Introduction: Capsule endoscopy (CE) is a minimally invasive exam suitable of panendoscopic evaluation of the gastrointestinal (GI) tract. Nevertheless, CE is time-consuming with suboptimal diagnostic yield in the upper GI tract. Convolutional neural networks (CNN) are human brain architecture-based models suitable for image analysis. However, there is no study about their role in capsule panendoscopy. Methods: Our group developed an artificial intelligence (AI) model for panendoscopic automatic detection of pleomorphic lesions (namely vascular lesions, protuberant lesions, hematic residues, ulcers, and erosions). 355,110 images (6,977 esophageal, 12,918 gastric, 258,443 small bowel, 76,772 colonic) from eight different CE and colon CE (CCE) devices were divided into a training and validation dataset in a patient split design. The model classification was compared to three CE experts’ classification. The model’s performance was evaluated by its sensitivity, specificity, accuracy, positive predictive value, negative predictive value, and area under the precision-recall curve. Results: The binary esophagus CNN had a diagnostic accuracy for pleomorphic lesions of 83.6%. The binary gastric CNN identified pleomorphic lesions with a 96.6% accuracy. The undenary small bowel CNN distinguished pleomorphic lesions with different hemorrhagic potentials with 97.6% accuracy. The trinary colonic CNN (detection and differentiation of normal mucosa, pleomorphic lesions, and hematic residues) had 94.9% global accuracy. Discussion/Conclusion: We developed the first AI model for panendoscopic automatic detection of pleomorphic lesions in both CE and CCE from multiple brands, solving a critical interoperability technological challenge. Deep learning-based tools may change the landscape of minimally invasive capsule panendoscopy.

O foco do trabalho pericial está direcionado em identificar e recolher os vestígios em cenas de crime. Todo o material recolhido pode passar por análises, como de DNA, que possibilita determinar autoria do delito, por meio da utilização de marcadores moleculares. O objetivo do presente trabalho é apresentar um relato de caso, destacando a viabilidade, eficiência do trabalho pericial tanto na cena de crime, quanto no Laboratório de Biologia em união com a investigação policial. Trata-se de um crime sexual envolvendo dois indivíduos do sexo masculino, em cuja cena do crime foram coletadas amostras questionadas para realizar a comparação entre as amostras dos envolvidos. Os perfis genéticos das amostras 2 (unhas postiças da vítima) e 3A e 3B (preservativo do quarto), apresentaram correspondência com a amostra referência da vítima. A amostra questionada 4A (preservativo sanitário parte externa) apresentou uma mistura de material genético de duas pessoas do sexo masculino e já as amostras questionadas 4B (preservativo sanitário parte interna) e 5 (lençol da cama) apresentaram perfil correspondente ao suspeito. É interessante mencionar que o fluxo das atividades investigativas permitiu a busca do suspeito mesmo após o término das análises laboratoriais. Assim, a garantia de que todos os procedimentos foram adotados para manter a cadeia de custódia, permitiu que novas análises do caso fossem realizadas com sucesso. As análises do suabe oral do suspeito encaminhado posteriormente pelo IML, levaram à sua inclusão juntamente com a vítima, como produtor de um dos perfis genéticos da mistura extraída no preservativo. Destacam-se pontos importantes para o desfecho exitoso apresentado no presente relato de caso. O primeiro se refere à adequada coleta e preservação dos vestígios biológicos encontrados no local, permitindo a adequada obtenção de produtos de amplificação de DNA nos procedimentos laboratoriais. O segundo se refere à garantia de manutenção da cadeia de custódia, além da integração entre atividades investigativa e pericial durante o trabalho policial.

Background: Capsule endoscopy (CE) is a valuable tool for assessing inflammation in patients with Crohn’s disease (CD). The current standard for evaluating inflammation are validated scores (and clinical laboratory values) like Lewis score (LS), Capsule Endoscopy Crohn’s Disease Activity Index (CECDAI), and ELIAKIM. Recent advances in artificial intelligence (AI) have made it possible to automatically select the most relevant frames in CE. Objectives: In this proof-of-concept study, our objective was to develop an automated scoring system using CE images to objectively grade inflammation. Design: Pan-enteric CE videos (PillCam Crohn’s) performed in CD patients between 09/2020 and 01/2023 were retrospectively reviewed and LS, CECDAI, and ELIAKIM scores were calculated. Methods: We developed a convolutional neural network-based automated score consisting of the percentage of positive frames selected by the algorithm (for small bowel and colon separately). We correlated clinical data and the validated scores with the artificial intelligence-generated score (AIS). Results: A total of 61 patients were included. The median LS was 225 (0–6006), CECDAI was 6 (0–33), ELIAKIM was 4 (0–38), and SB_AIS was 0.5659 (0–29.45). We found a strong correlation between SB_AIS and LS, CECDAI, and ELIAKIM scores (Spearman’s r = 0.751, r = 0.707, r = 0.655, p = 0.001). We found a strong correlation between LS and ELIAKIM ( r = 0.768, p = 0.001) and a very strong correlation between CECDAI and LS ( r = 0.854, p = 0.001) and CECDAI and ELIAKIM scores ( r = 0.827, p = 0.001). Conclusion: Our study showed that the AI-generated score had a strong correlation with validated scores indicating that it could serve as an objective and efficient method for evaluating inflammation in CD patients. As a preliminary study, our findings provide a promising basis for future refining of a CE score that may accurately correlate with prognostic factors and aid in the management and treatment of CD patients.

