Background: To evaluate the ability of therapeutic intensity score (TIS) in predicting the clinical outcomes of partial (PA) and total adrenalectomy (TA) for UPA. Methods: Between 2011 and 2022, a four-center adrenalectomy dataset was queried for "unilateral adrenal mass" and "UPA" (n = 90). Preoperative TIS of each antihypertensive medication were individually calculated and merged to create a single, cumulative variable. Probability of complete clinical, partial, and absent pooled success rates according to TIS were assessed for the overall cohort by Kaplan–Meier. Cox analyses were used to identify predictors of complete clinical and partial/absent success, respectively. For all analyses, a two-sided p < 0.05 was considered significant. Results: At a median follow-up of 42 months (IQR 27–54) complete partial, and absent clinical success were observed in 60%, 17.7%, and 22.3%, respectively. On Kaplan–Meier analysis, TIS < 1 predicted higher complete success rates (p < 0.001), while TIS ≥ 1 was predictor of either partial and absent clinical success (p = 0.008). On multivariable analysis, TIS < 1 (HR 0.25; 95% CI 0.11–0.57; p = 0.001) and adenoma size (HR 1.11; 95% CI 1–1.23; p = 0.0049) were independent predictors of complete clinical success, while TIS ≥ 1 (HR 2.84; 95% CI 1.32–6.1; p = 0.007) was the only independent predictor of absent clinical success. Conclusions: TIS score and adenoma size may help to identify patients who are likely to be at risk of persistent hypertension after surgery. [ABSTRACT FROM AUTHOR]

Objective: Primary aldosteronism (PA) is the most common cause of endocrine hypertension and adrenalectomy is the firstline treatment for unilateral PA. Suppression of aldosterone secretion of the nondominant adrenal gland at adrenal venous sampling (AVS), that is, contralateral suppression (CLS) has been suggested as a marker of disease severity. However, whether factors such as CLS, age, gender or comorbidities are associated with remission after surgery is controversial. The objective of this study is to investigate the prognostic value of CLS, age, gender, aldosterone‐to‐renin ratio, antihypertensives and comorbidities for clinical and biochemical remission following unilateral adrenalectomy in patients with PA. Design and patients: A retrospective study of patients with PA referred for AVS at Rigshospitalet from May 2011 to September 2020, who subsequently underwent adrenalectomy. Clinical remission was defined according to the PA surgical outcome criteria, whereas complete biochemical remission was defined as normalization of hypokalaemia without potassium substitution. Results: Eighty‐four patients were available for analysis of primary outcome. Among patients with CLS, 28/58 (48.3%) obtained complete clinical remission after surgery compared with 10/26 (38.5%) without CLS (p =.40). Complete biochemical remission was obtained in 55/58 (94.8%) of patients with CLS compared with 25/28 (89.3%) without CLS (p =.44). Female gender and lower number of antihypertensives at baseline were associated with higher odds for complete clinical remission, whereas none of the investigated variables were associated with biochemical remission. Conclusion: CLS was not significantly associated with complete clinical or biochemical remission in this cohort. Our results confirmed that female gender and lower number of antihypertensives were predictors of clinical remission. [ABSTRACT FROM AUTHOR]

This thesis sets out to investigate the development of the recent historical novel in Mexico by examining a corpus of five novels. Elena Garro’s 'Los recuerdos del porvenir' (1963) represents the final point of the novel of the Revolution and it is the link with the recent historical novel. Carlos Fuentes’ 'Terra Nostra' (1975) and Fernando del Paso’s 'Noticias del Imperio' (1978) belong to the group containing the postmodern historical novel. 'Terra Nostra' summarizes all the concerns of postmodernism and can be considered as a paradigm of this current of thought. 'Noticias del Imperio' seeks a reconciliation between history and literature in an attempt to get closer to the historical truth. Paco Ignacio Taibo II’s 'La lejanía del Tesoro' (1992) is a representative novel in the way it melds history with the mystery novel, developing the genre of the historical thriller. Lastly, Rosa Beltrán’s 'La corte de los ilusos' (1995), and in particular its treatment of history is pertinent to this thesis due to the fact that women have been traditionally silenced by official history. This novel gives them a voice. From its beginnings, the historical novel confronted the problem of being questioned for its lack of accuracy when dealing with the past. This skepticism sparked a long lasting debate that initially degraded the historical novel as secondary genre that could never contribute to historical knowledge. However, as a result of recent theories that seek to defend the poetic nature of history, a theory developed initially by Hayden White, the recent historical novel has sought to debunk historiography’s claim to be the only possible way to recount the past. This thesis advances the theory that the recent historical novel in Mexico is the result of a search for a genuine identity, as well as a quest to develop an alternative, yet truthful, interpretation of a past whose true nature has been distorted by decades of historical officialdom. This process is seen in a context of increasing democratisation and globalisation.

