Tiivistelmä. Arkkitehtuurin historian ja korjaussuunnittelun harjoitustyökurssilla tehtävänä oli laatia pieneen julkiseen rakennukseen korjaus- ja uudelleenkäyttösuunnitelma. Rakennuskohteena on Tuirantie 1 osoitteessa sijaitseva Oulun Tuiranpuiston entisen Ylilääkärin asunto. Puiston mäntykankaalle on sijoitettu erilaisia puu- ja kivirakennuksia, jotka muodostavat Oulun vanhan kunnallissairaalan kokonaisuuden. Hirsirunkoinen puurakennus valmistui vuonna 1909 ja sen suunnitteli Oulun silloinen lääninarkkitehti Viktor J. Sucksdorff. Ylilääkärin asunto on suojeltu merkinnällä sr-2. Tontin ulkopuolella sijaitsee myös vanha piharakennus, jota on mahdollisesti käytetty asunnon pesutupana. Pesutuvan suojeluluokka on sr-5, eli sitä ei saa purkaa. Tavoitteena oli pohtia arvottamisen perusteita sekä lähtökohtia. Tehtäväksi muodostui myös nykyaikaisten vaatimusten sovittaminen vanhaan historialliseen rakennukseen. Uuden ja vanhan välinen suhde rakennuksessa nousi myös esille. Ensin tehdyssä analyysivaiheessa selvitimme ryhmässä rakennuksen historiaa ja laadimme säilyneisyyskarttoja muutostöistä. Kurssin alussa tammi- ja helmikuussa suoritimme myös kaksi kenttäkäyntiä kohteessa. Saamalla tietoa rakennuksen ja alueen historiasta sekä rakennuksen käyttöhistoriasta ja muutoksista, pystymme suunnittelemaan rakennuksen arvoisen korjaussuunnitelman. Huomioinkin suunnitelmissa muun muassa rakennuksen kaupunkikuvallista, arkkitehtonista sekä kulttuurihistoriallista arvoa. Pyrin suunnitelmassa pienimuotoisiin muutoksiin, jotka korostaisivat rakennuksen alkuperäistä arkkitehtonista suunnitelmaa. Muuttamalla pohjaratkaisu avarammaksi pohjoisen sisäänkäynnin luota ja purkamalla kevyet väliseinät, saa rakennus uudenlaista tunnetta sekä laajemmat mahdollisuudet uudelle käyttökohteelle. Ratkaisuissa huomioin myös rakennuksen tulevaisuuden käyttömahdollisuuksia sekä muuntojoustavuutta. Rakennuksen sisäpintojen pinnalla näkyi 70- ja 80- lukujen muutostyöt, jotka pyrin joko palauttamaan entiselleen tai yhdistämään alkuperäiseen arkkitehtoniseen ilmeeseen saumattomasti. Yksinkertaistamalla pohjasuunnitelmaa sekä linjoja saatiin tuotua alkuperäinen jugend tyyli voimakkaammin näkyväksi. Suunnittelin entisen Ylilääkärin asunnon pieneksi yksityiseksi päiväkodiksi, joka erikoistuu luovien taideopintojen opetukseen ja toimii myös iltapäiväkerhona. Rakennus saadaan pienillä avartavilla muutoksilla sopimaan kasvatus- ja harrastustoimintaan. Asuinalueiden ja liikenneyhteyksien läheisyydessä toimivan rakennuksen käyttötarkoituksen muutos onnistuisi kevyiden seinien purkamisella sekä pintojen uusimisella puhtailla ja allergiasoimattomilla materiaaleilla. Piha-alueen rajaus rakennuksen suuntaisella alueella muodostetaan turvallinen, valoisa ja aktiivinen ulkotila. Leikkialue sijoittuisi rakennuksen luoteis- kulmaan mäntypuiden juurelle. Ympäröivä puisto toimisi vaihtelevana piha-alueena päiväkotilaisille ja mahdollistaisi retkeilyn ja opetuksen luonnontilassa. Työssä toimi ohjaajina Anu Soikkeli, Tanya Yliaho sekä Rosaliina Järvinen. Kurssiin sisältyi myös luentoja aiheesta ja aineistoakin oli laaja määrä, esimerkiksi Uki Arkkitehtien laatima rakennushistoriaselvitys kohteesta.