Background: The role of capsule endoscopy in the evaluation of the small bowel is well established, and current guidelines position it as a first-line test in a variety of clinical scenarios. The advent of double-headed capsules further enabled the endoscopic assessment of colonic mucosa and the opportunity for a one-step noninvasive examination of the entire bowel (pan-enteric capsule endoscopy [PCE]). Summary: We reviewed the technical procedure and preparation of patients for PCE, as well as its current clinical applications and future perspectives. In non-stricturing and non-penetrating Crohn’s disease affecting the small bowel and colon, PCE monitors disease activity by assessing mucosal healing, a major treatment outcome, with a higher diagnostic yield than cross-sectional imaging or conventional colonoscopy. Also in ulcerative colitis, double-headed capsules have been used to monitor disease activity noninvasively. Currently, validated scoring systems have been specifically devised for these double-headed capsules and permit a standardized assessment of the inflammatory burden. In suspected mid-lower digestive bleeding, some exploratory studies have demonstrated the feasibility and high diagnostic yield of PCE, which may work as a filter indicating which patients may benefit of further invasive procedures, namely, for planned hemostatic procedures. The possibility of using PCE is also discussed in the context of polyposis syndromes with simultaneous involvement of the small intestine and colon. Key Messages: PCE is a feasible, effective, and safe diagnostic procedure to evaluate the small bowel and colon. It has been increasingly explored in the setting of inflammatory bowel diseases and, more recently, in suspected mid-lower digestive bleeding. PCE is expected to reduce the demand for invasive procedures and expand the scope of noninvasive intestinal evaluation in the coming future.

Abstract Background Asymptomatic infection is the most common outcome of exposure to Leishmania parasites. In the Mediterranean region, where Leishmania infantum is endemic, studies on the prevalence of asymptomatic infection have often relied on serological testing in blood donors. In Spain, regional studies have shown seroprevalence in blood donors between 1 and 8%; in Portugal, values of 0 and 2% were suggested by two localized studies, in different populations. The purpose of this study was (i) to estimate the prevalence of asymptomatic Leishmania infection in blood donors in mainland Portugal, and (ii) to study the association between the detection of antibodies to Leishmania and sociodemographic factors, and also the knowledge, perceptions and practices (KPP) of the blood donors regarding leishmaniasis. Methods A cross-sectional study targeted the population of people who donated blood in mainland Portugal. Participants, distributed proportionally by municipality and aged between 18 and 65 years, were selected randomly in 347 blood collection points between February and June 2022, and completed a sociodemographic and a KPP questionnaire. Detection of anti-Leishmania antibodies in serum was performed using an ELISA commercial kit. Individual KPP scores were calculated by adding grades defined for each question. Results Globally, 201/3763 samples were positive. The estimated national true seroprevalence was 4.8% (95% CI 4.1–5.5%). The proportion of positive results was significantly different between NUTS (Nomenclature of Territorial Units for Statistics) regions. Models suggested that seropositivity was significantly higher in male sex, people older than 25 years, or residing in the Centro NUTS2 region, but not in dog owners nor people with lower KPP scores. Overall, 72.3% of participants had previously heard of leishmaniasis and, in multivariate analysis, a higher Knowledge score was associated with age 25–40 years, female sex, ownership of dogs, and higher education. Conclusions Global estimated true seroprevalence (4.8%) was similar to previous regional studies in blood donors in neighboring Spain. Higher seroprevalence values in the NUTS2 Centro region were consistent with incidence data from humans and seroprevalence studies in dogs. On the other hand, the low values in the Alentejo and the high values in the northern subregions may be the result of geographical shifts in parasite circulation due to climate change and should prompt localized and integrated, vector, canine, and human research, following a One Health approach. The role of capsule endoscopy and enteroscopy in managing various small-bowel pathologies is well-established. However, their broader application has been hampered mainly by their lengthy reading times. As a result, there is a growing interest in employing artificial intelligence (AI) in these diagnostic and therapeutic procedures, driven by the prospect of overcoming some major limitations and enhancing healthcare efficiency, while maintaining high accuracy levels. In the past two decades, the applicability of AI to gastroenterology has been increasing, mainly because of the strong imaging component. Nowadays, there are a multitude of studies using AI, specifically using convolutional neural networks, that prove the potential applications of AI to these endoscopic techniques, achieving remarkable results. These findings suggest that there is ample opportunity for AI to expand its presence in the management of gastroenterology diseases and, in the future, catalyze a game-changing transformation in clinical activities. This review provides an overview of the current state-of-the-art of AI in the scope of small-bowel study, with a particular focus on capsule endoscopy and enteroscopy.

Background and Aims: Capsule endoscopy (CE) revolutionized the study of the small intestine, overcoming the limitations of conventional endoscopy. Nevertheless, reviewing CE images is time-consuming. Convolutional Neural Networks (CNNs) are an artificial intelligence architecture with high performance levels for image analysis. Protruding lesions of the small intestine exhibit enormous morphologic diversity in CE images. We aimed to develop a CNN-based algorithm for automatic detection of varied small-bowel protruding lesions. Methods: A CNN was developed using a pool of CE images containing protruding lesions or normal mucosa/other findings. A total of 2565 patients were included. These images were inserted into a CNN model with transfer learning. We evaluated the performance of the network by calculating its sensitivity, specificity, accuracy, positive predictive value, and negative predictive value. Results: A CNN was developed based on a total of 21,320 CE images. Training and validation data sets comprising 80% and 20% of the total pool of images, respectively, were constructed for development and testing of the network. The algorithm automatically detected small-bowel protruding lesions with an accuracy of 97.1%. Our CNN had a sensitivity, specificity, positive, and negative predictive values of 95.9%, 97.1%, 83.0%, and 95.7%, respectively. The CNN operated at a rate of approximately 355 frames per second. Conclusion: We developed an accurate CNN for automatic detection of enteric protruding lesions with a wide range of morphologies. The development of these tools may enhance the diagnostic efficiency of CE.

