RESUMEN Objetivo: Estudiar la influencia del catastrofismo sobre el dolor lumbar crónico radicular y evaluar sus cambios a la inyección epidural de esteroides. Además, estudiar la relación entre catastrofismo y la respuesta al tratamiento. Material y métodos: Se realizó un estudio prospectivo sobre 52 pacientes con lumbociatálgia crónica unilateral, a los que se le realizó una inyección epidural lumbar de esteroides. En 39 de ellos se valoró en condiciones basales y al mes del procedimiento, el dolor y el catastrofismo mediante el Inventario Abreviado de Dolor y la Escala de Catastrofismo. Se definó como "respon­dedores" a los pacientes con disminución de 2 puntos o más en la intensidad del dolor. Para el catastrofismo se consideró un score de 30 como severo. Un valor de p < 0,05 se consideró de significancia estadística. Resultados: El catastrofismo total fue mayor en mujeres que en hombres. Se encontró una correlación lineal positiva moderada entre catastrofismo, intensidad del dolor e índice de interferencia, estadísticamente significativa. En los 39 pacientes tratados, se encontró una disminución estadísticamente significativa de la intensidad, interferencia funcional del dolor y catastrofismo total. Dieciseis pacientes se considera­ron respondedores. En estos, el índice de intensidad disminuyó de 7,8 ± 0,8 a 3,9 ± 3,1 (50 % de porcentaje de cambio, p = 0,0001); la interferencia funcional del dolor evaluada por el índice de interferencia disminuyó de 8,2 ± 1,6 a 5,0 ± 3,8 (40 % de cambio, p = 0,0027) y el catastrofismo total disminuyó de 37 ± 13 a 17,5 ± 16 (57 % de cambio, p < 0,0001). El catastrofismo total previo no mostró diferencias entre los pacientes respondedores y no respondedores a las inyecciones epidurales de esteroides. Conclusiones: La inyección epidural de esteroides en pacientes con dolor lumbar radicular crónico resultó efectiva para disminuir el dolor y sus repercusiones funcionales. Se produjo un descenso del catastrofismo, que mostró ser un constructo dinámico capaz de ser modificado por el tratamiento intervencionista. El catastrofismo total evaluado previamente a las inyecciones epidurales no afectó la respuesta de los pacientes a dicho tratamiento intervencionista.

RESUMEN Introducción: La algometría de presión es un método válido para evaluar el sistema sensorial de transmisión del dolor en seres humanos, mediante pruebas como la sumación temporal y la modulación condicionada del dolor. El objetivo de este trabajo fue la realización de una experiencia preliminar en el Servicio de Anestesio­logía de nuestro Hospital Universitario, utilizando la algometría de presión como test sensorial cuantitativo. Metodología: En una primera etapa, se realizó un estudio transversal de caso y control, sin aleatorización, donde se midió el umbral de dolor a la presión por algometría de presión a 58 pacientes que consultaron en la policlínica preoperatoria. Dieciocho tenían algún tipo de dolor crónico (grupo con dolor) y 40 no tenían dolor (grupo sin dolor). En una segunda etapa, se realizó un estudio observacional a 36 voluntarios sanos, residentes de anestesiología. En 16 se estudió la sumación temporal y en 20 la modulación condicionada del dolor, en ambos casos utilizando la algometría de presión. La aplicación de presión con manguito en el brazo se utilizó como estímulo heterotópico. Ambas pruebas fueron medidas a nivel del músculo trapecio derecho. Resultados: En el grupo con dolor el valor del umbral de dolor a la presión fue menor que en el grupo sin dolor en todos los puntos evaluados. El valor del umbral de dolor a la presión promedio de las cuatro regiones analizadas en el grupo con dolor, resultó ser menor que en el grupo sin dolor, 32,7 ± 10 Newtons versus 42 ± 11,7 Newtons (p = 0,004). El umbral de dolor a la presión promedio resultó más bajo en pacientes de sexo femenino que en los de sexo masculino, 36,6 ± 9 Newtons versus 48 ± 11 Newtons (p = 0,001). En los 20 voluntarios sanos en los que se realizó la prueba de modulación condicionada del dolor a nivel del trapecio, el umbral de dolor a la presión inicial fue de 63 ± 27 Newtons y el final fue de 72 ± 28 Newtons, con un aumento promedio de 9 Newtons (14,2 %). En los 16 en los que se realizó la prueba de sumación temporal se encontró que el nivel de dolor de la Escala Visual Numérica pasó de 4,1 ± 1,0 a 6,5 ± 1,5 entre el primer y décimo estímulo. Conclusiones: La aplicación de la algometría de presión en pacientes que consultaron en la policlínica preoperatoria mostró que los umbrales de dolor a la presión promedio fueron menores en pacientes de sexo femenino y con dolor crónico. En voluntarios sanos, la sumación temporal mostró un aumento en la intensidad del dolor, respuesta que podría extrapolarse a la expresión normal del fenómeno de "wind-up". En 80 % de los voluntarios sanos, la mo­dulación condicionada del dolor resultó en un aumento del umbral de dolor a la presión con la aplicación de un estímulo doloroso heterotópico. La algometría de presión constituye un instrumento útil y práctico. La información obtenida con los test sensorial cuantitativos puede ser de gran utilidad en las Unidades de Dolor Agudo y Crónico.

