4. Effect of Surgeon-Performed Thoracic Paravertebral Block on Postoperative Pain in Adolescent Idiopathic Scoliosis Surgery: A Prospective Randomized Controlled Trial.

Abstract

Posterior spinal fusion for adolescent idiopathic scoliosis (AIS) causes severe postoperative pain. Thoracic paravertebral block (PVB) provides excellent analgesia during various surgeries. We examined the effects of PVB on postoperative analgesia in children undergoing AIS surgery. In this study, 32 children scheduled for AIS surgery were randomly assigned to receive either PVB (PVB group) or no block (control group). The PVB group underwent surgeon-performed PVB with 0.5 mL/kg of adrenalized 0.2% ropivacaine on each side. The primary outcome was the pain score at rest at 6 h postoperatively. Secondary outcomes included pain scores both at rest and during movement and analgesic use for 48 h postoperatively. The postoperative resting pain scores at 6 h were comparable between the control and PVB groups (5.2 ± 2.0 and 5.1 ± 1.8, respectively), with no significant differences. However, at 1 h postoperatively, the control group showed significantly higher resting and mean moving pain scores than the PVB group (p < 0.05). The pain scores at other time points and analgesic use were comparable between the groups. Initial benefits of surgeon-performed bilateral PVB were observed but diminished at 6 h postoperatively. Abstract

Cholesterol sulfate (CS) is an activator of retinoic acid‐related orphan receptor α (RORα). CS treatment or RORα overexpression attenuates osteoclastogenesis in a collagen‐induced arthritis mouse model. However, the mechanism by which CS and RORα regulate osteoclast differentiation remains largely unknown. Thus, we aimed to investigate the role of CS and RORα in osteoclastogenesis and their underlying mechanism. CS inhibited osteoclast differentiation, but RORα deficiency did not affect osteoclast differentiation and CS‐mediated inhibition of osteoclastogenesis. CS enhanced adenosine monophosphate‐activated protein kinase (AMPK) phosphorylation and sirtuin1 (Sirt1) activity, leading to nuclear factor‐κB (NF‐κB) inhibition by decreasing acetylation at Lys310 of p65. The NF‐κB inhibition was restored by AMPK inhibitor, but the effects of CS on AMPK and NF‐κB were not altered by RORα deficiency. CS also induced osteoclast apoptosis, which may be due to sustained AMPK activation and consequent NF‐κB inhibition, and the effects of CS were significantly reversed by interleukin‐1β treatment. Collectively, these results indicate that CS inhibits osteoclast differentiation and survival by suppressing NF‐κB via the AMPK–Sirt1 axis in a RORα‐independent manner. Furthermore, CS protects against bone destruction in lipopolysaccharide‐ and ovariectomy‐mediated bone loss mouse models, suggesting that CS is a useful therapeutic candidate for treating inflammation‐induced bone diseases and postmenopausal osteoporosis. Abstract

[연구목적] 본 연구는 테마감리 회계이슈에 해당하는 기업에 대한 테마감리의 효과를 실증적으로 분석하였다. 테마감리는 회계오류 가능성이 높은 분야를 중점 감리분야로 미리 예고하여 감리를 실시하는 감리제도이다. 이에 따라 관련 기업은 해당 회계이슈에 대해 재무제표 작성단계부터 신중을 기할 것이므로 재무보고의 품질이 좋아질 것으로 기대된다. 또한 감사인은 테마감리 회계이슈를 감사위험으로 인지하여 감사노력을 증가시킬 것으로 예상된다. [연구방법] 2017년도 재무제표에 대한 4대 회계이슈 중 “개발비 인식·평가의 적정성”과 관련하여, 의료제조업을 영위하는 상장기업의 2014년부터 2017년까지 374개 기업-연도 자료를 이용하였다. [연구결과] 실증분석 결과, 첫째, 테마감리 시행 이전보다 이후에 연구개발비 자산화율이 유의하게 감소하였다. 한편, 감사보수는 2018년에 증가하였고, 감사시간은 2017년 및 2018년에 모두 증가하였다. 마지막으로, 추가분석에서는 테마감리 대상인 2017년도 재무제표의 연구개발활동 관련 기재정정 횟수가 이전 기간의 기재정정 횟수에 비해 유의하게 증가한 것으로 나타났다. [연구의 시사점] 본 연구는 테마감리를 실시한 결과, 회계오류 취약 분야를 개선하는 데 효과가 있었음을 실증적으로 제시하였다는 점에서 의의가 있다. 이러한 결과가 회계감사 관련 제도의 효과 및 회계보고품질 제고에 관심을 두고 연구하는 연구자들과 감독 당국에 의미 있는 시사점을 제공할 것으로 기대한다.

