[ES] En este trabajo se describe una metodología propuesta para la identificación semiautomática de discontinuidades y el posterior análisis cinemático y de estabilidad a través de su aplicación a una trinchera excavada en roca de una línea de ferrocarril. La adquisición de imágenes se ha llevado a cabo mediante un vehículo aéreo no tripulado de seis rotores para su posterior restitución fotogramétrica a través de la técnica digital Structure from Motion (SfM) mediante el programa Agisoft Metashape que proporciona una nube de puntos 3D. A partir de esta nube de puntos, se han identificado cuatro familias de discontinuidades (J1, J2, J3 y J4) que afectan al talud haciendo uso del programa de código abierto Discontinuity Set Extractor (DSE). Finalmente, se han llevado a cabo análisis cinemáticos y de estabilidad de las posibles roturas de bloques a favor de las discontinuidades identificadas en el talud. Los resultados muestran tres potenciales roturas por cuña y una plana que han sido validadas cualitativamente mediante el análisis de la geometría de las nubes de puntos., [EN] In this work, a methodology proposed for the semiautomatic identification of discontinuities and the later kinematic and stability analyses is described through its application to a rocky railway line cutting. Image acquisition has been performed using a six-rotors unmanned aerial vehicle for their subsequent photogrammetric restitution by means of the digital technique Structure from Motion (SfM) by means of the software Agisoft Metashape that provides a 3D point cloud. From this 3D point cloud, four discontinuity sets (J1, J2, J3 and J4) affecting the cutting have been identified using the open source software Discontinuity Set Extractor (DSE). Finally, kinematic and stability analyses of the potential block failures controlled by the discontinuities identified in the cutting. The results show three potential wedge and planar failures that have been qualitatively validated trough the geometric analysis of the 3D point cloud.

Special foundations, most prominently micropiles and soil anchors, are frequently used in construction today. In Spain, the grout for these special technical applications is generally prepared with portland cement, although the codes and standards in place stipulate only the minimum compressive strength required, with no mention of cement type. Those texts also establish a range of acceptable water:cement ratios. In the present study, durability and compressive strength in cement grout prepared with blast furnace slag cement at different w/c ratios are characterised and compared to the findings for a reference portland cement grout. The results show that slag grout exhibits greater durability than the portland cement material and complies with the compressive strength requirements laid down in the respective codes., Actualmente es muy frecuente el empleo de cimentaciones especiales, entre las que destacan los micropilotes y los anclajes. En España, las lechadas de cemento para estos trabajos geotécnicos especiales se preparan habitualmente con cemento Portland, aunque las diferentes normativas al respecto no restringen el tipo de cemento a emplear, siempre que se alcance una determinada resistencia a compresión. Respecto a la dosificación de las lechadas, la normativa permite emplear diferentes relaciones agua/cemento dentro de un determinado rango. En vista de ello, en este trabajo se han caracterizado las propiedades de durabilidad y resistencia a compresión de lechadas de cemento preparadas con un cemento con escoria de alto horno y con diferentes relaciones a/c, tomando como referencia de comportamiento lechadas de cemento Portland. El uso de un cemento con escoria conlleva una mejora en la durabilidad de las lechadas, cumpliendo los requisitos de resistencia a compresión establecidos por la normativa.