The sub-Antarctic islands of New Zealand are biodiversity hotspots in the Southern Ocean, containing numerous endemic species and providing breeding grounds for seabirds and marine mammals. However, due to their remoteness and harsh environments, many of their marine ecosystems are relatively unexplored and potentially at risk from alien invasive species. To better understand nearshore marine ecosystems at three New Zealand sub-Antarctic islands (Snares Islands, Auckland Islands and Campbell Island), we sampled nektobenthic fish and mobile macroinvertebrates at 40 sites (15–20 m depth) using baited remote underwater videos (BRUVs). MaxN of each species in the videos was recorded in 5-min intervals for 45 min, allowing analyses of MaxN over the whole deployment, as well as change through time during the deployment. Species distributions appeared to reflect both the geomorphological and biogeographic traits of the islands. The Auckland Islands and Campbell Island contain large inlets dominated by mobile crustaceans, and biological trends followed gradients in marine exposure along inlets. In contrast, the Snares Islands are mostly exposed coast and contained a higher diversity of fish species common with mainland New Zealand. We suggest that differences in nearshore marine communities between these islands are likely due to the combined effects of habitat availability, biogeography, and ocean temperature.

Background The administration of endocrine therapy for 5 years substantially reduces recurrence rates during and after treatment in women with early-stage, estrogen-receptor (ER)–positive breast cancer. Extending such therapy beyond 5 years offers further protection but has additional side effects. Obtaining data on the absolute risk of subsequent distant recurrence if therapy stops at 5 years could help determine whether to extend treatment. Methods In this meta-analysis of the results of 88 trials involving 62,923 women with ER-positive breast cancer who were disease-free after 5 years of scheduled endocrine therapy, we used Kaplan–Meier and Cox regression analyses, stratified according to trial and treatment, to assess the associations of tumor diameter and nodal status (TN), tumor grade, and other factors with patients’ outcomes during the period from 5 to 20 years. Results Breast-cancer recurrences occurred at a steady rate throughout the study period from 5 to 20 years. The risk of distant recurrence was strongly correlated with the original TN status. Among the patients with stage T1 disease, the risk of distant recurrence was 13% with no nodal involvement (T1N0), 20% with one to three nodes involved (T1N1–3), and 34% with four to nine nodes involved (T1N4–9); among those with stage T2 disease, the risks were 19% with T2N0, 26% with T2N1–3, and 41% with T2N4–9. The risk of death from breast cancer was similarly dependent on TN status, but the risk of contralateral breast cancer was not. Given the TN status, the factors of tumor grade (available in 43,590 patients) and Ki-67 status (available in 7692 patients), which are strongly correlated with each other, were of only moderate independent predictive value for distant recurrence, but the status regarding the progesterone receptor (in 54,115 patients) and human epidermal growth factor receptor type 2 (HER2) (in 15,418 patients in trials with no use of trastuzumab) was not predictive. During the study period from 5 to 20 years, the absolute risk of distant recurrence among patients with T1N0 breast cancer was 10% for low-grade disease, 13% for moderate-grade disease, and 17% for high-grade disease; the corresponding risks of any recurrence or a contralateral breast cancer were 17%, 22%, and 26%, respectively. Conclusions After 5 years of adjuvant endocrine therapy, breast-cancer recurrences continued to occur steadily throughout the study period from 5 to 20 years. The risk of distant recurrence was strongly correlated with the original TN status, with risks ranging from 10 to 41%, depending on TN status and tumor grade. Acute limb ischemia (ALI) is a serious medical emergency leading to high rate of complications, being not only limb- but even life-threatening, often despite early successful revascularization.1 Improvements in surgical techniques and perioperative patient care may have reduced the incidence of major complications in ALI patients over the years, but the results of trials published recently seem to document a persistent high risk, with