1. Population pharmacokinetics of intraperitoneal irinotecan and SN‐38 in patients with peritoneal metastases from colorectal origin

Abstract

Abstract Peritoneal metastases (PM) are common in patients with colorectal cancer. Patients with PM have a poor prognosis, and for those who are not eligible for cytoreductive surgery (CRS) with or without hyperthermic intraperitoneal chemotherapy (HIPEC), palliative chemotherapy is currently the only option. Recently, we conducted a phase I trial (INTERACT) in which irinotecan was administered intraperitoneally (IP) to 18 patients ineligible for CRS‐HIPEC. The primary objective was to evaluate covariates influencing the PK profile of irinotecan and SN‐38 after IP administration. Secondly, a population PK model was developed to support the further development of IP irinotecan by improving dosing in patients with PM. Patients were treated with IP irinotecan every 2 weeks in combination with systemic FOLFOX‐bevacizumab. Irinotecan and SN‐38 were measured in plasma (588 samples) and SN‐38 was measured in peritoneal fluid (267 samples). Concentration‐Time data were log‐transformed and analyzed using NONMEM version 7.5 using FOCE+I estimation. An additive error model described the residual error, with inter‐individual variability in PK parameters modeled exponentially. The final structural model consisted of five compartments. Weight was identified as a covariate influencing the SN‐38 plasma volume of distribution and GGT was found to influence the SN‐38 plasma clearance. This population PK model adequately described the irinotecan and SN‐38 in plasma after IP administration, with weight and GGT as predictive factors. Irinotecan is converted intraperitoneal to SN‐38 by carboxylesterases and the plasma bioavailability of irinotecan is low. This model will be used for the further clinical development of IP irinotecan by providing dosing strategies.

Abstract

Tanto en Chile como en el mundo, los grupos societarios más jóvenes han demostrado un flujo constante de cambios culturales y religiosos. Esto desafía las formas en que ellos se relacionan con las propuestas pastorales tradicionales, y si estas pueden responder, o no, a los desafíos de su transición secundaria-universitaria, etapa significativa para sus búsquedas vocacionales y religiosas. Este artículo presenta categorías que emergieron de una investigación cualitativa promovida por la Compañía de Jesús, cuyo objetivo fue caracterizar la forma en que los jóvenes, en sus primeros años de universidad, se relacionan con las propuestas pastorales ofrecidas por la espiritualidad ignaciana en Chile. El trabajo de entrevistas y grupos focales permitió identificar algunos temas: la importancia de los espacios de espiritualidad y servicio, la reflexión político-social en la espiritualidad, el impacto de la formación ignaciana en la vocación y elección de carrera, los cambios en disposición de tiempo y participación religiosa en esta transición y los desafíos que implica. En cuanto a la experiencia religiosa, se destacan los vínculos comunitarios, junto con la reflexión y el servicio. Al cierre de este texto se ofrecen algunos aspectos de discusión referidos a los estudios realizados en el área y se concluirá con prospectivas en la materia.

Abstract

Magnesium is an essential mediator of a vast number of critical enzymatic cellular reactions in the human body. Some clinical epidemiological studies suggest that hypomagnesemia accounts for declines in insulin secretion in patients with type 2 diabetes (T2D); however, the results of various experimental studies do not support this notion. To address this discrepancy, we assessed the short- and long-term effects of hypomagnesemia on β-cell function and insulin secretion in primary mouse islets of Langerhans and in a mouse model of hypomagnesemia known as Trpm6Δ17 /fl;Villin1-Cre mice. We found that lowering the extracellular Mg2+ concentration from 1.2 mM to either 0.6 or 0.1 mM remarkably increased glucose-induced insulin secretion (GIIS) in primary islets isolated from C57BL/6 mice. Similarly, both the plasma insulin levels and GIIS rose in isolated islets of Trpm6Δ17 /fl;Villin1-Cre mice. We attribute these rises to augmented increases in intracellular Ca2+ oscillations in pancreatic β-cells. However, the glycemic metabolic profile was not impaired in Trpm6Δ17 /fl;Villin1-Cre mice, suggesting that chronic hypomagnesemia does not lead to insulin resistance. Collectively, the results of this study suggest that neither acute nor chronic Mg2+ deficiency suppresses glucose-induced rises in insulin secretion. Even though hypomagnesemia can be symptomatic of T2D, such deficiency may not account for declines in insulin release in this disease.

