2. Immunosuppressive therapy after solid-organ transplantation: does the INTERMED identify patients at risk of poor adherence?

Abstract

Background: Lack of adherence to medication is a trigger of graft rejection in solid-organ transplant (SOT) recipients. Objective: This exploratory study aimed to assess whether a biopsychosocial evaluation using the INTERMED instrument before transplantation could identify SOT recipients at risk of suboptimal post-transplantation adherence to immunosuppressant drugs. We hypothesized that complex patients (INTERMED>20) might have lower medication adherence than noncomplex patients (INTERMED≤20). Methods: Each patient eligible for transplantation at the University Hospital of Lausanne, Switzerland, has to undergo a pre-transplantation psychiatric evaluation. In this context the patient was asked to participate in our study. The INTERMED was completed pre-transplantation, and adherence to immunosuppressive medication was monitored post-transplantation by electronic monitors for 12 months. The main outcome measure was the implementation and persistence to two calcineurin inhibitors, cyclosporine and tacrolimus, according to the dichotomized INTERMED score (>20 or ≤20). Results: Among the 50 SOT recipients who completed the INTERMED, 32 entered the study. The complex (N=11) and noncomplex patients (N=21) were similar in terms of age, sex and transplanted organ. Implementation was 94.2% in noncomplex patients versus 87.8% in complex patients (non-significant p-value). Five patients were lost to follow-up: one was non-persistent, and four refused electronic monitoring. Of the four patients who refused monitoring, two were complex and withdrew early, and two were noncomplex and withdrew later in the study. Conclusion: Patients identified as complex pre-transplant by the INTERMED tended to deviate from their immunosuppressant regimen, but the findings were not statistically significant. Larger studies are needed to evaluate this association further, as well as the appropriateness of using a nonspecific biopsychosocial instrument such as INTERMED in highly morbid patients who have complex social and psychological characteristics.

Abstract

The lakes of the basaltic plateaus of Patagonia represent the only reproductive habitat for the critically endangered Hooded Grebe (Podiceps gallardoi). More than 50 of these naturally fishless lakes have been stocked with the exotic Rainbow Trout (Oncorhynchus mykiss). While the available information suggests that the diet composition of trout and Hooded Grebes could overlap, to date no formal studies of the diet of Rainbow Trout in these aquatic environments have been published. We characterized the diet of trout in one of the primary reproductive areas of the Hooded Grebe based on the analysis of the gut contents of 177 individuals, from 9 lakes. Amphipods were clearly the most dominant group of prey, quantified as frequency of occurrence and proportion by weight. The condition factor of trout and the abundance of amphipods in the stomach contents showed a functional relationship, suggesting that the wellness of trout depends on the availability of this prey. Amphipods also represent a primary prey for the Hooded Grebe, suggesting a strong dietary overlap with the trout. The presence of trout may generate processes of competition that could affect the survival of the Hooded Grebe. This interaction may be particularly relevant in large vegetated lakes, where the Hooded Grebe reproduces.

10. Washback of Spanish University Entrance Examination on Chemistry Teaching in Upper Secondary Education

Abstract

The washback effect is particularly relevant when it comes to high-stakes tests, such as university entrance exams. This article presents a research study performed with the aim of determining the current influence of these exams on the teaching of chemistry in upper secondary education. The study was based on a questionnaire distributed among chemistry teachers --447 participants-- from across Spain. The results show that the chemistry content taught in upper secondary education is closer to what is requested in university entrance exams than to what teachers themselves believe to be important for students to learn. For the aspects examined, significant differences between regions were observed; in fact, relevant changes in the curriculum taught and the teaching methodology were apparent in those regions that incorporated content in their tests which was distinct from the traditional approach. Based on the findings, some implications and suggestions are drawn for future university entrance exams.

