyphoplastie (RFK). Unklar ist, wie gut sich mit dieser Methodik im Vergleich zur älteren Ballonkyphoplastie (BKP) wieder eine anatomische WK-Aufrichtung erreichen lässt. Ziel unserer retrospektiven Studie war daher der Vergleich beider Verfahren bez. der Grund-/Deckplattenwinkel post- vs. präoperativ.Anhand prä- und postoperativer Stehend-Röntgenaufnahmen wurden bei 87 Probanden (46 BKP und 41 RFK) bzw. 142 kyphoplastierten WK (67 BKP und 75 RFK) die Winkel von Grund- zu Deckplatte bestimmt. Hauptzielgröße war der Grad der Aufrichtung (Δpost−prä) bei der BKP vs. RFK. Außerdem wurden für die Bestimmung des sagittalen Alignments die angrenzenden gesunden Segmente ausgemessen.Sowohl bei der BKP (BKPprä: 11,5 ± 6,0°, BKPpost: 6,2 ± 4,6°, p 0,001) als auch RFK (RFKprä: 9,9 ± 6,2°, RFKpost: 6,3 ± 4,4°, p 0,001) kam es zu einer signifikanten Aufrichtung. Es zeigte sich im Verfahrensvergleich aber eine bessere Aufrichtung in der Gruppe BKP im Vergleich zur RFK (Δpost−prä BKP: − 5,3 ± 4,4°, Δpost−prä RFK: − 3,6 ± 4,4°, p = 0,03). Kein Verfahren führte aber im umgebenden Wirbelsäulensegment zu einer signifikanten Änderung des Winkels (Segment BKPprä: 13,8 ± 8,0°, Segment BKPpost: 12,5 ± 9,2°, p = 0,638; Segment RFKprä: 18,8 ± 14,3°, Segment RFKpost: 15,0 ± 13,2°, p = 0,330).Es konnte eine signifikant bessere Aufrichtung des Kyphosewinkels durch die BKP erreicht werden, wobei beide Verfahren für sich betrachtet zu einer hochsignifikanten Verbesserung des Kyphosewinkels geführt haben. Im angrenzenden Wirbelsäulensegment ist die Auswirkung auf den messbaren Winkel nicht signifikant ausgeprägt, sodass das Alignment durch den Eingriff tendenziell weniger beeinflusst wird.

Dual energy computed tomography (DECT) is a constantly evolving technology, which opens up new diagnostic possibilities. It is particularly valuable for musculoskeletal (MSK) imaging. Due to the lack of recognition and availability of dual energy scanners, routine use is only established in a few centres. The intention of this review is to show the possibilities and fields of applications of the DECT in MSK imaging, as well as to describe technical principles and typical indications. We mainly focus on the use of DECT in the context of fragility fractures of the pelvis. The use of the DECT in pelvic fractures of the elderly could combine the advantages of CT diagnostics - fast and continuous availability, lower costs by dispensing with a supplementary MRI examination - and the high sensitivity of MRI to oedema in fragility fractures. Furthermore, the latest DECT scanners are dose neutral, so that these examinations can also be carried out without increased radiation exposure.Mit der Dual-Energy-Computertomografie (DECT) steht seit Kurzem eine sich stetig fortentwickelnde Technik zur Verfügung, die neue diagnostische Möglichkeiten eröffnet. Insbesondere die muskuloskeletale (MSK) Bildgebung kann hiervon profitieren. Aufgrund des z. T. noch fehlenden Bekanntheitsgrads und der bislang nicht flächendeckenden Verfügbarkeit der Dual-Energy-Scanner ist ein routinemäßiger Einsatz der Möglichkeiten nur in wenigen Zentren etabliert. Diese Übersichtsarbeit soll die Möglichkeiten und Einsatzgebiete der DECT bei der MSK-Bildgebung aufzeigen sowie technische Grundlagen und typische Indikationen beschreiben. Einen besonderen Schwerpunkt legen wir dabei auf den Einsatz der DECT im Rahmen von Insuffizienzfrakturen des Beckens. Durch den Einsatz der DECT bei Beckenfrakturen des älteren Menschen könnten die Vorteile der CT-Diagnostik – gute Darstellung der knöchernen Situation, schnelle und allzeitige Verfügbarkeit, niedrigere Kosten durch die Einsparung einer ergänzenden MRT-Untersuchung – mit der hohen Ödemsensitivität der Kernspintomografie für Insuffizienzfrakturen verbunden werden. Darüber hinaus sind die neuesten DECT-Scanner dosisneutral, sodass diese Untersuchungen auch ohne erhöhte Strahlenexposition für die Patienten durchgeführt werden können.

