2. Assessment of Serum Magnesium Levels in Ischemic Stroke Patients and Its Correlation with Severity of Neurological Disability: A Case Control Study.

Abstract

Introduction: World Health Organization clinically defines a stroke as ‘the rapid development of clinical signs and symptoms of a focal neurological disturbance lasting more than 24 hours or leading to death with no apparent cause other than vascular origin’. Hypomagnesaemia leads to neuromuscular hyperirritability, tremors, increased vascular resistance, coronary vasospasm and hypertension. Hypomagnesaemia triggers vasoconstriction enhancing vascular endothelial injury and hence leads to atherosclerosis. Subjects and Methods: It was a case control study, undertaken in the Department of Medicine in S Nijalingappa medical college and HSK Hospital during the period of July 2021–December 2022. 30 patients with acute ischemic cerebral stroke fulfilling the inclusion criteria were selected. 2 ml venous sample for serum magnesium level was taken within 24 hours. Magnesium levels in patients after acute cerebral ischemic stroke were documented in patients, receiving standard management protocol of ischemic stroke. The correlation co-efficient of Hypomagnesaemia with modified Rankin Scale Score was calculated. Results: The mean age of patients was 45-80 years, 36 (60 %) patients were male and 24 (40 %) were females and there was male preponderance. The mean serum magnesium levels were measured. The present study observed a statistically significant correlation between modified Rankin Scale score measured at three months and serum magnesium level. Conclusion: It was concluded through the results of this study that Hypomagnesaemia in the body can cause more severe ischemic stroke. [ABSTRACT FROM AUTHOR]

Abstract

Introduction: Pancytopenia refers to the combination of anaemia, leukopenia and thrombocytopenia. Causes may be due to bone marrow failure, bone marrow infiltration, ineffective haematopoiesis or peripheral pooling/ destruction. A bone marrow aspirate is usually required to establish the diagnosis. Aetiologies of pancytopenia vary from one geographical region to other. Aim: Study of pancytopenia in patients admitted to a tertiary care hospital in north Karnataka. Methods: This study was conducted at SNMC and HSK, Bagalkot. This study was prospective, observational undertaken for 6-month period between July 2016-January 2017. History, physical examination, and primary blood investigations were done in all patients. Selected patients were evaluated with bone marrow examination. Materials: A total of 69 human subjects were enrolled. A thorough history, clinical examination and blood investigations were carried out. Results: Dimorphic anaemia is common than megaloblastic anaemia. Among those subjected for bone marrow megaloblastic anaemia was commoner than dual deficiency bone marrow. Other causes of pancytopenia were malaria, dengue, enteric fever, and less common causes included sepsis, MDS, TB, HIV, SLE. Conclusion: Nutritional anaemia is commonest cause for pancytopenia. This may be due to megaloblastic anaemia or deficiency of iron/vitamin B12/folate combined

7. Study of depression in patients with diabetes compared to non diabetics among elderly population and its association with blood sugar, HbA1c values.

Abstract

INTRODUCTION: Depression is very frequently observed in clinical practice especially in elderly individuals. Its prevalence is always underestimated and it is often undertreated resulting in a lot of suffering. It is one of important conditions resulting in reduced quality of living of diabetic subjects. AIM: To compare prevalence of depression in elderly patients with diabetes with elderly subjects without diabetes and find out its relation with blood sugar and HbA1c values. MATERIALS AND METHODS: Elderly patients (>65yrs) with diabetes attending out-patient departments as well as admitted in KIMS hospital were studied. RESULTS AND DATA ANALYSIS: Our study involved 300 elderly subjects. This included 146 diabetics and 154 nondiabetic subjects. Presence of depression in each group was evaluated by Zung depression scale. Depression was found in significantly more number of diabetic subjects than non- diabetic subjects and depression scores showed significant correlation with PPBS values but only insignificant correlation with FBS and HbA1c values. CONCLUSION: Depression in elderly diabetic population is significantly more compared to non-diabetic elderly population. Depression severity scores correlate with post prandial blood sugar values, but not with fasting blood sugar or glycated hemoglobin values.