reported 30-day amputation rate of 5% to 12%, mortality risk at 10% to 38%, combined incidence of amputation and death of 25% to 37.5%, at 1- to 6-month follow-up.2–7 Concomitant underlying diseases, the metabolic derangement that seems as a result of the acute insult, and a possible reperfusion injury following revascularization may account for this severe prognosis.8 Only anticoagulation, fasciotomy (when indicated), and perioperative supportive treatment are established strategies in ALI patients.1,8,9 Possible benefit from cardiovascular active therapies has recently been suggested in patients undergoing peripheral revascularization or noncardiac major surgical intervention.10,11 Moreover, several categories of compounds, potentially acting on pathobiological mechanisms of ischemia-reperfusion syndrome, have been tested in experimental models, but none of them has as yet been proven effective in clinical studies in patients with ALI.12–18 Because of their pharmacologic profile, prostanoids represent a potentially interesting category as adjuvant treatment of ALI patients.19 Several ischemia-reperfusion studies described the use of prostaglandins for reduction of postischemic tissue injuries, and even recently both PGE1 and PGI2 appeared as potent inhibitors of reflow-paradox in a preclinical model of reperfusion injury.20 Iloprost is a widely studied synthetic analogue of prostacyclin, with a 10-fold higher half-life than the native compound, and indicated in the treatment of severe chronic limb ischemia.1,21–23 Results from pilot studies and case reports also described the positive effects of iloprost in the management of acute ischemia secondary to various causes, particularly after accidental intra-arterial administration of drugs or toxic agents.24–26 Several preclinical studies have assessed the effects of iloprost in experimental ischemia-reperfusion injury and documented the actions of the compound on different pathophysiologic mechanisms potentially relevant for damage following ALI.27–32 A diagram indicating where iloprost can interfere in the mechanisms, leading from ischemia and reperfusion, to the development of no-reflow and reflow-paradox, is reported in Figure 1. 33 FIGURE 1. Some years ago, we performed a placebo-controlled, double-blind pilot study in 30 patients with ALI undergoing Fogarty's thromboembolectomy. Encouraging results were obtained with the use of intraoperative and postoperative iloprost (lower incidence of major clinical events, more evident metabolic improvement by means of transcutaneous tensiometry).34 In this paper, we report the results of ILoprost in Acute Ischemia of Lower Limbs (ILAILL) study, a larger, multicenter trial including patients undergoing all types of surgical revascularization, who received iloprost or placebo administration during intervention and therefore for 4 to 7 days, and were observed for a 3-month postoperative period.

Objectives To evaluate the effects of iloprost, in addition to surgery, on the outcome of acute lower limb ischemia (ALLI). Design Post-hoc analysis of a randomized, double-blind, placebo-controlled study. Methods In the context of the ILAILL (ILoprost in Acute Ischemia of Lower Limbs) study, 192 elderly patients (>70 years old) undergoing surgery for ALLI were assigned to receive perioperative iloprost (intra-arterial, intra-operative bolus of 3000ng, plus intravenous infusion of 0.5–2.0ng/kg/min for six hours/day for 4–7 days following surgery), or placebo (iloprost: n =100; placebo: n =92). Patients were followed-up for three-months following surgical revascularization. Results The combined incidence of death and amputation (primary study end-point) was significantly reduced in patients treated with iloprost (16.0% vs 27.2% in the placebo group; hazard ratio 1.99, 95% confidence interval 1.05–3.75, p =0.03). A statistically significant lower mortality (6.0%) was reported in patients receiving iloprost, compared to controls (15.2%) (hazard ratio 2.93, 1.11–7.71, p =0.03). The overall incidence of death and major cardiovascular events was lower in patients receiving iloprost compared to those assigned placebo (24.0% and 35.9%, respectively), at the limits of statistical significance (relative risk 1.64, 0.97–2.79, p =0.06). Conclusions These results confirm the poor outcome in elderly patients with ALLI. Based on a subgroup analysis iloprost, as an adjuvant to surgery, appears to reduce the combined end-point of death and amputation.