11. Intoxicación por triyodotironina: Estudio clínico y farmacocinético

A una mujer tiroidectomizada por carcinoma papilar de tiroides se le suspendió el tratamiento con L-tiroxina para realizar un estudio de extensión cinco semanas después. Para minimizar sus síntomas durante las tres primeras semanas se le prescribió tratamiento con cápsulas de 25 µg de triyodotironina cada 8 horas. Nueve días después consultó por dolor abdominal, náuseas, vómitos, fiebre de 40 °C, mareo y molestia torácica. Mediante inmunoanálisis quimioluminiscente se detectó una triyodotironina sérica total de 575,2 nmol/l a las 11 horas de la toma de la última cápsula (valor normal: 1,1-2,9 nmol/l). A lo largo de los diez días siguientes la enferma presentó delirio, agitación, taquicardia, hipertensión arterial, estreñimiento y luego diarrea, pero no arritmias ni temperatura superior a 38 °C. Se realizaron 59 determinaciones de triyodotironina total para determinar la cinética de eliminación de esta hormona. Se calcula que la máxima concentración sérica tras la ingestión de la última cápsula debió ser de 794,3 nmol/l, es decir, 397 veces superior al promedio de la normalidad. La vida media de eliminación fue de 24 horas y 40 minutos. La hemoperfusión con carbón activado no tuvo impacto alguno sobre la velocidad de eliminación. La concentración de triyodotironina se normalizó a las 200 horas de la toma de la última cápsula, pero las manifestaciones clínicas tardaron tres días más en desaparecer. Los datos farmacocinéticos sugieren que la intoxicación pudo deberse a que durante nueve días la paciente tomara cápsulas con 5 mg de triyodotironina, es decir, una dosis 200 veces mayor que la prescrita por su médico. A woman, thyroidectomised because of a thyroid papillary carcinoma, interrupted temporarily her levothyroxine intake in order to be subjected to an extension study five weeks later. To minimise her symptoms for the first three weeks, a treatment was prescribed consisting of one 25 µg-capsule of triiodothyronine every 8 hours. Nine days later she complained of abdominal pain, nausea, vomiting, fever of 40 °C and chest discomfort. A serum total triiodothyronine of 575.2 nmol/l was measured by chemoluminiscent immunoassay eleven hours after the intake of the latest capsule (normal level: 1.1-2.9 nmol/l). Along the following ten days the patient suffered from delirium, agitation, tachycardia, hypertension, constipation and later diarrhoea, but neither arrythmias nor axillary temperature over 38 °C. Fifty-nine measurements of the serum total triiodothyronine were performed in order to determine the kinetics of elimination of this drug. We estimate that the maximal serum concentration after the intake of the latest capsule could be 794.3 nmol/l, i.e. 397 times higher than the mean normal value. The elimination half-life was 24 hours 40 minutes. The charcoal haemoperfusion had no impact on the velocity of elimination. The concentration of triiodothyronine became normal 200 hours after the intake of the latest capsule, but the clinical manifestations still lasted three days more. The pharmacokinetic data suggest that this intoxication could be due to the intake of capsules containing 5 mg of triiodothyronine, i.e. a dose 200 times higher than that prescribed by her physician.

- Abstract
Having oxidized 3,4-dihydroxyphenylalanine (dopa) with sodium periodate or mushroom tyrosinase in a pH range from 3.5 to 6.0, it has been possible to detect spectrophotometrically 4-(2-carboxy-2-aminoethyl)-1,2-benzoquinone with the amino group protonated (o-dopaquinone-H+), a postulated intermediate in the melanogenesis pathway. When the pH value was greater than 4, the final product obtained was 2-carboxy-2,3-dihydroindole-5,6-quinone (dopachrome); however, for pH values lower than 4, two different products were identified by means of cyclic voltammetry: 5-(2-carboxy-2-aminoethyl)-2-hydroxy-1,4-benzoquinone and dopachrome. These products appeared when oxidation was achieved with the enzyme as well as with periodate. This suggests that two chemical pathways can arise from alpha-dopaquinone-H+, whose relative importance is determined by the pH. The steps of these pathways would be dopa leads to o-dopaquinone-H+ leads to o-dopaquinone leads to leukodopachrome leads to dopachrome, for the first one, and dopa leads to o-dopaquinone-H+ leads to 2,4,5-trihydroxyphenylalanine leads to 5-(2-carboxy-2-aminoethyl)-2-hydroxy-1,4-benzoquinone very slowly leads to intermediate compound leads to dopachrome, for the second one. The stoichiometry for the conversion of dopaquinone-H+ into dopachrome for pH values greater than 4 followed equation of 2 o-dopaquinone-H+ leads to dopa + dopachrome. No participation of oxygen was detected in the conversion of leukodopachrome (2,3-dihydro-5,6-dihydroxyindole-2-carboxylate) into dopachrome.

