This retrospective case series reports on French bulldogs diagnosed with patellar maltracking without patellar luxation. We describe clinical presentation, joint exploration, and surgical treatment with patellar groove replacement to alleviate maltracking in the first attempt. Three French bulldogs were included. Clinical presentation was similar in all cases, with intermittent skipping lameness of 1 hind limb, absence of patellar luxation, and patellar maltracking. Joint exploration revealed abnormal conflict between the patella and medial ridge of the trochlea, with secondary severe erosion of articular cartilages of both patella and medial ridge of the trochlea. Patellar groove replacement was completed without perioperative or postoperative complications. Follow-up results and owner satisfaction were excellent in all cases. Patellar maltracking should be included as a differential diagnosis for stifle lameness in French bulldogs. Patellar groove replacement should be considered to alleviate maltracking associated with secondary severe patellofemoral erosion. Key clinical message: Patellar maltracking seems to be a misdiagnosed component of patellar luxation in the dog, particularly in the French bulldog. Considering the prevalence of cranial cruciate ligament disease (CCLD) and concurrent medial patellar luxation (MPL) among dogs, and the concomitant bone deviations routinely identified, the aim of this study is to evaluate the efficacy of the combination between distal femoral osteotomy (DFO) and modified tibial plateau levelling osteotomy (mTPLO) for the treatment of MPL with distal femoral varus, tibial torsion, and CCLD. Dogs with concurrent MPL and CCLD that underwent DFO and mTPLO procedures were included. Subjective clinical evaluations of gait and pre-operative and immediate post-operative radiographic examinations were performed. Surgical data were documented. The anatomical lateral distal femoral angle (aLDFA), femoral varus angle, mechanical medial proximal tibial angle, and tibial plateau angle (TPA) were compared between the pre- and post-operative periods. A total of 12 dogs were included in this study. Closing-wedge DFO and mTPLO procedures were performed on 14 limbs. The mean pre-operative and post-operative aLDFA values were 104.8 ± 3.9° and 95.4 ± 2.4°, respectively. TPA values were 26.2 ± 3.7° pre-operatively and 6.3 ± 3.2° post-operatively. Bone healing was observed in 7.1 %, 71.3 %, and 85.5 % of patients at the first, second, and third follow-up, respectively. Lameness was resolved in 28.5 %, 78.5 %, and 100 % at 30, 60, and 90 days, respectively. No major peri- or post-operative complications were reported. The combination of mTPLO and DFO aligns the stifle extensor mechanism, promoting dynamic stabilization. Objective: To explain the relationship between cartilage erosion and medial patellar luxation (MPL) and to identify risk factors in dogs., Methods: A retrospective review was conducted on 90 dogs (103 stifles) surgically treated for MPL between January 2006 and March 2024. Data collected included signalment, side of operated stifle, patellar luxation grade, symptom duration, and lameness score. Cartilage erosion was evaluated for extent and location on the patella and femoral trochlea. Statistical analyses were conducted to identify risk factors., Results: The prevalence of cartilage erosion of the patella and femoral trochlea was 47.6% (49/103) and 54.4% (56/103), respectively, increasing with a higher grade of patellar luxation. Lesions were most prevalent in the distolateral patella and proximomedial trochlea, with generalized lesions more prevalent in grade IV. The extent of both lesions was significantly associated with age, patellar luxation grade, and symptom duration, while body weight significantly correlated only with the cartilage erosion of the patella. No significant correlation was observed with sex, side of operated stifle, or lameness score., Conclusions: Many patients with MPL exhibited cartilage erosion in the patellofemoral joint, likely due to biomechanical mechanisms. Surgery can be indicated for patients with MPL, as it may prevent cartilage erosion while improving patellofemoral alignment and gait. When selecting surgical candidates, it is important to consider risk factors, such as patellar luxation grade, body weight, age, and symptom duration., Clinical Relevance: Early surgical treatment is recommended, especially for dogs with higher body weight and higher grade of MPL, to prevent cartilage erosion and secondary osteoarthritis.

The aim of this study was to analyse a series of five cases of early surgical treatment in juvenile canine patients with high-grade medial patellar luxation. Canine patients, with immature skeleton, affected by grade III or IV medial patellar luxation and treated by multimodal surgical techniques were selected. Five dogs of different breeds, mean age 7.2 ± 3.0 months, met the inclusion criteria. Simultaneous distal femoral varus and external tibial torsion were identified in three of the five cases, whereas in the others, femoral varus with associated tibial valgus and isolated external tibial torsion were observed. Four of the five patients were treated surgically with closing wedge osteotomy of the distal femur, and three of them underwent tibial tuberosity transposition simultaneously. Only two animals required trochleoplasty. Mean time to beginning of weight bearing was 9.8 ± 5.5 days, whereas time to bone healing was 55 ± 24 days. Bone realignment of the extensor apparatus and return to limb support were achieved in all cases. Postoperative reintervention was needed in one patient; however, it was not linked to the initial surgery. Patients were followed up to 1 year postoperatively, with maintenance of extensor alignment and no late complications. Background: Various trochleoplasty techniques, including trochlear wedge recession (TWR) and trochlear block recession (TBR), are used to treat dogs with medial patellar luxation (MPL). However, the objective outcomes of these surgical procedures are underreported., Methods: Medical records were obtained for dogs weighing less than 10 kg that underwent either TWR or TBR and tibial tuberosity transposition to address grade I-III MPL. Long-term (at least 1 year after the last procedure) follow-up included orthopaedic and radiographic examinations, such as osteoarthritis score (OAS), ground reaction force (GRF) analysis and canine brief pain inventory (CBPI)., Results: Overall, 20 dogs (26 stifles) were followed up in the long term. Minor postoperative complications, medial patellar reluxation (MPR) and intermittent lameness occurred in 46.15%, 19.23% and 15% of the dogs, respectively. MPR occurred only in TWR-treated stifles, while mean OAS increased in all groups. Using the CBPI, the owners perceived an excellent or very good outcome in 95% of dogs., Limitations: The limitations of the study include its retrospective observational nature, a lack of randomisation and a small sample size., Conclusion: Surgical treatment resulted in a favourable outcome. GRF analysis could detect subtle differences in weight bearing in dogs treated for MPL, which might not be apparent clinically. There might be a higher risk for reluxation for TWR. Cranial cruciate ligament rupture (CCLR) is a common cause of stifle joint pain in dogs. This study assessed the short-term outcomes in the management of CCLR, with or without concurrent medial patellar luxation (MPL), in small-breed dogs, comparing surgical intervention using tibial plateau leveling osteotomy (TPLO) with non-surgical approaches. Dogs weighing less than 10 kg and suffering from CCLR were divided into two groups: surgical and non-surgical (nine stifles per group). Both groups followed the same rehabilitation protocol. Measurements were taken on days 0, 14, 28, 42, 56, 70, and 84. These measurements included the mean difference in orthopedic assessment score (diffOAS) and thigh muscle circumference (TMC) assessed by veterinarians, as well as the Canine Brief Pain Inventory score (CBPI) evaluated by the owners. Six stifles had concurrent MPL in the surgical group and five in the non-surgical group. No significant differences were observed in diffOAS (P=0.685), TMC (P=0.557), CBPI pain severity score (P=0.062), and CBPI pain interference score (P=0.29) between the two groups. However, both groups showed a significant decrease (P<0.05) in diffOAS and CBPI. The TMC remained relatively stable in the non-surgical group (P>0.05), but it significantly increased in the surgical group (P<0.05). Both groups demonstrated improved clinical outcomes in the short term, with no significant differences. However, cautious consideration should be given to a conservative treatment in small-breed dogs, particularly when considering the exclusive improvement observed in TMC within the TPLO group.