Introduction: Amyotrophic lateral sclerosis (ALS) is a neurodegenerative disorder with an inexorably progressive course which leads to a progressive neuromuscular weakness. Weight loss is one of the major bad prognostic factors in ALS. The placement of percutaneous endoscopic gastrostomy (PEG) is of paramount importance in patients with dysphagia to improve the disease outcomes, although some fear exists regarding the possible ventilatory complications during the procedure. The aim of this study was to evaluate the safety and effectiveness of PEG tube insertion under non-invasive ventilation (NIV) in patients with ALS and severe ventilatory impairment. Methods: A retrospective study of all consecutive PEGs placed in our department from May 2011 to January 2018 in patients with ALS was performed. The procedure was performed under non-invasive positive-pressure ventilation for ventilatory support. Results: We included 59 patients with ALS with severe ventilatory impairment, 58% were female, with a mean age of 67.2 ± 10.1 years and a median follow-up of 6 [2–15] months. The main indication for PEG placement was dysphagia (98%). The median time for PEG tube insertion since the established diagnosis of ALS was 12 [6–25] months and 4 [2–18] months since the beginning of bulbar symptoms. The majority of the patients had placed a 20-Fr PEG (63%) and under mild sedation with midazolam (80%), all under NIV. There were no immediate complications during and after the procedure (no episodes of aspiration or orotracheal intubation) and mortality. Conclusion: The placement of PEG is a very important procedure in patients with ALS and severe ventilatory impairment. The interdisciplinary department collaboration permitted the placement of PEG under NIV, in a safe and effective procedure in this special population.

Introduction: Capsule endoscopy has revolutionized the management of patients with obscure gastrointestinal bleeding. Nevertheless, reading capsule endoscopy images is time-consuming and prone to overlooking significant lesions, thus limiting its diagnostic yield. We aimed to create a deep learning algorithm for automatic detection of blood and hematic residues in the enteric lumen in capsule endoscopy exams. Methods: A convolutional neural network was developed based on a total pool of 22,095 capsule endoscopy images (13,510 images containing luminal blood and 8,585 of normal mucosa or other findings). A training dataset comprising 80% of the total pool of images was defined. The performance of the network was compared to a consensus classification provided by 2 specialists in capsule endoscopy. Subsequently, we evaluated the performance of the network using an independent validation dataset (20% of total image pool), calculating its sensitivity, specificity, accuracy, and precision. Results: Our convolutional neural network detected blood and hematic residues in the small bowel lumen with an accuracy and precision of 98.5 and 98.7%, respectively. The sensitivity and specificity were 98.6 and 98.9%, respectively. The analysis of the testing dataset was completed in 24 s (approximately 184 frames/s). Discussion/Conclusion: We have developed an artificial intelligence tool capable of effectively detecting luminal blood. The development of these tools may enhance the diagnostic accuracy of capsule endoscopy when evaluating patients presenting with obscure small bowel bleeding.

The present evidence-based guidelines are focused on the use of device-assisted enteroscopy in the management of small-bowel diseases. A panel of experts selected by the Spanish and Portuguese small bowel study groups reviewed the available evidence focusing on the main indications of this technique, its role in the management algorithm of each indication and on its diagnostic and therapeutic yields. A set of recommendations were issued accordingly.

Background and study aims Colon capsule endoscopy (CCE) is a minimally invasive alternative to conventional colonoscopy. However, CCE produces long videos, making its analysis time-consuming and prone to errors. Convolutional neural networks (CNN) are artificial intelligence (AI) algorithms with high performance levels in image analysis. We aimed to develop a deep learning model for automatic identification and differentiation of significant colonic mucosal lesions and blood in CCE images. Patients and methods A retrospective multicenter study including 124 CCE examinations was conducted for development of a CNN model, using a database of CCE images including anonymized images of patients with normal colon mucosa, several mucosal lesions (erosions, ulcers, vascular lesions and protruding lesions) and luminal blood. For CNN development, 9005 images (3,075 normal mucosa, 3,115 blood and 2,815 mucosal lesions) were ultimately extracted. Two image datasets were created and used for CNN training and validation. Results The mean (standard deviation) sensitivity and specificity of the CNN were 96.3 % (3.9 %) and 98.2 % (1.8 %) Mucosal lesions were detected with a sensitivity of 92.0 % and a specificity of 98.5 %. Blood was detected with a sensitivity and specificity of 97.2 % and 99.9 %, respectively. The algorithm was 99.2 % sensitive and 99.6 % specific in distinguishing blood from mucosal lesions. The CNN processed 65 frames per second. Conclusions This is the first CNN-based algorithm to accurately detect and distinguish colonic mucosal lesions and luminal blood in CCE images. AI may improve diagnostic and time efficiency of CCE exams, thus facilitating CCE adoption to routine clinical practice.

