Abstract

The assessment of neutrino absolute mass scale is still a crucial challenge in today particle physics and cosmology. Beta or electron capture spectrum end-point study is currently the only experimental method which can provide a model independent measurement of the absolute scale of neutrino mass. HOLMES is an experiment funded by the European Research Council to directly measure the neutrino mass. HOLMES will perform a calorimetric measurement of the energy released in the electron capture decay of the artificial isotope $^{163}$Ho. In a calorimetric measurement the energy released in the decay process is entirely contained into the detector, except for the fraction taken away by the neutrino. This approach eliminates both the issues related to the use of an external source and the systematic uncertainties arising from decays on excited final states. The most suitable detectors for this type of measurement are low temperature thermal detectors, where all the energy released into an absorber is converted into a temperature increase that can be measured by a sensitive thermometer directly coupled with the absorber. This measurement was originally proposed in 1982 by A. De Rujula and M. Lusignoli, but only in the last decade the technological progress in detectors development has allowed to design a sensitive experiment. HOLMES plans to deploy a large array of low temperature microcalorimeters with implanted $^{163}$Ho nuclei. Abstract

HOLMES is a new experiment aiming at directly measuring the neutrino mass with a sensitivity below 2 eV. HOLMES will perform a calorimetric measurement of the energy released in the decay of $^{163}$Ho. The calorimetric measurement eliminates systematic uncertainties arising from the use of external beta sources, as in experiments with spectrometers. This measurement was proposed in 1982 by A. De Rujula and M. Lusignoli, but only recently the detector technological progress has allowed to design a sensitive experiment. HOLMES will deploy a large array of low temperature microcalorimeters with implanted $^{163}$Ho nuclei. HOLMES, besides being an important step forward in the direct neutrino mass measurement with a calorimetric approach, will also establish the potential of this approach to extend the sensitivity down to 0.1 eV and lower. In its final configuration HOLMES will collect about $3\cdot 10^{13}$ decays with 1000 detectors characterized by an instrumental energy resolution of the order of few eV and a time resolution of few microseconds. To embed the $^{163}$Ho into the gold absorbers a custom mass separator ion implanter is being developed. The detectors used for the HOLMES experiment will be Mo/Cu bilayers TESs (Transition Edge Sensors) on SiN$_x$ membrane with gold absorbers. Microwave multiplexed rf-SQUIDs are the best available technique to read out large array of such detectors. An extensive R&D activity is in progress in order to maximize the multiplexing factor while preserving the performances of the individual detectors. The current activities are focused on the the single detector performances optimization and on the $^{163}$Ho isotope production and embedding. A preliminary measurement of a sub-array of $4\times 16$ detectors is planned late in 2017. In this contribution we present the HOLMES project with its technical challenges, its status and perspectives.

Abstract

Task-free connectivity analyses have emerged as a powerful tool in functional neuroimaging. Because the cross-correlations that underlie connectivity measures are sensitive to distortion of time-series, here we used a novel dynamic phantom to provide a ground truth for dynamic fidelity between blood oxygen level dependent (BOLD)-like inputs and fMRI outputs. We found that the de facto quality-metric for task-free fMRI, temporal signal to noise ratio (tSNR), correlated inversely with dynamic fidelity; thus, studies optimized for tSNR actually produced time-series that showed the greatest distortion of signal dynamics. Instead, the phantom showed that dynamic fidelity is reasonably approximated by a measure that, unlike tSNR, dissociates signal dynamics from scanner artifact. We then tested this measure, signal fluctuation sensitivity (SFS), against human resting-state data. As predicted by the phantom, SFS--and not tSNR--is associated with enhanced sensitivity to both local and long-range connectivity within the brain's default mode network., Comment: 27 pages, 4 figures, 2 tables. Abstract