Objectives:  To describe the implant characteristics and surgical application of a custom-made trochlear ridge prosthesis (TRP) and to report clinical outcomes in dogs affected by patellar luxation treated with TRP., Study Design:  Dogs affected by patellar luxation underwent computed tomography. A specific canine bone anatomical replica, a cutting guide, and a TRP were designed and provided for surgery. Surgical records, clinical and radiographic reassessments, complications, pre- and postoperative lameness, type and degree of patellar luxation, and TRP and patellar position after surgery were reviewed. Clinical outcomes were defined as full, acceptable, or unacceptable function., Results:  The TRP was implanted in 60 femoral trochleae: 48 unilateral and 12 bilateral. Successful correction of patellar luxation was achieved in 59/60 cases. TRP was applied with other surgical techniques in 36/60 of the cases and as the only surgical procedure in 24/60 cases. Overall, three complications were observed: two minor and one major (patellar luxation recurrence). Neither implant loosening nor infection was observed. The mean radiographic follow-up was 3.8 months. At the time of the final follow-up, 57/60 cases were scored as fully functional., Conclusion:  The TRP application either alone or in combination with other surgical techniques allowed for correction of patellar luxation and improvement in preoperative lameness with nominal complications. A 12 yr old male castrated miniature Australian shepherd dog presented for surgical consultation of historical bilateral medial patellar luxations with a 3 mo history of an acute onset of a left pelvic limb lameness. Physical examination confirmed medial patellar luxations and a mass effect of the left stifle medially. Radiographs showed medial distension of the joint capsule by a soft tissue opacity. Fine-needle aspirate of the left stifle revealed a mesenchymal cell population. Left medial parapatellar stifle arthrotomy found a fatty mass, which was excised at its base. A benign fibrolipoma was diagnosed on histopathology, and the excision was expected to be curative. The owners reported immediate improvement of perceived comfort postoperatively. At 2 and 24 wk, the dog returned to a normal level of function. Objectives:  This study aims to report complications and long-term outcomes of combined tibial plateau leveling osteotomy and tibial tuberosity transposition (TPLO-TTT) for treatment of concurrent cranial cruciate ligament rupture and medial patellar luxation (MPL)., Study Design:  This is a retrospective study. Dogs that underwent TPLO-TTT for treatment of combined cranial cruciate ligament rupture and grade III or IV MPL were included. Signalment, fixation method, and complications were recorded. Long-term outcome assessment (minimum 1 year postoperatively) was performed through clinical and radiographic reexamination at the authors' institution., Results:  Twenty-four stifles (22 dogs) were included. Twenty-one stifles had a grade III MPL, and 3 stifles had grade IV MPL. Four major complications consisting of surgical site infection ( n  = 3) and recurrent grade II MPL ( n  = 1) were observed. Dogs with surgical site infections were successfully treated with implant removal and oral antibiotics. No additional surgery was performed for the patient with recurrent MPL as no signs of lameness were observed. Minor complications occurred in five cases. At long-term evaluation (median: 27 months; range: 12-67 months), 21/22 dogs were clinically sound and 23/24 stifles had a complete resolution of MPL., Conclusion:  TPLO-TTT can be considered as an effective surgical option to treat patients with concurrent cranial cruciate ligament rupture and MPL with a relatively low major complication rate. Patellar luxation (PL) is a common orthopaedic condition in dogs. This study aimed to evaluate the incidence and cause-specific mortality rate, age at diagnosis, and risk factors for medial PL (MPL), lateral PL (LPL), and bidirectional PL (BPL). Other diagnoses in dogs with PL were also explored. The study population included just over 600,000 dogs insured by Agria Pet Insurance in Sweden (2011-2016). There were 2726 dogs with PL. Medial patellar luxation affected 90 % of the dogs with PL, followed by LPL (5.9 %), BPL (2.4 %), and unspecified PL (1.6 %). The median age at first diagnosis during the study period was 2.8 years for MPL, 2.7 years for LPL, and 1.5 years for BPL. In total, 168 (6.2 %) of the dogs with PL had cruciate ligament rupture. There were substantial breed-specific differences in the risk of PL: almost all breeds at increased risk of MPL were small-sized, while several of the breeds at increased risk of LPL were large-sized. The breeds at high risk of BPL varied in size. Females had an increased risk of MPL (RR 1.2, 95 % CI: 1.1-1.3, p < 0.001) and a decreased risk of LPL (RR 0.72, 95 % CI: 0.51-1.0, p = 0.042) compared to males. In total, 116 dogs were euthanised due to PL and the breeds with the highest risk of PL-related euthanasia were the Pyrenean mountain dog, Dogue de Bordeaux, and German pinscher. Objective:  The aim of this study was to describe partial parasagittal patellectomy for the surgical management of patellar luxation, including surgical technique, proposed indications, complications, and outcomes in a cohort of dogs., Study Design:  This is a single-center retrospective clinical case series., Methods:  Medical records (2019-2020) of dogs presenting for patellar luxation were reviewed. Clinical signs, luxation direction and grade, preoperative imaging and intraoperative findings, and complications were retrospectively retrieved. Owners were contacted via questionnaire for medium- and long-term follow-up., Results:  A total of 24 stifles in 19 dogs met the inclusion criteria. Intraoperative minor complications occurred in four stifles. A single minor complication occurred postoperatively with no major complications. Increased thickness and radiographic attenuation of the patellar ligament were noted in 14 stifles at radiographic reassessment. Re-luxation was not encountered, with long-term owner-assessed outcome as good to excellent in all cases., Conclusion:  Partial parasagittal patellectomy, used in conjunction with block recession trochleoplasty and tibial tuberosity transposition, provided reliable resolution of patellar luxation in canine stifles with grade 2 or 3 luxation where width mismatch between the patella and the trochlea was noted intraoperatively. Objective:  The aim of this study was to describe the use of a procedure-specific fixation method for tibial tuberosity transposition and report clinical outcome., Study Design:  This is a multi-institutional case series, evaluating 37 cases that were treated surgically for medial patellar luxation (MPL) and in which the tibial tuberosity transposition (TTT) was performed using the Rapid Luxation Plating System (RLPS). Surgical technique, implants, clinical outcome, and complications are reported., Results:  Surgery was successfully performed in dogs weighing 2.5 to 36.2 kg. Postoperative minor complications occurred in 13 cases (35%) and major complications occurred in 3 cases (8%). No implant-related complications or tibial tuberosity avulsions or fractures were seen. Introduction: A common technique for the treatment of medial patellar luxation is the lateral transposition of the tibial tuberosity. Two variations of a new surgical method with a retention plate and fixation with a wire or a security plate, were tested against each other and against the existing fixation with Kirschner wires and a tension band by the number of reoperations. The new method using a retention plate and security plate showed the lowest rate of reoperations (5 %), followed by the new technique using retention plate and wire (14 %) and the existing fixation with Kirschner wires and a tension band (34 %). A possible explanation for the good result of the new method can be the fixation of the implants on the medial side of the tibia and the non-use of Kirschner wires.