With modern society well entrenched in the digital area, the use of Artificial Intelligence (AI) to extract useful information from big data has become more commonplace in our daily lives than we perhaps realize. Medical specialties that rely heavily on imaging techniques have become a strong focus for the incorporation of AI tools to aid disease diagnosis and monitoring, yet AI-based tools that can be employed in the clinic are only now beginning to become a reality. However, the potential introduction of these applications raises a number of ethical issues that must be addressed before they can be implemented, among the most important of which are issues related to privacy, data protection, data bias, explainability and responsibility. In this short review, we aim to highlight some of the most important bioethical issues that will have to be addressed if AI solutions are to be successfully incorporated into healthcare protocols, and ideally, before they are put in place. In particular, we contemplate the use of these aids in the field of gastroenterology, focusing particularly on capsule endoscopy and highlighting efforts aimed at resolving the issues associated with their use when available.

Background and Objectives: Device-assisted enteroscopy (DAE) has a significant role in approaching enteric lesions. Endoscopic observation of ulcers or erosions is frequent and can be associated with many nosological entities, namely Crohn’s disease. Although the application of artificial intelligence (AI) is growing exponentially in various imaged-based gastroenterology procedures, there is still a lack of evidence of the AI technical feasibility and clinical applicability of DAE. This study aimed to develop and test a multi-brand convolutional neural network (CNN)-based algorithm for automatically detecting ulcers and erosions in DAE. Materials and Methods: A unicentric retrospective study was conducted for the development of a CNN, based on a total of 250 DAE exams. A total of 6772 images were used, of which 678 were considered ulcers or erosions after double-validation. Data were divided into a training and a validation set, the latter being used for the performance assessment of the model. Our primary outcome measures were sensitivity, specificity, accuracy, positive predictive value (PPV), negative predictive value (NPV), and an area under the curve precision–recall curve (AUC-PR). Results: Sensitivity, specificity, PPV, and NPV were respectively 88.5%, 99.7%, 96.4%, and 98.9%. The algorithm’s accuracy was 98.7%. The AUC-PR was 1.00. The CNN processed 293.6 frames per second, enabling AI live application in a real-life clinical setting in DAE. Conclusion: To the best of our knowledge, this is the first study regarding the automatic multi-brand panendoscopic detection of ulcers and erosions throughout the digestive tract during DAE, overcoming a relevant interoperability challenge. Our results highlight that using a CNN to detect this type of lesion is associated with high overall accuracy. The development of binary CNN for automatically detecting clinically relevant endoscopic findings and assessing endoscopic inflammatory activity are relevant steps toward AI application in digestive endoscopy, particularly for panendoscopic evaluation.

Colon capsule endoscopy (CCE) is a minimally invasive alternative to conventional colonoscopy. Most studies on CCE focus on colorectal neoplasia detection. The development of automated tools may address some of the limitations of this diagnostic tool and widen its indications for different clinical settings. We developed an artificial intelligence model based on a convolutional neural network (CNN) for the automatic detection of blood content in CCE images. Training and validation datasets were constructed for the development and testing of the CNN. The CNN detected blood with a sensitivity, specificity, and positive and negative predictive values of 99.8 %, 93.2 %, 93.8 %, and 99.8 %, respectively. The area under the receiver operating characteristic curve for blood detection was 1.00. We developed a deep learning algorithm capable of accurately detecting blood or hematic residues within the lumen of the colon based on colon CCE images.

Background: Colon capsule endoscopy (CCE) is an alternative for patients unwilling or with contraindications for conventional colonoscopy. Colorectal cancer screening may benefit greatly from widespread acceptance of a non-invasive tool such as CCE. However, reviewing CCE exams is a time-consuming process, with risk of overlooking important lesions. We aimed to develop an artificial intelligence (AI) algorithm using a convolutional neural network (CNN) architecture for automatic detection of colonic protruding lesions in CCE images. An anonymized database of CCE images collected from a total of 124 patients was used. This database included images of patients with colonic protruding lesions or patients with normal colonic mucosa or with other pathologic findings. A total of 5715 images were extracted for CNN development. Two image datasets were created and used for training and validation of the CNN. The AUROC for detection of protruding lesions was 0.99. The sensitivity, specificity, PPV and NPV were 90.0%, 99.1%, 98.6% and 93.2%, respectively. The overall accuracy of the network was 95.3%. The developed deep learning algorithm accurately detected protruding lesions in CCE images. The introduction of AI technology to CCE may increase its diagnostic accuracy and acceptance for screening of colorectal neoplasia.

Los programas de estímulo a la alimentación escolar son relevantes pues contribuyen a la formación de los(as) alumnos(as) y comunidades. Analizando el caso de Santa Lucía (en el Caribe), destaca el proyecto Fortalecimiento de los programas de alimentación escolar en el marco de la iniciativa América Latina y el Caribe sin Hambre 2025, desarrollado por la Cooperación del gobierno brasileño junto con la Organización de las Naciones Unidas para la Alimentación y la Agricultura (FAO). El proyecto, que se llevó a cabo entre 2009 y 2017, fue desarrollado en más de 10 países y tenía como objetivo principal fomentar la alimentación escolar sustentable. El proyecto se inició entre 2014 y 2015 en Santa Lucía, donde hay cuatro escuelas-piloto. Dos de ellas se ubican en áreas rurales, y son las que presentan los mejores resultados. La ponencia tiene como objetivo analizar los efectos del proyecto en Santa Lucía, profundizando en el análisis realizado por la evaluación externa conducida entre FAO y gobierno brasileño. La metodología es una revisión bibliográfica de los principales documentos oficiales y de la situación en Santa Lucía. Los principales resultados expresan una mejor participación de los alumnos en la escuela, fortalecimiento de la gestión escolar, entre otros.

