Symmetric drug-related intertriginous and flexural exanthema (SDRIFE) is a cutaneous drug reaction that presents with symmetrical erythema in the flexures. The reaction typically appears hours-to-days after drug exposure but has been reported to occur months after drug initiation. Diagnostic criteria include cutaneous reaction after exposure to a systemic drug, erythema of the gluteal region and/or V-shaped erythema of the inguinal areas, involvement of an additional intertriginous site, symmetry, and absence of systemic involvement. The rash typically presents as macular erythema. However, variations in morphology have been reported including papules, pustules, vesicles, and bullae. The histopathology of SDRIFE is non-specific and the diagnosis is made clinically. Cessation of the causative drug leads to gradual rash resolution. Beta-lactam antibiotics are the most implicated medications but case reports describe SDRIFE following monoclonal antibodies, chemotherapeutic agents, and various other medications. We present a patient with SDRIFE secondary to lenalidomide, an immunomodulatory agent. This case highlights the importance of considering SDRIFE in the differential diagnosis of patients presenting with intertriginous erythema.

Calcipotriol (calcipotriene) is a synthetic vitamin D3 derivative that is a standard treatment option for psoriasis. It is generally well tolerated with minimal side effects. Due to its ability to reduce keratinocyte proliferation and induce keratinocyte differentiation as well as its immunomodulatory effects, calcipotriol has been used to treat a variety of skin disorders such as atopic dermatitis, actinic keratoses, lichen planus, seborrheic keratoses, and vitiligo [1]. We surveyed the literature examining the use of calcipotriol for non-psoriatic dermatologic disease.

Pretibial myxedema, more generally thyroid dermopathy, results from mucopolysaccharide accumulation in the dermis, typically between the knee and dorsal foot. Thyroid dermopathy presents in Graves disease, but can occur in Hashimoto thyroiditis, primary hypothyroidism, and euthyroid patients. Treatment of thyroid eye disease with teprotumumab is established in the literature, with few case reports also showing improvement in pretibial myxedema. Reported is a 76-year-old man with thyroid eye disease and pretibial myxedema treated with teprotumumab; improvement was demonstrated in both conditions. He developed "muffled" hearing as an adverse effect, a complication not widely published in the dermatology literature. At 18 months post-treatment, his symptoms are stable without recurrence, but hypoacusis persists. Given the long-term efficacy and side-effects, dermatologists should recognize the potential benefits and risks of using teprotumumab for thyroid dermopathy. A baseline audiogram may be considered prior to therapy. Additionally, longitudinal data is needed to document the benefits and risks of this novel therapy.