Gracias al análisis planteado en el artículo, se argumenta que las medidas a corto plazo son la solución, sino una política sostenible de la misma manera que las inversiones de recursos sin objetivos claramente marcados no pueden mejorar el sistema de resocialización en Bogotá y Colombia Public policies are the ways in which the state intervenes in society, in order to meet its objectives and satisfy the needs of the civilian population, one of these public policies is that of prison resocialization, which has as its objective the reintegration into society of the people who have committed any contravention or crime. In Colombia, this public policy is materialized through the PASO system, whose objective is reintegration into civil life. This system has a large number of shortcomings, which results in the systematic violation of the human rights of persons deprived of freedom. The main objective of the article is to explain the shortcomings of the PASO system and how the recent governments of this country have done nothing more than try to solve the crises that have occurred within the prison system with short-term measures. Taking advantage of the methodological advantages of the descriptive qualitative research model, which will open a range of possibilities to explain the problems of the PASO system, in the same way the collection of information on how the PASO system is applied in the city of Bogotá. Thanks to the analysis proposed in the article, it is argued that short-term measures are the solution, but rather a sustainable policy, in the same way that resource investments without clearly defined objectives cannot improve the resocialization system in Bogotá and Colombia. - Abstract
The Pie de Vaca locality in the State of Puebla stands out for its important abundance of fossil mammal footprints, among them, the most representatives have been referred to camelids and felids. The purpose of the present study was the formal characterization of this set of footprints and comment on some paleobiological aspects related to their size, speed, and mode of progression of the track-makers. The sample consists of 233 footprints including 154 referable to nine trackways produced by camelids and 79 referable to three trackways produced by felids. The characterization of the tracks was made by comparing their size and morphology with others produced by fossil and recent taxa. The impressions of camelids, based on their shape and size, were designated to the icnospecies Lamaichnum guanicoe because they show the typical morphological pattern of this group of artiodactyls. It is suggested that they were produced by some member of the genus Hemiauchenia, which corresponds to the most common camelid of the Late Cenozoic of Mexico. On the other hand, the tracks of felids were only referred to the morphofamily Felipedidae due to the bad preservation of its ichnotaxonomic characters; however, being larger than traces of the ichnogenera Felipeda, Pycnodactylopus, Pumaeichnum and Mitsupes, it is suggested that they were produced by some form of medium to large size, a machairodontid or a pantherine, for instance. The mode of progression of both producers corresponds to that of relatively fast walking organisms that move at a speed lower than 4 m/s. The direction and number of individuals of camelids trackways, are indicative of gregarious behavior, probably associated with a certain social organization. In the case of felids, solitary or couple behavior is proposed. [ABSTRACT FROM AUTHOR]