Background and Objectives: Device-assisted enteroscopy (DAE) allows deep exploration of the small bowel and combines diagnostic and therapeutic capacities. Suspected mid-gastrointestinal bleeding is the most frequent indication for DAE, and vascular lesions, particularly angioectasia, are the most common etiology. Nevertheless, the diagnostic yield of DAE for the detection of these lesions is suboptimal. Deep learning algorithms have shown great potential for automatic detection of lesions in endoscopy. We aimed to develop an artificial intelligence (AI) model for the automatic detection of angioectasia DAE images. Materials and Methods: A convolutional neural network (CNN) was developed using DAE images. Each frame was labeled as normal/mucosa or angioectasia. The image dataset was split for the constitution of training and validation datasets. The latter was used for assessing the performance of the CNN. Results: A total of 72 DAE exams were included, and 6740 images were extracted (5345 of normal mucosa and 1395 of angioectasia). The model had a sensitivity of 88.5%, a specificity of 97.1% and an AUC of 0.988. The image processing speed was 6.4 ms/frame. Conclusions: The application of AI to DAE may have a significant impact on the management of patients with suspected mid-gastrointestinal bleeding.

Although infliximab (IFX) is an efficient therapy for ulcerative colitis (UC) patients, a considerably high rate of therapeutic failures still occurs. This study aimed at a better understanding of IFX pharmacokinetics and pharmacodynamics among clinically-asymptomatic UC patients. This was a multicentric and prospective study involving 65 UC patients in the maintenance phase of IFX therapy. There were no significant differences between patients with positive and negative clinical, endoscopic and histological outcomes concerning their IFX trough levels (TLs), area under the IFX concentration vs. time curve (AUC), clearance and antibodies to infliximab (ATI) levels. However, the need to undergo therapeutic escalation later in disease development was significantly associated with higher ATI levels (2.62 μg/mL vs. 1.15 μg/mL, p = 0.028). Moreover, and after adjusting for disease severity, the HR (hazard ratio) for therapeutic escalation was significantly decreased for patients with an ATI concentration below 3 μg/mL (HR = 0.119, p = 0.010), and increased for patients with fecal calprotectin (FC) level above 250 μg/g (HR = 9.309, p = 0.018). In clinically-stable UC patients, IFX pharmacokinetic features cannot predict therapeutic response on a short-term basis. However, high levels of ATIs or FC may be indicative of a future therapeutic escalation.

Com o objetivo de avaliar a morfologia, a morfometria e a integridade de membrana dos espermatozoides epididimários de paca, células espermáticas oriundas da cauda do epidídimo foram obtidas de cinco animais pelo método de flutuação, utilizando os diluidores ACP-123 ® e Botusemen Special ®, coradas em Panótico rápido e Eosina-Nigrosina. Os dados foram analisados por estatística descritiva e teste t de Student. A morfologia de 200 espermatozoides corados em Panótico rápido evidenciou que os mesmos possuíam: cabeça ovalada com três vesículas na região acrossomal, peça intermediária, cauda alongada, sendo os defeitos celulares de 27%. A morfometria de 100 células espermáticas (efetuada com microscópio óptico e softwares EZ Leica LAS de aquisição de imagens para sistemas operacionais Windows) apresentou as seguintes medidas (média ± d.p): comprimento total 43,87 ± 4,91 µm, cabeça 7,54 ± 0,82 µm, peça intermediária 5,35 ± 0,83 µm, cauda 30,72 ± 2,55 µm e largura da cabeça 5,30 ± 0,68 µm. Das 2.000 células coradas em Eosina-Nigrosina para avaliação da integridade da membrana, 83,8 % das diluídas em ACP 123 ® e 72,9 % das diluídas em Botusemen® estavam com as membranas intactas. Os resultados sugerem que espermatozoides epididimários de pacas podem ser utilizados em técnicas de reprodução assistida, agregam conhecimentos a esta área e suscitam novos estudos sobre o papel das três vesículas acrossomais características nesta espécie.

Background: The treatment goal of Crohn’s disease (CD) has moved towards achieving mucosal healing, resolution of transmural inflammation, and normalization of biomarkers. The purpose of this study was to evaluate how well computed tomography enterography (CTE) and fecal calprotectin (FC) correlated with endoscopic activity in newly diagnosed patients with CD and after 1 year of therapy. Methods: Consecutive patients with newly diagnosed CD were evaluated by endoscopy, CTE, and FC at diagnosis and 12 months after beginning immunosuppression. Endoscopic severity was assessed using the Simplified Endoscopic Score for Crohn’s Disease (SES-CD). Biomarkers, clinical indexes, and FC were recorded on the day of ileocolonoscopy at diagnosis and 1 year after diagnosis. We adapted a CTE score for disease activity based on radiological signs of inflammation (i.e. mural thickness, mural hyperenhancement, mesenteric fat proliferation, mesenteric fat densification, comb sign, presence of strictures, fistulas, abscesses, ascites, and lymphadenopathy). Correlations between endoscopy, CTE, and FC were assessed using Spearman’s rank correlation. Results: A total of 29 patients (48% women; median age 30 (24.5–35.5) years) were included in this prospective cohort. CTE findings significantly correlated with endoscopic findings. Endoscopic remission (ER) at 1-year follow up significantly correlated with improvement in mural hyperenhancement ( p = 0.004), mesenteric fat densification ( p = 0.001), comb sign ( p = 0.004), and strictures ( p = 0.008) in CTE. None of the CTE findings improved in patients without ER. FC correlated with SES-CD ( rs = 0.696, p < 0.001) and with CTE features of inflammation ( rs = 0.596, p < 0.001). A cut-off of 100 µg/g predicted ER with 92% sensitivity, 65% specificity, and 83% accuracy (area under curve 0.878, p < 0.001). Conclusions: CTE findings and FC levels correlated with endoscopic activity in CD both at diagnosis and at 1-year follow up. These two noninvasive markers of disease activity may be used as an alternative to endoscopy to monitor disease response to therapy.