17. 한국어 교육에서의 교사 발화 연구 동향 분석

Abstract

이 연구는 지금까지 한국어 교육에서 이루어진 교사 발화 관련 연구들을 종합적으로 고찰하여 검토하고 이를 통해 앞으로의 교사 발화 관련 연구의 방향성을 제안하는 데 그 목적이 있다. 이를 위해 2018년 12월까지 발표된 국내 석, 박사 학위 논문과 한국연구재단(KCI)의 등재(후보지) 학술 논문을 대상으로 하였다. 논문 검색을 위해서는 학술연구정보서비스(RISS, www.riss.kr) 사이트를 이용하였고, ‘주제어 검색’과 ‘원문 확인’ 작업을 바탕으로 학위 논문 54편(석사 학위 논문 48편, 박사 학위 논문 6편)과 학술지 논문 51편으로 총 105편을 선정, 검토하였다. 먼저 시기별 및 유형별로 분석한 결과 1998년을 시작으로 2001년, 2004년에 교사 발화에 대한 논의가 본격적으로 시작되었고 2012년과 2013년을 기점으로 수적인 증가를 보인다. 주제별 분류 결과로는 교사 발화의 언어적 특징(6편), 교사의 학습자 모어 사용(7편), 교실 내 상호작용에서의 교사말/발화 연구(28편), 스캐폴딩(7편), 오류 수정 피드백(36편), 질문/발문(21편)으로 나타났다. 연구 방법별 분류 결과로는 교실 안(59편), 교실 밖(46편), 교육에의 적용 목적(61편), 습득 과정의 기술 목적(44편), 질적(36편), 양적(31편), 혼합(38편)으로 나타났다. 이 외에 언어적 유형을 기술한 논문은 10편으로 나타났다. 지금까지의 한국어 교사 발화 관련 연구는 수정 피드백, 상호작용에서의 교사말/발화, 질문/발문의 유형과 기능에 대한 연구가 주를 이루었다. 그러나 이들 연구의 대부분은 교사 발화가 교실 내 수업을 기반으로 함에도 불구하고 교실 밖 상황에서 수집된 자료 방법을 많이 사용되었다. 앞으로 교실 내 자료에 근거하여 교사의 발화가 수업 진행 과정에서 교사 발화의 언어적 분석을 바탕으로 학습자의 반응이나 언어 습득에 미치는 효과 등을 논의하는 연구가 보다 필요하다고 하겠다.

