Abstract

Multisystem inflammatory syndrome in children (MIS-C) is an uncommon but emerging syndrome related to SARS-CoV-2 infection. While the presentation of MIS-C is generally delayed after exposure to the virus that causes coronavirus 2019, both MIS-C and Kawasaki disease (KD) share similar clinical features. Multisystem inflammatory syndrome in children poses a diagnostic and therapeutic challenge given the lack of definitive diagnostic tests and a paucity of evidence regarding treatment modalities. We review the clinical presentation, diagnostic evaluations, and management of MIS-C and compare its clinical features to those of KD.

12. A narrative review of the current available literature on the intersection between the climate crisis and paediatric surgical care.

Abstract

The climate crisis exacerbates health inequities, including in paediatric surgery, creating a vicious cycle. We sought to review (a) existing evidence on the connection between paediatric surgery and climate and (b) how this addresses the vicious cycle. A PubMed search was conducted on 23-08-2023. Articles not commenting on "climate change" and "paediatric surgery" were excluded. Included papers were categorised into emerging themes. Out of 151 search results, seven articles were included. The emerging themes related to "Effect of climate on paediatric surgical illness" (n = 3), "Effect of surgery on climate" (n = 2), and "Mitigating impact of paediatric surgery" (n = 2). Five articles were observational studies, and two were literature reviews, all papers published after 2020. We found limited primary research focusing on the intersection between climate change and paediatric surgery. Articles tend to focus on quantifying impact and mitigation, which does not lend itself to climate justice. The syndemic model of health focuses on the complex interconnections and pathways through which health conditions interact within populations to exacerbate adverse health outcomes. We suggest future research needs to be reframed, with the interconnection between health inequities, the climate crisis, and the wider health system addressed together. [ABSTRACT FROM AUTHOR]

17. iGreen : a social norms intervention to encourage pro-environmental behaviour

Abstract

Previous research indicates that social norms interventions provide a promising avenue to encourage behaviour change. This study examined the efficacy of a social norms intervention, with the inclusion of personalised individual feedback, to encourage pro-environmental behaviour change. A qualitative approach was used to gain an in-depth understanding of how people respond to social norms feedback and personalised individual feedback on environmental behaviours. Central to this research was an innovative Facebook app called iGreen, which was designed specifically by the author and a number of colleagues to provide a seven-week social norms intervention. This app comprised environmentally themed games, a quiz on aspects of everyday domestic behaviours that impact on the environment, and the ability to provide feedback on respondents’ previous quiz answers. Respondents were randomly allocated to either a no feedback group, a personalised individual feedback group, or a group in which feedback also included the average quiz answer of other iGreen users (social norms feedback group). A sample of fifty-one people who used iGreen completed all quizzes, forty-four of these respondents completed a post-intervention questionnaire, and thirty respondents were interviewed. Drawing on elements of a discourse analysis approach to analyse the interviews enabled an in-depth understanding of why a social norms intervention might, or might not encourage pro-environmental behaviour and how people respond to personalised individual feedback and social norms feedback. The major finding in this research is that the quiz encouraged behaviour change because the questions increased the salience of injunctive norms and personal norms. This supports the focus theory of normative conduct and norm activation theory, which both state that increasing the salience of norms influences behaviour. Another finding is that environmental behaviour change can be constrained due to people associating some behaviours with the stigmatisation of environmental activists. Lastly, respondents in all three intervention groups claimed to have changed some behaviour and there were no apparent differences between the groups. This suggests that increased salience (in this case induced by answering repeated quiz questions) encouraged behaviour change. This raises the question of whether increased salience, rather than feedback, may account for some of the behaviour change found in previous social norms research. This research identifies key elements of an intervention that can increase its potential to encourage pro-environmental behaviour which has potential practical application in the design of innovative social norms interventions. The main contribution of this research is the discovery that making people’s everyday behaviours more salient can encourage pro-environmental behaviour. A digital quiz is a simple, cost-effective and engaging method for increasing salience and encouraging behaviour change, and this should be explored in future research.

Abstract

ObjectiveCommunity-based participatory research is recommended for research on health disparities and to improve uptake of clinical research findings. We describe the development of a multicenter consortium designed to support a community agency-academic partner infrastructure to support community-based, health-services research on multiple sources of health and healthcare disparities in local communities.DesignWe describe the development of the Los Angeles Community Health Improvement Collaborative (CHIC).ResultsThe CHIC partners examined the research capacity and health priorities of its partners and developed a research agenda focused on four tracer conditions (depression, violence, diabetes, and obesity) and four areas for development of research capacity: public participation in all phases of research; understanding community and organizational context for clinical services interventions; practical clinical services trial methods; and advancing health information technology for clinical services research. The partners pooled resources to develop these areas for the tracer conditions.ConclusionsThe challenges of a participatory approach to community-based clinical services research go beyond the significant methodologic and operational issues for specific projects and include building a sustainable capacity for research, community programs, and partnership across diverse communities and stakeholder organizations even when funding sources are not fully aligned with these goals.

38. Effective Cache Placement for Content Delivery Networks in Fog Computing.

Abstract

Fog computing (FC) is a concept that encompasses cloud paradigm-like amenities to the edge of the network. The fog layer that complements the cloud-based concept aids in strengthening the standards of services (QoS) in time-sensitive activities. One such application of FC arises in the deployment of content delivery networks for reducing the latency in transferring content to customers and enhancing the user experience. This paper presents a fog-based system where fog nodes act as caches for content storage. Appropriate placement of cache nodes is important for fast content distribution to users and maintaining the QoS. Another challenge is to minimize energy expenditure while deploying caches at scale. To deal with these issues, a Connected Dominating Set (CDS) and popularity-based caching strategy in content distribution fog networks are suggested in this work. The content is sorted based on its past popularity and then CDS based rules have been developed to recommend the most ideal fog nodes (FNs) to place popular content or files. The selection of fog nodes as caches is performed based on factors of storage capacity, energy consumption, and user density (i.e., the number of neighbors of a fog node). Experiment outcomes authenticate the efficiency of the anticipated scheme without caching and random method of cache placement. In comparison to the without caching and random method, the proposed method produces better fog nodes as cache nodes, incurs a lower energy consumption and consumes less bandwidth.