Tiivistelmä Lähtökohdat: Fysioterapeuttien suoravastaanotot ovat yleistyneet nopeasti. Tarkastelimme muutoksen vaikutusta lääkärien ja fysioterapeuttien työnjakoon. Menetelmät: Selvitimme työnjakoa suoravastaanottotoiminnassa kirjallisuushaulla ja Suomessa myös verkkokyselyllä. Tulokset: Kirjallisuuskatsauksen mukaan potilaat olivat tyytyväisiä ja pääsivät nopeammin hoitoon. Toimintamallin ei ole todettu lisänneen fysioterapiapalvelujen tarpeetonta käyttöä eikä heikentäneen potilasturvallisuutta tai hoidon jatkuvuutta. Suomessa suoravastaanotolle ja lääkärille ohjautumisen kriteerit vaihtelivat erityisvastuualueilla. Potilaiden omahoitoisuuden ja yksikön toiminnan arvioitiin parantuneen huomattavasti suoravastaanoton ansiosta. Lääketieteellinen tuki fysioterapeuteille ei aina ollut toimivaa. Päätelmät: Kokemusten mukaan fysioterapeuttien suoravastaanottotoiminta on vaikuttavaa. Lisätutkimusta kuitenkin tarvitaan. Abstract Background: Direct access to physiotherapy (DAP) has increased greatly both in Finland and worldwide, and therefore practical issues related to the division of labour between physiotherapists and general practitioners (GPs) are of great interest. Methods: We performed a literature review of the division of labour between physiotherapists and GPs in DAP. We also carried out a Digium -survey on the national practices of DAP in Finland. Results: The literature review found 24 articles, and 59 health care professionals responded to the survey. Based on the review, patients were satisfied and had quicker access to care. DAP did not seem to increase the use of physiotherapy services or decrease patient safety or continuity of care. In Finland, the criteria for DAP and GP referral vary greatly by region. The respondents considered that patients’ self-management ability and health care units’ operational effectiveness had improved considerably as a result of DAP. Medical support for physiotherapists was usually available but not always made use of. Conclusions: On the basis of both the literature review and national experiences DAP is regarded as an effective activity, but further research is needed.

Kevätpöly aiheuttaa terveysongelmia sekä likaantumista alueilla, missä liukkauden torjuntaan käytetään hiekkaa ja suolaa. Hiekka sekä kuluttaa tienpintaa että muodostaa jauhautuessaan pienempiä partikkeleita, jotka nousevat ilmaan renkaan tai ajoneuvon ilmanvirran vaikutuksesta. Hiekoitusmateriaalin raekoolla sekä kivilajilla on myös vaikutusta hiukkaspäästöihin. Suolaaminen pitää tienpinnan kosteana, joka kiihdyttää pinnan kulumista. Tämä korostuu erityisesti nastarenkaiden kohdalla. Osa katupölystä on kaukokulkeuman mukanaan tuomia hiukkasia sekä palo- ja kulumistapahtumissa vapautuneita hiukkasia. Kevätpölykauden kestoon voidaan vaikuttaa poistamalla pölyävä materiaali kaduilta, minkä aloitusajankohta riippuu lämpötilasta ja lumien sulamisesta. Epätasaisen sulamisen takia jäisiä alueita joudutaan harjaamaan uudelleen, koska polanteessa olevan hiekan poistaminen on hankalaa. Mahdolliset yöpakkaset hankaloittavat veteen perustuvien pölyntorjuntamenetelmien käyttämistä. Kevätpuhdistuksessa arvioidaan kerättävän 50…70 % levitetystä materiaalista ja loppu on poistunut lumenajossa, sulamisvesien mukana tai on tullut harjatuksi tiensivuun. Harjalaitteilla poistetaan lähinnä karkeaa aineista kaduilta. Hienoaines pestään painepesureilla tai kerätään erilaisia imulakaisuautoja käyttäen pois. Harjatessa on käytettävä pölyntorjuntamenetelmiä, jottei puhdistus nostaisi ilman hiukkaspitoisuuksia. Paras keino ehkäistä pölyämistä olisi estää pölyn syntyminen. Vettä käyttävien menetelmien on todettu olevan tehokkaimpia, mutta suuri veden kulutus aiheuttaa kuljetuskustannuksia. Kuivamenetelmät eivät lisää kerättävän materiaalin massaa, mutta kauhan tyhjennys aiheuttaa tällöin pölyämistä. Harjaamista voidaan tehostaa ottamalla käyttöön paikannusjärjestelmät, jotka mahdollistavat työpäiväkirjat sekä tehokkaamman työnjohdon. Tällöin on mahdollista käyttää kustannusperusteista laskutusta yhdistämällä paikkatietoja vaa’an tietoihin. Myös harjalaitteen ohjauksen kehittämisellä voidaan tehostaa toimintaa, koska nykyiset laitteet vaativat poistumista ohjaamosta esimerkiksi kierrosnopeuden säätämiseksi. Road dust causes problems in areas where the sand and salt are used to prevent slipperiness in winter. The dust causes health hazard and surroundings get dirty in the spring. The sand wears the road surface and it is also be crushed to smaller particles. The salt increases the wearing of the road because it keeps the surface longer wet. Also using studded tires increases the wearing of the asphalt. Some particles of the road dust are from burning processes, some are from distant sources and some particles are made by corrosion and mechanical wearing. These particles are reabsorbed up in the air by the tires or by airflow. It is possible to reduce road dust problems by using sweepers or other road maintenance equipments. It has been estimated that 50…70 % of used sand is collected every spring. The rest of sand is transported with snow or brushed to the side of the road. The collecting of the sand also lifts particles up in the air. The best way to prevent this is using water. But when using it must the temperature be above zero. To prevent dusting is also possible to use chemicals or vacuum. Before using those methods it is vital to design good housing, because it also improves their efficiency.

