Abstract

Aim To compare clinical presentation, operative management and short- and long-term outcomes of congenital bile duct cysts (BDC) in adults with children. Methods Retrospective multi-institutional Association Francaise de Chirurgie study of Todani types I+IVB and IVA BDC. Results During the 37-year period to 2011, 33 centers included 314 patients (98 children; 216 adults). The adult population included more high-risk patients, with more active, more frequent prior treatment (47.7% vs 11.2%; p < 0.0001), more complicated presentation (50.5% vs 35.7%; p = 0.015), more synchronous biliary cancer (11.6% vs 0%; p = 0.0118) and more major surgery (23.6% vs 2%; p < 0.0001), but this latter feature was only true for type I+IVB BDC. Compared to children, the postoperative morbidity (48.1% vs 20.4%; p < 0.0001), the need for repeat procedures and the status at follow-up were worse in adults (27% vs 8.8%; p = 0.0009). However, severe postoperative morbidity and fair or poor status at follow-up were not statistically different for type IVA BDC, irrespective of patients' age. Synchronous cancer, prior HBP surgery and Todani type IVA BDC were independent predictive factors of poor or fair long-term outcome. Conclusion BDC is a more indolent disease in children compared to adults, except for Todani type IV-A BDC.

Abstract

Resume Objectif de l'etude Cette etude vise a definir les arguments concordants ou discordants avec le concept generalement admis de filiation entre les cystadenomes mucineux (CM) et les cystadenocarcinomes pancreatiques (CK). Materiel et methodes Cette etude a ete realisee a partir d'une revue de la litterature et d'une enquete retrospective multicentrique ayant reuni 150 CM, 79 CK et 55 tumeurs intracanalaires papillaires et mucineuses (TIPMP). Les elements comparatifs entre CM et CK etaient : l'âge moyen au moment du diagnostic, le sex ratio, l'association d'un diabete sucre, la coexistence d'une autre tumeur maligne, la localisation de la tumeur et sa taille, le caractere communiquant de la tumeur, l'existence d'une pancreatite d'amont, l'elevation du taux serique du CA 19-9. L'analyse a porte egalement sur les examens anatomopathologiques des 58 CK reseques. Resultats Deux arguments sont apparus en faveur de la filiation dans cette etude : l'âge plus eleve des CK (65 ans vs 50,5 ans) et la decouverte histologique de formes de passage entre CM et CK dans 55 % des cas. A l'oppose, deux elements contradictoires sont apparus: la proportion plus elevee des CK chez l'homme (40 % vs 13 %) et la frequence plus grande des localisations cephaliques des CK (49 % vs 27 %). Les autres differences entre CM et CK (diabete associe, elevation du CA19-9, alterations genetiques) n'etaient pas en contradiction avec la filiation CM-CK. Les TIPMP etaient caracterisees par une majorite de cas masculins (67 %) et de localisations cephaliques (67 %). Conclusion La filiation CM-CK ne peut pas etre remise en cause malgre la constatation de differences sur le sex ratio et la localisation tumorale. Celles-ci peuvent s'expliquer par l'integration de TIPMP malignes dans les CK. Le cadre nosologique des CK communiquant avec le canal de Wirsung est tres proche de celui des TIPMP.

Abstract

International audience; PURPOSE: Several phase II studies have shown the feasibility of neoadjuvant chemoradiation regimens for resectable localized pancreatic adenocarcinoma. However, there is to date no completed phase III study to validate this approach and treatment effects evaluation still remains an active area of investigation. From the mature results of the SFRO-FFCD 9704 trial, we explored the antitumoral effect of a 5-fluoro-uracil and cisplatin-based preoperative chemoradiation regimen, with a special highlight on the histopathological response and performed a literature review. PATIENTS AND METHODS: Treatment consisted of concurrent radiotherapy (50 Gy within five weeks) and chemotherapy with 5-fluoro-uracil (300 mg/m(2)/day, five days/week, weeks 1-5) and cisplatin (20mg/m(2)/day, days 1-5 and 29-33), followed by surgical resection of the pancreatic tumour in patients without progression. RESULTS: In all, 41 patients were enrolled, 26 patients (63%) underwent surgical resection with curative intent and 21 (80.7%) had R0 resection. A total of 13 of 26 specimens (50%) presented a major pathologic response (≥ 80% of severely degenerative cancer cells), with one complete pathologic response. The local recurrence and two-year survival rates were 4 and 32%, respectively, for the 26 operated patients. CONCLUSION: Our results suggest that preoperative chemoradiation provides antitumoral effect associated with major histopathological response in 50% of patients and a high R0 resection rate. Evaluation of histopathological response to neoadjuvant chemoradiation may serve as a surrogate marker for treatment efficacy and further research is needed to determine new prognostic and predictive factors of treatment response.