Objective: To describe the dome trochleoplasty procedure and report the short-term outcomes and complications associated with a novel technique to correct patellar luxation and patella alta., Animals: 13 dogs (16 stifle joints) diagnosed with medial patellar luxation with concurrent patella alta in dogs > 20 kg., Clinical Presentation: Medical records of dogs weighing > 20 kg that underwent a dome trochleoplasty for correction of a medial luxating patella were prospectively evaluated. The procedure described involves an osteotomy of the femoral trochlea, which is then translated and/or rotated to correct patellar luxation. Clinical results were assessed using subjective lameness scoring, radiographic evaluation, and the Canine Brief Pain Inventory (CBPI) tool., Results: 16 stifle joints were included in this study. The overall complication rate was 50%. Major complications occurred in 43.8% of stifle joints due to pin migration and recurrent luxation of the patella. One dog experienced a catastrophic complication 4 months postoperatively. Uncomplicated osteotomy healing was present in 94% of dogs. The median initial lameness score was 2 (mean, 1.81; range, 0 to 4) and at the final recheck was 0 (mean, 0.31; range, 0 to 2). The CBPI scores were available for 50% of stifle joints. The median initial CBPI score was 45.5 (mean, 48.8; range, 32 to 74) and at the final recheck was 17.5 (mean, 20.5; range, 0 to 43)., Clinical Relevance: The dome trochleoplasty procedure offers an alternative technique for surgical correction of patellar luxation secondary to patella alta in large-breed dogs, but due to its higher complication and reluxation rates, it should be used cautiously and probably in combination with other corrective procedures, such as tibial tuberosity transposition, soft tissue imbrication, and/or soft tissue release rather than as a stand-alone procedure.

Objective:  The aim of this study was to determine the factors related to functional patella alta, with which the proximodistal patellar position exceeds the reference range of healthy small dogs proximally when the stifle is at full extension., Study Design:  Mediolateral-view radiographs of dogs weighing less than 15 kg were obtained and classified into the medial patellar luxation (MPL) and control groups. The reference range of the proximodistal patellar position was determined from the control group. In both groups, a patellar position exceeding this reference range proximally was considered functional patella alta. Multiple logistic regression analysis was performed to examine the factors related to functional patella alta. A receiver operating characteristic (ROC) curve was made for each factor., Results:  Overall, radiographs of 127 stifles of 75 dogs were obtained. Eleven stifles in the MPL group and one stifle in the control group were determined to be functional patella alta. The factors associated with functional patella alta included a greater full extension angle of the stifle joint, longer patellar ligament, and shorter femoral trochlear length. The full extension angle of the stifle joint had the biggest area under the ROC curve., Conclusion:  Mediolateral radiographs of the stifle joint in full extension are clinically important in dogs with MPL because some dogs might have a proximally positioned patella that is recognizable only when the stifles are extended., Competing Interests: None declared., (Thieme. Objective: To report a modification of the tibial tuberosity transposition (m-TTT) technique used to treat medial patellar luxation (MPL) in dogs, and report its complications., Study Design: Retrospective case series., Sample Population: Dogs (n = 235) undergoing MPL correction, using m-TTT (n = 300 stifles)., Methods: Medical records and client surveys were reviewed to determine complications associated with this technique and they were compared with previously reported complications using similar techniques., Results: Short-term minor complications included low-grade reluxation (11 stifles, 3.6%), incisional seroma (nine stifles, 3%), pin-associated swelling (seven stifles, 2.3%), patellar desmitis (six stifles, 2%), superficial incisional infection (four stifles, 1.3%), pin migration (three stifles, 1%), tibial tuberosity (TT) fracture (two stifles, 0.6%), tibial tuberosity displacement and patella alta (one stifle, 0.3%), pin-associated discomfort (one stifle, 0.3%), trochlear block fracture (one stifle, 0.3%). Short-term major complications included pin migration (three stifles, 1%), incisional infection (two stifles, 0.6%), tibial tuberosity fracture (two stifles, 0.6%), and high grade reluxation (two stifles, 0.6%). Long-term follow-up examination data were available for 109/300 (36.3%) stifles. One minor complication and four major complications were documented. All long-term complications were due to pin migration. The overall major complication rate was 4.3% (13/300 stifles), with a minor complication rate of 15% (46/300 stifles). Objective:  The main aim of this study was to determine the effective magnitude of proximal tibial segment medialization achievable during tibial plateau levelling and medialization osteotomy (TPLO-M) with Fixin 1.9-2.5 mm pre-contoured T plates with three different offsets., Methods:  In this in vitro study, 36 tibia bone models reconstructed using stereolithography from hindlimb CT scans of a 5 kg dog and a 10 kg dog without orthopaedic disease were used. TPLO-M was performed using plates with three different offsets (2, 4 and 6 mm). Post-osteotomy radiographic and bone models measurements were performed., Results:  Regardless of patient weight, the +4 mm offset plates provided a translation of 2.93mm (± 0.51) while the +6 mm offset plates provided a translation of 5.03mm (± 0.47). In the 5kg dog bone model group limited bone contact at the osteotomy site was documented when using the +6mm offset plate., Conclusion:  The +4mm and +6mm offset Fixin plates may be considered for TPLO-M in dogs weighing between 5 and 10 kg. Objective: To determine the signalment and musculoskeletal morphology of small-breed dogs affected by medial patellar luxation (MPL) grade IV based on the age of the CT scan., Animals: 40 small-breed dogs (54 limbs) with MPL grade IV., Procedures: Dogs that had undergone corrective surgery for MPL grade IV and had performed CT of the hind limb before surgery were included. Signalment (age, body weight, sex, laterality, and breed) and concomitant cranial cruciate ligament rupture (CrCLR) were recorded. Femoral inclination angle, anatomical lateral distal femoral angle (aLDFA), femoral torsion angle, quadriceps muscle length to femoral length ratio (QML/FL), and patellar ligament length to patellar length were obtained by CT images. The dogs were categorized into 2 groups based on their age at the time of the CT scan, the skeletally immature group and the skeletally matured group. Signalment and group were included in the multiple regression analysis to determine the factors associated with each measurement parameter. A logistic regression analysis was conducted to determine the risk of CrCL concomitant with age., Results: The multiple regression model demonstrated that the group was associated with the value of aLDFA and QML/FL. aLDFA was higher, and QML/FL was lower in group SI than in group SM. CrCLR was present in 5/54 limbs (9.2%), with a mean age of 70.8 months and it was associated with increasing age., Clinical Relevance: In Singleton's classification, dogs classified as grade IV can be categorized into 2 groups based on musculoskeletal morphology and pathophysiology: the skeletally immature and skeletally matured types.