Abstract

Objectives: To evaluate the ability of the Assessment of Different NEoplasias in the adneXa (ADNEX) model and the International Ovarian Tumour Analysis (IOTA) two‐step strategy to predict malignancy in adnexal masses detected in an outpatient low‐risk setting, and to estimate the risk of complications in masses with benign ultrasound morphology managed using clinical and ultrasound follow‐up. Methods: This single‐center study was performed at Hospital Universitari Dexeus, Barcelona, Spain, using interim data from the ongoing prospective observational IOTA Phase‐5 (IOTA5) study. The primary aim of the IOTA5 study is to describe the cumulative incidence of complications during follow‐up of adnexal masses classified as benign on ultrasound examination. Consecutive patients with an adnexal mass detected between June 2012 and September 2016 in a private center offering screening for gynecological cancer were included and followed up until February 2020. Tumors were classified as benign or malignant based on histology (if patients underwent surgery) or the outcome of clinical and ultrasound follow‐up at 12 (range, 10–14) months. Multiple imputation was used when outcomes were uncertain. The ability of the ADNEX model without CA125 and of the IOTA two‐step strategy to distinguish benign from malignant masses was evaluated retrospectively using the prospectively collected data. We assessed performance with regard to discrimination (area under the receiver‐operating‐characteristics curve (AUC)), calibration, classification (sensitivity and specificity) and clinical utility (Net Benefit). In the group of patients with a mass judged to be benign who were selected for conservative management, we evaluated the occurrence of spontaneous resolution or any mass complication during the first 5 years of follow‐up by assessing the cumulative incidence of malignancy, torsion, cyst rupture and minor mass complications (inflammation, infection or adhesions) and the time to occurrence of an event. Results: A total of 2654 patients were recruited to the study. After application of exclusion criteria, 2039 patients with a newly detected mass were included for the model validation. Of those, 1684 (83%) masses were benign, 49 (2%) masses were malignant and, for 306 (15%) masses, the outcome was uncertain and therefore imputed. The AUC was 0.95 (95% CI, 0.89–0.98) for ADNEX without CA125 and 0.94 (95% CI, 0.88–0.97) for the two‐step strategy. Calibration performance could not be meaningfully interpreted because the small number of malignancies resulted in very wide confidence intervals. The two‐step strategy had better clinical utility than did the ADNEX model at malignancy risk thresholds < 3%. There were 1472 (72%) patients whose mass was judged to be benign based on pattern recognition by an experienced ultrasound examiner and were managed with clinical and ultrasound follow‐up. In this group, the 5‐year cumulative incidence was 66% (95% CI, 63–69%) for spontaneous resolution of the mass, 0% (95% CI, 0–0.2%) for torsion, 0.1% (95% CI, < 0.1–0.4%) for cyst rupture, 0.2% (95% CI, 0.1–0.6%) for a borderline tumor and 0.2% (95% CI, 0.1–0.6%) for invasive malignancy. Abstract

Due to the growing popularity of optical superchannels and software defined networking, reconfigurable optical add-drop multiplexer (ROADM) architectures for superchannel switching have recently attracted significant attention. ROADMs based on micro electro-mechanical system (MEMS) and liquid crystal-on-silicon (LCoS) technologies are predominantly used. Motivated by requirements for low power, high-speed, small area footprint and compact switching solutions, we propose and demonstrate spatial and wavelength flexible superchannel switching using monolithically integrated silicon photonics (SiP) micro-ring resonators (MRR). We demonstrate the MRRs capabilities and potential to be used as a fundamental building block in ROADMs. Unicast and multicast switching operation of an entire superchannel is demonstrated after transmission over 50 km of standard single mode fiber. The performance of each sub-channel from the 120 Gb/s QPSK Nyquist superchannel is analyzed and degradation in error vector magnitude performance was observed for outer sub-channels due to the 3-dB bandwidth of the MRRs, which is comparable with the superchannel bandwidth. However, all sub-channels for all switching cases (unicast, multicast and bi-directional operation) exhibit performance far below the 7% FEC limit. The switching time of the SiP MRR chip is such that high capacity superchannel interconnects between users can be setup and reconfigured on the microsecond timescale.

Abstract

In this work the strategy of the Latin American Giant Observatory (LAGO) to build a Latin American collaboration is presented. Installing Cosmic Rays detectors settled all around the Continent, from Mexico to the Antarctica, this collaboration is forming a community that embraces both high energy physicist and computer scientists. This is so because the data that are measured must be analytical processed and due to the fact that \textit{a priori} and \textit{a posteriori} simulations representing the effects of the radiation must be performed. To perform the calculi, customized codes have been implemented by the collaboration. With regard to the huge amount of data emerging from this network of sensors and from the computational simulations performed in a diversity of computing architectures and e-infrastructures, an effort is being carried out to catalog and preserve a vast amount of data produced by the water-Cherenkov Detector network and the complete LAGO simulation workflow that characterize each site. Metadata, Permanent Identifiers and the facilities from the LAGO Data Repository are described in this work jointly with the simulation codes used. Abstract

The Latin American Giant Observatory (LAGO) is an extended cosmic ray observatory composed by a network of water-Cherenkov detectors spanning over different sites located at significantly different altitudes (from sea level up to more than $5000$\,m a.s.l.) and latitudes across Latin America, covering a huge range of geomagnetic rigidity cut-offs and atmospheric absorption/reaction levels. This detection network is designed to measure the temporal evolution of the radiation flux at ground level with extreme detail. The LAGO project is mainly oriented to perform basic research in three branches: high energy phenomena, space weather and atmospheric radiation at ground level. LAGO is built and operated by the LAGO Collaboration, a non-centralized collaborative union of more than 30 institutions from ten countries. 38. Five-Beam Fully Metallic Nolen Matrix-Based Array Antenna for 5G Applications at 26 GHz

Abstract

The communication explores the development of a Nolen matrix (NM) operating at 26 GHz (25.25–26.75 GHz) using E-plane rectangular waveguides (RWs). The Nolen network comprises five coaxial input ports and seven outputs, each linked to a linear array of nine elements. This design yields a compact fully metallic $7 \times 9$ slot array with low profile, high efficiency, and beam-switching capabilities. Experimental results closely align with theoretical and simulated data, demonstrating gain values surpassing 21 dBi and measured total mean efficiencies of 80% across all scenarios. Moreover, the antenna’s steering ranges from −40° to +35°. Given its performance and frequency versatility, potential applications for this antenna include wireless communications, radar systems, satellite communications, and remote sensing.