Introduction: Cholangiopancreatoscopy with SpyGlass™ Direct Visualization System (SGDVS) is being used in specialized centers for improving the sensitivity of endoscopic retrograde cholangiopancreatography (ERCP) in patients with indeterminate pancreatobiliary strictures (PBS). The aims of this study were to report our initial experience with SGDVS in the evaluation of indeterminate PBS, and discuss the improvements of ERCP brought by this technique in our center. Methods: The usefulness of SGDVS in patients with indeterminate PBS (defined after nondiagnostic previous ERCP with brush cytology) was evaluated in a prospective observational cohort study conducted at a single tertiary biliopancreatic unit. The accuracy of diagnosis by the SGDVS visual findings, SGDVS-guided biopsy, technical success, image quality, change in patient management after the procedure, and complication rate were assessed. Results: In our single-center cohort, there were 13 SGDVS procedures for evaluating indeterminate PBS. Technical success, defined by the ability to progress with the SpyScope to the target lesion, was achieved in all the cases. The diagnostic accuracy of visual findings (87.5%) was superior to SGDVS-guided biopsy (55%). In 11 (85%) procedures, the image quality was considered good. The procedure permitted exclusion of malignancy and avoiding surgery in 9 patients (69%). There were no complications during the procedures. However, in the post-procedure monitoring, 3 patients developed acute pancreatitis (19%) and 2 patients developed acute cholangitis (13%). Conclusion: The SGDVS can be considered useful in the context of indeterminate PBS. The intervention is associated with high procedural success and alters clinical outcome compared to conventional approaches.

Background: Therapeutic drug monitoring (TDM)-based algorithms can be used to guide infliximab (IFX) adjustments in inflammatory bowel disease (IBD) patients. This study aimed to explore a rapid IFX-quantification test from a clinical perspective. Methods: This manuscript describes a prospective cohort study involving 110 ulcerative colitis (UC) patients on the maintenance phase of IFX. IFX trough levels were quantified using a rapid quantification assay and a commonly-used reference kit. Results: Irrespective of the assay used to measure IFX, its through levels were statistically different between patients with and without endoscopic remission (Mayo endoscopic score = 0), as well as between patients stratified by their faecal calprotectin (FC) levels. Despite the fact that the two methods correlated well with each other [Spearman’s rank correlation coefficient = 0.843, p < 0.001; intraclass correlation coefficients = 0.857, 95% confidence interval (CI): 0.791–0.903], there was a discernible systematic variation; values obtained with the reference kit were on average 2.62 units higher than those obtained with the rapid assay. Notwithstanding, 3 µg/ml was shown to be an acceptable cut-off to assess endoscopic status and inflammatory burden levels using both assays. The percentage of patients that had a positive outcome when the IFX concentration measured by the rapid assay ranked above 3 µg/ml was 88% both for a Mayo endoscopic score ⩽ 1 and for an FC concentration

O presente artigo tem como objetivo analisar os avanços e os limites na análise da queda recente verificada na desigualdade no Brasil. É sabido que durante os anos 2000, a alteração verificada no Índice de Gini teve como efeitos a redução expressiva da pobreza extrema, com um crescimento de quase 20% na renda desses indivíduos. No entanto, apesar da melhora nos rendimentos - derivado da conjugação entre o aquecimento da economia nacional (com expansão do consumo, melhora dos preços relativos e maior atuação das políticas sociais) e internacional - alguns trabalhos argumentam que a desigualdade deve ser vista por um prisma multidimensional. Nesse sentido, pretende-se sintetizar e analisar a literatura acerca do tema, demonstrando as suas limitações e propondo novos enfoques para o estudo da realidade brasileira.

Abstract Introduction A significant portion of patients treated in emergency departments have nasal fracture. It is important that the otolaryngologist know how to treat such damage. Objectives To evaluate the effectiveness of nasal fracture reduction under local anesthesia and tolerance to the procedure. Methods Twenty-four patients treated in the emergency department with closed reduction under local anesthesia were prospectively followed. Epidemiologic information and data regarding pain and complications during the management were noted. The degree of satisfaction was researched by visual analog scale. Results The majority of patients were male (75%), and the most common cause of injury was motor vehicle accident. We found a significant association between time to reduction and referred pain during the procedure. In patients in whom the procedure was delayed (over 3 days), there was less pain, and those who bled during the procedure had a shorter average time to reduction than the group of patients who did not bleed. Most patients were very satisfied, with more than 95% of these willing to undergo the same process again, if necessary. Conclusions The closed approach in the clinic under local anesthesia was effective and safe in restoration of the nose.