Pathobionts associated with periodontitis, such as Treponema denticola, must possess numerous sensory transduction systems to adapt to the highly dynamic subgingival environment. To date, the signaling pathways utilized by T. denticola to rapidly sense and respond to environmental stimuli are mainly unknown. Bis‐(3′–5′) cyclic diadenosine monophosphate (c‐di‐AMP) is a nucleotide secondary messenger that regulates osmolyte transport, central metabolism, biofilm development, and pathogenicity in many bacteria but is uncharacterized in T. denticola. Here, we studied c‐di‐AMP signaling in T. denticola to understand how it contributes to T. denticola physiology. We demonstrated that T. denticola produces c‐di‐AMP and identified enzymes that function in the synthesis (TDE1909) and hydrolysis (TDE0027) of c‐di‐AMP. To investigate how c‐di‐AMP may impact T. denticola cellular processes, a screening assay was performed to identify putative c‐di‐AMP receptor proteins. This approach identified TDE0087, annotated as a potassium uptake protein, as the first T. denticola c‐di‐AMP binding protein. As potassium homeostasis is critical for maintaining turgor pressure, we demonstrated that T. denticola c‐di‐AMP concentrations are impacted by osmolarity, suggesting that c‐di‐AMP negatively regulates potassium uptake in hypoosmotic solutions. Collectively, this study demonstrates T. denticola utilizes c‐di‐AMP signaling, identifies c‐di‐AMP metabolism proteins, identifies putative receptor proteins, and correlates c‐di‐AMP signaling to osmoregulation. OnabotulinumtoxinA (onabotA) is approved in the US for 12 therapeutic indications. Real-world data on onabotA multi-indication use are limited, often leading to delayed or reduced treatment. This study provides real-world evidence on the safety of onabotA when treating multiple indications concomitantly. SYNCHRONIZE was a multicenter, retrospective, chart-review study evaluating onabotA's safety for adults treated for ≥2 therapeutic indications within a 3-month period. The primary outcome was treatment-emergent adverse events (TEAEs) within 6 months post-treatment. A total of 279 patients were included. The most common concomitant indications treated were cervical dystonia and chronic migraine (43.4%). The average 3-month cumulative dose for multiple indications was 282.2 U. The treatment interval for multiple indications was ≤24 h for most patients (62.4%). Overall, 28.7% of patients reported ≥1 TEAE with no apparent trends in TEAEs and dose interval or cumulative dose. Reported TEAEs included UTI (5.7%), neck pain (5.0%), and headache (4.3%). No patient had a lack of effect according to clinical objective measurements. SYNCHRONIZE described the real-world safety of onabotA for patients treated concomitantly for ≥2 indications within a 3-month period. Heterosis has long been harnessed in crop production, and while crop fertility lays the groundwork for leveraging heterosis, there remains a dearth of comprehensive data regarding genic male sterility in mungbean. Mungbean (Vigna radiata (L.) Wilczek), being a self‐pollinating crop, exhibits significant hybrid vigour in F1 hybrid seed yield, suggesting the potential for developing hybrid varieties to propel mungbean yield beyond existing plateaus. A new male sterile mutant was induced, isolated, stabilized and evaluated, via gamma irradiation at a rate of 600 Gy, identified in the M3 generation of the GM 4 accession. A uniform male sterile line was crossbred with the wild‐type parental plant to explore the inheritance pattern of male sterility. Pollen sterility was validated through the acetocarmine test, while stigma viability was ascertained using the hydrogen peroxide. While all F1 plants displayed fertile flowers, the F2 generation showed a clear 3:1 segregation ratio for fertile to male sterile plants, indicating the control of male sterility by a single recessive gene, mms. Also, a novel existence of a truncated stigma nestled within the anther column inhibits the effective reception of pollen during anthesis, offering potential in reducing emasculation time by a slight cut on the flower bud and enhancing cross pollination during hybrid seed production. Background and Aim: There is a scarcity of Indian studies on infective endocarditis. Additionally, the majority of research has focused on the paediatric age group. The purpose of this study was to investigate the microorganisms responsible for infective endocarditis in patients with rheumatic heart disease in patients attending Medicine OPD of Smt NHL Municipal Medical College, Ahmedabad. Additionally, the study aimed to assess the effectiveness of medical treatment and evaluate the prognosis of the cases. Material and Methods: Our study took place at the Department of General Medicine, a Smt. NHL Municipal Medical College, Ahmedabad, over the course of one year. We thoroughly investigated patients who were clinically suspected of having infective endocarditis. A study was conducted on 40 patients with Rheumatic heart disease who met the diagnostic criteria for infective endocarditis. A comprehensive set of medical tests were performed, including a complete hemogram, urinalysis specifically to check for microscopic haematuria, chest Xray, and EGG. Prior to starting treatment, blood cultures were collected. A comprehensive M mode and 2 D echocardiographic examination was conducted for all individuals suspected of having the condition, with additional TEE performed for cases that required further clarification. Results: A significant number of patients presented with a combination of mitral and aortic valve lesions. The most frequently observed combined lesions in our patients were mitral and aortic regurgitation. The patients included in our study experienced mild to moderate grade fever. At the time of admission, nineteen patients in the study group had a leukocyte count exceeding 10,000 cells per cubic millimetre. The majority of patients in the study group exhibited elevated ESR levels. Out of the total 40 patients, 11 had a positive blood culture (27.5%). Surprisingly, in 29 patients, the blood culture remained negative even after multiple sample collections and a seven-day incubation period. Most patients initially received a standard recommended dose of injection Penicillin G and Gentamicin on an empirical basis. However, as per the sensitivity pattern, appropriate antibiotics were administered thereafter. Conclusion: Treatment of endocarditis was more successful when suitable antibiotics were started after culture and sensitivity rather than the empirical treatment. [ABSTRACT FROM AUTHOR]

The current study was initiated to assess mungbean [Vigna radiata (L.) R. Wilczek] mutant lines for yield and their attributes in the mutation generation third (M3) generation using an augmented block design. Note that 1200 mutated mungbean lines were selected from the γ-irradiated (400–700 Gy) population and distributed into 21 blocks, along with three popular varieties (checks) replicated in each block. To evaluate mean performance, variance, and population genetics, the observations on days to flowering, days to maturity, plant height (cm), number of clusters per plant, number of pods per plant, pod length (cm), protein percentage, seed index, and seed yield (g) were measured and analyzed. All the lines under consideration showed highly significant variations for all the variables, except for days to flowering. Seventy-nine mutant lines were found to have significantly better yield attributes than checks and are currently being evaluated under station trial. Skewness and kurtosis analysis unveiled the presence of gene interactions, offering opportunities for targeted improvement and efficacy of γ rays as a mutagen, facilitating the release of variability within the population. Future mungbean breeding programs will benefit from the successful isolation of mutant plants with yield-enhancing traits, such as up to 20 clusters per plant, 71 pods per plant, 8.55 cm pod length, 26.69% protein content (5% higher than parent), and 29 gram seeds per plant. A neurofibroma with focal glomus‐like body differentiation is an unusual phenomenon recently encountered in an excision specimen from the right lateral distal forearm of a 26‐year‐old man. Glomus cells are modified smooth muscle cells normally present in glomus‐like bodies but can also be found in glomus tumors (GT) or lesions considered in the spectrum of GT, including myopericytoma, myofibroma, and angiolipoma. Neurofibromas are peripheral nerve sheath tumors derived from the neural crest cells. While both GT and its variants and neurofibroma are thought to be derived from different cell types, there is growing evidence that glomus cells have a neural crest origin. This is based on multiple theories, with some overlapping pathways, including neural crest cell differentiation, Schwann cell reprogramming, VEGF expression, and NF1 gene biallelic inactivation. This report adds to the growing evidence of possible neural crest origin for glomus cells and would help explain finding glomus‐like bodies scattered through a neurofibroma. [ABSTRACT FROM AUTHOR]