PURPOSE: Our purpose was to investigate the normal volumes of the human entorhinal, perirhinal, and temporopolar cortices on MR imaging studies using a customized program. METHODS: We designed a protocol in which the volumes of the entorhinal, perirhinal, and temporopolar cortices were determined from coronal MR images using anatomic landmarks defined on the basis of cytoarchitectonic analyses of 49 autopsy cases. MR volumetry of these cortical areas was performed in 52 healthy volunteers. RESULTS: The overall mean volumes were 1768 +/- 328 mm3/1558 +/- 341 mm3 (right/left) for the entorhinal cortex, 2512 +/- 672 mm3/2572 +/- 666 mm3 for the perirhinal cortex, and 2960 +/- 623 mm3/3091 +/- 636 mm3 for the temporopolar cortex. The right entorhinal cortex was 12% larger than the left. The volume of the temporopolar cortex was reduced bilaterally by 13% in the older age group compared with younger subjects, while the volumes of the entorhinal and perirhinal cortices were unaffected by age. There were no differences between men and women in the volumes of any of the three cortices. CONCLUSION: Our method provides a tool by which to measure volumes of the entorhinal, perirhinal, and temporopolar cortices on coronal MR images.

PURPOSE: To examine the value of measurement of the interuncal distance in the diagnosis of mild to moderate Alzheimer disease. METHODS: We measured interuncal distance from coronal MR scans acquired on a 1.5-T imager. We estimated interuncal distance from a total of 141 subjects: 54 patients diagnosed according to the National Institute of Neurological and Communicative Disorders and the Alzheimer's Disease and Related Disorders Association criteria of probable Alzheimer disease, 40 subjects fulfilling the National Institute of Mental Health criteria of age-associated memory impairment, 27 healthy cognitively normal older control subjects, and 20 control subjects younger than 50 years of age. For comparison we normalized interuncal distance for a horizontal line drawn through the inner cranium at the level of the uncus (interuncal distance/intracranial width ratio), for the brain area (interuncal distance/brain area) and for the intracranial area (interuncal distance/intracranial area). RESULTS: The standard interuncal distance and the interuncal distance/intracranial width differed between the young control subjects and the other groups, but did not differ among the control, age-associated memory impairment, and Alzheimer disease groups. The Alzheimer disease group had significantly greater interuncal distance/intracranial area and interuncal distance/brain area compared with age-matched controls. A considerable overlap was found, however, in the values of patients with Alzheimer disease and control subjects. The cutoff point of 30 mm for interuncal distance yielded 37% sensitivity and 72% specificity to distinguish patients with Alzheimer disease from nondemented elderly subjects. Interuncal distance was not significantly related to the clinical severity of Alzheimer disease as assessed by Clinical Dementia Rating Scale and Mini-Mental Status Examination. Instead, there was a strong correlation between standard and normalized interuncal distance and age in the whole study population and in nondemented subjects. CONCLUSIONS: Our results showed that in a series of 54 patients with mild to moderate Alzheimer disease, interuncal distance was not a reliable diagnostic tool. The study also confirmed the strong age dependence for interuncal distance.

PURPOSE: To evaluate different carotid stenosis estimation methods with digital subtraction angiography. METHODS: We assessed the intraobserver reproducibilty and interobserver variability of visual interpretation and the measurement methods used by the European Carotid Surgery Trial and the North American Symptomatic Carotid Endarterectomy Trial. Angiographic stenosis measurements according to both criteria were performed twice by a radiologist, a neurologist, and a vascular surgeon. Eighty bifurcations of consecutive symptomatic patients underwent 480 pairs of measurements. In addition, four radiologists estimated the stenoses visually. RESULTS: Intraobserver consistency was slightly better by the European (kappa, 0.86 to 0.94) than by the North American (kappa, 0.68 to 0.91) trial criteria or by visual interpretation (kappa, 0.79 to 0.81). No significant interobserver variability was found, except in the subgroup of mild stenoses by the North American Trial criteria. By kappa statistic, the interobserver agreement was excellent by the European trial method (kappa, 0.72 to 0.86), good by the North American trial method (kappa, 0.59 to 0.77), and good to excellent by visual evaluation (kappa, 0.68 to 0.88). The visual estimation agreed more closely with the European (kappa, 0.73 to 0.92) than with the North American trial (kappa, 0.55 to 0.74) criteria measurements. CONCLUSIONS: All three methods have good reproducibility in digital subtraction angiography. Interobserver differences become more important in the estimation of mild stenosis.