Abstract

Alcohol use disorder (AUD) constitutes a critical public health issue and has sex-specific characteristics. Initial evidence suggests that progesterone and estradiol might reduce or increase alcohol intake, respectively. However, there is a need for a better understanding of how the menstrual cycle in females and the ratio of progesterone to estradiol in females and males influence alcohol use patterns in individuals with AUD. In this sex-separated multicenter longitudinal study, the authors analyzed 12-month data on real-life alcohol use (from 21,460 smartphone entries), menstrual cycle, and serum progesterone-to-estradiol ratios (from 667 blood samples at four individual study visits) in 74 naturally cycling females and 278 males with AUD between 2020 and 2022, using generalized and general linear mixed modeling. Menstrual cycle phases were significantly associated with binge drinking and progesterone-to-estradiol ratio. During the late luteal phase, females showed a lower predicted binge drinking probability of 13% and a higher predicted marginal mean of progesterone-to-estradiol ratio of 95 compared with during the menstrual, follicular, and ovulatory phases (binge drinking probability and odds ratios vs. late luteal phase, respectively: 17%, odds ratio=1.340, 95% CI=1.031, 1.742; 19%, odds ratio=1.523, 95% CI=1.190, 1.949; and 20%, odds ratio=1.683, 95% CI=1.285, 2.206; difference in progesterone-to-estradiol ratios, respectively: −61, 95% CI=−105.492, −16.095; −78, 95% CI=−119.322, −37.039; and −71, 95% CI=−114.568, −27.534). In males, a higher progesterone-to-estradiol ratio was related to lower probabilities of binge drinking and of any alcohol use, with a 10-unit increase in the hormone ratio resulting in odds ratios of 0.918 (95% CI=0.843, 0.999) and 0.914 (95% CI=0.845, 0.988), respectively. These ecologically valid findings suggest that high progesterone-to-estradiol ratios can have a protective effect against problematic alcohol use in females and males with AUD, highlighting the progesterone-to-estradiol ratio as a promising treatment target. Abstract

Abstract Food security is under increased pressure due to the ever-growing world population. To tackle this, alternative protein sources need to be evaluated for nutritional value, which requires information on digesta peptide composition in comparison to established protein sources and coupling to biological parameters. Here, a combined experimental and computational approach is presented, which compared seventeen protein sources with cow’s whey protein concentrate (WPC) as the benchmark. In vitro digestion of proteins was followed by proteomics analysis and statistical model-based clustering. Information on digesta peptide composition resulted in 3 cluster groups, primarily driven by the peptide overlap with the benchmark protein WPC. Functional protein data was then incorporated in the computational model after evaluating the effects of eighteen protein digests on intestinal barrier integrity, viability, brush border enzyme activity, and immune parameters using a bioengineered intestine as microphysiological gut system. This resulted in 6 cluster groups. Biological clustering was driven by viability, brush border enzyme activity, and significant differences in immune parameters. Finally, a combination of proteomic and biological efficacy data resulted in 5 clusters groups, driven by a combination of digesta peptide composition and biological effects. The key finding of our holistic approach is that protein source (animal, plant or alternative derived) is not a driving force behind the delivery of bioactive peptides and their biological efficacy.

16. Forward planning under uncertainty in a population-based alcohol use disorder sample

Abstract

Altered decision-making is a defining component of addiction, but there is little evidence whether these alterations affect multi-step planning in individuals with alcohol use disorder (AUD). We used a recently developed planning task in a cross-sectional approach to test the planning performance of 30 individuals diagnosed with AUD relative to 32 healthy control subjects, both sampled from the general population. To gain insight into the factors underlying behavioral performance, we inferred the parameters of a reinforcement learning agent performing rational planning, using a Bayesian inference scheme. This approach allowed us to differentiate between separate factors determining planning performance, e.g., planning depth, decision noise, and bias for certain action choices. Contrary to our hypothesis, we did not observe reduced planning depth in AUD subjects. Instead, our results show a small effect in the opposite direction: Healthy controls were slightly less efficient in the planning task. Importantly, subjects in the control group allocated less time for planning than AUD subjects, potentially indicative of motivational differences between groups. The group difference in planning depth persisted when controlling for both reaction times and general cognitive performance, albeit at a lesser magnitude. Altogether, our results do not favor the view that mild-to-moderate alcohol use disorder in general-population individuals generally involves impairments in cognitive tasks requiring forward planning across multiple steps.

