There is emerging evidence that the cardiac interatrial septum has an important role as a thromboembolic source for ischemic strokes. There is little consensus on treatment of patients with different cardiac interatrial morphologies or pathologies who have had stroke. In this paper, we summarize the important background, diagnostic, and treatment considerations for this patient population as presented during the Federation of American Societies for Experimental Biology (FASEB) Virtual Catalytic Conference on the Cardiac Interatrial Septum and Stroke Risk, held on December 7, 2022. During this conference, many aspects of the cardiac interatrial septum were discussed. Among these were the embryogenesis of the interatrial septum and development of anatomic variants such as patent foramen ovale and left atrial septal pouch. Also addressed were various mechanisms of injury such as shunting physiologies and the consequences that can result from anatomic variants, as well as imaging considerations in echocardiography, computed tomography, and magnetic resonance imaging. Treatment options including anticoagulation and closure were addressed, as well as an in-depth discussion on whether the left atrial septal pouch is a stroke risk factor. These issues were discussed and debated by multiple experts from neurology, cardiology, and radiology.

22. Glycerophospholipid oxidation and production of aldehydes in oesophageal adenocarcinoma

- Abstract
Oesophageal adenocarcinoma (OAC) has an unmet clinical need, with five year survival in the UK remaining at 15%. There has been little improvement with advances in surgical practice or systemic chemotherapeutic regimens. Early diagnosis holds the key to radical treatment, the clinical utility of breath testing has highlighted aldehydes as a potential early marker of cancer. The emerging field of lipidomics has identified variations in lipid composition between cancer and benign tissue. These observed changes have highlighted phospholipids as particularly important class responsible for structural membrane stability, cell signalling and replication. In this research, multiple mass spectrometry techniques were implemented to identify and correlate lipid abundance with increased aldehyde quantitation. Desorption Electrospray Ionisation- Mass Spectrometry (DESI-MS) was utilised for lipid profiling in oesophageal adenocarcinoma tissue to reveal a prevalence of Phosphatidic acids (PA) and Phosphatidylglycerol (PG) species. Comprehensive bioinformatics analysis highlighted the PG pathway with significantly dysregulation and positive phenotype to PG production. The investigation of aldehydes was performed in vivo by lipid oxidation and corroborated in OAC tissue by a targeted Liquid Chromatography mass spectrometry (LC-MS) method. This identified medium and long chain aldehydes (Pentanal, Nonanal, Un-decanal) at particularly increased concentration. To investigate the lipid product correlation, the chemistry of lipid oxidation was defined and characterised. To explored the origin of the increased PA and PG a targeted LC-MS method was created and patient tissue and surface mucus samples were collected at paired sites. The analysis confirmed a relative increase of PAs and PGs in OAC tissue and mucus of representative intensities suggesting a correlation between mucus sampling and cell phospholipid concentration. These data highlight the Phospholipid products of a genetically dysregulated pathway in OAC, which may contribute to the production of unstable polyunsaturated lipids which are prone to oxidation and formation of aldehydes.