Abstract

Objectives: To assess the ability of the International Endometrial Tumor Analysis (IETA)-1 polynomial regression model to estimate the risk of endometrial cancer (EC) and other intracavitary uterine pathology in women without abnormal uterine bleeding. Methods: This was a retrospective study, in which we validated the IETA-1 model on the IETA-3 study cohort (n = 1745). The IETA-3 study is a prospective observational multicenter study. It includes women without vaginal bleeding who underwent a standardized transvaginal ultrasound examination in one of seven ultrasound centers between January 2011 and December 2018. The ultrasonography was performed either as part of a routine gynecological examination, during follow-up of non-endometrial pathology, in the work-up before fertility treatment or before treatment for uterine prolapse or ovarian pathology. Ultrasonographic findings were described using IETA terminology and were compared with histology, or with results of clinical and ultrasound follow-up of at least 1 year if endometrial sampling was not performed. The IETA-1 model, which was created using data from patients with abnormal uterine bleeding, predicts four histological outcomes: (1) EC or endometrial intraepithelial neoplasia (EIN); (2) endometrial polyp or intracavitary myoma; (3) proliferative or secretory endometrium, endometritis, or endometrial hyperplasia without atypia; and (4) endometrial atrophy. The predictors in the model are age, body mass index and seven ultrasound variables (visibility of the endometrium, endometrial thickness, color score, cysts in the endometrium, non-uniform echogenicity of the endometrium, presence of a bright edge, presence of a single dominant vessel). Abstract

Nd3+-doped, LaF3-based, transparent oxyfluoride glass-ceramics (GCs) containing Ag nanoparticles (NPs) have been prepared by a melt-quenching method. Different Ag-containing precursors were employed, with AgNO3 as the most suitable for obtaining Ag NPs segregated in the glassy matrix. Transmission electron microscopy and X-ray absorption spectroscopy determined the presence of Ag0 NPs, with higher concentrations in glass-ceramics produced in an inert atmosphere (N2). The spectral features of emission and excitation spectra, in addition to the different lifetimes of the 4 F3/2 state in the co-doped samples with Ag NPs, demonstrate the incorporation of Nd3+ ions into the LaF3 nanocrystals. The relative contribution to the emission from Nd3+ in the nanocrystals depends on the Ag concentration and atmosphere. The glass-ceramic co-doped with 0.2 wt% Ag, thermally treated in N2, exhibits the highest contribution of emissions attributable to Nd3+ ions in the crystalline phase. The crystalline fraction is similar to that of a Ag-free sample synthesized under analogous conditions. This increase of the luminescence is likely attributable to the enhancement of the local electric field in the LaF3 nanocrystals due to the presence of Ag0 NPs.

Abstract

Background Bacteroides fragilis , an anaerobic gut bacterium and opportunistic pathogen, comprises two genetically divergent groups (or divisions) at the species level. Differences exist both in the core and accessory genomes and the beta-lactamase genes, with the cephalosporinase gene cep A represented only in division I and the carbapenemase gene cfi A only in division II. Methods Multidrug resistance in a clinical B. fragilis strain was examined by whole-genome sequencing. Results Strain CNM20200260 carried the antimicrobial resistance genes cep A, cfi A2, ant (6'), erm (F), mef (En2), est (T), tet (Q) and cat (A), along with 82-Phe mutation in gyr A (together with 47 amino acid changes in gyr A/B and par C/parE). bex A/B and other efflux pump genes were also observed. None of the detected insertion sequences was located upstream of cfi A2. The genome-based taxonomy coefficients (average nucleotide identity, DNA–DNA hybridization similarity and difference in genomic G + C%) with respect to genomes of the strains of B. fragilis division II and the novel species Bacteroides hominis (both cfi A-positive) met the criteria for CNM20200260 to belong to either species (>95%, >70% and <1%, respectively). No such similarity was seen with type strain NCTC 9343 or the representative genome FDAARGOS 1225 of B. fragilis (division I, cfi A-negative). Strain CNM20200260 harboured four out of nine Kyoto Encyclopedia of Genes and Genomes orthologues defined for division I and one of two defined for division II. Conclusions This is the first description of the co-occurrence of cep A and cfi A in a Bacteroides strain, confirming the complexity of the taxonomy of this species. [ABSTRACT FROM AUTHOR]