Abstract

Cinchonine (CN) has been known to exert antimalarial, antiplatelet, and antiobesity effects. It was also recently reported to inhibit transforming growth factor β‐activated kinase 1 (TAK1) and protein kinase B (AKT) through binding to tumor necrosis factor receptor‐associated factor 6 (TRAF6). However, its role in bone metabolism remains largely unknown. Here, we showed that CN inhibits osteoclast differentiation with decreased expression of nuclear factor of activated T‐cells, cytoplasmic 1 (NFATc1), a key determinant of osteoclastogenesis. Immunoblot and quantitative real‐time polymerase chain reaction analysis as well as the reporter assay revealed that CN inhibits nuclear factor‐κB and activator protein‐1 by regulating TAK1. CN also attenuated the activation of AKT, cyclic AMP response element‐binding protein, and peroxisome proliferator‐activated receptor‐γ coactivator 1β (PGC1β), an essential regulator of mitochondrial biogenesis. Collectively, these results suggested that CN may inhibit TRAF6‐mediated TAK1 and AKT activation, which leads to downregulation of NFATc1 and PGC1β resulting in the suppression of osteoclast differentiation. Interestingly, CN not only inhibited the maturation and resorption function of differentiated osteoclasts but also promoted osteoblast differentiation. Furthermore, CN protected lipopolysaccharide‐ and ovariectomy‐induced bone destruction in mouse models, suggesting its therapeutic potential for treating inflammation‐induced bone diseases and postmenopausal osteoporosis. 43. Enantioselective light switch effect of Δ- and Λ-[Ru(phenanthroline)2 dipyrido[3,2-a:2′, 3′-c]phenazine]2+ bound to G-quadruplex DNA

Abstract

The interaction of Δ- and Λ-[Ru(phen)2DPPZ]2+ (DPPZ = dipyrido[3,2-a:2′, 3′-c]phenazine, phen = phenanthroline) with a G-quadruplex formed from 5′-G2T2G2TGTG2T2G2–3′(15-mer) was investigated. The well-known enhancement of luminescence intensity (the ‘light-switch’ effect) was observed for the [Ru(phen)2DPPZ]2+ complexes upon formation of an adduct with the G-quadruplex. The emission intensity of the G-quadruplex-bound Λ-isomer was 3-fold larger than that of the Δ-isomer when bound to the G-quadruplex, which is opposite of the result observed in the case of double stranded DNA (dsDNA); the light switch effect is larger for the dsDNA-bound Δ-isomer. In the job plot of the G-quadruplex with Δ- and Λ-[Ru(phen)2DPPZ]2+, a major inflection point for the two isomers was observed at x ≈ .65, which suggests a binding stoichiometry of 2:1 for both enantiomers. When the G base at the 8th position was replaced with 6-methyl isoxanthopterin (6MI), a fluorescent guanine analog, the excited energy of 6-MI transferred to bound Δ- or Λ-[Ru(phen)2DPPZ]2+, which suggests that at least a part of both Ru(II) enantiomers is close to or in contact with the diagonal loop of the G-quadruplex. A luminescence quenching experiment using [Fe(CN)6]4- for the G-quadruplex-bound Ru(II) complex revealed downward bending curves for both enantiomers in the Stern–Volmer plot, which suggests the presence of Ru(II) complexes that are both accessible and inaccessible to the quencher and may be related to the 2:1 binding stoichiometry.

44. Usefulness of non-invasive cardiac output monitoring in elderly patients undergoing monopolar transurethral resection of the prostate: a pilot study

Abstract

Objective To assess the impact of irrigating fluid on hemodynamic profiles using real-time non-invasive cardiac output monitoring (NICOM) in elderly patients undergoing monopolar transurethral resection of the prostate (TURP).Methods Twenty patients between 65 and 80 years of age who were scheduled for monopolar TURP and received spinal anesthesia up to T10 were enrolled. Irrigating fluid (2.7% sorbitol with 0.5% mannitol solution) was used. Hemodynamic profiles including cardiac index, and stroke volume variation (SVV) using NICOM were obtained. Estimated irrigating fluid absorption was indirectly calculated.Results The median amount of irrigating fluid used was 6000 mL. The median SVV was 11%, which increased to 12% at 10 minutes after initiating surgery. No significant changes in the cardiac index were observed. The estimated absorption of irrigating fluid was almost zero.Conclusions Although the estimated amount of irrigating fluid that was absorbed was negligible, the increase in SVV may indicate intravascular volume depletion with diuresis resulting from mannitol in the irrigating fluid early during irrigation. Therefore, even during short irrigating times, intensive hemodynamic monitoring should be performed to monitor the possibility of intravascular volume depletion as well as volume overload, especially immediately after large amounts of irrigating fluid are used.