This study analyzes seasonal and spatial patterns of column carbon dioxide (CO2) over North America calculated from aircraft and tall tower measurements from the NOAA Global Greenhouse Gas Reference Network from 2004 to 2014. Consistent with expectations, gradients between the eight regions studied are larger below 2 km than above 5 km. The 11-year mean CO2 dry mole fraction (XCO2) in the column below ~ 330 hPa (~ 8 km above sea level) from NOAA's CO2 data assimilation model, CarbonTracker (CT2015), demonstrates good agreement with those calculated from calibrated measurements on aircraft and towers. Total column XCO2 was attained by combining modeled CO2 above 330 hPa from CT2015 with the measurements. We find large spatial gradients of total column XCO2 during June to August, and the north and northeast regions have ~ 3 ppm stronger summer drawdown than the south and southwest regions. The spatial gradients of total column XCO2 across North America mainly reflect large-scale circulation patterns rather than regional surface sources and sinks. We have conducted a CarbonTracker experiment to investigate the impact of Eurasian long-range transport. The result suggests that the large summer time Eurasian boreal flux contributes about half of the north-south column XCO2 gradient across North America. Our results confirm that continental-scale total column XCO2 gradients simulated by CarbonTracker are realistic and can be used to evaluate the credibility of spatial patterns from satellite retrievals, such as the long term average spatial patterns from satellite retrievals reported for Europe which show larger spatial difference (~ 6 ppm) and scattered hot spots.

BACKGROUND Ruptures of the anterior cruciate ligament (ACL) lead to an anteromedial rotational instability in the knee joint and, thus, to a deterioration of the patient's ability to stand (postural control). It still remains unclear whether postural control can be restored by ACL reconstruction. The aim of this study was to investigate the effect of a unilateral ACL graft on the ability to stably stand on the injured leg using computer-aided dynamic posturography (CDP); the stability indices on standing on one leg were compared pre- and postoperatively. PATIENTS AND METHODS A total of 25 patients were studied after unilateral ACL injury and subsequent ACL reconstruction using CDP. The average time of follow-up was 608 days (range 357-821 days). The ability to stably stand on the healthy and injured leg was compared and evaluated for significant differences. RESULTS There was a significant improvement of postural control after ACL reconstruction. The Overall Stability Index (OSI) for the leg with ACL rupture was 3.7° ± 1.6° preoperatively and 3.0° ± 1.1° postoperatively (p < 0.05). In the postoperative comparison with the healthy leg, the value in the operated leg was 3.0° ± 1.1° and the healthy leg was 3.0° ± 1.4° (p = 0.99). CONCLUSIONS ACL replacement can be improve stability compared to the preoperative value by about 21% and even the stability level of the contralateral healthy knee can be achieved.

The authors conducted this cross-sectional study to assess the influence of submaximal exercise on postural stability as measured by computerized dynamic posturography (CDP) and compared this study's CDP results with clinical balance tests such as the Berg Balance Test and the Timed Up and Go test using a descriptive/explorative approach.Forty-three participants performed two exercise tasks (6-min walk test and stair climbing). CDP and established scores (Berg Balance Test and Timed Up and Go test) were used to assess postural instability.Despite significant pulse rate increases (approximately 20%, P0.05), no clear differences in CDP results were found after completion of either exercise task. The overall stability index scores for all subjects remained unchanged (P = 0.98). A descriptive analysis showed that excellent Berg Balance Test and Timed Up and Go test results were associated with good CDP results.Submaximal exercise did not adversely affect postural stability in these older adults.