The integration of data from different sources can be very helpful in understanding the mechanism, the geometry, the kinematic, and the area affected by complex instabilities, especially when the available geotechnical information is limited. In this work, the suitability of different techniques for the study of a deep-seated landslide affecting a bridge in Alcoy (Spain) is evaluated. This infrastructure presents such severe damage that has rendered the bridge unusable, which prevents normal access to an important industrial area. Differential SAR Interferometry (DInSAR) and terrestrial Light Detection and Ranging (LiDAR) remote sensing techniques have been combined with ground displacement monitoring techniques, such as inclinometers and conventional geological and geotechnical investigation, electrical-seismic tomography, damage, and topographic surveys, to determine the boundaries, mechanism, and kinematics of the landslide. The successful case study that is illustrated in this work highlights the potential and the need for integrating multi-source data for the optimal management of complex landslides and the effective design of remedial measurements. El espectro de las aves cazadas cambia a lo largo de los tres niveles arqueológicos determinados durante la excavación, seguramente debido al calentamiento climático del Alleröd y a sus conse- cuencias medioambientales. La observación de los huesos con una lupa binocular ha puesto de manifiesto la existencia de numerosas marcas de carnicería y de cocción. Estas últimas sirven para la reconstrucción de las cadenas operativas del tratamiento puesto en práctica por los cazadores-recolectores magdaleniense y azilienses. La preparación de las carcasas se muestra minuciosa y codificada. Se puede apreciar una cierta variabilidad de una especie a otra. Se discute el uso de las aves, a lo largo del Tardiglaciar, como recurso alimenticio o técnico (huesos, plumas).; La grotte de Santa Catalina livre une abondante avifaune marine et terrestre principalement accumulée par l’Homme. Au sein des trois niveaux archéologiques définis à la fouille, le spectre chassé évolue sans doute sous l’influence du réchauffement climatique de l’Alleröd et de ses conséquences environnementales. L’observation des surfaces osseuses sous une loupe binoculaire a mis en évidence de nombreuses traces de boucherie et de cuis- son. Ces dernières servent de supports à la reconstitution des chaines opératoires de traitement mises en œuvre par les chasseurs-cueilleurs magdaléniens et aziliens. La préparation des carcasses apparait minutieuse et codifiée. Une variabilité est parfois perceptible d’une espèce à l’autre. L’utilisation, au cours du Tardiglaciaire, des oiseaux comme ressources alimentaire et technique (os, plumes) est discutée.

22. [Integration of the nursing process in the electronic health record in an university hospital].

- Abstract
Aim: To describe the process of implementation of Individualized Care Plan in the Electronic Health Record and its impact on the University Hospital Alcorcón Foundation., Methodology: Working groups of staff nurses who analyzed activities usually performed to create a catalog of diagnoses, outcomes and interventions. A group of referents that refined the catalog to make it manageable was created. A training plan, nursing assessment forms and the Nursing Discharge Report were designed., Results: In February 2016 the new methodology was implemented in inpatient units of adults. Between 74.86 and 88.18% of the patients underwent a care plan with the new methodology. Between 69.41 and 76.25% of patients are discharged with a Nursing Discharge Report accordance with regulations. An increase of 24.1% of patients with Nursing Discharge Report after implantation is observed (P=.000; RR: 1.46; 95% CI 1.36-1.56). A total of 116 nurses has been trained., Conclusions: In the study conditions, the use of nursing taxonomies has generated thinking skills and allowed nurses to issue judgments, ensure quality of care, and implementing interventions with a planned results. The nursing taxonomy and care plan in the Electronic Health Record have increased interprofessional communication to improve continuity of care through improved Nursing Discharge Report., (Copyright © 2017 SECA. - Abstract
Although hybridisation through genome duplication is well known, hybridisation without genome duplication (homoploid hybrid speciation, HHS) is not. Few well-documented cases have been reported. A possible instance of HHS in Medicago prostrata Jacq. was suggested previously, based on only two genes and one individual. We tested whether this species was formed through HHS by sampling eight nuclear loci and 22 individuals, with additional individuals from related species, using gene capture and Illumina sequencing. Phylogenetic inference and coalescent simulations were performed to infer the causes of gene tree incongruence. We found no evidence that phylogenetic differences among M. prostrata individuals were the result of HHS. Instead, an autopolyploid origin of tetraploids with introgression from tetraploids of the M. sativa complex is likely. We argue that tetraploid M. prostrata individuals constitute a new species, characterised by a partially non-overlapping distribution and distinctive alleles (from the M. sativa complex). No gene flow from tetraploid to diploid M. prostrata is apparent, suggesting partial reproductive isolation. Thus, speciation via autopolyploidy appears to have been reinforced by introgression. This raises the intriguing possibility that introgressed alleles may be responsible for the increased range exploited by tetraploid M. prostrata with respect to that of the diploids., (Copyright © 2016 Elsevier Inc. - Abstract
A woman, thyroidectomised because of a thyroid papillary carcinoma, interrupted temporarily her levothyroxine intake in order to be subjected to an extension study five weeks later. To minimise her symptoms for the first three weeks, a treatment was prescribed consisting of one 25 micro g-capsule of triiodothyronine every 8 hours. Nine days later she complained of abdominal pain, nausea, vomiting, fever of 40 degrees C and chest discomfort. A serum total triiodothyronine of 575.2 nmol/l was measured by chemoluminiscent immunoassay eleven hours after the intake of the latest capsule (normal level: 1.1-2.9 nmol/l). Along the following ten days the patient suffered from delirium, agitation, tachycardia, hypertension, constipation and later diarrhoea, but neither arrythmias nor axillary temperature over 38 degrees C. Fifty-nine measurements of the serum total triiodothyronine were performed in order to determine the kinetics of elimination of this drug. We estimate that the maximal serum concentration after the intake of the latest capsule could be 794.3 nmol/l, i.e. 397 times higher than the mean normal value. The elimination half-life was 24 hours 40 minutes. The charcoal haemoperfusion had no impact on the velocity of elimination. The concentration of triiodothyronine became normal 200 hours after the intake of the latest capsule, but the clinical manifestations still lasted three days more. The pharmacokinetic data suggest that this intoxication could be due to the intake of capsules containing 5 mg of triiodothyronine, i.e. a dose 200 times higher than that prescribed by her physician.