Simple Summary: Cancer of the upper digestive system is associated with poor survival due to difficulties in diagnosing the disease early, before it has spread around the body. Previous research has shown that the healthy bacteria within the body changes in response to medications, diet and infection. Some of these changes are associated with a greater risk of developing cancers of the oesophagus (gullet) and stomach. This occurs, at least in part, through inflammation. This process is the body's natural response to infection, injury and exposure to potentially harmful substances, which, if uncontrolled can lead to diseases including cancer. This review article aims to explain the ways in which this happens. By developing a deeper understanding of these processes, advances in early diagnosis of this disease can be made, which may lead to better survival. Introduction: The upper gastrointestinal microbiome is a dynamic entity that is involved in numerous processes including digestion, production of vitamins and protection against pathogens. Many external and intrinsic factors may cause changes in the proportions of bacteria within the microbial community, termed 'dysbiosis'. A number of these have been identified as risk factors for a range of diseases, including oesophago-gastric carcinoma. Materials and Methods: A narrative review was conducted to elucidate the current evidence on the role of the microbiome in promoting oesophago-gastric tumourigenesis. Significant causes of dysbiosis including age, medications and GORD were examined and key pro-inflammatory pathways implicated in tumourigenesis and their interaction with the microbiome were described. Results and Discussion: An association between microbial dysbiosis and development of oesophago-gastric cancer may be mediated via activation of pro-inflammatory pathways, the inflammasome and the innate immune system. Advances in sequencing technology allow microbial communities to be fingerprinted by sequencing the 16S rRNA gene, enabling a deeper understanding of the genera that may be implicated in driving tumourigenesis. Conclusions: Developing a greater understanding of the influence of the microbiota on oesophago-gastric tumourigenesis may enable advances to be made in the early detection of malignancy and in the development of novel systemic therapies, leading to improved rates of survival. Aim: This study aims to provide a comprehensive overview of real‐world evidence pertaining to disparities in the utilization of continuous glucose monitors (CGMs)/insulin pumps to highlight potential evidentiary gaps and discern emerging themes from the literature. Materials and Methods: A systematic review of published manuscripts and abstracts was conducted from: MEDLINE, EMBASE, Nursing and Allied Health, Web of Science and CINHAL. Attributes related to patients, outcomes, interventions (CGMs/pumps/both) and study type were captured. In addition, factors associated with disparities in device utilization were examined. Results: Thirty‐six studies were included in the final analysis; the studies predominantly focused on people living with type 1 diabetes. Only two studies included individuals with type 2 diabetes. Almost two‐thirds of the studies reported outcomes associated with disparities (e.g. glycated haemoglobin, diabetic ketoacidosis, resource utilization). Most studies highlighted disparities across race, ethnicity and insurance type. Evidentiary gaps were identified, particularly in the evidence for people with type 2 diabetes, the continuation of CGM/pump use and limited studies addressing disparities among Native Americans/American Indians. Conclusion: This study reveals critical disparities in diabetes technology use across race, ethnicity and insurance type, particularly among people with type 1 diabetes. Evidentiary gaps assessing disparities in diabetes technology use persist, particularly concerning people with type 2 diabetes, Native American/American Indian and LGBTQ+ populations, and in outcomes related to continuation of use. Social and digital determinants of health, such as income, transportation, residential location and technological literacy, are crucial to achieving equitable access. Cyberspace odyssey (CSO) is a novel serious game supporting computer networking education by engaging students in a race to successfully perform various cybersecurity tasks in order to collect clues and solve a puzzle in virtual near-Earth three-dimensional space. Each team interacts with the game server through a dedicated client presenting a multimodal interface, using a game controller for navigation and various desktop computer networking "tools of the trade" for cybersecurity tasks on the game's physical network. Specifically, teams connect to wireless access points, use packet monitors to intercept network traffic, decrypt and reverse engineer that traffic, craft well-formed and meaningful responses, and transmit those responses. Successful completion of these physical network actions to solve a sequence of increasingly complex problems is necessary to progress through the virtual story-driven adventure. Use of the networking tools reinforces networking theory and offers hands-on practical training requisite for today's cyber operators. This article presents the learning outcomes targeted by a classroom intervention based on CSO, the design and implementation of the game, a pedagogical overview of the overall intervention, and four years of quantitative and qualitative data assessing its effectiveness.

Cardiovascular diseases are the leading cause of fatalities in the United States. Atherosclerotic plaques are one of the primary complications that can lead to strokes and heart attacks if left untreated. It is essential to diagnose the disease early and distinguish vulnerable plaques from harmless ones. Many methods focus on the structural or molecular properties of plaques. Mechanical properties have been shown to change drastically when abnormalities develop in arterial tissue. We report the development of an acoustic radiation force optical coherence elastography (ARF-OCE) system that uses an integrated miniature ultrasound and optical coherence tomography (OCT) probe to map the relative elasticity of vascular tissues. We demonstrate the capability of the miniature probe to map the biomechanical properties in phantom and human cadaver carotid arteries.