Objective: To determine the quadriceps muscle length (QML) to femoral length (FL) ratio (QML/FL) and its associated factors in small breed dogs with medial patellar luxation (MPL) and to compare the differences in the QML/FL between MPL grades., Study Design: Retrospective study., Sample Population: Small breed dogs (<10 kg) with MPL: 78 dogs (134 limbs)., Methods: Medical records and computed tomography (CT) images from 2008-2020 were reviewed. Furthermore, age, bodyweight, sex, laterality, MPL grade, femoral inclination angle (FIA), femoral torsion angle (FTA), anatomical lateral distal femoral angle (aLDFA), and patellar ligament length to patellar length ratio were included in the regression analysis to determine the factors associated with QML/FL. Each measurement parameter was compared among the four MPL grade groups., Results: The final model demonstrated that the QML/FL increased with increasing age (p = .004) and decreased with increasing FTA and aLDFA (p = .015 and p < .001, respectively). Objectives: To determine cutoff values in small (SB) and medium/large (MLB) breed dogs with and without medial patellar luxation (MPL) for identifying abnormal femoral trochlea morphology., Study Design: Original research., Animals: A total of 80 computed tomographic (CT) scans from client-owned dogs METHODS: Four groups of 20 dogs were created: (1) control SB, (2) control MLB, (3) MPL-SB, and (4) MPL-MLB. Two authors measured the femoral trochlear groove angle (FTGA), femoral trochlear angle (FTA), and femoral trochlear ridge inclination angle (FTRIA) in two points with CT. ANOVA and ROC-analysis were tested to the control and MPL groups to assess sensitivity, specificity, and cutoff values. Statistical significance was set to p < .05. Intraclass correlation coefficients evaluated the inter-rater agreement., Results: FTGA (± SD) in control SB (128.8° ± 4.7°) and control MLB (119.2° ± 5.6°), was smaller (p < .0001) than in MPL-SB (139.4° ± 4.4°) and MPL-MLB (133.7° ± 5.1°). FTA and FTRIA were decreased (p = .12, p = .23) in MPL-SB (2.1° ± 6.8; -0.3° ± 3.3°) and MPL-MLB (3.8° ± 5.6°; 1.7° ± 4.5°) compared to control SB (0.2° ±4.1; -0.1° ± 2.6°) and control MLB (5.3° ± 2.8°; 3.1° ± 1.3°). Cutoff values for FTGA, FTA, and FTRIA were > 134°, < -5.9°, < -2 ° (SB), and > 128.3°, < -0.4°, < -0.4° (MLB). Sensitivity, specificity, and inter-rater agreement were superior for FTGA than FTA and FTRIA., Conclusions: Dogs without MPL had a deeper femoral trochlear groove than MPL dogs. Objective: To determine the influence of radiographic examination on the recommendations made at the time of planned re-evaluation of dogs after medial patellar luxation (MPL) surgery., Study Design: Retrospective multi-institutional case series., Animals: Client-owned dogs (N = 825) that underwent MPL surgery., Methods: Records of 10 referral institutions were searched for dogs that had been treated surgically for unilateral MPL and underwent a planned follow-up visit, including radiographs. The frequency of, and reasons for, changes in further recovery recommendations were investigated., Results: Follow up was performed at a median of 6 (range, 4-20) weeks postoperatively. Isolated radiographic abnormalities were identified in 3.3% (27/825) of dogs following MPL surgery and led to a change in recommendations in 3% (13/432) of dogs that were presented without owner or clinician concerns. Lameness, administration of analgesia at follow up, and history of unplanned visits prior to routine re-examination were associated with a change in postoperative plan (P < .001). In the absence of owner and clinician concerns, the odds of having a change in convalescence plans were not different, whether or not isolated radiographic abnormalities were present (P = .641)., Conclusion: Routine radiographs at follow up did not influence postoperative management of most dogs after MPL surgery in the absence of abnormalities on clinical history or orthopedic examination., Clinical Significance: Dogs that were presented for routine follow up after unilateral MPL surgery without owner concerns, lameness, analgesic treatment or a history of unplanned visits, and for which examination by a surgical specialist was unremarkable, were unlikely to benefit from radiographs., (© 2023 American College of Veterinary Surgeons.)

Objective:  The aim of this study was to identify the ideal anchor point for patellar anti-rotational sutures for adjunctive stabilization of medial patellar luxation in both small and large breed dogs., Study Design:  Retrospective radiographic survey was performed on 110 stifles from 101 dogs. Radiographs were grouped based on patient weight (≤15 kg; >15 kg) and diagnosis (medial patellar luxation, cranial cruciate ligament rupture, and normal joints). Radiographic measurements included: the proximal, middle, and distal points of the trochlear ridge, the caudal aspect of Blumensaat's line (roof of the intercondylar notch), the centre of the lateral fabella, as well as the "best-fit" centre of a circle overlying the trochlea. These landmark coordinates were used to calculate radii for comparison, and for scaling between joints., Results:  Use of the fabellar centre resulted in larger radii (corrected p <0.001) than those from the best-fit circle centre for all but one combination of patient group and trochlear end point locations. Using the best-fit circle centre, radius variation was less marked than with the fabellar centre. Significant differences in location for centres of the best-fit circle and fabella were seen across all patient categories (Pillai's trace p <0.001)., Conclusion:  The fabella is unlikely to be the best choice for anchoring a patellar anti-rotational suture. Objective: The aim of this study was to test the low sensitivity of the Allberg and Miles index to the stifle opening angle, evaluate the displacement of the patella after a Modified Maquet Technique using this index and assess the incidence of patellar luxation post-Modified Maquet Technique in dogs., Study Design:  Medical records were reviewed from 2012 to 2017. Allberg Miles index were determined for each stifle pre- and postoperatively, as well as the stifle joint opening of each case. Occurrence of patellar luxation was recorded., Results:  137 stifles on 116 dogs were reviewed. The stifle opening angle did not influence the Allberg Miles index. Pre- and postoperative index showed a distal displacement of the patella after a Modified Maquet Procedure, especially at 135° of stifle opening angle. Only 1/137 cases demonstrated patellar luxation after the surgery., Conclusion:  Based on our statistical analysis, we were able to conclude that within the maximum stifle opening angle range recorded in our series of cases; the Allberg Miles index variation was not significant. Objective: To document and compare preoperative and postoperative stance analysis measurements in animals with naturally occurring patellar luxation., Animals: 131 client-owned dogs surgically treated for naturally occurring unilateral or bilateral patella luxation between March 30, 2015, and February 12, 2020., Procedures: Weight bearing as a percent body weight on each limb was recorded with the use of a platform device for analyzing stance (PetSafe Stance Analyzer; LiteCure LLC, Companion Animal Health) preoperatively and postoperatively for all dogs. Signalment, limb affected, lameness grade, luxation direction, luxation grade, and surgical procedure were collected from patient records and assessed for the effects of these variables on weight bearing preoperatively or at the first or second postoperative recheck examination., Results: There were 61 males and 70 females, with a mean age and body weight of 4.62 years and 13.01 kg, included in the study. As age increased, body weight decreased in these dogs (P = .025). There was a statistically significant improvement in lameness after surgery (P = .008) at the second postoperative recheck examination. Lameness significantly decreased as postoperative time increased (P < .001, r = 0.503). As age increased, lameness at the initial visit decreased compared to younger dogs but not significantly (P = .062). There was no significant effect of preoperative luxation grade, luxation direction, surgical procedure, or sex when comparing initial lameness or lameness at recheck examination., Clinical Relevance: Surgical correction of patella luxation improves lameness as measured by postoperative stance analysis. Preoperative luxation grade or direction, surgical procedure performed, and sex of the animal did not significantly affect outcome in this group of dogs.