17. Novel Dietary Proteins Selectively Affect Intestinal Health In Vitro after Clostridium difficile-Secreted Toxin A Exposure

Abstract

Bacterial gastroenteritis forms a burden on a global scale, both socially and economically. The Gram-positive bacterium Clostridium difficile is an inducer of gastrointestinal bacterial infections, often triggered following disruption of the microbiota by broad-spectrum antibiotics to treat other conditions. The clinical manifestatiaons, e.g., diarrhea, are driven by its toxins secretion, toxin A (TcdA) and toxin B (TcdB). Current therapies are focused on discontinuing patient medication, including antibiotics. However, relapse rates upon therapy are high (20–25%). Here, eighteen dietary proteins were evaluated for their capacity to restore gut health upon C. difficile-derived TcdA exposure. We used bioengineered intestinal tubules to assess proteins for their beneficial effects by examining the epithelial barrier, cell viability, brush-border enzyme activity, IL-6 secretion, IL-8 secretion and nitric oxide (NO) levels upon TcdA challenge. TcdA effectively disrupted the epithelial barrier, increased mitochondrial activity, but did not affect alkaline phosphatase activity, IL-6, IL-8 and NO levels. Intervention with dietary proteins did not show a protective effect on epithelial barrier integrity or mitochondrial activity. However, bovine plasma and potato protein increased alkaline phosphatase activity, egg-white protein increased IL-6 and IL-8 release and wheat, lesser mealworm and yeast protein increased NO levels after TcdA exposure. Hence, dietary proteins can influence parameters involved in intestinal physiology and immune activation suggesting that supplementation with specific dietary proteins may be of benefit during C. difficile infections.

Abstract

Self-regulation, the ability to guide behavior according to one’s goals, plays an integral role in understanding loss of control behaviors a pertinent example being substance use disorders (SUD). Yet, experimental tasks that measure processes underlying self-regulation are not easy to deploy in contexts where such behaviors often occur, namely in real life situations outside the laboratory. Moreover, lab-based experimental tasks are criticized for poor test–retest reliability and a lack of construct validity. These concerns might in part explain why ecological validity of experimental measures—their ability to predict real-life behavior—is low. To address these shortcomings, we assessed the reliability and construct validity of four smartphone-based experimental tasks designed to measure cognitive control and decision-making. To facilitate future clinical applicability we recruited a large (N=488) sample of individuals with SUD. Joint modeling of measurement sessions increased the reliability of task measures from moderate to good and often excellent levels. In line with theories of cognitive control and motivation, three latent factors reflecting cognitive control and decision-making in the context of gains and losses best described the data. As proof of concept, we show that a latent cognitive control score based on joint modeling, yielded stronger correlations with drinking behavior than single task scores based on separate modeling. These findings indicate that in individuals with SUD, smartphone-based ambulatory experimental assessments can reliably index functions of cognitive control and decision-making, with plausible construct validity. Our findings provide evidence for rich possibilities arising from longitudinal experimental studies in SUD as well as in psychiatry, neuroscience, and psychology more generally.

Abstract

Self-regulation, the ability to guide behavior according to one’s goals, plays an integral role in understanding loss of control over unwanted behaviors, for example in alcohol use disorder (AUD). Yet, experimental tasks that measure processes underlying self-regulation are not easy to deploy in contexts where such behaviors usually occur, namely outside the laboratory, and in clinical populations such as people with AUD. Moreover, lab-based tasks have been criticized for poor test–retest reliability and lack of construct validity. Smartphones can be used to deploy tasks in the field, but often require shorter versions of tasks, which may further decrease reliability. Here, we show that combining smartphone-based tasks with joint hierarchical modeling of longitudinal data can overcome at least some of these shortcomings. We test four short smartphone-based tasks outside the laboratory in a large sample (N = 488) of participants with AUD. Although task measures indeed have low reliability when data are analyzed traditionally by modeling each session separately, joint modeling of longitudinal data increases reliability to good and oftentimes excellent levels. We next test the measures’ construct validity and show that extracted latent factors are indeed in line with theoretical accounts of cognitive control and decision-making. Finally, we demonstrate that a resulting cognitive control factor relates to a real-life measure of drinking behavior and yields stronger correlations than single measures based on traditional analyses. Our findings demonstrate how short, smartphone-based task measures, when analyzed with joint hierarchical modeling and latent factor analysis, can overcome frequently reported shortcomings of experimental tasks.