A linfadenopatia no lúpus eritematoso sistêmico (LES) é um achado benigno encontrado comumente em jovens, com atividade cutânea e sintomas constitucionais, apresentando boa resposta à corticoterapia. O achado mais frequente à biópsia é a hiperplasia folicular reacional. Relatamos o caso de um paciente que, desde os 13 anos de idade, apresentava surtos recorrentes de linfadenopatia, acompanhados de hepatoesplenomegalia, febre e emagrecimento. Na evolução, apareceram artrite, hipertensão arterial, proteinúria, miocardiopatia e neuropatia periférica. Foi amplamente investigado sem esclarecimento diagnóstico e submetido a tratamento empírico de tuberculose. Somente após cinco anos de evolução firmou-se o diagnóstico de LES e recebeu tratamento específico. O diagnóstico precoce nestes casos é difícil, pois a investigação laboratorial pode ainda não demonstrar presença de autoanticorpos ou hipocomplementemia.Lymphadenopathy is a benign finding in systemic lupus erythematosus (SLE), commonly seen in young patients with cutaneous involvement and constitutional symptoms, with good response to corticosteroids. Reactive follicular hyperplasia is the most frequent finding in biopsies. We report the case of a patient with recurrent episodes of lymphadenopathy associated with hepatosplenomegaly, fever, and weight loss since the age of 13 years. The patient also developed arthritis, hypertension, proteinuria, cardiomyopathy, and peripheral neuropathy. His condition was investigated extensively without diagnostic clarification; he was treated, empirically, for tuberculosis. The patient received a diagnosis of SLE only five years after the original presentation and received the specific treatment. Early diagnosis in those cases is difficult because laboratorial exams may not show the presence of auto-antibodies and low complement levels.

INTRODUÇÃO: A esclerose sistêmica (ES) é uma doença reumatológica caracterizada por inflamação autoimune, vasculopatia e fibrose da pele e de vários órgãos. Poucos tratamentos são considerados eficazes para a doença com intenso envolvimento de pele como a ES na forma difusa. Portanto, avaliamos a eficácia da ciclofosfamida no tratamento de pacientes com ES difusa grave. PACIENTES E MÉTODOS: Nove pacientes com ES difusa (Critérios do Colégio Americano de Reumatologia [ACR]) com escore de Rodnan de pele modificado (ERM) > 30 (0-51) foram submetidos ao tratamento com ciclofosfamida (CFM) EV na dose de 0,5 a 1,0 g/m² mensal por 18 meses, totalizando 18 pulsos. Além da realização do ERM a cada seis meses, exames laboratoriais e avaliação de efeitos adversos foram também analisados. Pacientes com doença pulmonar e envolvimento cardíaco foram excluídos. RESULTADOS: A maioria dos pacientes era do sexo feminino (77%), com média de idade de 41,7 anos e tempo de doença de 2,2 anos. Observou-se redução significativa do ERM de 37,7 ± 4,08 para 29,1 ± 8,13 após 12 meses de tratamento (P = 0,009). Sete pacientes completaram 18 meses de CFM e com redução persistente do ERM (média ERM = 26,4; P = 0,01). Houve também uma redução da proteína C-reativa (PCR) (média inicial PCR = 8,9 mg/dL) quando comparada com valores após 18 meses de tratamento (média PCR = 3,09 mg/dL; P = 0,04). Não foram observadas infecções, alterações urinárias e/ou quadro hematológico importante durante todo o tratamento. CONCLUSÃO: Nesta amostra de pacientes, o tratamento com a CFM foi eficaz e pode ser considerado como terapia alternativa para pacientes com ES e envolvimento cutâneo grave.INTRODUCTION: Systemic sclerosis (SSc) is a rheumatologic disease characterized by autoimmune inflammation, vasculopathy and tissue fibrosis of skin and various organs. There are few effective treatments available for a severe cutaneous involvement of diffuse SSc. Therefore, we evaluated the efficacy of cyclophosphamide in the treatment of severe diffuse SSc. PATIENTS AND METHODS: Nine diffuse SSc (American College of Rheumatology [ACR] criteria) patients with a modified Rodnan skin score (MRSS) > 30 (0-51) were submitted to treatment with cyclophosphamide (CPM), at a monthly dose of 0.5 to 1.0 g/m² IV for 18 months. MRSS was performed every six months during 18 months. Laboratory evidence of inflammatory activity and adverse events were also retrospectively assessed. Patients with severe pulmonary and cardiac involvement were excluded. RESULTS: Most patients were female (77%) with mean age of 41.7 years and duration of disease of 2.2 years. There was a significant reduction in the MRSS from 37.7 ± 4.08 to 29.1 ± 8.13 after 12 months of treatment (P = 0.009). Seven patients completed 18 months of CPM and had persistent reduction of the MRSS (mean MRSS = 26.4. P = 0.01). There was also a reduction of C-reactive protein (CRP) (initial mean CRP = 8.9 mg/dL) compared with values after 18 months of treatment (mean CRP = 3.09 mg/dL, P = 0.04). There were no infections, urinary disorders and/or important hematological problems throughout the treatment. CONCLUSION: The treatment with cyclophosphamide was effective and should be considered an alternative for the treatment of severe cutaneous involvement of SSc.