Background Ablation of cardiac arrhythmias in children and teenagers often necessitates the use of anesthesia, which can suppress ventricular arrhythmias (VAs), making it difficult to map the site of origin using activation time (AT). Pace mapping, a technique employed to assist with VA origin localization, depends on subjective comparison of paced and targeted QRS morphology. We assessed the utility of a quantitative approach to paced QRS to VA morphology matching using the PaSo software (Carto 3, Biosense Webster), to localize the VA site of origin. Methods Twenty-four patients underwent 26 procedures for frequent VAs, 29 for targeted VA. If AT mapping was precluded due to infrequent VA, pace mapping was executed using the PaSo software, after regionalization based on targeted VA QRS morphology. Results Subjects were aged 1-32 (mean 14 ± 6) years; 10 were male. Heart disease was present in six patients. PVC frequency prior to onset of anesthesia was 15 ± 16/min, decreasing to 0-1 PVC/min in 17 cases prior to ablation. Arrhythmia localization was performed by AT mapping + PaSo (12) or PaSo only (17). Pace mapping exhibited an intraventricular gradient of percent QRS morphology match. Highest achieved QRS match averaged 96 ± 2%. Successful ablation (> 1-month follow-up) was achieved in 24/29 targeted VAs, 11/12 ablated using AT and pace mapping, and 13/17 VA ablated using pace mapping only, P = 0.29. Conclusions (1) Spontaneous VA frequency was markedly reduced following anesthesia, despite catecholamine administration. (2) Notwithstanding the ability to perform AT mapping, successful ablation can still be performed using pace mapping only, facilitated by the PaSo software. [ABSTRACT FROM AUTHOR]

Assuming that the current Castilian language is defined as one of the Romance languages with the weakest linguistic resources to express the crossing of borders, it will be necessary to identify the modes of expression of the passage in ancient Castilian (nouns, verbs, prepositions, phrases, verbal tenses...) and to follow its permanence and evolution in a corpus of varied texts, related to the theme of travel, and which extend from the 13th to the 15th century. How was the passage in ancient Castilian said? From which morphosyntactic and lexical strategies? What representations of spatial displacement does the language portray? How does the verbalization of the passage in ancient Castilian involve a certain conceptualization of the dynamics of the world and space-time?

The question of passage to China refers here to the establishment of Christian missionaries in the Middle Kingdom in order to obtain its conversion in the last third of XVIth century. At that time, the West rediscovered China thanks to Iberian oceanic navigation around the world. Europeans and first Iberians started realizing that China with whom direct relations were interrupted since mid XIVth, was the widest, the most populated and the wealthiest realm in the world of their time. After the conquest of the Aztec Confederation in the 1520’s and of Inca Empire in the 1530’s, what kind of contact the Spanish could establish with that major power on their western frontier? This paper aims to study the first mission attempted from Spanish franciscans in Chinese territory from the colonial Manila, in 1579, through the augustinian Juan González de Mendoza’s relation in Book II of the second part of his famous Historia del gran reino de la China, published in Roma in 1585. This mission had to face the Chinese isolationism and xenophobia of that time. We will question, in an historical and philological perspective, the meaning of Juan González de Mendoza’s transcription-rewriting of Friar Agustín de Tordesillas’s ocular testimony. Tordesillas was Pedro de Alfaro’s companion. Then we will study the complex notion of entrada used by the missionaries to qualify their project, together with the favorable and adverse factors to their « passage ». Finally we will highlight the crucial question of the Chinese interpreters who were negociating the mission settling, at the crossroads between two different cultural worlds and their personal interests.

Background: To propose a trifecta that summarizes endpoints and predicts their maintenance after adrenalectomy (n = 90) for unilateral primary aldosteronism (UPA). Methods: Trifecta was defined as coexistence of: ≥50% antihypertensive therapeutic intensity score reduction (∆TIS), no hypokalemia at 3 months, and no Clavien grade 2–5. Logistic regression was used to identify predictors of trifecta. Probability of clinical, biochemical, and simultaneous success according to trifecta were assessed by Kaplan–Meier. Cox regression was used to identify predictors of long-term clinical, biochemical, and simultaneous success. For all analyses, a two-sided p < 0.05 was considered significant. Results: Simultaneous success rate was 50%. On multivariable analysis, TIS was an independent predictor of trifecta achievement (HR 3.28; 95% CI 1.07–10.9; p = 0.03). At Kaplan–Meier, trifecta predicted higher success for all endpoints (each p < 0.03). On multivariable Cox analysis, adenoma size (AS) ≥6 cm and trifecta were independent predictors of biochemical (AS: HR 2.87; 95% CI 1.53–5.36; trifecta: HR 2.1; 95% CI 1.13–3.90; each p < 0.02) and simultaneous success (AS: HR 3.81; 95% CI 1.68–8.65; trifecta: HR 4.29; 95% CI 2.08–8.86; each p < 0.01), while trifecta was an independent predictor of complete clinical success (HR 2.84; 95% CI 1.45–5.58; p < 0.01). Conclusions: Trifecta and AS are independent predictors of either long-term complete clinical, biochemical, or combined success after adrenalectomy for UPA.