Abstract

Importance Alcohol consumption (AC) leads to death and disability worldwide. Ongoing discussions on potential negative effects of the COVID-19 pandemic on AC need to be informed by real-world evidence. Objective To examine whether lockdown measures are associated with AC and consumption-related temporal and psychological within-person mechanisms. Design, Setting, and Participants This quantitative, intensive, longitudinal cohort study recruited 1743 participants from 3 sites from February 20, 2020, to February 28, 2021. Data were provided before and within the second lockdown of the COVID-19 pandemic in Germany: before lockdown (October 2 to November 1, 2020); light lockdown (November 2 to December 15, 2020); and hard lockdown (December 16, 2020, to February 28, 2021). Main Outcomes and Measures Daily ratings of AC (main outcome) captured during 3 lockdown phases (main variable) and temporal (weekends and holidays) and psychological (social isolation and drinking intention) correlates. Results Of the 1743 screened participants, 189 (119 [63.0%] male; median [IQR] age, 37 [27.5-52.0] years) with at least 2 alcohol use disorder (AUD) criteria according to the Diagnostic and Statistical Manual of Mental Disorders (Fifth Edition) yet without the need for medically supervised alcohol withdrawal were included. These individuals provided 14 694 smartphone ratings from October 2020 through February 2021. Multilevel modeling revealed significantly higher AC (grams of alcohol per day) on weekend days vs weekdays (β = 11.39; 95% CI, 10.00-12.77; P < .001). Alcohol consumption was above the overall average on Christmas (β = 26.82; 95% CI, 21.87-31.77; P < .001) and New Year’s Eve (β = 66.88; 95% CI, 59.22-74.54; P < .001). During the hard lockdown, perceived social isolation was significantly higher (β = 0.12; 95% CI, 0.06-0.15; P < .001), but AC was significantly lower (β = −5.45; 95% CI, −8.00 to −2.90; P = .001). Independent of lockdown, intention to drink less alcohol was associated with lower AC (β = −11.10; 95% CI, −13.63 to −8.58; P < .001). Notably, differences in AC between weekend and weekdays decreased both during the hard lockdown (β = −6.14; 95% CI, −9.96 to −2.31; P = .002) and in participants with severe AUD (β = −6.26; 95% CI, −10.18 to −2.34; P = .002). Conclusions and Relevance This 5-month cohort study found no immediate negative associations of lockdown measures with overall AC. Rather, weekend-weekday and holiday AC patterns exceeded lockdown effects. Differences in AC between weekend days and weekdays evinced that weekend drinking cycles decreased as a function of AUD severity and lockdown measures, indicating a potential mechanism of losing and regaining control. Abstract

Background/objectives: The aim of the present study was to validate a short questionnaire on habitual dietary salt intake, to quickly and easily identify individuals whose salt consumption exceeds recommended levels. Subjects/methods: A total of 1131 hypertensive subjects participating in the MINISAL-SIIA study were included in the analysis. Anthropometric indexes, blood pressure, and 24-h urinary sodium excretion (NaU) were measured. A fixed-sequence questionnaire on dietary salt intake was administered. Results: NaU was significantly associated with scores, with a linear association across categories (p for trend <0.0001). In addition, participants who achieved a total score above the median value (eight points) had significantly higher NaU than those whose score was below median (p < 0.0001). In the total sample, the prevalence of “high NaU” (NaU > 85 mmol/day) and “very high NaU” (NaU > 170 mmol/day) was 86 and 35%, respectively. The score of the questionnaire had a significant ability to detect both “high NaU”—with a specificity of 95% at the score of 10 points—and “very high NaU”—with a specificity of 99.6% at score of 13 points. Conclusions: The main results of the study indicates that a higher score of this short questionnaire is distinctive of habitual high salt consumption in hypertensive patients.