Objectives:  Medial patellar luxation (MPL) is the most common developmental cause of hindlimb lameness in cats. The association between femoral and tibial conformation and MPL measured on computed tomography (CT) has not been reported in cats. The aims were to report femoral and tibial conformation in cats with and without MPL and to report normal femoral and tibial angles., Methods:  Angle of inclination of femoral neck (AI), anatomical lateral distal femoral angle (aLDFA), femoral trochanteric angle (FCT), angle of anteversion of femoral neck (AA), distal and proximal anteversion angle (DAA/PAA), overall tibial valgus (TV), tibial torsion (TT), tibial tuberosity displacement (TTD) and trochlear depth:patellar thickness ratio (T:P) were measured by three observers on CT of cats with and without MPL. Comparisons were made between groups. Inter-observer intraclass correlation coefficient (ICC) was calculated., Results:  Sixteen cats were recruited: eight control and eight with MPL. The aLDFA, PAA, TT, TTD and T:P were significantly less in cats with high-grade MPL. The AI, FCT, AA, DAA and TV were not significantly different. A high correlation was shown with inter-observer ICC in 33.33% and good correlation in 26.67% when comparing measurements between observers., Clinical Significance:  This study suggests that cats with high-grade MPL have decreased TT, TTD and T:P and may require tibial tuberosity transposition and femoral trochleoplasty. The PAA, TT and aLDFA were decreased, although clinical significance may vary and these cats may not require correctional osteotomies. Study aims were to evaluate if medial patellar luxation clinical grades changed with oestrus status, and to determine interobserver agreement for different classification methods for grading patellar luxation. Intact female dogs were recruited for grading by 3 independent observers on 2 occasions (pro-oestrus/oestrus and dioestrus/anoestrus) using a published grading system. Observers were blinded to oestrus status. Oestrus status was confirmed by vaginal cytology. Observer agreement was determined using Fleiss' multirater kappa on the original grading scores, simplification to the Norwegian Kennel Club system, and further simplification to a binary suitable/not-suitable for breeding system. The exact McNemar's test was used to assess the effect of oestrus on classification. Of 70 dogs recruited, 53 underwent paired observations. Interobserver agreement was considered moderate to very good for the study sub-groups, with overall kappa values of 0.68 (95% CI 0.63-0.72), 0.79 (0.73-0.84) and 0.92 (0.85-0.99), and percentage agreements of 65%, 81% and 94%, for the original, simplified and binary classifications. Oestrus status did not have a significant effect on classification of breeding suitability. Clinicians and owners should not be concerned about the timing of patellar luxation grading in relation to oestrus. Objectives: To describe a computed tomographic (CT) methodology for planning the correction of femoral and tibial torsion and report the clinical outcomes after femoral (FDO) and tibial (TDO) detorsional osteotomy in dogs affected by torsion malalignment and patellar luxation (PL)., Study Design: Multicenter retrospective study., Animals: Eighteen client-owned dogs., Methods: Dogs underwent CT to measure femoral (FTA) and tibial torsion angle (TTA). Abnormal femoral external torsion was defined when FTA <20°, abnormal femoral internal torsion if FTA >35°; abnormal tibial external torsion was defined when TTA < -10°, and abnormal tibial internal torsion when TTA >2°. The cortical arch length (CAL) was measured with CT and used intraoperatively to determine the magnitude of correction. The medical records and radiographs were reviewed and used to report clinical and radiographic outcomes. Radiographs were reviewed to evaluate postoperative limb alignment, patellar position, and bone healing., Results: Twenty-two detorsional osteotomies were performed. Mean preoperative FTA was 14° for medial-PL and 45.2° for lateral-PL. Mean preoperative TTA was 11° for medial-PL. Physiological patellar tracking was restored in 22/22 of cases. CAL measurement allowed for correction of abnormal torsion in 19/22 of cases. Seventeen out 18 dogs had full or acceptable functional outcome. The median radiographic follow-up was 3 months. Major complications occurred in 2/22 cases, which suffered an iatrogenic abnormal femoral internal torsion and a persistent hindlimb lameness., Conclusions: CAL can be measured with CT and used intraoperatively to guide the correction of abnormal torsion in dogs., Clinical Relevance: Abnormal femoral and tibial torsion are predisposing factors for PL. Trochleoplasty is often performed in dogs with medial patellar luxation (MPL); however, the current guidelines on when to perform a trochleoplasty in dogs are vague. The sulcus angle (SA) is used to assess the femoral trochlear morphology in humans. The aim of this study is to describe a method to measure the SA and other parameters of trochlea morphology in dogs using computed tomography. First, we searched for a suitable measuring location for the SA. Transverse images of the femurs were obtained as perpendicular planes to the tangent of the femoral trochlea which was 0 to 60 degrees (every 5 degrees) to the anatomical axis of the femur. The deepest point of the femoral trochlea was found in the transverse images perpendicular to the tangent of the femoral trochlea which was at 15 degrees to the anatomical axis of the femur. The SA and the other parameters of femoral trochlea morphology were measured at the deepest point of the femoral trochlea. The SA of the stifle joints with grade 3 and 4 MPL was significantly higher than the SA of stifle joints not affected by MPL. There was no significant difference in the SA between dogs affected by grade 1 and 2 MPL and dogs not affected by MPL. Background: Medial patellar luxation (MPL) is a common orthopedic condition in dogs. After surgical correction, the recurrent rates appear to be higher and outcomes worse in grade IV than in lower grade MPL., Aim: To describe the outcomes of surgical correction of canine grade IV MPL., Methods: This retrospective study analyzed the medical records of 16 dogs (11 females, 5 males; mean age = 5.05 years) with 22 stifle joints (8 left and 14 right) that underwent MPL surgery at Rakuno Gakuen University between May 2015 and October 2020. The procedures included capsulorrhaphy, trochlear block recession, tibial tuberosity transposition, medial retinacular release, and lateral fabellotibial suturing. The types of postsurgical complications, including a recurrence of MPL, were recorded., Results: Twelve of the 16 (75 %) dogs had concurrent contralateral MPL, and 2 dogs (12.5%) had concurrent cranial cruciate ligament rupture. The mean follow-up was 12.1 months. No lameness was observed. The outcomes of surgery for grade IV MPL were not associated with bilateral surgery, age, sex, body weight, or the preoperative quadriceps angle. Although none of the dogs showed any major or catastrophic complications following surgery, postoperative minor and major complications were observed in 8 of the 22 stifle joints (36.4%)., Conclusion: Grade IV MPL is a complex orthopedic disease in dogs, which requires surgical patellar realignment to improve limb function. The surgical techniques used in this study were associated with a good prognosis without a surgical revision.