Time Series Forecasting has become vital in various industries ranging from weather forecasting to business forecasting. There is a need to research database storage solutions for companies in order to optimize resource allocation, enhance decision making process and enable predictive data storage maintenance. With the introduction of Artificial Intelligence and a branch of AI, Machine Learning, Time Series Forecasting has become more powerful and efficient. This project attempts to validate the possibility of using time series forecasting on database storage data to make business predictions. Currently, predicting capabilities of database storage is an area which is not fully explored, despite the growing necessity of databases. Currently, most of the optimization of databases is left to human touch which is ultimately slower and more error prone. As such, this research will investigate the possibilities of time series forecasting in database storage. This project will use Machine Learning and Time-series Forecasting to predict the future trend of database storage to give information on how the trend of the data will change. Examining the pattern of database storage fluctuations will allow the respective owners an overview of their storage and in turn, make decisions on optimizing the database to prevent critical problems ahead of time. Three distinct approaches - employing a traditional linear model fore forecasting, utilizing a Convolutional Neural Network (CNN) to detect local changes in time series data, and leveraging a Recurrent Neural Network (RNN) to capture long term temporal dependencies - are implemented to assess which of these techniques is better suited for the provided dataset. Furthermore, two settings (single step and multi step) have been tested in order to test the changes in accuracy from a small prediction step to a major. BACKGROUND: We assessed in-hospital mortality and utilization of invasive cardiac procedures following Acute Coronary Syndrome (ACS) admissions on the weekend versus weekdays over the past decade in the United States. METHODS: We used data from the Nationwide Inpatient Survey (2001-2011) to examine differences in all-cause in-hospital mortality between patients with a principal diagnosis of ACS admitted on a weekend versus a weekday. Adjusted and hierarchical logistic regression model analysis was then used to identify if weekend admission was associated with a decreased utilization of procedural interventions and increased subsequent complications as well. RESULTS: 13,988,772 ACS admissions were identified. Adjusted mortality was higher for weekend admissions for Non-ST-Elevation Acute Coronary Syndromes [OR: 1.15, 95% CI, 1.14-1.16] and only somewhat higher for ST-Elevation Myocardial Infarction [OR: 1.03; 95% CI, 1.01-1.04]. Additionally, patients were significantly less likely to receive coronary revascularization intervention/therapy on their first day of admission [OR: 0.97, 95% CI: 0.96-0.98 and OR: 0.75, 95% CI: 0.75-0.75 for STEMI and NSTE-ACS respectively]. For ACS patients admitted during the weekend who underwent procedural interventions, in-hospital mortality and complications were higher as compared to patients undergoing the same procedures on weekdays. CONCLUSION: For ACS patients, weekend admission is associated with higher mortality and lower utilization of invasive cardiac procedures, and those who did undergo these interventions had higher rates of mortality and complications than their weekday counterparts. This data leads to the possible conclusion that access to diagnostic/interventional procedures may be contingent upon the day of admission, which may impact mortality.

Previous research has shown that roughly 15% to 30% of those with heart failure (HF) develop atrial fibrillation (AF). Although studies have shown variations in the incidence of AF in patients with HF, there has been no evidence of mortality differences by race. The purpose of this study was to assess AF prevalence and inhospital mortality in patients with HF among different racial groups in the United States. Using the National Inpatient Sample registry, the largest publicly available all-payer inpatient care database representing >95% of the US inpatient population, we analyzed subjects hospitalized with a primary diagnosis of HF from 2001 to 2011 (n = 11,485,673) using the International Classification of Diseases, Ninth Edition (ICD 9) codes 428.0-0.1, 428.20-0.23, 428.30-0.33, 428.40-0.43, and 428.9; patients with AF were identified using the ICD 9 code 427.31. We assessed prevalence and mortality among racial groups. Using logistic regression, we examined odds of mortality adjusted for demographics and co-morbidity using Elixhauser co-morbidity index. We also examined utilization of procedures by race. Of the 11,485,673 patients hospitalized with HF in our study, 3,939,129 (34%) had AF. Haplotyping of human chromosomes is a prerequisite for cataloguing the full repertoire of genetic variation. We present a microfluidics-based, linked-read sequencing technology that can phase and haplotype germline and cancer genomes using nanograms of input DNA. This high-throughput platform prepares barcoded libraries for short-read sequencing and computationally reconstructs long-range haplotype and structural variant information. We generate haplotype blocks in a nuclear trio that are concordant with expected inheritance patterns and phase a set of structural variants. We also resolve the structure of the EML4-ALK gene fusion in the NCI-H2228 cancer cell line using phased exome sequencing. Finally, we assign genetic aberrations to specific megabase-scale haplotypes generated from whole-genome sequencing of a primary colorectal adenocarcinoma. This approach resolves haplotype information using up to 100 times less genomic DNA than some methods and enables the accurate detection of structural variants.

The identification of traffic signs is an essential step in the process of autonomous driving and the development of sophisticated driver support systems. In this research, investigate the possibility of using the YOLOv8 object identification algorithm to the problem of recognizing traffic signs. Because of its high level of performance and speed, the most cutting-edge object recognition algorithm known as YOLOv8 is well suited for use in the detection of real-time traffic signs. When it comes to object detection, CNN-based algorithms, such as Faster R-CNN, provide a low level of accuracy and poor performance. It has been shown that these techniques are effective in the context of traffic sign recognition (TSR) systems. Single-stage detection approaches, such as SSD and YOLO, are not recommended for the problem of recognition owing to their poor accuracy and difficulties in detecting smaller objects. This is despite the fact that these methods provide the most cutting-edge real-time detection speed available. This is because there is a problem with detecting very tiny objects. We trained the YOLOv8 algorithm using a GTSDB dataset consisting of images of traffic signs, and on our test set, we were able to get a map of 95.6 %. The Performance of our model was also examined on a real-world driving dataset and discovered that our model was able to reliably recognize traffic signs in a variety of different conditions.