Results: NaU was significantly associated with scores, with a linear association across categories (p for trend 85 mmol/day) and “very high NaU” (NaU > 170 mmol/day) was 86 and 35%, respectively. The score of the questionnaire had a significant ability to detect both “high NaU”—with a specificity of 95% at the score of 10 points—and “very high NaU”—with a specificity of 99.6% at score of 13 points. Conclusions: The main results of the study indicates that a higher score of this short questionnaire is distinctive of habitual high salt consumption in hypertensive patients.

Abstract

Organic cation transporter 1 (OCT1) is a membrane transporter that affects hepatic uptake of cationic and weakly basic drugs. OCT1 transports structurally highly diverse substrates. The mechanisms conferring this polyspecificity are unknown. Here, we analyzed differences in transport kinetics between human and mouse OCT1 orthologs to identify amino acids that contribute to the polyspecificity of OCT1. Following stable transfection of HEK293 cells, we observed more than twofold differences in the transport kinetics of 22 out of 28 tested substrates. We found that the β2-adrenergic drug fenoterol was transported with eightfold higher affinity but at ninefold lower capacity by human OCT1. In contrast, the anticholinergic drug trospium was transported with 11-fold higher affinity but at ninefold lower capacity by mouse Oct1. Using human–mouse chimeric constructs and site-directed mutagenesis, we identified nonconserved amino acids Cys36 and Phe32 as responsible for the species-specific differences in fenoterol and trospium uptake. Substitution of Cys36 (human) to Tyr36 (mouse) caused a reversal of the affinity and capacity of fenoterol but not trospium uptake. Substitution of Phe32 to Leu32 caused reversal of trospium but not fenoterol uptake kinetics. Comparison of the uptake of structurally similar β2-adrenergics and molecular docking analyses indicated the second phenol ring, 3.3 to 4.8 Å from the protonated amino group, as essential for the affinity for fenoterol conferred by Cys36. This is the first study to report single amino acids as determinants of OCT1 polyspecificity. In this paper, a high-resolution P-band Pol-InSAR data set acquired by the airborne RAMSES system over pine forest stands of different height is investigated. A significant penetration depth in all the polarimetric channels and a wide range of polarimetric phase center heights are observed, attesting of an interaction of the radar waves with different forest structural elements. The main objective of this paper concerns forest height inversion at P-band. First, forest-modeling assumptions are evaluated using a priori information, such as ground-level and forest height measurements. The full extend of the forest height is shown to be responsible of the volume decorrelation, and a significant orientation effect is dearly identified over the highest stands. As a consequence, the Oriented Volume over Ground model (OVoG) is determined to be the most appropriated model for the forest height inversion. At P-band, the ground contribution is present in all the polarimetric channels due to the important penetration at this frequency. To overcome this difficulty, a time--frequency optimization method based on sublook decomposition is developed to separate the pure ground and canopy contributions, allowing forest height estimation with OVoG with an rms error on the order of 2 m. In the last section of this paper, a sensitivity analysis of the inversion with respect to two important system parameters, the signal-to-noise ratio and the resolution, is presented, leading to a discussion on the inversion robustness in spaceborne conditions, where these system parameters are the most deteriorated as compared to airborne configurations. In spaceborne synthetic aperture radar (SAR), a single-polarization on-transmit offers twice the swath width compared to full polarization. This is linked to SAR system design issues, and, without getting into the technical details deserving by themselves a full paper, we can just mention the swath characteristics of ALOS PALSAR (the Advanced Land Observing Satellite, Phased Array L-Band Synthetic Aperture Radar), reducing from 70 km for the dual-pol mode to 30 km for the full polarization mode. The reduced coverage in the full polarization mode has a harmful impact on the revisit time, which is always a major drive for the Earth-observing community. The options chosen up to now for dual-pol system designs (or single-polarization on-transmit) rely on a linear polarization on-transmit [either horizontal (H) or vertical (V)], with two orthogonal polarizations on-receive. Souyris and Raney in earlier papers proposed more pertinent alternatives for the selection of the transmit polarization leading to a better characterization of the scattering mechanisms. In this paper, the analysis is pursued in more depth by including the effect of the ionosphere on the wave propagation and extending the applications to polarimetric interferometry SAR (PolInSAR). A compact mode is developed where the transmit polarization is circular, whereas the only constraint on the two receiving polarizations is independence. Indeed, the choice of the polarizations of the two receive channels does not matter, as any polarization on-receive can be synthesized from these two measurements. This is, however, not the case for the unique transmit polarization. At a low frequency, where the ionosphere has a significant effect, the circular transmit polarization is the only sensible option, as it provides an effective constant polarization as seen by the scattering surface. This is an essential condition for a meaningful multitemporal analysis. Both the polarimetric SAR applications and the PolInSAR applications in the context of this compact polarimetry (CP) mode are explored. A pseudocovariance matrix can be reconstructed following Souyris' proposed approach for distributed targets and is shown to be very similar to the full polarimetric (FP) covariance matrix. The reconstruction of the cross-polarized Sigma0 is shown to be reliable and to have very low sensitivity to Faraday rotation. A PolInSAR vegetation height inversion for P-band is presented and applied to the CP data with a level of performance that is similar to the one derived from FP (a 1.2-m root-mean-square height error on the ONERA Airborne radar (RAMSES) data over the Landes Forest). A procedure is developed to correct for the ionospheric effects for the PolInSAR acquisition in the FP or CP mode and is assessed on the data simulated from an airborne acquisition. The results demonstrate that the technique is efficient and robust. The calibration of CP data is identified as an important challenge to be solved, and some clues are provided to address the problem. Abstract