Objective:  The aim of this study was to determine the degree of bone deformities and hindlimb postural abnormalities in a standing position in awake Toy poodles with and without grade 2 medial patellar luxation (MPL) using high speed 320-row computed tomography (CT)., Methods:  The limbs with grade 2 MPL (MPL-G2 group) and without any orthopaedic disorders (control group) were imaged in a standing position, without sedation or anaesthesia, using CT. In MPL-G2 group, images were obtained when the patella was luxated (G2-L group) and reduced (non-luxation, G2-NL group). Bone morphologies of the femur and tibia were quantified three-dimensionally. Hindlimb standing posture was evaluated by measuring femoral rotation and abduction angles, tibial rotation angle, metatarsal rotation angle, foot rotation angle, angle between the femoral anatomical axis and the mechanical axis of hindlimb and stifle joint line convergence angle., Results:  There were no significant differences in bone morphologic parameters between the MPL-G2 group (5 limbs) and the control group (6 limbs). In the G2-NL group, there were no significant hindlimb postural abnormalities. In contrast, in the G2-L group, significant hindlimb postural abnormalities including external rotation of femur, internal rotation of tibia and foot, external rotation of tarsal joint, large stifle joint convergence angle, genu varum and toe-in standing were observed., Conclusion:  Dogs with grade 2 MPL have no bone deformities but show abnormal standing posture when the patella is luxated., Competing Interests: None declared., (The Author(s). Aim: To evaluate the clinical and radiographic outcome of pes varus deformity correction in dachshunds managed with acute medial opening wedge osteotomy of the distal tibia and stabilised with a mini hybrid external skeletal fixator (HESF)., Methods: Surgical correction involved a transverse osteotomy over the premeasured location at the distal tibia and application of a IMEX mini HESF. All corrections were evaluated using centre of rotation and angulation methodology., Results: Medical records and radiographies of 20 dachshunds (28 pes varus corrections) were reviewed. All osteotomies healed and fixators were removed between 6 and 12 weeks. Lameness resolved in 18 dachshunds (90%) and significantly improved in two dachshunds (10%). Lateral patella luxation (LPL) was detected in 11/28 (39.2%) of the involved pelvic limbs, all of which resolved following pes varus correction. Mean frontal plane alignment (FPA) of the normal and abnormal tibiae were 12.3° valgus (range: 4°-18°) and 25° Varus (range: 16°-41°) respectively. Angular correction ranged between 30° and 50° (Mean: 39°) and the mean post-operative FPA was 13° valgus (range: 5°-21°)., Clinical Significance: Pes varus deformity in dachshunds can be corrected by medial opening wedge osteotomy of the distal tibia stabilised by HESF. Single-session bilateral pes varus corrections can also be performed with minimal morbidity. Objectives:  The purpose of this study was to compare the load at failure, stiffness and mode of failure between three types of tibial tuberosity transposition fixation techniques: (a) pin and figure-8 tension band wire (Pin-TBW), (b) locking plate with pin and a tension band wire (Plate-Pin-TBW) and (c) locking plate with a pin (Plate-Pin)., Methods:  Six pairs of raccoon dog cadaveric tibiae were tested in Phase I Pin-TBW versus Plate-Pin-TBW and seven pairs in Phase II Plate-Pin-TBW versus Plate-Pin. One limb of each pair was randomly assigned to one of two groups for each phase. A tensile force was applied to the patellar ligament until construct failure., Results:  Pin-TBW (342N ± 54.7N) failed at a lower load than Plate-Pin-TBW (469N ± 77.3N), p  = 0.00748, with all Pin-TBW failing by fracture and the majority of Plate-Pin-TBW failing by rupture of patellar ligament. Pin-TBW group Phase I, normalized with Plate-Pin-TBW Phase I, failed at a lower load than Plate-Pin group Phase II, normalized with Plate-Pin-TBW Phase II, p  = 0.00467. The aim of this study was to evaluate the transposition distance in in vitro canine medial patellar luxation (MPL) models of different severity grades using a single radiograph of the stifle joint. Each MPL grade model was generated with manipulation and evaluated by palpating the stability of the patella. An anterior-posterior radiograph of the entire hind limb with extended hip and stifle joints, including the iliopubic eminence, was used for all measurements. The quadriceps angles and the percentage of the transposition distance to the maximum width of the proximal tibia were measured. Our results demonstrated that when the tibial tuberosity transposition procedure is performed in a clinical setting, the required distance to correct the abnormal direction of the extensor mechanism in the canine stifle joint was greater in severe grade MPL than in mild grade MPL., (Copyright © 2022 Elsevier Inc. Objective:  The aim of this study was to evaluate the bone deformities of the distal femur of Toy Poodles with medial patellar luxation (MPL) using computed tomography., Methods:  Computed tomography data from 37 stifles of Toy Poodles (16 normal, 10 grade 2 MPL and 11 grade 4 MPL) were analysed. For trochlear morphology, sulcus angle, medial or lateral trochlear inclination and trochlear angle were compared. For condylar morphology, distance between the apex of the medial or lateral trochlear ridge and the most caudal point of the medial or lateral femoral condyle (MC or LC), the length of the cranial/middle (anterior) part (MC(a) or LC(a')) and caudal (posterior) part (MC(p) or LC(p')) of distal femur and medial or lateral condyle width (MCW or LCW) were measured, and the ratios of medial-to-lateral condyles and anterior-posterior parts were compared., Results:  Sulcus angle was significantly higher, and medial trochlear inclination was significantly lower in grade 4 MPL group than other groups. Lateral trochlear inclination was not significantly different among 3 groups. Trochlear angles at proximal trochlear groove were significantly lower in grade 4 MPL group than other groups. Objective: To assess the role of the proximodistal and caudocranial relative position of the patellar ligament insertion on the tibia and patellar ligament length-to-patellar length ratio (PLL:PL) in small-breed dogs with and without grade II medial patellar luxation (MPL)., Study Design: Retrospective study., Sample Population: Dogs weighing ≤15 kg, including 43 stifles with MPL and 34 control stifles., Methods: The proximodistal and caudocranial relative position of the patellar ligament insertion was determined as a ratio using the vertical (VTT) and horizontal distance (HTT) between the tibial tuberosity insertion and the tibial plateau divided by the tibial plateau length (TPL). In addition, PLL:PL and tibial plateau angle (TPA) were determined., Results: The VTT:TPL ratio was lower in affected stifles (95% CI: 0.86-0.94) than in the control group (0.93-1.01; p = .01). No other difference was identified between affected and normal stifles., Conclusion: The only difference identified in this study consisted of a more proximal position of the patellar ligament insertion in the stifles of small-breed dogs with grade II MPL., Clinical Significance: The more proximal position of the patellar ligament insertion will result in a more proximal position of the patella in the trochlear groove and may contribute to the development of MPL. Objective:  The aim of this study was to describe an alternative method for trochleoplasty, semi-cylindrical recession trochleoplasty (SCRT), and compare it to trochlear block recession (TBR) in regard to recessed trochlear depth, patellar depth, patellar articular contact with the trochlea, recessed trochlear surface area, procedure time, complications and functional outcome., Study Design:  Ten dogs with bilateral grade II-III medial patellar luxations underwent bilateral simultaneous correction surgery including tibial tuberosity transposition, lateral imbrication and trochleoplasty with one stifle each undergoing TBR and SCRT. Patients received pre-and postoperative stifle computed tomography scans and pre- and 8-week postoperative pressure platform analysis and physical examinations. Recessed trochlear depth, patellar depth, trochlear surface area, patellar articular contact, procedure time, complications and functional outcomes as measured by gait analysis were compared between the two techniques., Results:  There was no significant difference in measured outcome variables between techniques at any time point. At the time of the 8-week reassessment, no patella had reluxated., Conclusion:  Short-term follow-up shows similar functional outcomes between both techniques regarding rate of reluxation and limb function. The SCRT was subjectively easier to perform than TBR, particularly in small patients. Background: Patellar luxation (PL) is a common orthopedic affection among farm and pet animals with mostly congenital (environmental and/or genetic) background., Aim: We report here the first observation of lateral PL in Hejazi goats bred in Libya., Methods: Five Hejazi goats aged between 4 months and 2 years with severe hind limb lameness were admitted to Al-Sorouh veterinary clinic in Tripoli during the period from 2016 to 2018. The goats were thoroughly examined clinically and radiographically. Two goats were surgically treated, and the other three cases were not because of either the cost limitation or expected poor prognosis. The surgical intervention involved femoral trochlear sulcoplasty, medial joint capsule imbrication, and tibial tuberosity transposition., Results: The clinical examination showed grade III-IV lateral PL. Radiologically, there were unilateral or bilateral, ventrocaudal, and dorsal PLs. Two cases were referred to surgical correction. One case almost restored the normal movement of stifle joint together with a good general status 1 year postsurgery. However, the surgical treatment was not effective in correcting the luxated patella in the second case., Conclusion: Lateral PL is common among orthopedic affections in Hejazi goats in Libya, and its surgical treatment provided a quite convenient approach. Quadriceps contracture (QC) is reported in dogs mainly as a complication of trauma or parasitic infection. QC causes progressive hind limb deviation, muscular hypotrophy and degenerative joint disease and, in puppies, bone hypoplasia. The aim of this clinical case series is to describe the radiographic and computed tomographic (CT) changes in hind limb ossification centres in thirteen 55 to 57-days-old Doberman Pinscher related littermates induced by QC after repeated intramuscular injections. The presence, size, and shape of ossification centres of affected and unaffected hind limbs were compared. Affected limbs were hyperextended and externally rotated, with genu recurvatum and proximo-medial patellar luxation. QC had no influence on the time of appearance of ossification centres however, it was associated with femoral head flattening, hip subluxation, flattening of the femoral distal epiphysis. The tibial plateau was tilted caudoproximally-craniodistally and wedged into the growth plate. Thirty-two out of fifty-five ossification centres (including diaphyseal and epiphyseal centres, such as femoral head and tibial plateau) were significantly smaller in affected limbs (p < 0.05). Lack of weight-bearing could account for the smaller size of ossification centres in affected tarsi and metatarsi. Progressive limb hyperextension and external rotation might have induced gradual loading withdrawal on the medial aspect of the foot justifying the reduced size observed only for the medial ossification centres of the digits of affected limbs. Background: French bulldogs exhibit significantly larger femoral external rotation and abduction than other breeds. We were curious as to whether this peculiar leg kinematic affects patellar motion and/or might induce medial patellar subluxation (MPSL) or medial patellar permanent luxation (MPPL). We hypothesized that the more abducted leg posture during stance causes an unusual medial pull direction of the rectus femoris muscle during stance, and that this may facilitate the occurrence of MPSL or even MPPL during locomotion. To test our hypothesis, we analyzed existing stifle-joint X-ray-sequences collected during the treadmill walk and trot of seven adult female French bulldogs. We estimated 3D-patellar kinematics using Scientific Rotoscoping., Results: The three-dimensional motion of the patella comprises rotations and translations. From the seven dogs analyzed, three exhibited MPSL and one MPPL during the gait cycle. Medial patellar luxation (MPL) occurred mostly around toe-off in both gaits studied. Patellar position was generally not gait-related at the analyzed timepoints. In dogs with MPL, the patella was placed significantly more distally (p = 0.037) at touch-down (TD) and at midswing (p = 0.024), and significantly more medial at midswing (p = 0.045) compared to dogs without MPL., Conclusions: Medial patellar luxation seems to be the consequence of the far from parasagittal position of the stifle joint during stance due to a broad trunk, and a wide pelvis. This peculiar leg orientation leads to a medial sideway pull caused by the rectus femoris muscle and the quadriceps femoris and may initiate plastic deformation of the growing femur and tibia. Thus, a way to avoid MPL could be to control breeding by selecting dogs with lean bodies and narrow pelvis. Actual breeding control programs based on the orthopedic examination are susceptible to errors. Systematic errors arise from the fact that the grading system is highly dependent on the dog's condition and the veterinarians' ability to perform the palpation on the stifle. Based on our results, the position of the patella at TD, or even perhaps during stand might offer a possibility of an objective radioscopic diagnostic of the MPL.