[EN] The final degree project has been based on the concern that caused me the lack of knowledge of the origin of the festivities of the patron saint of my locality, Albal. The project is based on carrying out research on the origins of the feast of Santa Anna, a study on the parish church of Nostra Senyora dels Ànagels, as well as a comprehensive in situ study, restoration proposal and preventive conservation plan of the Santa Anna Pass. The passage is composed by a throne on which four images rest. The carvings and the passage, present multiple damages which has been appered along the years, processions and the bad manipulation by the brotherhood and for not being in a place where the environmental conditions are optimal for their conservation. Due to the fire in the local archive in the years 50, no information regarding this step is available, so the information that could be collected has been through interviews with the elderly persons in the locality.

- Abstract
[Abstract] Background: The effect of Leg-Length Discrepancy (LLD) on dynamic gait parameters has been extensively discussed. Podobarography is the study of foot-to-ground pressure distribution. It has been used to test plantar footprint deviations that could reveal pathology. Purpose: The aim of this study is to determine the effects of simulated LLD on dynamic gait parameters measured with a pressure platform in healthy subjects. Methods: Thirty-seven healthy subjects participated in observational cross-sectional research. A procedure was performed to capture the dynamic parameters of each participant under five different simulated LLD conditions. Support time, mean pressure, and peak pressure measures were registered on three trials for each foot and LLD level per session. An analysis of variance (ANOVA) test for repeated measures was performed to check for differences between the different simulated LLD levels. Results: The stance time of the short leg had no significant changes. The stance time of the long leg increased by 3.51% (p < 0.001), mean pressure of the short leg increased by 1.23% (p = 0.005), and decreased by 5.89% in the long leg (p < 0.001). Peak pressure of the short leg decreased by 2.58% (p = 0.031) and the long leg decreased by 12.11% (p < 0.001). Conclusions: This study demonstrates that increasing LLD causes an asymmetrical foot-loading pattern, with decreased mean and peak pressure on the longer limb, and consequently an overload on the short side. Furthermore, an increasing LLD causes increased stance time on the long leg.

- Abstract
[Abstract] Reduced plantar foot sensation, postural instability, and gait difficulties are characteristic of Parkinson´s disease patients. A systematic review was carried out to determine the effect of the different types of insoles and shoes in these patients. Several databases were used to search for relevant articles reporting Parkinson´s disease patients undergoing treatment with any type of insole and footwear. All titles and abstracts were reviewed independently by two reviewers and the available data were extracted. The study eligibility criteria were any type of experimental study that included Parkinson’s disease patients treated with any type of insole or footwear. Eight studies were selected. Interventions used were textured insoles, footwear modifications, and habitual footwear. Three different outcomes were evaluated in each study: gait parameters, balance, and plantar sensation. According to the data available from this systematic review, the most important conclusion is that more controlled studies are needed in this research field. There are indications to suggest that textured insoles have positive effects on gait parameters, balance, and plantar sensation in Parkinson’s disease patients.