Objective Postural stability can be quantified using posturography systems, which allow different foot platform stability settings to be selected. It is unclear, however, how platform stability and postural control are mathematically correlated. Design Twenty subjects performed tests on the Biodex Stability System at all 13 stability levels. Overall stability index, medial-lateral stability index, and anterior-posterior stability index scores were calculated, and data were analyzed using analysis of variance and linear regression analysis. Results A decrease in platform stability from the static level to the second least stable level was associated with a linear decrease in postural control. The overall stability index scores were 1.5 ± 0.8 degrees (static), 2.2 ± 0.9 degrees (level 8), and 3.6 ± 1.7 degrees (level 2). The slope of the regression lines was 0.17 for the men and 0.10 for the women. Conclusions A linear correlation was demonstrated between platform stability and postural control. The influence of stability levels seems to be almost twice as high in men as in women.

This is a preliminary report of five patients diagnosed with locally advanced nonresectable pancreatic cancer who achieved improved quality of life, delay of local progression, and reduction of biomarker CA 19-9 after infusion of colloidal phosphorus 32 ( 32 P) and administration of combined chemoradiotherapy. A phase II trial using intratumoral colloidal 32 P delivery for nonresectable pancreatic cancer without metastases is in progress. Patients initially were given infusions of decadron followed by macroaggregated albumin and 30 mCi colloidal 32 P to the interstitial space of the tumor by two infusions I week apart. Through this method, doses ranging from 750,000 to 1,800,000 cGy were delivered. After administration of colloidal 32 P, external radiation to a dose of 6000 cGy minimum tumor dose, including regional lymph nodes, was given concomitantly with four intravenous infusions of 500 mg bolus 5-fluorouracil on alternating days within the first 2 weeks after initiation of external radiation. All five of these patients demonstrated cessation of local tumor growth or regression of disease on serial computed tomography scans for a minimum of 10 months from completion of therapy. Three of these patients have survived without local disease progression over 24 months from initiation of therapy, with one patient approaching 36 months. CA 19-9 values for all patients declined within weeks after completion of therapy. This new method of isotope delivery has resulted in reduction of tumor volume, normalization of the biomarker CA 19-9, and improved performance status in those patients who have localized nonresectable disease without dissemination.

Purpose: Selective high-dose radiation of solid tumors has been a goal of radiation oncology. The physiological barriers of solid tumors (high interstitial tumor pressure, reduced tumor vascularity, and poor perfusion) have been major barriers in achieving significant tumor dose of systemically infused radioconjugates. Direct tumor infusional brachytherapy overcomes these barriers and leads to selective high tumor doses. Methods and Materials: The development of interstitial tumor infusion of macroaggregated albumin (MAA) followed by colloidal chromic phosphate 32 P has overcome solid tumor obstacles in 47 patients with nonresectable pancreatic cancer in a Phase I dose escalation study. The colloidal 32 P infusion was followed by external radiation and five fluorouracil. Results: Of the 28 patients with cancer limited to the pancreas, 15 of 16 patients retained 86-100% (mean 96%) of the infused colloidal 32 P isotope. While the other 12 patients had partial shunting to the liver, shunting to the liver was due to high interstitial resistance with tumor dose deposition of 17-88% (mean 52%). Of the 19 patients with metastatic pancreas cancer, colloidal 32 P tumor deposition ranged from 22 to 100% of the infused dose (mean 79%). The less than optimal tumor deposition led to our increasing the MAA from 600,000 to 1.5-2.5 million particles. Interstitial dexamethasone 2 mg and later 4 mg was infused first and prevented liver shunting by somehow reducing tumor resistance. The median survival in 28 Phase I patients with nonresectable pancreas cancer without metastasis, was 12 months. No significant toxicity occurred when treatment was limited to two infusions with as much as 30 mCi each. The maximum tumor dose was 17,000 Gy (1.700,000 cGy). In 19 nonresectable pancreatic cancer patients with metastasis, a 6.9 months median survival was observed. Conclusion: Infusional brachytherapy is an outpatient procedure that delivers high-dose radiation selectively to pancreatic cancer. Results of the Phase I study in nonresectable pancreas cancer has led to a national multiinstitutional Phase II trial.
