Abstract

Cytomegalovirus (CMV) commonly infects immunocompromised individuals, such as cancer patients. We present a case involving a 60‐year‐old male with Stage 3A lung adenocarcinoma and chronic obstructive pulmonary disease (COPD) diagnosed with CMV tracheobronchitis, initially suspected as cancer progression. Treatment with ganciclovir led to partial improvement in symptoms of shortness of breath and cough, as well as bronchoscopic findings. However, due to ganciclovir‐induced neutropenia, the therapy was switched to foscarnet. Distinguishing between cancer progression and infectious tracheobronchitis through physical examination and chest CT scans remains challenging. In lung cancer patients presenting with airway and bronchial narrowing along with ulcerative mucosal lesions, CMV infection should be considered. A bronchoscopic biopsy is crucial for accurate diagnosis and determining the appropriate treatment in these patients. [ABSTRACT FROM AUTHOR]

Abstract

Pulmonary large cell neuroendocrine carcinoma (LCNEC) is a rare and aggressive subtype of non‐small cell lung cancer with a poor prognosis. Spontaneous regression, that is, partial or complete disappearance of a malignancy without medical intervention, is extremely rare in LCNEC. Herein, we present a case of spontaneous complete regression in a 71‐year‐old male patient with recurrent LCNEC after surgical resection. The patient was diagnosed with stage IB LCNEC and underwent surgical resection. At 1‐year follow‐up, chest computed tomography revealed a recurrent lesion next to the stump site and enlargement of lymph nodes 4R and 7; recurrent LCNEC was confirmed. The patient declined chemoradiation therapy. One year after recurrence, the patient experienced severe multifocal necrotizing pneumonia and was treated with antibiotics, resulting in a gradual decrease in the size of the recurrent lesion. Five years after the initial diagnosis, positron emission tomography/computed tomography revealed no hypermetabolic lesions, indicating the spontaneous complete regression of LCNEC. Abstract

The demand for liquefied natural gas (LNG) ships is increasing for various reasons. Despite their enormous size, cargo holds inside freighters transporting LNG have traditionally been constructed by welding to high-quality standards for safety. This process traditionally relies on manual labor or semi-automatic moving devices. In this study, a methodology was designed for robot-based automated laser welding inside large LNG cargo holds. The developed approach offers a practical solution to challenging issues such as the corrugation of the membrane that forms the inner walls of LNG cargo holds and the inter-floor movement of robots. This study analyzes and restructures the work for laser welding using mobile robots inside LNG cargo holds composed of membranes. For realistic constraints, such as inter-floor movement of robots and high-quality welding of membrane corrugations, methods integrated with manual work have been proposed. Additionally, for the overall membrane laser welding of the LNG cargo hold space, an automated method using robots was suggested. Abstract

Background: Lung cancer is the most common cause of cancer deaths worldwide and accounts for approximately 2 million deaths annually. Despite advances in lung biopsy methods using bronchoscopy, a transthoracic needle biopsy continues to be used widely owing to its excellent accessibility and cost-effectiveness. Current Concepts: Various guidance methods are used during a transthoracic needle biopsy to guide the biopsy needle toward the target lesion. Commonly used modalities include conventional computed tomography, computed tomography fluoroscopy, cone beam computed tomography, and ultrasonography. Complications of a transthoracic needle biopsy include pneumothorax (20.0%), hemorrhage or hemoptysis (11.0%), delayed pneumothorax (1.4%--4.5%), air embolism (0.02%--1.8%), and tumor seeding (0.12%--0.061%). Discussion and Conclusion: Careful selection of the guidance method and needle type, based on the risk factors, the patient's condition, and location of the lesion is important to achieve high accuracy and low complication rates during a transthoracic needle biopsy. If possible, the bronchoscopic approach should initially be attempted in high-risk groups. [ABSTRACT FROM AUTHOR]

However, there is concern over whether a single measure of blood eosinophils is sufficient for outlining a treatment plan. Here, we evaluated the association between variability in blood eosinophils and the effects of ICS in stable COPD cohorts.Methods: COPD patients in the Korean COPD Subtype Study and the Seoul National University Airway Registry from 2011 to 2018 were analyzed. Based on blood eosinophils at baseline and at 1-year follow-up, the patients were classified into four groups with 250/μL as a cutoff value: consistently high (CH), consistently low (CL), variably increasing (VI), and variably decreasing (VD). We compared rates of acute exacerbations (AEs) according to ICS use in each group after calibration of severity using propensity score matching.Results: Of 2,221 COPD patients, 618 were analyzed and a total of 125 (20%), 355 (57%), 63 (10%), and 75 (12%) patients were classified into the CH, CL, VI, and VD groups, respectively. After calibration, we found that ICS users tended to have a lower AE rate in the CH group (RR 0.41, 95% CI 0.21– 0.74) and VI group (RR 0.45, 95% CI 0.22– 0.88), but not in the CL group (RR 1.42, 95% CI 1.08– 1.89) and VD group (RR 1.71, 95% CI 1.00– 2.96).Conclusion: More than one-fifth of patients had an inconsistent blood eosinophil level after the 1-year follow-up, and the AE-COPD rate according to ICS differed based on variability in eosinophils.