- Abstract
[Abstract] BACKGROUND: Leg length discrepancy (LLD) may play a key role in exercise biomechanics. Although the Podoprint platform has been used in dynamic pressure studies, there are no data regarding the reliability and repeatability of dynamic measurements under simulated LLD conditions. OBJECTIVES: To determine the intra and intersession repeatability and reliability of dynamic parameters of the Podoprint pressure platform under simulated LLD conditions. DESIGN AND SETTING: Observational cross-sectional study at a public university. METHODS: Thirty-seven healthy volunteers participated in this study. LLD was simulated using ethyl vinyl acetate plantar lifts with heights of 5 mm, 10 mm, 15 mm and 20 mm located under the right shoe of each volunteer. The procedure was performed to capture the dynamic parameters of each participant under five different simulated LLD conditions. Stance time, mean pressure and peak pressure measurements were registered in three trials for each foot and each LLD level. Data were collected during two separate testing sessions, in order to establish intrasession and intersession reliability. RESULTS: The intraclass correlation coefficients (ICCs) for intrasession reliability ranged from 0.775 to 0.983 in the first session and from 0.860 to 0.985 in the second session. The ICCs for intersession reliability ranged from 0.909 to 0.990. Bland-Altman plots showed absence of systematic measurement errors. CONCLUSIONS: The results from this study indicate that the Podoprint platform is a reliable system for assessing dynamic parameters under simulated LLD conditions. Future studies should evaluate plantar pressures under LLD conditions, in association with exercise, biomechanics and musculoskeletal disorders.

Stroke is one of the main causes of disability. Telerehabilitation could face the growing demand and a good strategy for post-stroke rehabilitation. The aim of this study is to examine the possible effects of therapeutic exercises performed by an App on trunk control, balance, and gait in stroke survivors. A preliminary 12-week randomized controlled trial was developed. Thirty chronic stroke survivors were randomly allocated into two groups. Both groups performed conventional physiotherapy, in addition to, the experimental group (EG) had access to a telerehabilitation App to guide home-based core-stability exercises (CSE). Trunk performance was measured with the Spanish-Trunk Impairment Scale (S-TIS 2.0) and Spanish-Function in Sitting Test. Balance and gait were measured with Spanish-Postural Assessment Scale for Stroke patient, Berg Balance Scale and an accelerometer system. In EG was observed an improvement of 2.76 points in S-TIS 2.0 (p = 0.001). Small differences were observed in balance and gait. Adherence to the use of the App was low. CSE guided by a telerehabilitation App, combined with conventional physiotherapy, seem to improve trunk function and sitting balance in chronic post-stroke. Active participation in the rehabilitation process should be increased among stroke survivors. Further confirmatory studies are necessary with a large sample size. info:eu-repo/semantics/publishedVersion

International audience; Smart Contracts are computer programs which implement and execute transactions and manage business logic on a decentralized public ledger. Smart Contracts can be written in different programming languages and for different Blockchains. Currently the most used language for Smart Contracts is Solidity and the most used platform is the Ethereum Blockchain. Assessing the quality of Smart Contract programs is an important task required to professional programmers, especially when a programming language has so powerful economic implications. It is therefore crucial to provide professional programmers with tools for the evaluation of Smart Contracts. In software engineering, software metrics has been defined and used to measure software quality and, more in general, to qualify software under the principle "You Can't Manage What You Don't Measure". For the Solidity programming language there are only a few Standalone Applications to analyse the Smart Contract metrics. The aim of this paper is first to build a tool for the practical computation of a specific set of Solidity source code metrics, so that the set will be extensible in the future according also to Solidity compiler evolution, second to fully enable a webbased usage of the tool to access the metrics of the Solidity programming language. The tool, PASO, differently from the existing application, is able to give software metrics values for Smart Contracts written in Solidity programming language just using a web browser.

Background: This systematic review and metanalysis was conducted to assess differences between perioperative and functional outcomes in patients undergoing minimally-invasive partial (mi-PA) and total adrenalectomy (mi-TA) for unilateral primary aldosteronism (uPHA). Material and Methods: Multiple scientific databases (PUBMED, Web of Science, and Cochrane Library) were searched up to November 2021 for surgical series comparing mi-PA vs. mi-TA for uPHA according to the PRISMA statement. Primary outcomes of interest were perioperative and functional outcomes. Results: Overall, a total of 802 patients from six eligible studies were identified, with mi-PA and mi-TA performed in 40.4% (n = 324) and 59.6% (n = 478) of cases, respectively. No differences were recorded between the two groups according to number of transfusions, EBL and Clavien–Dindo complications ≥2. Similarly, no differences in clinical success, persistence of postoperative hypokalemia and improvement in HTN were reported between mi-PA and mi-TA. Conclusions: In a uPHA setting, mi-PA and mi-TA provide comparable perioperative and functional outcomes despite the use of mi-PA remains limited to patients with small adenoma size, or hereditary/bilateral disease. Due to limited use of standardized reporting criteria in most of current series, the quest for a superiority of mi-PA over mi-TA in the treatment of uPHA still remains open. [ABSTRACT FROM AUTHOR]