Objective: To report the preoperative evaluation, treatment with transarticular elastic external skeletal fixation (ESF), and outcome of a dog with bilateral medial patellar luxation (MPL) and stifle rotational deformity., Animal: One nonambulatory, 2.5-month-old, 7.5-kg mixed-breed intact female dog., Study Design: Case report METHODS: Complex pelvic limb deformities were assessed with physical examination and computed tomography (CT) and characterized as bilateral grade 4 MPL and 90° internal rotation of the tibia in relation to the femur. Three-dimensional imaging and bone models were used to quantify the deformity and rehearse surgical correction. Transarticular elastic chains between ESF pins in the femur and tibia were used to gradually correct stifle rotational malalignment on each pelvic limb. Soft tissue releases, imbrication, and a transphyseal staple were used to correct the patellar luxation (PL) and femoral varus. The dog was reevaluated for 1 year postoperatively., Results: Short- and long-term management included intensive physical rehabilitation to increase pelvic limb strength and function. Surgical treatment resulted in resolution of the PL and neutral stifle alignment. The dog was able to ambulate with persistent decreased range of motion of the stifle., Conclusion: Transarticular elastic chains and femoral physeal stapling improved the function of a dog with severe bone deformities and PL in a growing dog., (© 2020 American College of Veterinary Surgeons.)

Objective:  The aim of this study was to investigate the prevalence of cranial cruciate ligament rupture and patellar luxation and the associated risk factors in dogs., Materials and Methods:  A total of 13,072 clinical records of dogs were reviewed from School Hospital (Faculty of Veterinary Sciences, National University of La Plata). Data of age, breed, sex, body weight, patellar luxation and cranial cruciate ligament rupture condition were registered. Chi-squared and Fisher's exact tests were used to compare the prevalence of cranial cruciate ligament rupture and patellar luxation with the variables and then univariable logistic regression was used to evaluate the risk of having cranial cruciate ligament rupture and patellar luxation. Multivariable logistic regression was used including all variables to assess the odds of having patellar luxation and cranial cruciate ligament rupture., Results:  Of 13,072 patients treated, 72 and 51 had cranial cruciate ligament rupture and patellar luxation respectively. Sex was not a major risk factor for either condition. Adult (odds ratio [OR] = 8.2) and senior (OR = 4.3) patients had increased risk of having cranial cruciate ligament rupture, while for patellar luxation age was not a risk factor. Groups 2, 3 and 8 were more likely to have cranial cruciate ligament rupture (OR = 5.5, OR = 9.1 and OR = 2.6), and group 11 had lower risk of having patellar luxation (OR = 0.08). Maxi (OR = 2.4) and giant (OR = 6.0) breeds had higher risk of having cranial cruciate ligament rupture, and medium and maxi breeds had higher risk of patellar luxation (OR = 0.05 and OR = 0.3). Multivariate OR test confirmed that age (adult), body size (giant and maxi) and breed group (Group 3) were significantly associated with having cranial cruciate ligament rupture, and age was associated with having patellar luxation., Clinical Significance:  This is the first epidemiological study of cranial cruciate ligament rupture and patellar luxation in dogs from School Hospital (Faculty of Veterinary Sciences, National University of La Plata). Background: Femoral varus deformities complicating the realignment of the quadriceps muscles are frequently associated with medial patellar luxation (MPL) in dogs. Therefore, distal femoral osteotomy (DFO) is recommended in dogs affected with severe MPL and a distal femoral varus deformity. The presence of an anatomic lateral distal femoral angle (aLDFA) of ≥ 102° has been anecdotally recommended as an indication for performing corrective DFO in large-breed dogs. However, the effect of a femoral varus deformity on MPL has not been scientifically evaluated. We aimed to evaluate the influence of a femoral varus deformity on MPL using a finite element method based computer model. Three-dimensionally reconstructed computed tomographic images of a normal femur from a Beagle dog were deformed using meshing software to create distal varus deformities. A total of thirteen aLDFAs, including 95°, 98° and 100°-110°, were simulated. The patellar positions and reaction force between the patella and trochlear grooves were calculated for all finite element models under constant rectus femoris muscle activation., Results: The patella was displaced medially from the trochlear groove at an aLDFA of ≥103°. With an aLDFA of 103° to 110°, the reaction force was equal to zero and then decreased to negative values during the simulation, while other models with aLDFAs of 95°, 98°, and 100°-102° had positive reaction force values. The patella began to luxate at 24.90 seconds (sec) with an aLDFA of 103°, 19.80 sec with an aLDFA of 104°, 21.40 sec with an aLDFA of 105°, 20.10 sec with an aLDFA of 106°, 18.60 sec with an aLDFA of 107°, 15.30 sec with an aLDFA of 108°, 16.60 sec with an aLDFA of 109°, and 11.90 sec with an aLDFA of 110°., Conclusion: Severe distal femoral varus with an aLDFA of ≥103° caused MPL when other anatomical factors were controlled. Thissimplified computer model provides complementary information to anecdotal cutoffs for DFO, hence it should be applied to clinical patients with caution.

Objective: To quantify changes in the patellar tendon length following surgical correction of medial patellar luxation in dogs and evaluate potential risk factors associated with patellar tendon elongation., Study Design: Retrospective case series (n = 50)., Methods: Dogs that underwent surgery for medial patellar luxation correction and had 2-3 months follow up were included. Digital radiographs were utilized to quantify the patellar tendon length to patellar length ratio at various follow-up points. Odds ratio comparisons between potential risk factors associated with changes in patellar tendon length were performed., Results: Post-operative patellar tendon lengthening of ≥ 5% was observed in 20% of stifles and post-operative patellar tendon shortening of ≥ 5% was observed in 22% of stifles at the 2-3 month follow up period. The risk factors including age, body weight, trochleoplasty and grade of medial patellar luxation were not significantly associated with risk of patellar tendon elongation. Patellar tendon lengthening was not associated with recurrence of luxation., Conclusion: Patellar tendon lengthening and shortening can be observed in dogs following common medial patellar luxation corrective procedures in the short term follow up period. Objective:  The aim of this study was to describe a new surgical method of segmental femoral ostectomy for the reconstruction of the femoropatellar joint (FPJ) in dogs with medial patellar luxation (MPL) grade IV., Study Design:  The medical records of six dogs that underwent segmental femoral ostectomy to reconstruct the FPJ for MPL grade IV were retrospectively reviewed. The data included breed, age, body weight, comorbidities, history of orthopaedic abnormalities, pre- and postoperative radiographic findings, morphological indicators measured on three-dimensional computed tomographic multiplanar reconstruction images, surgical methods and postoperative complications., Outcomes:  Eleven hindlimbs with MPL grade IV in six dogs underwent segmental femoral ostectomy to reconstruct the FPJ with the four basic surgical procedures. Six stifle joints with femoral varus deformity were treated by closing wedge ostectomy and segmental ostectomy, while five stifle joints were treated by segmental ostectomy alone. The mean resected femoral length was 11.7 mm. The mean resected femoral length relative to the preoperative femoral length was 14.3%. The stability and congruity of the FPJ was markedly improved, with an increased range of motion in all dogs compared with their preoperative status., Conclusion:  Segmental femoral ostectomy was a useful treatment method for MPL grade IV with shortened quadriceps muscle length relative to the femoral length.