We propose a new approach for evaluating the contribution of the different channels of polarimetric and interferometric synthetic aperture radar (PolInSAR) images. For that purpose, we demonstrate that the Bhattacharyya distance between the probability density functions of neighboring regions in the image provides an efficient scalar contrast measure. We show that the analysis of this contrast measure allows one to precisely characterize the contribution of each channel for different system configurations, including intensity, polarimetric, and interferometric images. We illustrate this approach using a real synthetic aperture radar image to compare several polarimetric system architectures. Since PolInSAR imaging configurations can correspond to complex and expensive systems, the proposed method can be helpful in system imaging optimization. Abstract

A sparse pine forest is investigated at X-band on a single-pass polarimetric synthetic aperture radar interferometry (PoIInSAR) data set using HH and HV channels. These first preliminary results show that the associated phase centers present a significant vertical separation (about 6 m) allowed by penetration through gaps in the canopy. Forest parameter inversion using the random volume over ground (RVoG) model is evaluated and adapted at this frequency. The forest height can be retrieved accurately by supposing a high mean extinction coefficient (around 1.6 dB/m). The penetration depth is estimated to be around 4 m, based on the forest height ground measurements. Finally, a time-frequency analysis using a sublook decomposition is performed to increase the vertical separation of the polarimetric phase centers. As a consequence, RVoG-inversion performance is improved, and a penetration depth that is in better accordance with a previous work (of the order of 2 m) is found. This paper has shown that the height inversion of a pine forest was possible using PolInSAR X-band data and that the performance was more dependent on the forest density than at lower frequencies. Abstract

Bacterial gastroenteritis forms a burden on a global scale, both socially and economically. The Gram-positive bacterium Clostridium difficile is an inducer of gastrointestinal bacterial infections, often triggered following disruption of the microbiota by broad-spectrum antibiotics to treat other conditions. The clinical manifestatiaons, e.g., diarrhea, are driven by its toxins secretion, toxin A (TcdA) and toxin B (TcdB). Current therapies are focused on discontinuing patient medication, including antibiotics. However, relapse rates upon therapy are high (20&ndash, 25%). Here, eighteen dietary proteins were evaluated for their capacity to restore gut health upon C. difficile-derived TcdA exposure. We used bioengineered intestinal tubules to assess proteins for their beneficial effects by examining the epithelial barrier, cell viability, brush-border enzyme activity, IL-6 secretion, IL-8 secretion and nitric oxide (NO) levels upon TcdA challenge. TcdA effectively disrupted the epithelial barrier, increased mitochondrial activity, but did not affect alkaline phosphatase activity, IL-6, IL-8 and NO levels. Intervention with dietary proteins did not show a protective effect on epithelial barrier integrity or mitochondrial activity. However, bovine plasma and potato protein increased alkaline phosphatase activity, egg-white protein increased IL-6 and IL-8 release and wheat, lesser mealworm and yeast protein increased NO levels after TcdA exposure. Hence, dietary proteins can influence parameters involved in intestinal physiology and immune activation suggesting that supplementation with specific dietary proteins may be of benefit during C. difficile infections.