Background: Italy has one of the world’s oldest populations, and suffered one the highest death tolls from Coronavirus disease 2019 (COVID-19) worldwide. Older people with cardiovascular diseases (CVDs), and in particular hypertension, are at higher risk of hospitalization and death for COVID-19. Whether hypertensionmedicationsmay increase the risk for death in older COVID 19 inpatients at the highest risk for the disease is currently unknown. Methods: Data from 5,625 COVID-19 inpatients were manually extracted from medical charts from 61 hospitals across Italy. From the initial 5,625 patients, 3,179 were included in the study as they were either discharged or deceased at the time of the data analysis. Primary outcome was inpatient death or recovery. Mixed effects logistic regression models were adjusted for sex, age, and number of comorbidities, with a random effect for site. Results: A large proportion of participating inpatients were ≥65 years old (58%), male (68%), non-smokers (93%) with comorbidities (66%). Each additional comorbidity increased the risk of death by 35% [adjOR = 1.35 (1.2, 1.5) p < 0.001]. Use of ACE inhibitors, ARBs, beta-blockers or Ca-antagonists was not associated with significantly increased risk of death. There was a marginal negative association between ARB use and death, and a marginal positive association between diuretic use and death. Conclusions: This Italian nationwide observational study of COVID-19 inpatients, the majority of which ≥65 years old, indicates that there is a linear direct relationship between the number of comorbidities and the risk of death. Among CVDs, hypertension and pre-existing cardiomyopathy were significantly associated with risk of death. The use of hypertension medications reported to be safe in younger cohorts, do not contribute significantly to increased COVID-19 related deaths in an older population that suffered one of the highest death tolls worldwide.

Abstract

Background The burden of cardiovascular (CV) complications in patients hospitalised for community-acquired pneumonia (CAP) is still uncertain. Available studies used different designs and different criteria to define CV complications. We assessed the cumulative incidence of acute of CV complications during hospitalisation for CAP in Internal Medicine Units (IMUs). Methods This was a prospective study carried out in 26 IMUs, enrolling patients consecutively hospitalised for CAP. Defined CV complications were: newly diagnosed heart failure, acute coronary syndrome, new onset of supraventricular or ventricular arrhythmias, new onset hemorrhagic or ischemic stroke or transient ischemic attack. Outcome measures were: in-hospital and 30-day mortality, length of hospital stay and rate of 30-day re-hospitalisation. Results A total of 1266 patients were enrolled, of these 23.8% experienced at least a CV event, the majority (15.5%) represented by newly diagnosed decompensated heart failure, and 75% occurring within 3 days. Female gender, a history of CV disease, and more severe pneumonia were predictors of CV events. In-hospital (12.2% vs 4.7%, p p = 0.0001) mortality was higher in patients with CV events, as well as the re-hospitalisation rate (13.3% vs 9.3%, p = 0.002), and mean hospital stay was 11.4 ± 6.9 vs 9.5 ± 5.6 days (p p = 0.009). Conclusion Cardiovascular events are frequent in CAP, and their occurrence adversely affects outcome. A strict monitoring might be useful to intercept in-hospital CV complications for those patients with higher risk profile. Abstract

Purpose ofInvestigation: Solid ovarian tumors represent a clinical challenge, in particular in case of young patients who require a fertility sparing treatment. The authors report a case of hypercellular mitotically active ovarian fibrothecoma in a very young woman, successfully treated with a fertility sparing surgery.A 21-year-old nulliparous woman presented at the present hospital with a 14-cm right ovarian mass, consisting of solid and pseudo-cystic components. There was neither an elevation of tumor markers nor evidence of metastatic disease. A laparotomic right salpingo-oophorectomy was performed. Uterus and left adnexa were preserved.The neoplasm consisted of a prevalent population of spindle-shaped elements and of a minor component of cells with wider cytoplasms, attributable to a thecomatous differentiation. The mitotic activity was focally elevated. Cytological atypia was mild to focally moderate. Clear areas of coagulative necrosis were not observed. At present 48 months after surgery, the patient is alive with no evidence of recurrence.The authors reported the lesion as a hypercellular and mitotically active fibrothecoma. The uneventful follow-up confirms the low malignant potential of the lesion. Caution is required reporting hypercellular stromal ovarian tu- mors, in order to avoid overdiagnosis and overtreatment, particularly in young patients.
