6. The Current State of Non-Invasive Measurement of Intracranial Pressure in Patients with Craniosynostosis: A Systematic Review.

Abstract

Introduction: Despite being invasive, direct measurements remain the gold standard to measure intra-cranial pressure (ICP) in patients with craniosynostosis. However, there has been persistent effort to develop non-invasive modalities to measure ICP, possibly avoiding some of the risks of direct measurements. Here, we conduct a systematic review of the evidence behind various non-invasive modalities to monitor ICP in patients with craniosynostosis. Methods: A systematic review was conducted using PubMed, Cochrane, and Web of Science databases to identify studies describing the use of non-invasive ICP measurements in patients with craniosynostosis. Studies were included if they assessed a non-invasive method of ICP monitoring against a direct/invasive ICP monitoring technique in patients with craniosynostosis. Non-English and non-human studies were excluded. Results: A total of 735 studies were screened, of which 52 were included in the study. Nine methods of non-invasive ICP measurement were identified, with varying sensitivities and specificities in detecting elevated ICP. Specifically, optical coherence tomography (OCT), and ocular ultrasonography demonstrated ability to accurately measure ICP when compared to direct measurements. Conclusion: Here, we present the first systemic-review of the current literature surrounding non-invasive modalities to measure ICP in patients with craniosynostosis. While direct measurement remains the gold-standard, multiple reviewed modalities have shown promise in accurately measuring ICP. Of these, OCT has the most rigorous evidence supporting its use. Ocular sonography has also shown promise, albeit without as robust evidence supporting its use. 18. Patient Perceptions of Cancer and Reconstructive Care During the COVID-19 Pandemic

Abstract

Background:The Coronavirus-2019 (COVID-19) pandemic profoundly impacted care for breast cancer patients. Oncologic and reconstructive surgeries were delayed as hospitals attempted to preserve personal protective equipment and bed capacity. Little is known about how this unprecedented time has affected breast cancer patients’ mental health and perceptions regarding their care. Methods:A survey was sent to surgical oncology and plastic surgery patients who received breast cancer care between March and May 2020, during which our institution suspended elective surgery. The survey questions included patient demographics, as well as questions focused on worries related to COVID-19, individualized cancer and reconstructive care, and patient satisfaction with telemedicine. Descriptive statistics were used to assess patient responses. Results: 56 breast cancer patients completed the survey, which reflected a 25% response rate. A majority of patients expressed moderate concern about contracting COVID-19, accessing high-quality cancer care (78%), and receiving timely surgical care (68%). Only 43% of patients reported delays in their surgical cancer and reconstructive care, when in actuality 57% of patients experienced surgical delays. Overall, patients were satisfied with telemedicine and did not feel it negatively impacted their care (69%). Conclusions:The COVID-19 pandemic has led to disruptions of routine oncologic and reconstructive care for breast cancer patients. Patients were concerned about receiving high-quality cancer and reconstructive care, and the results of this study uncovered gaps in patient–physician communication. The implementation of telemedicine was received positively. These data can be used to improve future health system practices as the medical community faces new potential shutdowns of surgical services.

Abstract

Aim: The aim of the present study was to assess the Correlation between Neutrophil Lymphocytes Ratio (NLR) and Severity, Progression & Outcome of Covid-19 disease. Methods: The Prospective, Cross-Sectional, Analytical study was conducted at OPD/IPD patients of Geetanjali Medical College and Hospital, Udaipur. Patients with COVID-19 RTPCR positive of OPD/IPD at Geetanjali Medical College and Hospital, Udaipur were included. For the study, 103 patients of Covid-19 Positive were considered. Results: The study was carried out in 103 COVID patients. Out of total patients, 56 i.e. 54.37% were of age group 50-69 and 28(27.18%) of age 30-49 years.15.53% patients were of age more and 70 years and only 2.91% patients were observed under age of 30 years. Out of 103 patients, 71(68.93%) were males and 32(31.07%) were females. Regarding place of admission, 59(57.28%) were in ward and 44 (42.72%) were in ICU. Out of 103 patients, 69 (66.99%) patients had moderate CT severity score, 28(27.18%) had mild CT severity score and 6 (5.83%) had severe CT score. Fever, cough, shortness of breath and weakness were found to be the most common symptoms. Sore throat, headache, vomiting, diarrhea, body ache, cold, chest pain, abdomen pain and loss of taste were also seen in some cases. Conclusion: It was concluded that Neutrophil-Lymphocytes Ratio (NLR) has significant correlation with Clinical profile, Progression of disease and Outcome in Covid-19 and Neutrophil Lymphocytes Ratio (NLR) also had significant correlation with Biochemical markers, Radiological findings and Co-morbidities.