Abstract

Objectives Few studies have analyzed factors associated with delirium subtypes. In this study, we investigate factors associated with subtypes of delirium only in patients with dementia to provide insights on the possible prevention and treatments. Design This is a cross-sectional study nested in the "Delirium Day" study, a nationwide Italian point-prevalence study. Setting and participants Older patients admitted to 205 acute and 92 rehabilitation hospital wards. Measures Delirium was evaluated with the 4-AT and the motor subtypes with the Delirium Motor Subtype Scale. Dementia was defined by the presence of a documented diagnosis in the medical records and/or prescription of acetylcholinesterase inhibitors or memantine prior to admission. Results Of the 1057 patients with dementia, 35% had delirium, with 25.6% hyperactive, 33.1% hypoactive, 34.5% mixed, and 6.7% nonmotor subtype. There were higher odds of having venous catheters in the hypoactive (OR 1.82, 95% CI 1.18-2.81) and mixed type of delirium (OR 2.23, CI 1.43-3.46), whereas higher odds of urinary catheters in the hypoactive (OR 2.91, CI 1.92-4.39), hyperactive (OR 1.99, CI 1.23-3.21), and mixed types of delirium (OR 2.05, CI 1.36-3.07). We found higher odds of antipsychotics both in the hyperactive (OR 2.87, CI 1.81-4.54) and mixed subtype (OR 1.84, CI 1.24-2.75), whereas higher odds of antibiotics was present only in the mixed subtype (OR 1.91, CI 1.26-2.87). Conclusions and implications In patients with dementia, the mixed delirium subtype is the most prevalent followed by the hypoactive, hyperactive, and nonmotor subtype. Motor subtypes of delirium may be triggered by clinical factors, including the use of venous and urinary catheters, and the use of antipsychotics. Future studies are necessary to provide further insights on the possible pathophysiology of delirium in patients with dementia and to address the optimization of the management of potential risk factors.

Abstract

In this paper, we show that polarimetric and interferometric SAR (PolInSAR) images can be efficiently partitioned into homogeneous regions with a statistical technique based on minimization of a parameter-free criterion. This technique consists of finding a polygonal partition of the image that minimizes the stochastic complexity, assuming that the image is made of a tessellation of statistically homogeneous regions. The obtained results demonstrate that a global partition in statistically homogeneous regions of PolInSAR images can provide better results than a partition based on a single characteristic such as polarimetry or interferometry only. Abstract

In this study, the critical state friction angle in a direct shear box was tested at vertical effective stresses between 4 kPa and 150 kPa for seven sands of different origins. The results show a stress dependency on the angle of friction at low vertical stresses, and that the critical state friction angle decreases with increase in the vertical effective stress. The largest drop occurs for vertical effective stresses between 4 kPa and 50 kPa and varies between 4° and 10° depending mostly on the shape of the sand particles and the difference in the extreme void ratios.

Abstract

Radical ring opening polymerization is a powerful tool to achieve a polyester via radical polymerization. We used it to obtain a dimethylated version of poly(caprolactone) (PdmCL) from dimethylated MDO (DMMDO). First, we revisited monomer synthesis and achieved a milder synthetic protocol by introducing a cobalt-based catalyst. We also developed a new route towards DMMDO via a cyclic carbonate using the Petasis chemistry. Amphiphilic block-copolymers were then generated by free radical polymerization of DMMDO with a PEG-based macroinitiator. The resulting polyesters self-assembled into nanoparticles that were biodegradable as well as biocompatible. The nanoparticles proved to be an effective protective shell for an entrapped enzyme that was released upon degradation of the polyester by esterase. We are confident that our results will spur further research into block-copolymers resulting from RROP.