[ABSTRACT FROM AUTHOR]

International audience; This article examines the critical reception of the film adaptation of West Side Story (Jerome Robbins and Robert Wise, 1961) in Spanish culture and cinematography. It looks into two other screen versions of Romeo and Juliet: Los Tarantos (Francesc Rovira-Beleta, 1963) and No somos ni Romeo ni Julieta (Alfonso Paso, 1969). West Side Story remains in the Spanish cultural memory as a filmic event associated with the arrival of foreign influence amidst national-catholic dictatorship. I will use the variables “place” and “space,” which distinguish, on one hand, proper orders established by authority and, on the other hand, areas transformable by interactions and changes, and will refer to Shakespeare’s presentist criticism, French cultural theory and French Film Theory as lenses. I will examine West Side Story’s 1963 Spanish reception material as well as its possible interactions with the webwork of cultural forces in operation amidst the 1960s Spanish culture. Then, I will examine the mise en scenes of Los Tarantos and No somos ni Romeo ni Julieta focusing on the forms in which characters, places and camerawork contribute to create spaces, i.e. places permeable to change and transformation through human interaction.

Falls in the elderly are one of the main geriatric syndromes and a clear indicator of fragility in the older adult population. This has serious consequences, leading to an increase in disability, institutionalization and death. The purpose of this cross-sectional study was to analyze the prevalence of risk of falling and associated factors in a population of 213 non-institutionalised, able older adults with a history of falling in the previous year. We used the following assessment tools: Questionnaire of the WHO for the study of falls in the elderly, Geriatric Depression Scale and Tinetti’s Gait and Balance Assessment Tool. Age, using ambulatory assistive devices, polymedication, hospital admission following a fall and depression were significantly associated with risk of falling. In order to prevent fall reoccurrence, community-based fall prevention programs should be implemented.

This article seeks to understand the design of the PASO (open, simultaneous and compulsory primaries) which were introduced within the Peruvian political reform in 2019. First, we trace the descriptive characteristics of the Peruvian PASO and determine that the design of the instrument is part of a process of policy transfer. We then approach the logic of its design transfer through the available evidence on the instrument and the actors that worked on its domestic adaptation. A bibliographic and archival review was conducted, and interviews to a lesser extent. We conclude that the design of the Peruvian PASO takes the Argentinian formula as a reference, adding several adjustments to it. Moreover, it highlights the central role of an interested actor in the design of the instrument, a role that was eased thanks to a tense and urgent political environment.

- Abstract
Problem: In the World Athletics Championships, the racewalkers participate with the highest level of technical execution and with the best physical conditions, so the analysis of the steps of a racewalker is key to obtain an optimal result. Objective: To analyze the technique of an elite racewalker and the behavior of the variables of the athletic racewalking, pre-competition to the World Athletics Championships. Method: A test on an electronic track was given to one athlete, evaluating the kinematic variables: step length, step frequency, flight time, and speed. ANOVA was used to determine significant differences (p ≤0.05). Result: In the three tests carried out, all the variables increased significantly, although the most significant difference was found between flight time and step frequency p = 0.0000001, p <0.05. Conclusion: Since the flight time increases rapidly, it must be careful that this not influence the technical execution of the racewalkers, to be more effective, and thus avoid a warning or disqualification., Problema: No mundial, os marchistas participam com o maior nível de execução técnica e com as melhores condições físicas, é por isso que a análise dos passos de um marchista é a chave para a obtenção de melhores resultados.