- Abstract
Introduction: The neuroblastoma (NB) is one of the most common pediatric malignant neoplasms. The most commonly used tumor markers in the diagnosis and follow-up of this tumor are the serum neuron-specific enolase (NSE), ferritin and lactic dehydrogenase and urinary vanillymandelic and homovanillic acid. The common imaging modalities are CT, MRI and 123I or 131I-meta-iodobenzylguanidine scintigraphy., Aim: The aim of this study is to assess the value of 123I-meta-iodobenzylguanidine (MIBG) scintigraphy and serum determinations of NSE and ferritin in the diagnosis and evolution of NB patients., Material and Methods: 20 patients (8 female, 12 male) whose ages ranged from 2 months to 9 years with a mean age of 2.64 years diagnosed of NB. 47 123I-MIBG scans, 47 NSE determinations and 47 ferritin ones were selected., Results: At the time of diagnosis, 100% of the 123I-MIBG scans were positive. 65% of NSE determinations presented clearly pathological levels and 15% were very near to the cut-off point. Only 45% of the ferritin levels were increased. The differences between the lesions visible by 123I-MIBG scanning before and 3 months after treatment as well as NSE and ferritin levels were studied. When the Student's T test was applied, we found statistically significant pre and post-treatment differences in 123I-MIBG scanning and NSE. In the case of ferritin, there was no statistical significance in spite of the decrease in the values. The direct correlation and Spearman correlation between laboratory data and 123I-MIBG scanning as well as correlation between NSE and ferritin were also studied. There was a good correlation between 123I-MIBG and NSE and between NSE and ferritin. We have also studied the data in 7 relapses., Conclusions: 123I-MIBG scintigraphy and serum determination of NSE are two successful diagnostic tools for the diagnosis and evolution of NB patients.

Introduction: Retinoblastomas (RB) are the most common malignant intraocular tumors in childhood. The neuron specific enolase (NSE) is used as a tumor marker in different malignant diseases (mainly in neuroblastomas and lung carcinoma of small cells). In this work we report our experience using NSE and the role we consider that it plays in the determination of this enzyme in RB cases., Materials and Methods: NSE determinations in aqueous humor (AH) and serum were carried out in 17 RB by means of a radioimmunometric assay and were compared with those values obtained in a control group composed of 8 subjects in whom the RB diagnosis as well as other diseases that could alter its determination had been ruled out. The possible correlation between NSE levels and different tumor stages (Reese-Elsworth and histological-clinical), as well as the tumor type (exophytic and mixed) were also studied., Results: NSE levels were increased in AH and serum in 88% of RB patients and were normal in all the subjects in the control group (when a sample which was considered to be non-valid due to the presence of red cells in AH was excluded). No correlation between NSE levels in AH and serum and the different tumor stages was found. Nevertheless, the RB group of mixed tumor type showed a significantly higher mean in AH (p<0.01) than the RB exophytic group., Conclusion: NSE determination in AH is of value in the differential diagnosis in patients in whom RB is suspected but difficult to diagnose as well as in the diagnosis of relapses. Serum determinations are mainly useful in the follow-up and in the early diagnosis of possible metastases.

- Abstract
An HPLC method for the simultaneous determination of safrole (S), dihydrosafrole (DHS), and chloromethyldihydrosafrole (CM-DHS) in piperonyl butoxide, with fluorimetric detection and elution gradient, is described. Samples with internal standard (piperonyl isobutyrate) are adsorbed on silica cartridges, then eluted with an appropriate solvent. Internal standard, S, DHS, and CMDHS are eluted together and injected into a reversed-phase chromatography column. The concentrations of the three products are calculated from calibration graphs, and linearity and reproducibility are verified. The limit of detection is 2 mg.kg-1. This analytical method allows for control of the synthesis process of piperonyl butoxide and determination of some carcinogenic substances, such as S and DHS.