OBJETIVO: Analisar a percepção, hábitos e conhecimentos auditivos da população usuária do Sistema Único de Saúde das cidades do Vale do Aço. MÉTODOS: Participaram deste estudo, 255 indivíduos distribuídos por Unidades Básicas de Saúde, na faixa etária de 20 a 60 anos. Foram aplicados questionários com 14 perguntas sobre percepção, hábitos e conhecimentos auditivos. RESULTADOS: Em relação às questões de percepção auditiva, foram obtidos os seguintes resultados: 74,9% escutam bem; 63,1% têm dificuldade em acompanhar conversa; dor de ouvido (54,5%); zumbido (46,7%); plenitude auricular (41,2%). Com relação a hábitos dos entrevistados, constatou-se: utilizar objetos para limpar o ouvido (80,4%); ouvir som alto (31,4%); usar fones de ouvido (7,1%), julgar prejudicial ouvir som alto (84,7%); considerar saudável o uso do cotonete (71%); acreditar na cura da surdez (66,9%). Encontrou-se relação estatisticamente significativa entre percepção auditiva de não ouvir bem e dificuldade em acompanhar conversa (pPURPOSE: To analyze the perception, habits and knowledge about hearing of a population of Brazilian Unified Health System users at cities from the Vale do Aço region. METHODS: Two hundred and fifty five individuals distributed by Primary Care Units and with ages varying from 20 to 60 years participated on the study. They answered a questionnaire that contained 14 questions regarding perception, habits and knowledge about hearing. RESULTS: Regarding the questions about hearing perception, 74.9% of the subjects reported to have a good hearing; 63.1% have some difficulty accompanying conversations; 54.5% have earache, 46.7% have tinnitus, and 41.2% have auricular plenitude. Regarding habits, 80.4% of the subjects reportedly use objects to clean their ears, 31.4% hear loud music, 7.1% use earphones, 84.7% judge that high volumes are harmful; 71% consider the use of cotton swabs to be healthy, and 66.9% believe that deafness is treatable. Pesquisa descritiva-exploratória, de delineamento transversal, que teve como objetivo identificar o perfil dos motoristas caminhoneiros que trafegam na BR-381 quanto aos fatores de riscos modificáveis e não modificáveis para o desenvolvimento da hipertensão arterial. Utilizou-se instrumento para coleta e registro de dados demográficos, fatores de risco e níveis pressóricos de 257 motoristas; os dados foram tabulados e expressos em frequência absoluta e percentagem. A faixa etária variou entre 30 a 39 anos; 54,1% relataram possuir parentes diretos hipertensos; 49,4% e 23,7% relataram uso bebida alcoólica e tabaco, respectivamente. Em relação aos hábitos alimentares, 39,3% informaram ingerir alimentos processados e in natura e 13,2% alimentos fritos; 26,8% relataram acrescentar sal à alimentação depois de preparada. Os fatores de risco identificados podem favorecer o desenvolvimento da hipertensão arterial, fazendo-se necessária a realização de ações de saúde para esse grupo da população.

Objetivo: identificar as dificuldades encontradas pelo profissional de enfermagem no processo de humanização da assistência e descrever a importância da atuação do enfermeiro na assistência humanizada, sendo feito desta forma estaremos contribuindo para a sensibilização da sociedade a qual vivemos. Métodos: foi feita uma pesquisa exploratória descritiva bibliográfica com abordagem qualitativa realizada na Biblioteca Virtual de Saúde (LILACS e BDENF). Após a coleta de dados, foi realizada uma leitura exploratória, seletiva, crítica e análise temática. Resultados: emergiu as seguintes categorias: Dificuldades encontradas pelo profissional de enfermagem no processo de humanização da assistência e a importância da atuação do enfermeiro na humanização da assistência. Conclusão: concluímos, então, que o ato de humanizar é de extrema importância para recuperação do paciente, o qual necessita de atenção e carinho.

Glycogenic hepatopathy is a rare and underecognized complication in long-standing poorly controlled type 1 diabetes mellitus patients. This is a distinct entity from other causes of hepatomegaly and elevated liver enzymes in diabetics, such as nonalcoholic fatty liver disease. Glycogenic hepatopathy is characterized by the combination of poorly controlled diabetes, acute liver injury with marked elevation in serum aminotransferases, and the characteristic histological features on liver biopsy. It is important to distinguish this entity as it has the potential for resolution following improved glycemic control. In this report, we describe four cases of adult patients presenting elevated serum transaminases and hepatomegaly with a history of poorly controlled type I diabetes mellitus. One of the patients had also elevated amylase and lipase in the serum, without clinical or imagiologic evidence of acute pancreatitis. Liver biopsy was performed in all patients and revealed glycogenic hepatopathy. Clinician's awareness of glycogenic hepatopathy should prevent diagnostic delay or misdiagnosis and will provide better insight and management for this condition.

INTRODUÇÃO: Crianças com Síndrome de Down adquirem o sentar independente por volta dos 10 meses de idade, seguindo uma curva de desenvolvimento motor própria para essa síndrome. OBJETIVO: Esta pesquisa objetiva avaliar a eficácia do tratamento por meio do balanço para a aquisição do sentar independente em crianças com Síndrome de Down, sem o uso de quaisquer técnicas de fisioterapia convencional. METODOLOGIA: O estudo caracteriza-se como uma pesquisa descritiva do tipo estudo de caso, realizado no Núcleo de Assistência Integral ao Paciente Especial (NAIPE) de Joinville, SC. Os atendimentos foram individualizados, com duração de 30 minutos, nos quais a criança permanecia sentada num balanço composto por plataforma revestida com Etil vinil acetato (EVA®) colorido e texturizado, realizando-se deslocamentos lineares no sentido ântero-posterior. A amostra do estudo foi composta por três crianças com Síndrome de Down, com idade entre 6 e 7 meses, do sexo feminino, sem controle de tronco para a sedestação sem apoio. RESULTADOS: A média de sessões foi 15 (± 2). CONCLUSÃO: Observou-se que, quando estimuladas precocemente por meio do balanço, essas crianças adquiriram o sentar antes do tempo descrito pela literatura.
